    Here’s an idea somebody should’ve thought of. There are thousands of us out there who continued to take hormone replacement therapy,. I was much more afraid of osteoporosis, which is prevalent in my family Than I was breast cancer which is not. So that was in 1998; it is now 2024 and I am osteopenia not osteoporosis and do not have breast cancer. So why doesn’t somebody start a survey of all of those who have been taken hormone replacement-for 20 or 30 years and see if they have gotten breast cancer. I have not. It would not cost anything. I am much more afraid of these new bone drugs than I am Estradiol which is what I’m taking now.

      Thank you for sharing your experience. I am on hrt for the same reasons, afraid of osteoporosis, which I have but more of the bone drugs than hrt. Two years, so far, so good have slowed none loss but not reversed. Started pilates, and here drops. Keeping fingers crossed. Studies the these would help such a large population!

      Yes, God forbid anyone do any new real studies on women's health! I've had to fight with doctors about continuing to take HRT because they still think it causes breast cancer, which has been debunked. I'm more worried about osteoporosis and heart disease than I am about breast cancer.

      I've been on hormone replacement for over a decade, except for one short period when I tried alendronate (Fosomax), which was pure disaster: 3 years of pain from taking two (just two!) doses. I worry about cancer, but my estradiol dose is low, and I only take 12 days of micronized progesterone once every 4 months.

    So eternally grateful. Hrt is definitely for me but I just want to tweak dosages of Estradiol, Progesterone and Testosterone to suit my labs. And heading to a Masterclass on hormones. The positive impact you have made oin my life where there is basically no other medical support here for osteoporosis 😊🙏🙏

    The women that participated in the study had underlying health conditions, had been menopausal for more than 10 years, were smokers or former smokers, had high blood pressure, were obese, etc. if I recall correctly the average age was 65 😮

    Thanks for doing the deep dive into the studies! I am 64 with ER+ PR+ Her2 low metastatic breast cancer. I have had serum and stool testing for hormones and hormone metabolism. I literally have no measurable estrogen. This testing was done during a time I was off of any hormone blockers for at least two years. The idea of hormone blockers for postmenopausal women who have very little estrogen to start with always bothers me. It just does not make sense. Why did I not get this when I had more circulating estrogen? ( premenopausal) 0:59
    Taking the hormone blockers further starves my body of estrogen which will cause bone loss and the answer is prolia. I just don’t like the idea of taking a drug to manage the side effects of another drug. Especially a drug with that side effect profile.
    It just does not make sense. Maybe the natural estrogen was holding it back in some way who knows 0:59

    Great analysis! Thank you. This topic is essential for women to understand. I’ve studied it for a few years and my takeaway is the same. For most women, topical estradiol and micronized progesterone is the way to go. No one wants breast cancer but we also don’t want brittle bones, depressed or anxious mood, brain fog or dementia, slow metabolism, no sex drive and the list goes on. I plan on being in HRT till I die.

    There are many different types of breast cancer. With the second study, it might also depend on the type of breast cancer that it was protective against. Too much estrogen increases tumor proliferation if it is ER/PR positive. But HER2 would likely not be affected negatively by Estrogen/progestins. I am glad you explained to people how data sets can be manipulated. Some people may benefit from taking Estrogen, but if you have cancer cells that are ER/PR+, and did not know they had these tumors yet, because they had not been detected, it (the Estrogen supplement) can increase the growth of that tumor or cells if they have this type of cancer.

    I was one of the millions of women who were “ripped off hormone replacement.” If I had stayed on BHRT, I probably wouldn’t have osteoporosis! 💔

      Me too!!! Lost 18.8% bone loss in 2 yrs bc of being ripped off of HRT- which my provider couldn’t figure out why and had me go through many other labs to fine NOTHING wrong! I had to figure it out on my own!!! Maddening!!

      If it's any comfort, unless you plan to stay on HRT for life (probably not a good idea), when you discontinue there will typically be a large drop in bone density in the first few years, and then the rate of bone loss will slow. So staying on HTR through menopause will only delay, not prevent bone loss.

      @@kathleensutherland6593, this idea of stopping estrogen at a certain age has been debunked. Unless there is some contraindication, there is no reason to age out of HRT.

    I know someone who was in estrogens, and later on she got breast cancer. She is doing fine after treatment. But I personally prefer not to take chances.

      I was taking estrogen for my bones and also got breast cancer. I'm also doing fine but am challenged about my bone health as I don't want to take the meds. I have osteoporosis -3.3 in my spine.

      Sadly, I know several family members who experienced taking estrogen and developed breast cancer.Also some who took osteoporosis medicine only to find out 20 years later they needed to take more because they had osteoporosis again.

    Hi Dr Doug : you might be interested in a paper published dec 2023 , lead auther Dr. Avrum Bluming ( oncologist /hematologist) disputing the statical analysis of the CEE /MPA arm of the WHI. He said that the supposed elevation was in breast cancer risk in this group is an illusion caused by the control group having a collective lower risk of breast cancer. Unfortunately some of the control group had been on estrogen for n the past which would lower their risk of breast cancer. If these women were removed from the control group there was no real increased risk of breast cancer.

    I took a combination birth control pill for 15 years and got breast cancer. My grandmother was given estrogen replacement to help her with menopausal symptoms and she got ovarian cancer. Excess estrogen causes female cancers and no one can tell me differently. We are supposed to go through “the change” of life. Yes, it sucks. Hot flashes, insomnia, weight gain, mood swings, etc. However, it is something we go through. These symptoms do dissipate with time. We reach a “new normal”. Trust me..chemo, radiation, mastectomy, hysterectomy, reconstructive surgeries, endocrine therapy, targeted therapy…it’s all a hell of a lot worse than dealing with menopausal symptoms! All these doctors now saying hormone replacement therapy is safe..just wait. Same as they’re now saying old studies were wrong, 20 years from now, they’ll be saying we were wrong again. As for our bones, calcium, vitamin D, and lift weights! Go walking, do jumping jacks, lunges. Those exercises are best thing to prevent bone loss and help with menopause weight gain too!

    Thank you for all this. At age 61, I've been on low-dose transdermal estriodal and micronized progesterone for over a decade, and I'm glad to know it's relatively safe. But I may switch to raloxifene, which seems even safer. And thank you for saying you wouldn't wish breast cancer on anyone, rather than "on my worst enemy." When people say the latter, I wonder, "Just how many enemies do you have?" And what WOULD you wish on your worst enemy? It just doesn't seem a healthy mentality.

    I have been taking various forms of Estradiol and Progesterone tablets (never Progestin or horse hormones-always natural) for about 17 years. Primary care physicians have typically cautioned me with doom and gloom for continuing to take these meds. Gynecologists have typically been fine with my continuing to Rx these meds. No cancer, test yearly, BP is low normal. Over the years it has been so difficult to see how friends and acquaintances suffer with menopausal symptoms and post menopause symptoms because they could benefit from hormone therapy.

    I wish my gyno had been up on of this during my years of being compliant to all of the Pap smears and mammograms. My hormones were never checked, but I sure as s*** had those routine exams done! So frustrating. I now have osteoporosis at 58.

    This 2023 report from Harvard is alarming, although it does not mention estradiol. It says estrogen is both a generator and catalyst of breast cancer news.harvard.edu/gazette/story/2023/05/estrogen-a-more-powerful-breast-cancer-culprit-than-we-realized/

    My aunt in Norway in the 80s took very high doses of estrogen and died of breast cancer. So far she is only woman in family to have breast cancer.

      Most breast cancer cases are not hereditary anyway. Cancer is a very personal disease, and there is so much to take into account- weight, diet, lifestyle choices, chemical exposure, exercise, detox pathways, occupation, how many kids a woman has and when, stress level, water filtration, etc. Sorry about your aunt!

    I never took HRT. I have lifted weights since my mid 20’s. I am now 65, still lift weights. In 2012 I was diagnosed with Estrogen positive breast cancer. I was 51 at the time, no osteoporosis. I took Anastrozol for 5 years. My bone density went down a little. I continued to lift weights. I’ve been off Anastrozol for almost 7 years and my bone density has increased and have never had osteoporosis. I was barely osteopenia in one area of hip during treatment. No longer a problem. I believe for myself- estrogen would have been dangerous had I taken it earlier on. Everyone is different with different risks. I truly wish I could take HRT for its benefits, but have had to come to peace with my own situation and make other healthy choices that can benifit me.

      If you are cancer free now, you may still be able to take some HRT. As gilliandowney said, estrogen does not cause breast cancer, and HRT can reduce your risk of heart disease and dementia. It would be worthwhile for you to at least search out a qualified professional to help you explore what options may be appropriate for you, while taking into consideration your medical history.

    Dr.Doug I would like to ask you 2 questions. I have severe osteoporosis induced by corticosteroids and all my vertebraes have compression fractures. Prolia doesn't help, fractures are still happening. Can I start with hormone replacement treatment in my 57 years? I already had hysterectomy. Combination of osteoporosis medications plus HRT would be more effective?

    Hello, just want to give an additional train of thought. I get what Dr. Doug is saying and am also steeped in the hormone breast cancer studies.. I am also positive ER and progesterone and have had breast cancer a couple of times. I learned that if I understood my actual personal hormone levels, if the system monitored them, my hormone levels, I would have known that I was on a steep precipice cliff, that taking any more was going to push me over, in my particular case. We are are all unique and indivulized ..Its like, I don't want to eat antibiotics or pestisides in meat, produce, and grains or eat charred meat, or drink contaminated water, etc. There are some of us who do not cycle out excess levels systematically. There are things we can do if we are one of them. I built up a huge hormone reserve that started the process. OH MY I love how estrogen makes me feel. I am finding safe alternatives for me, now sure my bones might be stronger if I was still on it..? But my risk of yet another metastatic occurrence or stroke would be heightened. This taking responsibility for our own bodies and health outcomes is more than a full-time job, has helped me stay alive another 10 years after recurrence. I want to live long enough to enjoy 90 like my Mom did, who died in her sleep last month. Intuition is our highest healing treatment.

I wish there were experts we could all trust or at least a great place to learn ourselves. May I ask what estrogen alternatives you have found that work for you?     What a god send you are sir! I've been worried about my wife who has recently been diagnosed with Osteoporosis quite early at the age of 45 and following an early menopause at the age of 40. She stayed away from HRT initially but I'm wondering if that was a mistake. We go to see our GP next week following the diagnosis and we will be more knowledgable with the information you have shared in this video. We both exercise regularly and eat the right food so I am hoping in combination with medication we can reverse this illness. Thanks again from the UK.

    Hello, just wanted to share my thoughts. I am 62, I have severe osteoporosis. I had endometriosis treated in the Uk by norethesvterone and in France by Lutheran and Lutenyl (which are progesterone), I ended up with 2 breast cancers which from what I understand where mostly caused by being put on primolut nor (norethisterone). When they treat your hormone dependant cancer, they give you either tamoxifen or anti aromataze, which are both designed redirect (if I understand well) the oestrogen towards other parts of your body in order to protect your breasts; If ostregeon doesn't increase breast cancer why the need to such drugs to be taken for years? thank you for answers.       Estrogen does not cause breast cancer, but if you currently have an estrogen receptive breast cancer, taking estrogen will stimulate the cancer cells to grow faster. You just need to understand that there is a difference in the two concepts. Causation with respect to cancer is not the same thing as stimulation of an already existing cancer.

    What are your thoughts about women who have already had estrogen sensitive breast cancer? I had a lumpectomy for this 7 years ago and was given a 98% chance of it not recurring. I am 75 and have osteoporosis.

    I have been told over and over that I cannot ever take estrogen because I had breast cancer 20 years ago. I have osteoporosis now. I am considering taking a SARMS called Osterine, start OsteoStrong and change my diet. Do you know anything about SARMS and Osterine? Btw, the last blood test years ago showed no testosterone or estrogen in my body. Thank you

    There is theory out about the type of estrogen that are causing and feeding fhe breast cancer the estroegn from viseral fat and from ovaries.. any studies on that?

    One can easily manipulate the outcome of a study by changing the hypothesis language. Not saying they did.
    I'm curious if the researchers have conflict of interests-- who funded their study?

    I cannot thank you enough for providing us with this analysis of the literature!!! I have one question for you….When I started on an estradiol patch I already had in place a mirena IUD, which releases small amounts of progestin in a localized fashion. Therefore I am not taking any progesterone.
    What is your take on this approach…estradiol patch and mirena IUD? Since the IUD releases progestin, which as you discuss, is most convincingly associated w breast cancer risk, would it better to remove the IUD and start taking the micronized progesterone?
    I was thinking the localized release of progestin into the uterus is much safer than taking it orally and the progestin would not even reach the breast tissue, negating the breast cancer risk. I have not heard much discussion of this option and would love to know your thoughts.

    Patches don't cause stroke, which is nice. And not having it doesnt stop you getting breast cancer. My aunt got her mono at 35 and sisnt take hrt she got osteo at 40 and is now 84 ...shes in bits and agony and a hunchback. She should definitely have been given estrogen ...or why her periods stopped at 35. We also had lead pipes groiwng up. Anyway she didn't take estrogen and still got cancer but uts gone now. Cancer is pretty easy these days to treat and you can also take estrogen blockers that block estrogen uptake in the breast ...

    Interesting, confusing, but helps a lot. I will continue with my bio identical hrt , including testosterone, it seems to be helping so i will ignore the so called medics !! Muchas gracias de España xx

    I was told. I was fat which increased estrogen which caused estrogen. Which caused cancer. I was 59.

    No family history of BC, Very thin and fit, daily exercise, non smoker, non drinker, organic, pasture raised, grass fed food, no jab, etc. Only bioidentical HRT for 9 years. Then BC!!! Er and Pr positive and Her2 low. So stop telling yourself that it is women who MUST have had some other risk factor who get BC.

      Very LOW RISK. Same here, thin 5’2” 112# had my children young and nursed them, ate clean like you did, no jab. Surgical menopause at 47. Bio-identicals for several years, never synthetic type. I do feel possibly my taking the BHRT kept my BC from being aggressive and it was tiny 4mm ILC in left breast and 2 tiny DCIS in rt breast, atypical lobular hyperplasia in both. The DCIS/hyperplasia was found via a breast reduction/lift and then I chose a bil mastectomy which discovered the tiny 4mm ILC, node neg, etc. Was ER+, PR+ HER2-. My mammogram was clear prior to my breast reduction surgery!! I am not taking the AI meds,(med onc is not happy with me). I am 64, 1 yr out from my dbl mast. I already have penia. I was off my estriol/prog creams about 9 mos and was just awful, no quality of life. Back on them. I have not taken estradiol in years, I have read estriol is a safer and milder estrogen and there are studies it too can help with bone health, protective of breasts. I am also able to be sexually active thanks to compounded vag estriol 3x a wk. Thank you for this video.

    Over weight women and men have excess estrogen and that can lead to breast cancer in both sexes. If for some reason you choose to take HRT then keep your body fat low

    I started HRT for early menopause and continued to take it for 18 years during which I was diagnosed with osteoporosis I still had a hip fracture after taking fosamax for 8 years I ended up with non genetic ovarian cancer which is an oestrogen fed cancer Neither the HRT or fosamax prevented the hip fracture and I'm' of the opinion HRT caused the ovarian cancer

    Thank you for this overview. I am a 2x survivor of estrogen + breast cancer, one before and one 25 years later. The question: can there be any inferences from these studies or others that show BC risk vs osteoporosis risk with taking these HRTs? I guess I’m asking, if no HRT what is the risk of osteoporosis/Penia as compared to the risk of taking HRT and BC? Not sure if this is a viable analysis question, never took statistics. But I guess weighing the two risks someone could make a decision. Too late for me and HRT, I’m not even supposed to eat phytoestrogen foods.😢

      God you can get beast cancer treated if it happens. It's so hard to fix bones. I mean pepple are on the contraceptive pill for years and years and years and then they pull estrogen for osteo? Thats wack.

      That is such a great test. There's nothing else that tells you about detox pathway, methylation, amount of actual hormone metabolites you have, etc. My 16 OH went up from 2 years ago and we're trying to figure out why. Every woman should have the DUTCH!
