    A lady is concerned about the sudden appearance of multiple folds on what used to be double fold upper eyelid, and fine lines on her lower eyelid. She wants to know if this is due to excess skin, and if fat injection can improve its appearance.
    New York oculoplastic and reconstructive surgeon Amiya Prasad, M.D. reviews her concerns in this video, and explains the likely cause of the sudden development of multiple creases on her upper eyelid, how they can be treated by a specialist, and why the fine lines on her lower eyelid don’t need treatment:
    1:15 - Dr. Prasad details his background training in cosmetic oculofacial plastic and reconstructive surgery, as well as his experience as an eyelid rejuvenation specialist
    2:00 - Dr. Prasad explains how he determines whether to perform incisional or non-incisional surgery for Asian double eyelids
    2:27 - The goal of both incisional and non-incisional approaches
    2:38 - Dr. Prasad talks about the likely causes for excess creases on the eyelid
    3:02 - Why Dr. Prasad usually opts for the non-incisional Asian blepharoplasty for younger individuals, and how this procedure is performed
    3:51 - How fat grafting has no benefit in this scenario, and how the lines present there are only a natural result of facial expression
    4:23 - Why it is advisable to consult with a specialist such as an oculoplastic surgeon who does a lot of Asian eyelid surgery, and Dr. Prasad’s recommendation

      Hi Mark Lee's Eyebrows,

      Hi M Shaz,
      Eyelid tape can help the appearance of a double eyelid, but this is temporary and won't encourage the eyelid skin to make stronger connection to the eyelid muscle. A stronger connection between the eyelid skin and muscle has to be done through surgery. I hope this clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  5 лет назад +10

      Hi Odette Zuma,
      Multiple creases are not caused by a weak eye muscle called the levator muscle, but a poor connection between the levator muscle and the upper eyelid skin. If you have a weak levator muscle, it's a different condition known as eyelid ptosis, where the eyelid droops. In eyelid ptosis, the muscle can't be strengthened, but it can be shortened or reattached. I hope this clarifies things for you.

    • @AmiyaPrasadMD

      Hi Kimi Wu,
      Massage is not recommended for your eyelids as they can loosen the connection between the eyelid skin and muscle that lifts the eyelid, and this connection creates the crease or fold. If you loosen it by massaging it, you will weaken the connection further, and you may lose your crease. The only way to create a double eyelid crease, or a more defined crease, is through double eyelid surgery, as featured in the video. I hope this clarifies things for you.

      It is possible to have a loose connection between your eyelid skin and eyelid muscle that creates the crease, even if you are not Asian. A proper evaluation is needed to determine the cause, and the cost. There are no set costs for surgery as each patient has different needs, and some surgeries are more extensive than others. Hi March Seffelly Pestano Paclibar,
      Sorry to hear about your problem. It is possible for aggressive eye rubbing to loosen the connection between the muscle that lifts the eyelid called the levator muscle, and the eyelid skin, as this connection is what creates the double fold. If this is the case, only surgery can reestablish and strengthen that connection. A physical examination is needed to confirm your issue. I hope this clarifies things for you.

      Hi Muskan Achhava,
      Having multiple folds in your eyelid is a cosmetic issue, and does not affect eye or eyelid function. It may be due to a weakened connection between your eyelid skin, and the muscle that lifts the eyelid. You can have it treated for a more defined eyelid crease, if you want to change its appearance, but there usually is no health or functional issue if you leave it as it is. I hope this clarifies things for you.

      Hi Watch Out,
      It is highly unlikely as multiple creases are caused by a weak internal connection between the eyelid skin, and the muscle that lifts the upper eyelid called the levator muscle, and a topical serum would not be strong enough to cause this issue. I hope this clarifies things for you.

I have hooded double eyelids but I noticed that when I press on them or even blink the crease position changes and I get multiple eyelid creases. Hi @SomiarOfDreamers,
      Sorry to hear about your problem. Intermittent or temporary multiple, shallow creases are likely due to weak skin-to-muscle connection. The connection can be strengthened only with surgery for a stable, defined, and deep eyelid crease. Surgery can also treating hooding due to excess or stretched eyelid skin, as well as to reduce fat behind the upper eyelids. I hope this clarifies things for you.

      Hi Eman the baller Obika,

    Hi Doc, if i will apply eyelid glue everyday for years, would it somehow develop the eyelid shape that I want overtime? Would it also be permanent? I did have a stye several years ago and it resulted of uneven eyelid on my right eye 😞 and another thing, is the non-incisional procedure invisible in the eyes and permanent? Does it leave scars overtime or produce any side effects?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  3 года назад

      Hi E Ph,
      No, because eyelid glue is only used externally on the skin, while a stable eyelid crease is created internally with a connection between the eyelid skin, and the muscle that lifts the eyelid called the levator muscle.
      Depending on how non-incisional eyelid surgery is performed, and who performs it, obvious signs of surgery should not be seen at all, and the crease looks natural.
      Expecting permanent results is not realistic, as eyelid surgery does not stop aging, so there could be skin laxity, and more fat in the upper eyelid as one gets older, which can obscure the eyelid crease. I hope this clarifies things for you.

    Is there any way I could fix it or treat it at home. I woke up one morning like a month ago or so and my eyelid was tripled, now I didn’t worry because it had happened before and would go away, but now it’s stayed like this for about a month. Idk if it has anything to do with how hard I rubbed my eye because I did that I think, and also there has been dry skin that has flaked off my eye when I rubbed it too hard. I think these might be related but I really want to get rid of the triple eye lid. I’m still young and I wouldn’t really consider surgery, so is there any way I could fix it? Or what should I do?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  Год назад +1

      Hi Yeti,
      Sorry to hear about your problem. It is possible for aggressive eye rubbing to loosen an already weak fibrous connection between the eyelid skin, and the eyelid muscle. Only surgery can establish a connection between the eyelid skin and the muscle to create a stable and defined eyelid crease. Some people use eyelid tape or glue to artificially create a temporary crease, but I don't recommend this as pulling the adhesive on and off can cause more damage, and even damage the eyelid muscle, which can cause the upper eyelids to droop. I normally don't recommend double eyelid surgery until the early 20s when the face has stopped growing, and maturing. I hope this clarifies things for you.

      Orbital fat around the eyes, which does cause eye bags when they prolapse forward, is usually not lost when you are trying to lose body weight and reduce fat in other areas. Hi @@Vishwa_S_11,
      Surgery is the only way to create a stronger connection between the upper eyelid skin, and the muscle that lifts the eyelid called the levator. The point where this connection is made is where your eyelid folds, thereby creating a visible crease. There is eyelid tape to temporarily create a crease, but this has its own set of issues, such as daily application, unpredictability and inconsistency of results, and not knowing when it can come loose. Consideration for surgery for someone your age needs to be considered on a case-by-case basis, as I normally perform cosmetic surgeries on people who have matured to the point where the face has already stopped growing, so results are more predictable. I hope this helps you.

      Hi RUclips Videos,
      This happens most commonly in people of Asian descent, but it can happen in other ethnicities as well. A similar procedure can be done for Caucasian eyes, but the exact procedure can be determined after a proper physical exam of your eyelids. I hope this clarifies things for you.

    My multiple eyelids (there are more than three) was like on and off but now it seems they extra lids don't go away(I've been wearing contact lenses for ages can that be the cause?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  3 года назад +1

      Hi Nita,
      Sorry to hear about your problem. It is possible that your contact lenses further weakened the connection between the eyelid skin, and the muscle that lifts the eyelids called the levator. It's the strength of this connection that's needed for a stable and defined eyelid crease. I hope this clarifies things for you.

    One of my eyelids have been like that for 8 months. But my other eyelid is normal. I am going to a eye doctor in February. Is there a natural way other than surgery? And if I get this surgery will it count as plastic surgery bc I don’t want to say I got plastic surgery.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  3 года назад +1

      Hi Hi Be,
      Surgery is the only way to strengthen the connection between the eyelid skin, and the muscle that lifts the eyelid called the levator muscle. Technically, if you had a double eyelid before, and lost it, it would be counted as plastic surgery because it is correction or restoration. If you had a monolid before, and want a double eyelid created now, it would be considered cosmetic surgery. I hope this clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  3 года назад +1

      Hi Fahim,
      An extra crease is usually due to a weak connection between the eyelid skin, and the muscle that lifts the eyelid called the levator muscle. An inadvertent injury may have caused this, but a proper diagnosis through a physical examination is needed to determine the exact issue. A weak eyelid skin-to-muscle connection usually does not resolve on its own. I hope this helps you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  4 года назад

      Hi abcdefghijklmnop,
      A natural crease since you were born is due to fibers making a connection between they eyelid skin and the levator muscle, and the eyelid skin folds into this crease when your eyes are open. An examination is needed to determine whether your new double eyelid is due to a weakened connection, stretched or redundant skin, and if the eyelid skin is folding into this new crease. I hope this clarifies things for you.

      A physical examination is needed to determine the best procedure for you, but for your eyelid creases it is likely incisional, or non-incisional double eyelid surgery.
      If you believe your upper eyelid is sagging, it is especially important to have it examined by an oculoplastic/ oculofacial surgeon for eyelid ptosis. In the absence of these specialized eyelid surgeons, you should see an ophthalmologist about eyelid ptosis. Hi Manmeet Kaur,
      As described in the video, multiple eyelid creases are most often due to a weak connection between the eyelid skin, and the muscle that lifts the eyelid. Eyebrows can have an asymmetrical position due to bone structure as the two halves of the face form separately before birth. Your face is still growing at your age, so normally I don't recommend treatment until after the face stops growing in the early 20s. I hope this clarifies things for you.
