How to Keep (if possible) the Ability to Burp After Botox Treatment for R-CPD

  • Inability to burp (aka R-CPD or no-burp) can thankfully be treated successfully with a simple outpatient injection of Botox into a muscle in the upper esophagus. Since the Botox effect on the muscle serves as "training wheels" for burping, about 4 out of 5 people are "cured" by a single treatment. That's why we aim for "once and done" treatment.

    I had a second dose of botox 48 hours ago and am now burping like a champ it has literally changed my life! Thank you for these tips, keeping my fingers crossed I never go back to how it was before

    I had a botox injection about 10 months ago and ever since i can burp !!! Hallelujah!! 🙏
    And this after spending nearly 40 years of my life without being able to burp at all…
    Each burp now feels so precious…… 😄👍👍👍

    I had my procedure done about 4 1/2 weeks ago, and I watched this a few days ago when you posted it. I’d been able to burp mainly when I turn my head to the left, but not as easily while facing straight forward. Your advice on moving the larynx as if in preparing to talk in a deep voice has helped tremendously! I am now able to burp facing straight forward with that technique. Thank you for sharing 😁

    Hi, I am about a week post botox round #2. Is it normal for every burp to sound like a bubble and then have to let it out? Basically, the burps are not really automatic, it’s more of a conscious action. I am getting these burps with every full breath I take.

      I tell people "expect weird" for the first week or two while you figure out the changes...

      No, doctors all over the world are reported to have offered the procedure, several whom we are aware of are located in Europe, Philippines, and Australia.

    QUESTION:
    I got Botox 6 months ago and now, I can only micro burp (on demand). So I can harness and expel air but so very little. Would a second Botox intervention help at all? It seems like I have learned how to burp, but it looks like my sphincter is either too tight or too big to fully open.
    Also, when the Botox was active, I could burp all the time. The third month was a golden era: I burped only when I wanted to (no more burps when laughing or singing)
    Lastly, when a gurgle comes up, my sphincter seems to tense even more and I can’t burp at all, I have to wait until the air bubble comes down and then I can let go of a little bit of air but it’s really not satisfying…
    Would a second Botox injection weaken the sphincter and allow it to stay supple? Should I do SHAKER exercises instead, as physiotherapy?

    Oh! And when I air vomit, it’s explosive!!! So definitely not due to a lack of need to burp!

      Yes, while 80% of people get a single injection and burp "forever," we do offer a second injection to those who lose most or all of the ability. And as for air vomiting, it is usually LOUD. People call it "Let the Devil Out." And it is problematic socially for many due to the noise of it.

    Ive watched multipe videos of this condition. Ive been experiencing appetite killing discomfort in my stomach for a year. 10-15 minutes before my usual time to eat, this pressure builds up and im unable to force a burp.
    Can this condition develope in adulthood? Maybe cause of severe stress and anxiety at some point?
    I had ALOT of stress about a year ago that twisted my stomach up for over a month. I had to force myself to eat something everyday. Even a slice of bread was hard to finish.
    During the stressful time, this discomfort was the only thing i could rely on to be their all day, everyday. Some days worse than others.
    Today is the first day that I've come across something that has very similar symptoms to what I've had.
    I get gurgles but it's not like I've seen in the videos. Almost like pop rocks deep in my throat. Not as fast as pop rocks though.

      This is a spectrum disorder and the approximately ten common symptoms are arrayed differently in different people but usually with a lot of commonality too. Some, for example, have what we call "quiet internal gurgles" like the slow pop rock phenomenon you describe. And some think that the disorder starts only when they develop the syndrome. They say "this started when I was 18" but when asked if they could burp before that, the often say 'no.' There is also a very small group who could burp earlier in life and lose the ability...

    Hello. Would there be an age limit on when you could receive the botox injections for R-CPD? Our daughter is 14, and she a tiny little thing. But this sounds so much like what she has, constantly gurgling, never burped.. Im just afraid of the Botox at her age???

      Not at all. We love to find people with this when they are young, so that they don't have to suffer the misery into high school and college. Beside the misery, there is anxiety and social disruption. Our youngest so far is seven and he was very small; we have at least two nine-year-olds. Also, the syndrome often gets worse over time.

    Hi Doctor, I’ve done 2 injections before with some success, after recent manometry I was diagnosed with major weak peristalsis (weak oesophagus) I heard you talk about the stretched oesophagus and would like to know should I rather get myotomy or would another injection even work? Thanks

      In our patients, we consider myotomy after a minimum of 2 extremely effective Botox injections. In other words, very good burping, relief of symptoms followed by return of the R-CPD symptoms. I’d even prefer 3 “perfect” injections in such a young Person…

      Hello thanks for the response, I did not have an extremely effective injection only partial relief with microburps with only a couple of good burps and now back with noburps, I’m wondering if it is because my weak oesophagus playing a role, and asking if it is indeed my weak oesophagus wether another injection could work and if not could a myotomy also not work is there still hope ? Thanks again for the information above.

      @@zaneaspeling9750 It is hard to know but a 24 year old ought to be able to generate really good burps eventually. Sometimes a change of dose or targeting. “It isn’t what is in the syringe, but what makes it into the muscle…”

      Thanks, I’ve been diagnosed with Major motility disorder of frequent failed peristalsis, could I ask if your patients that had the over stretched oesophagus were effectively treated with botox ?

      @@zaneaspeling9750 Yes. And keep in mind that virtually everyone with R-CPD has an overstretched esophagus. That’s why the nanometers findings of “slow esophageal transit,” which isn’t the diagnosis, but the result of the R-CPD diagnosis.

      Yes indeed. For some the upper abdomens becomes very hard; many also protrude especially by the end of the day.

      @beepboopbop1234 I didnt get it. Insurance didn't cover it and I couldn't afford it.
