    Dr. Bastian you’re a hero to the no burp community. I’ve suffered with R-CPD since I can remember. I searched far and wide for answers and always fell short. Or got the IBS diagnosis. I have not had the procedure yet, but I now have hope. A better quality of life is possible. I spent years thinking I was just disgusting for all these symptoms. My brother also has R-CPD. We also both have Emetophobia and I have Pseudodysphagia. I often excessively swallow or feel it is impossible to swallow. I have left dinner tables in panic. I am curious as if you know any researchers pursuing a link between childhood trauma and R-CPD? I noticed a flair up of Pseudodysphagia when I am experiencing an anxiety attack. I have been in touch with your office regarding treatment. I hope to shake your hand one day.

    I always get flatulence but my main issue is the fact that I can feel the air moving inside me popping in my intestines and just makes me uncomfortable...

    I’m in middle school and I am getting more final tests soon, we think that I have R-CPD but we just have to make sure with these final tests. This has made my personality completely change since I was younger, I used to be super outgoing but now I’m so anxious during school around my peers that I hardly even talk all day except when I’m around my friends. Honestly don’t take burping for granted, which I know sounds stupid, but I blame R-CPD for most of my social anxiety. That is if I DO have R-CPD. I have all of the main 4 symptoms, and it’s especially hard in middle school around all of my peers. Thank you so much for making these videos, otherwise I would have no idea why I feel so bad every day and would have no hope that I would be able to be fixed.

      Aside from the terrible physical symptoms, the two most common non-physical issues with R-CPD are anxiety (including emetophobia and social anxiety), and disturbance to social life such as you describe.     I’m suffering with this since 2020, apparently it’s a symptom my IBS. Extremely challenging and I’m 29 yet it’s affecting my social relationships and confidence. Such an important area to provide videos, thanks.

      There is more than one cause for excessive flatulence. I'm trying to help in particular the flatulence caused by the inability to burp, which also typically causes gurgling and bloating. All the best to you.

    Farting has ruined my life i cannot burp so i fart all the time i cannot socialize because of this i have to isolate myself glad it has been diagnosed thanks doctor you are hero

    I’ve developed this post Covid. After I eat I’m having difficulty burping. This makes my shortness of breath much more difficult. I have attributed this to gastroparesis.. Thank you.

      If people fill with air they cannot burp up, that air takes up space in abdomen and chest, and causes what I call "mechanical" shortness of breath due to inability to fill the lungs deeply. I really hope this resolves for you or that you will continue to study R-CPD (see other videos) and if it matches, ask your doctor(s) about treatment. All the best.

    Hi Doctor,
    I have been suffering from ur exact explanation immediately after nissen gastric fundoplication surgery 10 years ago. The surgery relieved the GERDS but came with a list of CONs i have been in a out of the ER for years an no one could tell me why the abdominal pain an flatulence other than thats what was expected after fundoplication surgery. I cant believe i found this awnser i will consult my PCP an arm my self with Info. I cant wait to resolve this pain its been so so many years of embarrassment an pain.

    I think I could have R-CPD but don't have all of the symptoms. I definitely cannot burp and experience pressure and hypersalivation, though not as much distension or grumbling as some others have described. Where I differ is in this topic right here - I have extreme flatulence. Cartoonish, even. Flatulence that could end a marriage. Intermittent constipation, too. I wish I were joking because of how ridiculous it sounds, but I'm not. This is especially a problem at night and overnight. I've even been able to record sounds using an app intended to diagnose snoring. It's horrible. I have to excuse myself to deflate (southbound) or have my sleep disrupted because of it. I recently went for my first appointment to an ENT practice here in New York City, and they referred me immediately to a GI specialist to ensure there is nothing wrong with the anatomy in the digestive system. What should I say to the GI specialist to advise that I don't believe this is a GI problem but rather a cricopharyngeal one? It's been a struggle to get folks to know what I'm talking about!

      I'd print out the initial description of R-CPD. It is in a peer reviewed but open source journal. Just search "Bastian belch" and it comes right up. Describes the first 51 of a current 670-patient caseload. Or search "can't burp" on RUclips for my detailed description. You will be more likely to find help from an ENT. GI doctors are not yet very aware of this disorder and tend to order a lot of tests. If you match the syndrome of R-CPD and it has been essentially "lifelong" the initial test is (arguably) Botox. Validates the diagnosis and treats the disorder in one step. All the best...

      @@Laryngopedia Thanks for responding so quickly! I will do that. Has anyone reported simethicone making the flatulence worse/louder? The ENT asked me if I have been using Gas-X or the like, and I told him I had to stop doing that when people in adjoining rooms could hear. I appreciate their desire to eliminate all other possibilities, but it does definitely sound like R-CPD! Regards

    I have the complete opposite. Been crying. I can't stop belching, I belch non stop. I have GERD symptoms, nausea, vomiting, regurgitation etc.. and been to gastroenterologists and done all tests, G-scopes, manometery, endoscopy, Swallow, CT scans etc... No one knows what's wrong with me, I'm so desperate I recently had a surgeon remove my Gallbladder and still have the symptoms. My life is miserable.

    I don't have all of these symptoms but I do have an inability to burp. I fart all day loonnnggg though. It's embarrassing 😳 but I have no way of releasing the pressure. Sometimes it's just air. Other times it's a horrific odor. Even with eating semi healthy, drinking water and taking high quality supplements, no relief. What to do?

    I didn't ever really mind the flatulence, what I do mind is the cramps and bloating. I remember times as a kid being in pain and not being able to move, and even now, eating can be quite unpleasant.

    I am having flatulence, involuntary gas release lightly , done endoscopy, colonoscopy all good but Rifaxinin worked after colonoscopy and polyps removal , what should I do sir , following low food map and this is giving so much stress

    ive been struggeling with this for as long as i can remember, ive been going to doctors and hospital clinics for YEARS and ive always gotten the answe " its all in your head, your all good "
    im in malta and i dream about the day this can be treated here one day
    ( also i dont have excessive flatulance but i dont burp ether and the pain everyday is olmost paralizing, i havent been able to have a job for years because its gotten soo bad )

    Hi Dr Bastian, thank you for your work on this area. I've had this condition for as long as I can remember. I got the excessive gurgling and painful hiccups all the time as a teenager. Now I only notice it when I'm sick and take standard cold and flu medicine. Do you know why that would be? I also now thankfully only get the hypersalivation symptom if I drink certain types of alcohol and I can pretty much fix this with a trip to the bathroom and "air vomiting" as I recently learned it's called. I always thought I was alone with this strange quirk. Do you know if it can naturally decline in strength as a person ages, like I have noticed, or might that be because I've naturally learnt which food and drink to avoid?

    I have the opposite problem. I have a really hard time passing gas. I always get gas trapped in my chest and the pressure is extreme. There are also days were I can’t burp or pass gas. 😢

      @@stevejones9662 Still have the same issues. However; I did a ton of research since then and I found out I have hypertonic pelvic floor dysfunction! I will start physical therapy next week and I hope I will get more answers especially the relation between pelvic pain and lack of burping.

    I haven't been able to burp for well over fifteen years, but I don't experience excessive flatulence. This said, can one have R-CPD without excessive flatulence?

      Yes. The four 'nearly universal' symptoms are 1) can't burp; 2) gurgling noises; 3) bloating often with some abdominal distention late in the day; and 4) excessive flatulence. A small percentage may say gurgles are only quiet and internal. Some say they feel bloated but don't distend; and a few say not increase of flatulence because they are deflating while asleep and don't know it. Other symptoms like hiccups, nausea, etc. can be greater than the 'big four' in some...

    Question: can R-CPD also be linked to excessively foul/malodorous flatulence?
    I meet all criteria for R-CPD but my flatulence odor is also fairly astounding; is there a link?

      Odor would be more related to the fermentation process-food eaten and micro biome; there are reasons the latter could be affected by large volumes of air passing through the gut; all speculative at this point but interesting to consider. Aside from very complicated “study” that wouldn’t provide relief of R-CPD, the main approach would be to fix the R-CPD and see what happens to the odor.

    I definitely fart a lot. My farts are LOUD and stink to high heaven. At first I thought this was funny because I could stink out a room and send everybody headed for the exits. Now I'm starting to worry this could be a more serious condition. I should mention I also have to shit frequently usually after a cup of joe in the morning.

    I rarely do burp so I dont know if its R CPD. I can also vomit. But I do have excessive flatulence which has become less severe after drinking Pipal leaf tea daily to control gas. But it doesnt go away. My father had excessive flatulence when he got into middle age.
    Is it possible that I passcgas from anus and dont burp because maybe the gas issue is not in the stomach but in small or large intestines?

      People with R-CPD may micro-burp occasionally, but never to the point of relief of the gurgling, bloating, flatulence, hiccups, etc.

      @@Laryngopedia I never even feel the urge to burb. My gas is in the lower part of the abdomen and it only leads to flatulence with no urge to burp. So, I don't know if its inabiility to burp. But, I'll keep this theory in mind and explore it further.

    just found out that i have a lactose issue that i havent had before. got tested a couple years ago but all was good. just wondering if r-cpd could have been a contributing factor or could have just been in the genes.

      Flatulence is such a common symptom of R-CPD. That doesn't remove the possibility that lactose intolerance is another source. You would know by going strictly lactose free. If the flatulence is unchanged and you fulfill the other criteria for R-CPD (can't burp, bloating, gurgling, etc.) then it could call the lactose contribution into question.

    Extreme flatulence affects your socializing because you'll be the life of every farty - I mean, party. Just ask someone to pull your finger, and they will be completely blown away. If you think I'm tooting my own horn and are saying, "This guy is full of hot air," then I don't really give a rip. There's no reason to create a big stink over something that will eventually blow over.

      For me laying down helps me fart more, but they're so loud it is very embarrassing if around anybody else.

    Hi dr ,is it possible to have minor rcpd? I’ve been experiencing this no burp problem for six months since having covid 19…..I’m matching up with 2 symptoms , I can’t burp and I feel like I might be swallowing air with my saliva, although I do not have the flatulence, most of the time feel like I have trapped air bubbles in my chest I do get some release but very rarely am I able to burp if anything I can just about manage to take in air…to release some of the excess air in an almost half burp if that makes any sense?, This Is the closest set of symptoms I can match my problem with, can’t seem to find anything else on the Internet about not being able to burp, I’m chatting with my doctor in the morning and will be mentioning RCPD, thank you

      As for most disorders, R-CPD is a spectrum. We patients with severe R-CPD who are extremely motivated to proceed with botox, and they may have a relative who also has it but symptoms are not as severe and they aren't ready (yet, or ever) to undergo treatment. You also reference recent onset. That can happen but is far less common than the "lifelong" sufferer. By lifelong, I mean they don't remember being able to burp, but may have only developed severed distress in middle or high school or even later...

      Sorry to hear this. The key is whether the symptoms interfere with quality of life via daily discomfort, social embarrassment (gurgles and flatulence). Some with mild symptoms live with R-CPD, but those with signifcant to severe daily symptoms should consider seeking a physician like at BVI to undergo treatment.

    Lol I have had this condition and never realised it was called RCPD
    It was misdiagnosed as gerd in me
    Cause the symptoms are slightly similar
    Btw does this cause mild acidity

      If a person has the syndrome--can't burp, gurgling, bloating, and flatulence is just one or the primary symptom, then ask your personal physician about R-CPD as a potential dx.     Hi, I think I have this. I have Small Intestinal Bacterial overgrowth(believe I had gastroparesis like symptoms from this) and overgrowths in my large intestine. I also have lpr reflux. I’ve had an episode that woke me up where i felt like I was choking and fell to the floor. It felt as if I was dying. I believe this to be my UES tightening from reflux and so much that it almost chokes me. When I treat the SiBO & take a prokinetic drug with Pepcid, things have improved. I take Ativan for when I get the tightening in the esophagus and I start burping the air out right away. Is it possible the tightening all started from reflux from the bacterial overgrowth? I wonder if Botox would be a good replacement for the ant anxiety med. Any good doctors for this near or in Los Angeles area?

      Keep in mind that R-CPD patients are often told they have SIBO, IBS, etc. etc. They key is to find out if in actuality the problem is R-CPD or not...since it can produce similar symptoms.

    Hey doctor my case is little different..... Although i can burp multiple times a day , sometimes i have to force myself to burp because i feel its stuck.....I have been diagnosed with GERD and Gastritis for over a decade....Though sometimes i can burp normally, it is only when my GERD worsens i have to face this difficulty.....P.s i have neved had any gurgling sound coming from my throat.....Dont know whether it is RCPD or not?......I would really appreciate ur help

    I’m glad to know I’m not alone in being unable to burp. For as long as I can remember it’s just not been a thing I can do. It can be quite painful sometimes. I can very VERY rarely burp. And it’s of course not intentional, and it surprises me every time 😂 always very small though, and also more likely to happen if I’ve had alcohol. My mum also can’t burp, so I guess it could be inherited?

      Yes some with R-CPD have a family member who also has it. We've had siblings, cousins, parents and grandparents, etc. (A person can also have an isolated case.) Those with R-CPD sometimes report the occasional random micro-burp that is non-relieving and the person cannot "harness" these micro-burps. So sorry and I hope you can find a source of help near you.

    Sir, i really wondering by watching this video because my husband has been suffering from this problem that he can't burp instead he will push some air out from mouth like Ufff Ufff for a long , what can be the fix and treatment ann any serious health condition to be going forward , please let us know if possible we can get treatment, thanks in advance.

    I'm laughing so hard I'm crying over here. The Dr. is so serious, but I know people that fart like a farm animal and they're happy as can be about it. I think it gives them joy in their life. Rather than try to control flatulence, I'd like to find out how to fart MORE! LMAO!!!

      Perhaps a group therapy session would be of some benefit, kinda like AA, only it would be labeled "FA" LMAO.
      Maybe they could trade recipes and do a potluck lunch with a plan to meet that same evening and share their experiences.
      Personally, I prefer great white northern bean soup. It's mighty powerful and can swiftly clear a room if need be.
      So I've found that when I eat Publix brand apricots dried with sulfur dioxide, it's nothing short and atomic reaction and I once broke the window crank off my pickup truck bc of having to open and close the window so many times. There's truly nothing that compares to this noxious and odiferous combination.
      Does anyone else have some tips to share please? Bean salads, etc.?

    Of all possible animations they could have chosen to represent farting, it's odd that they used a leaf blower. When I hear a leaf blower, I don't think, "my neighbor needs to cut back on the bean burritos." A whoopee cushion or an animation of someone asking people to pull their finger would have been better for that fart - I mean, part.
