    The most-common cause of shoulder pain that I treat in my physical therapy clinic is a condition known as “impingement”.
    Your shoulder is a ball and socket joint. That “ball” should stay firmly seated in that “socket” during shoulder motions, especially overhead. If the head of the humerus (the ball) does not stay seated properly in the glenoid cavity (the socket) during motion it can “pinch” on the tendon and other structures up on the top of your shoulder.
    Over time, this can lead to pain, inflammation, and even a rotator cuff tear. It’s important that you address the cause of this pain quickly to avoid exacerbating the injury.
    The primary causes of shoulder impingement include poor posture, muscle weakness, joint tightness, and muscle imbalance. The most effective way to cure this impingement syndrome is to perform exercises that address these deficits.
    Posture is probably the number one cause of shoulder impingement. To demonstrate why this is an issue I want you to stand up and slump forward (exaggerate poor posture). Now try to reach your hands all the way over your head. You won’t be able to do it! You’ll get to about 160 degrees of motion and run into a block.
    That’s the impingement I’m talking about. You’re pinching those structures under the roof of your socket and it’s not allowing you to go any further.
    Now stand with great posture. Tuck your shoulder blades down and back and make sure your head is squarely over your shoulders. Now reach both arms up in over your head. You should find that you are able to reach a lot higher with a lot less pain.
    In order to achieve full overhead motion, your shoulder blade needs to upwardly rotate. This will enable you to achieve those last 20° of motion without pain.
    The second primary cause of shoulder impingement that I see in my clinic is rotator cuff weakness. You have 4 muscles that make up the “rotator cuff”. The primary function of the rotator cuff is to keep the head of the humerus (ball) seated squarely in the middle socket of the shoulder.
    Oftentimes muscle imbalance and shoulder/rotator cuff weakness can cause the head of the humerus to move towards the front of the shoulder. During overhead motions, there’s less room under the “roof” in the front of the shoulder than there is in the middle of the shoulder. This can cause shoulder pain and impingement during overhead motions.
    SO WHAT DO WE DO ABOUT IT?
    The best things you can do to eliminate shoulder impingement are to stretch the tight structures in the front of the shoulder and chest while strengthening the rotator cuff muscles in the back. This will promote a better anatomical position for the shoulder joint and enable you to move through these overhead motions with decreased pain and less “pinching” symptoms.
    The stretches and exercises that I recommend to patients experiencing this type of shoulder pain include:
    1. Doorway Stretches
    2. Foam Roller
    3. Scapula Pinches
    4. YTA
    5. Resisted External/Internal Rotation

    Iv'e had shoulder pain for about six months now, the result of some extreme burpee exercises. I was convinced I'd need surgery. So your video popped up, I paused and did the exercises minus the stretch rope for about 10 minutes. Previously, when I would reached up at tip toe for an item, I would come down with shoulder agony.
    There's NO WAY I believe what has happened in 10 minutes, NO WAY. A little discomfort remains, but the agony has gone, and there I was thinking I'm just marking time till I can afford the surgery. Words can't describe how I feel. I am now subscribed, and I'm paying attention. THANKS A MILLION.

    just developed a shoulder pain as i was lifting a bicycle onto high rack on the wall a few months, pain is deep and I have been icing, but definitely work it through. this one along with Bob & Brad's videos, I found, very consistent and helpful.       wanted to follow up with a strong precaution with the foam roller. as i was laying down on the foam roller as shown around the 7 minute mark, i lost my balance, fell on my right side where i had the impinged shoulder, i tried to get off the foam roller too.
      Felt a sharp pain on my low back, and the next day I have the symptoms of a herniated disc which i didn't know right away until I talked to my doctor. this was on 12/23/20. now attending a much more painful injury and hoping it will heal itself in 4-6 weeks without a need for surgery. But just wanted to caution anyone the risk of injuring yourself in other places as these exercises should be done with care and instructed. A lumbar disc can be herniated at any point and mine probably have some history. Still don't do what i do. Heal away!

Some days it causes so much discomfort and distress that I can’t even go 10 mins without constantly thinking about the pain and position of my shoulder .     2 years later and your still helping people out. I just wanna say thank you so much. I was having a lot of pain in my shoulder all the time even during my sleep. But after doing exactly what you said in the video, it’s all gone now. My main issue was my posture. Again thank you.

    Shoulder Anatomy - 1:10
    Pathophysiology (Reason for pain) - 1:42
    Posture rehabilitation stretches
    1) Doorway Stretch - 4:48
    2) Foam roller stretch - 6:50
    Though I advise supporting great medical staffs in youtube however you can, a bolster works almost as good as a foam roller. If not, buy a yoga mat and roll it accordingly
    Rotator Cuff Strenghtening (Shoulder abuse resistance training)
    1) Scapular Pinches - 8:01
    2) Y, T, A stomache facing back stretch - 9:04
    3) External Rotation - 11:02
    Low resistance/assistance version (no band) is lying to the side of your body you are pulling away from and resting your elbow on the side and just lifting a liter of liquid using mainly your shoulder with a fixed wrist position
    4) Internal Rotation - 12:00
    Low resistance/assistance method (no band) is a little more technical for this exercise. On a normal on your back sleeping position, lift a liter of liquid from an open arm position toward your chest, closing it. The technical part is how open your initial open position is. In normal comfort circumstance its usually 120-150 degrees and not 180 degress. For this exercise specifically, i recommend you follow the DPT's advice and use a band if you are new to the gym or exercise in general.

      @@toneandtighten Ive been suffering myself but mine ended up as an associated symptom after a viral gastroenteritis attack can u believe? after whats been going on, the entire viral family sure has us by the balls, of course this was Norovirus and not the big cahuna C. Anyway that disease led to an on set of gastritis im already dealing with since last year due to inappropriate diet in uni. Eventually that cause me a mini lavatory lockdown which forced a bad posture from me for longer than usual that led to bursitis impingement. So while im stuck on lockdown, i researched and formatted your clips and provide easy reference for both myself and others. I repeat, if I may give advice, hopefully you can incorporate methods of relieving acute pain when its active because we all know when it is, its like back pain, it takes a while to relieve itself even if you are doing things right. What i found is mainly some gentle ball position greasing exercise (pendulum), adjusting rest on sitting and sleeping position, and your scapular pinches with the downward push being important even during attack. Thank you for your vids bruv. Many a shoulders are finally free from abuse #metooshouldermovement

    I am 62 and have cerebral palsy and I have a lot of pain in my left shoulder
    Also my Life physios have told me to do exercises which I haven’t been able to do

    Excellent. Massive back shoulder pain (up into my neck, as well) the past three days... it's been unbearable. Did your exercise... Helped immediately, but the pain came back after I stopped. Will continue to do the movements you've recommended to see if things improve. Thanks.

    I personally want to thank you for this episode. I am 52, and basically fit. I was diagnose with Hypothyroidism, and along with that came muscle cramps and my poor posture led to impingement and trapezoids knots in my back - these stretches push the shoulder and traps to soar but a relaxed mode. I have been putting on cold compress in the day and heating pad and elevation for the impinge shoulder. This stretch is now my go-to when I awake and hour before bed.     I just tried the exercises and it worked. Wow! A whole year in pain and all i needed was stretching. But not just any stretching the right stretching and strengthening!     Thank you for the great video, your time, effort and knowledge! I have been suffering from shoulder impingement for more than a year. I received all kind of medical intervention beside operation and I was still in pain. I tried your stretching and strengthening exercises and woala! My pain is still with me but it’s getting better.       Hi. May I ask what kind of weightlifting you did and how your shoulder rotation was improper? I have the same issue for a few weeks now. I always thought you need to external rotate your shoulders before a a lift to stabilize the shoulders. I would really appreciate an answer. Thanks in regard.

    Guys, take care of your shoulders. If you hurt them, that means no more pull ups or push ups.
    I thought that I've injured my shoulders, since I was never giving them the attention that they deserve. But, in my case it was just tight muscles in my left shoulder.
    The pain disappeared after one week from starting doing the exercices in this video. Thank you so much for sharing such an awesome content.

    I have watched more than 40 shoulder pain videos over the months and this is by far my favorite and most recommended video for shoulder pain and impingement. He takes you through stretch and then strengthen exercises that are simple and effective. Both my shoulders were grinding and in great pain, my left now is close to 100% and the right is still lagging but I think It to will be 100% if I continue with these exercises over the course of another few weeks to a month.     Oh man I hope this works. Had a fall running in the dark a month ago and my shoulder has been sore since. The doorway stretches are very accessible. I actually found myself doing these instinctually prior to seeing the RUclips video. Pretty exciting to consider the foam roller excersizes. The ideas to get minimal equipment and do this at home is a good way to keep the workout part of my daily routine with or without the trip to the gym. Got me considering if my mattress is too soft...hmmm. Great help explaining and identifying the causes for impingement. Pretty sure that is the issue for me. I feel like yeah, on the mend faster than before. Thanks!

    Jared, I'm a side sleeper!!!!!!
    I really appreciate this, I need to be disciplined and do these. I learned them in physical therapy.
    Laying on my bed and pushing a pillow into the wall then into my side helps a lot.
    I struggle with the strengthening exercises because I have a seriously messed up neck, so with any movements, muscles in my shoulders, neck and shoulder blades are trying to protect my poor neck; they tighten, and spasm, ugh!! But I NEED to stretch!!! Thank you, thank you, dear Jared! ❤

    Amazing. Just tried to cooy your movements while sitting and watching this video and it already helped relieve my pain. Pain which developed from being hyper active to kind of sedentary during the long lockdowns. Thanks for giving me hope

    Great video! Over the last 3 months I’ve been having impingement issues so I’ve been watching a lot of vids, one thing I’ve noticed is nobody really mentions truck driving as a very good reason for poor posture and heavy use of arms when steering. I say this cuz my exact situation been driving garbage truck for 23 years and it’s catching up to me, good exercising and stretching like seen here are helping ❤

    Thank you! I liked the logical process you took for this. Do the external and internal rotation exercises hit all the rotator cuff muscles? I’ve been doing these daily for a month. My shoulder is much improved but I still have some impingement pain, especially if I move my arm quickly.

    Just wanted to leave a comment to thank you for this excellent video. In the past, I went through PT for my rotator where I learned similar exercises. You’ve captured and explained them so well, and I watch it every few months as a refresher.
    Quick Q: what’s your take on frequency? For active PT and also maintenance.

    Doing the door exercises (and I've looke at other videos) my symptoms presented as bursitis arthritis and tendonitis but after that door exercise my shoulder - popped- and it's like everything has re set, some residual issues but quite amazing - many thanks

    Many thanks. I went to a doctor who did an x-ray and said I had arthritis. He said there is no gap caused by arthritis. My shoulder pain was not gradual, it appeared over night and was excruciating. The trouble was range. Arthritis does not affect the range as I was experiencing. No, after watching RUclips and reading on the internet, I believe I have impingement. My new doctor is going to do an ultrasound but has not told why. So I looked up ultrasound and impingement and I believe the doc is looking at impingement. I will now begin your exercises because I looked at my posture and I am rounding as you have shown. Thank you because when I have delt with this, I can sleep better and do weight training again. God bless

      I want to tell you how quick your exercises worked. I did the first door exercise and the first attempt hurt, but I have been in so much pain since August last year, I just held my ground. Another reason is because I could actually feel the stretch you talked about but, more, I could feel the joint opening up. I did this three times. The last had very very little pain. The second door exercise hurt proper, so I just did a small step inside. I did both door exercises three times and held for 15-20 seconds. The other exercise is the turning the shoulders out. Again I felt stretch and the joint opening. I had my best night sleep since August. I even slept painless on my left shoulder. I am now doing house activities using my left shoulder. The second door exercise is sore but is getting less pain. I also pushed out 3 reps of 20 push ups which I could only muster 10. The pain is worth it. God bless

    The doorway exercises really helped. On each first set, the pain in my shoulder was pretty substantial, but the pain gradually lessened with the second and third set. How many times a day can I do these stretching exercises? Thank you for posting these!

      @@deborahnickrand4538 cold presses help too, but whatever I do, sleeping usually reverses it. Had to learn to not sleep on my side. It is getting better, but slowly.

    Can't tell if mine is this or more deltoid tendons. This has been going for years, now.
    Truthfully, I can't see these doing any help; usually they aggravate it more. Maybe initial healing needs to happen.
    I believe the cause is from sleeping on my side. The first time switching sides (yesterday) had an immediate effect; I could at least lift my arm the next morning (which was shocking (no pun)).
    The dooming action was roller painting a ceiling - almost didn't even finish the job.
    Will try to follow up with results.

    What I’ve found is rest only for a day, but stretch it, then move it, stretch and even lift weights, but no overhead presses and no upright rows. Just keep it moving, and loosen it up it, along with these kind of exercises. Resting it too much causes it to seize up and it doesn’t heal. I’ve been dealing with this on and off for 4 years now. I’ll have periods of no pain at all and then most recently compete agony where I could barely move, but stretching it and then today 3 days after the initial impingement I’m lifting heavy weights again, and today it really does feel so much better. Keep it moving guys.

      Its your pec muscles, but you’re going at it too hard. Lighten up a bit. You just want to feel a comfortable stretch.

      My pecs are tight and shoulders are pronated, they roll in, I bet you're just super tight there. I'd roll a lacrosse ball over the chest to help it release. You can do it right in a doorway. Then add the stretch you were doing. Hope it's gotten better.

    I have been dealing with this Shoulder Impingement for the past 6months, my primary doctor prescribed me a ton of painkillers and said it would go away on its own. After taking the pills a couple of times, I stopped because it was getting high, and after it wore off the pain was the same. After trying these exercises I can feel some relief and it's only my 1st try.
