    What is skin purging or acne purging.
    Skin purging happens to your skin when an active ingredient triggers cell turnover which is just fancy for pushing everything underneath the skin to the surface, think of it like cleaning out the pipes. So out with the old skin, and in with the new. So things like excess sebum, flakes, and buildup all rise to the top of the skin and cause breakouts.

    Its going to look different from person to person, but essentially you can get a mix of whiteheads, blackheads, papules, pustules, cysts, and even the tiny ‘pre-pimples’. Generally speaking, it’s said that the skin renews itself approximately every 28 days, while that might be true, as we grow older, this skin cycle slows to about 45-60 days in our 40's and 50's. But remember, this entire cycle is about to get revved up and things are going to turnover faster, but not everyone will be at the same pace, which explains why some of you purge for only 1-2 weeks, while some of you can purge up to 6 or more weeks.

    First Tip is after youv'e done a patch test to make sure you dont react to it: You want to start slow. Slowly introduce a new product into your routine to give your skin some time to adjust. So for example with a retinol, try using the product once in the first week, maybe twice in the second week, then three times in the third week, and so on until you're using the product every day or every other day depending on how sensitive your skin is. For other products, going in for the lower concentration first and starting at a smaller amount, maybe just a tad smaller than a pea sized amount before gradually increasing to the recommended amount over some time, can be a good practice.

    Second Tip is to Absolutely don't touch your face. If you absolutely have to, make sure your hands are clean first. Adding extra oil or bacteria to your face can throw everything off which could make the breakouts worse.

    Third Tip is to Stay hydrated, 3.7 liters of water a day for men, and 2.7 liters of water a day for women. The 8 cups of water per day advice, lacks evidence, and personally I think that's not enough, especially if you are active. You see, People who are adequately hydrated with water are less likely to form acne scars, wrinkles, and guess what, they won't show as many signs of aging as those who drink little amounts of water.

    Fourth tip is to apply your product over a moisturizer. This allows the moisturizer to act as a buffer and prevent the chances of having a tough purge and overall irritation. This is especially true with tretinoin as it can help minimize harsh drying effects that a lot of people experience which leads to giving up the product.

    Once you start with the active ingredient that is said to purge, you want to have literally the most gentle skincare routine you can possibly get while still delivering the results you want, what do I mean? Be aware of every single product in your skincare routine, familiarize yourself with all the ingredients you use, as some can be aggravating and you may never even know. So what I would do is, For the first six weeks, only use a gentle cleanser and sensitive skin moisturizers, and avoid all harsh products. This means peels, exfoliating scrubs, and any other AHAs. You want to account for everything.

    I watched like 100 vidoes of the purging phase to remind my self its going to get better, it came first as dry flaky skin,then purging -it was so hard mentally to get thru , waking up to new whiteheada and bumpy skin for 2 months and now im officially out of that phase. Now i have a lot of hyperpigmentation from them but i can see good skin coming up. Anyone going thru purging - keep pushing girl ❤

    Thank you do much for this info. They are really helpful. I got Retin A but somehow scared to use it because of the purging stage. At least with this vid of yours, my fears have lessen. Presently, i use CeraVe acne foaming cleanser. Can i use it alongside this or do i get another type of cleanser? Is there any difference between Tetinon & Retin A?

    The video is really helpful but a quick question. So I use the cerave normal to oily skin cleanser and the cerave normal to dry skin moisturizer. So as the tretinoin will dry my face and cause irritation, should I use the normal to dry skin cleanser?

    Thank you for the video. I'm trying to get to a dermatologist but I had get a consult from my PCP first. They wanted to help me out since it's going to be a long wait to see one and didn't want to start from scratch so they prescribed tretinoin 0.025% and spironolactone to cover all bases. PCPs are jack of all trades and masters of none so they can't answer my ultra specific questions I can't google myself. I knew I was going to have to start on some retinoid so prior I worked on building a good barrier with using products with nicinamides, ceramides, hyaluronic acid, etc. because I have sensitive skin already. I hope it's not a bother but I'm curious if I'll get an answer. Prior to applying tretinoin instead of a moisturizer would it be beneficial at all to apply hyaluronic acid. I have been using Vichy Mineral 89 hyaluronic acid and I had thought it would be ok to use prior to seeing videos that all specifically say use a moisturizer. I've only used tretinoin once. I used Vanicream and La Roche Thermal Water mist then waited 20 min and applied directly. Waited 20 min again then did serum Cerve ultra light pm, and Eucerin emollient for extremely dry skin. I plan to do it for my 2nd time tomorrow which would be 2 nights post. It just feels weird to do moisturizer before serum lol. What are your thoughts about it? I found a couple of sites that just mention it's ok to use it but not in a specific order. I know hyaluronic acid absorbs better with wet skin vs. dry so is that's why it's not a good idea? Sorry for the lengthy post

    You are great Dr. Arsalan, based on my experience after using tretinoin my skin get irritation because I don’t know the proper way, very important to watch before to start after learning my skin slowly adopt the tolerance purging really scary but please don’t give up after all your skin become soft and clear.

      Thanks for your question. For that specific product, use it at night and apply it after a gentle cleanser and before moisturizing. You can also apply it after your moisturizer but wait 15 minutes after. With washed hands, use a pea-sized amount on your forehead, nose, cheeks and chin and evenly distribute it.

    Hi Handsome doctor. Should I persist with using my tretinoin as I build tolorence if my skin is dry and scalp in some areas but not itchy or stinging? I am ready mixed opinions.
    Also, how many days can you go without using tretinoin before you have to ‘start again’ and lose the progress of the tolorence you are building? ❤

      Hello and thank you for your question! The dry skin is expected with products that increase skin cell turnover. It is a normal process. Stinging and itching are some signs of irritation/reactions. So from what I hear, you seem like you are tolerating the product so far. 🙂
      It depends on the strength. But a good rule of thumb is to 2-3 days while listening to how your skin adapts to the on and off periods, this is so you minimize the length of time with purging.

I had perfect flawless skin (I was using tretinoin 0.1% for 10 years and then I lost tolorence because a medication gave me dermatitis (on my face).       No please, use the vitamin c only on the days you're not using the treat, but most importantly avoid any other actives during your test days

    Where can you get the Tretinoin without having to make a appt with a Derm, waiting months to get an appt, and then finding they won't order it. My pcp won't order it either. It frustrates me. Very strict in Massachusetts. Not sure why. It seems like no one else is having this problem getting the Tretinoin. Anywhere online I can buy it. Your help would truly be appreciated. I am 64 and looking to use it in your suggested skin regime. I always have just used a cleaner and moisturizer. Neutrogena cleanser and moisturizer. But I lately notice I am getting some wrinkles and need help. I also have been getting clogged pores on my eyes and dark circles. Can you suggest any treatment for that as well.     i was on 0.025% of tretinoin for approximately 2.5 months. my derm increased it to 0.05% a few weeks ago. i purged pretty badly with 0.025% and have been having more breakouts recently after switching to 0.05% however, they haven’t been cystic like i’m used to. mostly whiteheads. is it normal to have a purge after increasing the percentage? i feel like i have been purging forever. my skin care routine in the morning consists of a hydrating face wash by la roche posay, 2% salicylic acid, followed by sunscreen. at night it is the same cleansers, tretinoin after my skin dries, and moisturizer about 20 minutes later. is there anything i should change or add in?

      I think you have to change your facial wash to mild facial wash. Salicy acid is active ingredients that doesnt suit to use with retinol . Use the mild facial wash like cerave

    I have 3 important questions, doctor please advice me.
    1. Does purging causes cystic acne..? How many months this cycle takes if I start tretinon 2 times a week for 15 days.
    3 times a week for next 30 days. 5 times a week then onwords.
    So technically i would start applying daily after 45 days of exposure..! Slow but steady.
    2. If i used adaplene erlier for 2-3 months , and now i switch to tretinoin do i still get purge and cyctic acne..?
    3. How many monthd it takes to see visible diffrance on skin texture, pigmentation,acne and other issues..?

    Can you tell me if this composition is good for me from RetinA cream ( PEG 400, PEG 4000, Polysorbate 80, Retinyl Palmitate 200.000) I am 36 years old, I have no wrinkles, except for a slight nasolabial fold and a couple of spots that are visible to me, but I want to remove them as well as two wrinkles on my neck. First of all, RetinA treatment is important to me as a preventive measure. My pores are slightly enlarged, and my skin is mostly dry. I use moisturizers for sensitive skin because my skin is normal to dry, but sensitive at the same time. I use serums based on Retinol 1% in squalane almost every day, except when I use peptides. Serums based on peptides occasionally twice a week, serum Vitamin C 15% daily. Pro-retinol creams that contain SPF 25 daily and also SPF 50+ in particular. I use mix booster serum for the area around the eyes based on caffeine, collagen and retinyl every day, morning and evening. The sunscreen 50+ that I use is intended for children because it also bothers me if it is not medical.

      He said apply the product over the moisturiser. That means first apply the moisturiser and then apply the product so that your moisturiser acts as a shield and helps keep your skin moisturised

    I am six weeks into Differin and I'm at my wits end. The purge has caused acne that is the worst I've ever experienced in my life. What I can't figure out is much purging is normal? At what point does someone need to stop?

      Adapalene is not studied for anti-aging do I wouldn't recommend banking on Differin. At 27 you can def already start at least the otc retinol or retinal and perhaps eventually go on prescription tretinoin which is the gold standard for anti-aging.

      For some skin types, it may cause skin purging, redness, and burning if used improperly. A tip to prevent this when using glycolic acid: Start with a lower-concentration product and use it once or twice a week. Hope this helps!

      For best results consult with your dermatologist, this way they can see that It may be that you need a dose adjustment. Most seem to do well with the vanicream moisturizer. But others can be great solutions too.

      I would say that a dermatologist is best to advise you but I went to 2 and they wanted to give me the strongest and 2nd strongest ones. I don't know why. It would seem to make sense to start low and get accustomed to using it then go higher when ready. Starting this week. I'm scared...I should have said I wanted the low dose 😢

      Girl just try egg white on face like a mask and leave it off for 7min and wash your face and at night time use Benzoyl peroxide and wake up and touch your face you'll not feel your face like rock I've alot of whiteheads and texture skin i wasted alot of money in serums and I get breakouts everytime I add new products on my skin now I'm sticking to the simplest step by step routine my face is glowing and smooth and no sebum I hope it helps

      And tret takes alot of time and I've no patience to wait that long to work on my face if you're a risk taker go for it but I won't use tret without derm adv and this is too strong for me but might work for others

    If I’ve used retinol for a couple years and now have to stop due to pregnancy, will I purge again once I start using retinol again?

    Could I use adapalene only once a week or is it not effective at such a low rate? I started adapalene and have used it once a week for the past two weeks, and my skin is reacting well so far. I’m really scared of purging 😂 Thank you Dr. Arsalan!

      @@danaroberts3643 Ya! After using it once a week for two weeks, I increased the usage to every 4th night (like skin cycling). My skin is pretty clear now but not completely clear yet; I still have hyperpigmentation/acne scars, and occasionally get one or two pimples. I didn’t go through a crazy purge (I’d say for a couple weeks it was normal amount of acne for a bad day and then it just slowly decreased). I use vanicream cleanser and lotion which has really helped keep my skin moisturized and I didn’t have any dry skin or peeling!
