    These two are amazing, very knowledgeable. I still have a solid head of hair but have noticed a small amount of thinning over the last few years, currently I am 27. I know this will continue and its nice to know I can take a smaller amount of finasteride weekly, I was prescribed 1mg a day so 7mg per week. After watching this i'm confident a lower dose will help continue to keep things stable.

    I really found your previous video about Finasteride starter dosages informative. It would be really helpful if you could do a followup similar video for Dutasteride as well. Finasteride doesn't work for a small percentage of people but in terms of sheer numbers there are a lot of them now and these discussions have been popping up on forums a lot lately.

    Hello, thank you for these very informative videos. You are very much appreciated. I would like to ask you a question - what are your thoughts about reflex hyperandrogenicty while on finasteride? Is there a such phenomenon? Have you ever had a patient expiriencing it? Thank you for sparing time answering the questions.

    22 years old here. Dealing with hair loss for 2 years. Started Finasteride 0.5mg every other day last year so far no sides with regrowth and stabilization.

      @@1sad_latvianboi707 stabilization started after around 6 months but for regrowth it started around 10th months.

      @José José not topical anti androgen, but i am using topical Minoxidil (which is growth stimulant)

      @José José and i really couldn't tell if i had initial shedding or not because 1 week after I started using Finasteride I shaved my hair with razor :D

    Thank you so much for all the work you do, doctors! I was just wondering about the reaction finasteride might have with other drugs taken by the patient. I am currently taking oxybutynin because I suffer from excess sweating. Could these drugs react potentially cause either of them to be more or less affective?

    Can you please comment on Finasteride use for post menopausal women? Are daily doses needed or just a few times a week? We finally get to the age where we can have therapy that wasn't available for us before. Would love to hear anything you can say about what's available for us women at post menopause. Thank you :)

    There seems to be some evidence in bloodwork on topical dutasteride users (anecdotally at least) that the systemic levels of DHT are only being decreased by 20-30% but scalp DHT levels are being decreased by 60-70%. This due to the large dutasteride molecule having less systemic absorption which isn't the case with topical finasteride as its a smaller molecule. And Vikram is wrong about finasteride and dutasteride requiring metabolization by the liver. It IS metabolized by the liver into its various metabolites which have 5ar inhibition, but the base molecule of finasteride and dutasteride are also inhibitors themselves, therefore the drugs aren't required to be metabolized to get 5ar inhibition.

    What I notice in charts is that DHT gradually keeps going down if you take Fin everyday, so every other day it can prevent the accumulation...

    Dutasteride doesn't need to be metabolized by liver to become effective. As for the larger size of Dutasteride and struggle to get into skin, why not do a needling session and then apply Dutasteride from capsule or first mix Dutasteride into minoxidil?

    Thank you for the video doctors. I have a question. For guys who have been using fin for a while (~1 year), when they switch from fin to dut, many people report more shedding. Do you know what that is? I would appreciate your response.

    So you say the goal isn’t to bring DHT at the level of the hair follicle to zero because of other potential effects to the system. But if someone is completely bald at a young age; Norwood 6-7 diffuse balding, what other option is there? It would seem to you have to block as much of the DHT as humanely possible to try to regenerate the hair.

    Out of all your patients what percentage would say got away with 3mg/week? How long does finasteride work for (not in studies/trials) but in your own clinic? Do you believe most of patients experience no hair loss with JUST finasteride over the course of a decade?

    in a previous episode you commented on "Topical Finasteride" that the benefits should come from lowering the DHT levels produced by the liver so that Topical Use may not provide good results.
    but here you're saying that the drug concentration in the plasma does not matter as the main focus should be at blocking DHT generation at the level of the hair follicles.
    I see a paradox in those statements, could you please explain it?

    I use topical Dutasteride 0.1%, Pyridoxine 2%, Biotin 0.5%, 60 ml of Trichosol. I use Retinol A prior to help it absorb. I found absolutely no side effects from using it topically. As you can see topical Dutasteride contains several ingredients which are not in the pill form as they point out.

      @@MyLife-ij4tf finasteride. Is first basic thing to take. It's easy to do a prescription for happy head or something with minoxdil and dusteride and use topical.

    After the side effects with oral Fin(1mg), I make my own topical Fin/Minoxidil solution. I had the same side effects with 0.15% concentration but lowered it to 0.1% and now I got my morning wood back.

      @@sherlockva I assume it should. 0.1% topical is the equivalent on 1mg oral. And it suppresses scalp DHT as much as oral' Without decreseang blood serum DHT as much and causing side effects.

      ​@@sherlockva It's also much more convenient, you take a pill and you are done for the day. whereas the topical solution needs to b compounded and sometimes pre-made formulas are expensive. at the end of the day, you need to realize they are very few people that will get side effects on oral and the fear of side effects might actually get you a nocebo effect. So my suggestion to every one who is afraid of side effects would b just take the oral and don't worry, you will must likely not have any side effect. even if you encounter side effect, you can always switch to topical and customize the dosage.

    Duta lauded as 'powerful' but not explained one factor is its unusually long 'half life' of 5 weeks so that it banks up in your body so my urologist concedes taking every second day ok for treating enlarged prostate gland. Its adverse side-effects more diverse than previously recognised - unsurprisingly.

    I do have a question on what are your thoughts on finnasterides affect on beard hair?
    Heard it has a negative affect and I currently have a beard and am wondering if I'm gonna keep my head hair but loss my beard.

      Beard hair is unaffected.

      From personal experience the beard becomes hair falls out more on a daily basis, but it's not a huge issue

      My understanding is it doesn’t hurt current beard hair, but it may slow/prevent new beard growth in the future.

    Great info as usual guys.. 40 days on fin now and going great! No side effects whatsoever!
    Update: 24/04/24
    I stopped taking them, just couldn’t be bothered anymore to be honest. I had no side effects at all whilst on them and none when I stopped.

    Hi guys,
    Could you do a video on how to properly take care of Hair?
    1. How should we use shampoo and what kind of shampoo? Especially on DHT blocking shampoos
    2. How to use Hair conditioners?
    3. How to use Hair oils like coconut oils, Almond oils, etc? Is it advisable to use hair oils daily, overnight, and washing them off in the morning or applying it 50min or 2hrs before shampooing it?
    4. After how many weeks is it advisable to use hair oils? if it's useful at all?

      They answered it in their ketokonizole video. According to the guy shampoo has little to no impact.
      Anything with active ingredient should be left in the hair for at least 5 minutes as that's the minimum time required for your scalp to absorb

      I know about the active ingredient absorption...what I'm interested in on the effect of having a healthy scalp...for example when you apply hair oils on your scalp and hair your scalp feels moisturised but, when you wash your hair and don't apply any hair oils it feels dehydrated and the hairs feel curly and brittle

    I’ve been a Fin for years and just recently added 0.5mg DUT once a week and I’ve experienced ED, low libido and can’t maintain a hard erection. Side effects are the strongest the day I took DUT and gets better throught the week but after 1 month I’ve decided to stop because the side effects comes back everytime I take it. I have zero side effects with Fin. My routine now is microneedling, 1mg fin daily, scalp massage and nizoral EOD. I know the half life of DUT is like 5 weeks so hopefully I will go back to baseline and I havent done any permanent harm. 😢😢😢

    From what I understand dutasteride is a molecular weight that won’t penetrate thru for the scalp.. topical finistride will though.. basically finistride will still affect u systemically.. dutastride won’t go past the scalp

    Help me understanding: about the 5-a redutase type, considering that finasteride works on the type II enzyme, and that is more present in prostate, otherwise, type I enzyme is the most present in skin, right? So a drug that can act blocking the type I, wont give a better result, thinking about the blocking effect in skin/hair? Like dutasteride that acts in both enzymes

    I was prescribed Propecia/Finasteride for my hair when I was 22 (I'm 38 now) and have been on and off it a few times over the years with no very serious issues and then recently I got moved over to Dutasteride last April (taking for 15 months) and it was normal until about a few months ago it was like I was getting way more erections than usual with some very very minor pain in a small area in the middle off to the left..kinda just figured the guys been working a little too much and brushed it off...then pretty much OUT OF NOWHERE the thing is almost like a kinked hose in that spot going more to the left and down.. Now here's the kicker - the Dutasteride did absolutely jack to help my hair and I had better results/less problems with propecia/finasteride lol

    Im using finesteride...if and until i thin in front and top...id rather have a sharp buzz cut with face stubble then hair in my crown but none in front...that looks sad in my opinion...

    My regime is topical minoxidil and finasteride daily. I am thinking about adding in premixed topical dytasteride and will microneedling it in once a week. Any thoughts?

    What is the recommended dosage of Durasteride 0.5 mg (Avodart ) ? Once a day or every alternate day ?
    I remember dosage of fenisteride is mentioned as 3mg per week that is 1mg tablet alternative day .. but not sure of Duasteride ? Thank you

    Hi guys , love the show. Could you please answer a question for me? I use the theradome LLLT cap along with minoxidil. I was told by someone that you should wait 12 between using either, as LLLT works by stimulating collagen production, while minoxidil halts this.
    Is there any truth in this statement, or is it bs? I have wanted to use the LLLT cap sooner but have been holding off.

    about dutestiride i have prostate enlargement and the doc told me that i need to undergo for laser last 2019 i am get a laser but after do the laser to me my feeling is not back to normal.urine up to i taking medicine for my .prostate like dutestiride finarid for my enlarge prostate thats means the laser doing to me is not gurantee that effective bcos up to now i suffering abnormal.urination.how i take this medicine tnks doc pls explain

    Is true that the rate of sides from dutasteride was actually lower than finasteride in new study. Before I believe the rate was believed to be the same. Can you address the study and whether it makes sense to just go on dutasteride instead of finasteride for better DHT blocking but with the same if not lower sides?

    Iam 26(not married), with hair loss & planning for a Hair Transplant(FUE).Doctor said , I must use oral dutasteride(0.5mg daily once) through out my lifelife after my transplant..Is that okey, or Should I change to 1 tab per week, like that??     Hi,
    I've been taking 1mg finasteride for the last 4 years but would like to start using 0.5mg Dutasteride. What is your recommendation for this migration? Should I start by taking Dutasteride 2-3 times a week? If so, during the days that I'm not on Dutasteride, should I still be taking the 1mg finasteride?

    Dear doctors thanks a lot for the valuable information. I would be really greatful if you could answer my question:
    Can 1 mg finasteride per day be more beneficial than 3 times per week or is almost the same thing ?
    And secondly do you believe that by customizing the dose everyone can benefit from finasteride or are there people that cannot tolerate it at all?

      They answer your first question in the video. There would be no additional benefit to going past 1mg three times a week because of how long it shuts down the production of dht in the scalp. Secondly some people will not be able to tolerate any finasteride at all. Just based on researching the topic people have gone down to .25 milligram doses and spread them out and still gotten sides. But the percentage of people that respond that way is very, very low. Basically if you got side effects from 1mg every three days, you could try .75 milligrams every three days, then .5mg, then .25. The majority of people will be able to tolerate at least one these doses. The best idea If you are worried about side effects and just in general is to reverse that and increase your doses slowly. Start at .25mg for a month, then move to .5mg, then .75, then 1mg. If you start getting sides at one of the doses higher than .25mg, but could tolerate the smaller dose you were on previously, just go back to that dose and stick with it. That’s your safe dose. Any amount of finasteride is better than none so that’s the thing to keep in mind here.

      Dosed every day can be subtly more effective at a percentage level but not at real world noticeable level....and dosed everyday it can accumulate and lower DHT more than you want

    So if tissue level reduced DHT lasts for a whole month, the point of taking Finasteride three times a week is to keep serum levels of DHT lower as well as it seems both tissue and serum levels can effect hair loss....correct?

      The DHT in the serum level is what travels to your follicle via blood vessels and accumulates in the dermal pepilae

      As I know duta blocks more enzymes than fin...which means it is more effective.

    I use finasteride 3x a week and minoxidil 1x everyday. Getting good results. Can I have your view on caffeine lotions/shampoo as I also use this to help but am I waiting my time? I also see a new product serioxyl, can I also use this to help?

    Would like to know what you think about the topical Dutasteride formula that inhibits high amounts of scalp dht without systemic effect because of molecular size. I recently ordered a strut version (0.1% topical gel). Going for blood test to find baseline DHT levels then 10 days after to see what systemic effect it has and how much my dht has decreased. Topical Dut might be the holy grail for individuals who had sides with oral/top fin.

      Finasteride and dutasteride work only they get in the bloodstream they are not topical androgen like RU make a proper research

      @@berkkargn1080 incorrect. The main target is SCALP DHT not serum dht... RU is a whole different ballgame with how it reacts to androgen receptors. There’s a reason why individuals use topicals, to reduce scalp dht more. You’re looking at at a 50% scalp dht reduction while on oral fin 1 mg while your serum dht is getting shot at 70% but when you hop on topical you reduce around 70% but your serum dht still gets nuked over time because finasteride penetrates deeper which goes systemic meanwhile topical dut reduces scalp dht around 80%+ with little to no systemic effect but it depends on dosage and timing. Individuals are taking 0.1-0.5% once a week and getting results. I suggest you do some research on the hairlosstalks forum and possibly join the discord where people are doing blood test and sharing there results on topical Dut, the results are in and topical dut may be a good alternative for no side effects and low systemic absorption. So to answer your statement, no a topical does not have to go systemic it’s all about scalp DHT..

      @@vanishx1537 you didn’t understand what i said. Dutasteride and finasteride don’t kill 5 alpha reductase when they are in a lotion. They kill the enzyme when they get in the bloodstream ı mean systemic they are indirect antiandrogens. If you want to kill the androgens only in your scalp go with RU ıt is a direct antiandrogen.

    Thank you for your informative content Drs. I am an active blood donor and I do whole blood donations every 56 days. I would like to start on finasteride but unfortunately, I have to stop the medication 30 days prior to donation. Can I maybe reduce my blood donation frequency and start finasteride? If yes, how many months would you suggest for the finasteride on/off months? I took finasteride for 8 months 7mg/week and I have been off it for the last 6 months. I have started increased hair loss as a result.

      How long after u stopped did u notice increased loss? I heard some can do 3 mo off without increased loss. I think it'd just be trial and error. Maybe take a month. Off a month. donate. Repeat....?
      It is supposed to stay in your skin longer than in your blood.

    I started on 0.5mg eod thats not even 3mg a week and had to come off the mental side effects that im still suffering from to this day I just hope I'm not scared for life biggest regret I ever made trying finasteride, use at own risk my friends certainly comes with a huge risk of chance of side effects and the dick should be the least of your worries panic attacks were daily never experienced anything like it in my life I just hope it doesn't last forever

    if you are thinning then dont waste your precious hair and time take finasterid and if you can take minoxidil it will stop your hairloss problem but you have to use it everyday but if you dont then try Hair transplant

      I'm currently using Finasteride 3x a week for 2,5 months now. I think it's too early to say, but haven't experienced or noticed any halt yet.. After 4 months you should see some effect I've heard/read. But the side effects are very minimal. Tried it everyday, and definitely did notice the sides much more.

      @@1sad_latvianboi707 some people here in the comments are positive, although not sure if they are doing it 3x a week.
      But the guys here said that they don't notice a difference between 7x a week versus 3x a week in terms of effects of Finasteride. So I do think, it'll help to regrow too. but it might be smart to combine that with minoxidil.

      Long time finasteride user here - my experience has been stabilisation.
      More recently, I've been having PRP injections and have seen some regrowth and thickening, but as I waited too long to act on PRP, that's as good as I'm going to get as you can't wake up follicles that have died. Next option is FUE... haven't pulled the trigger on that yet.

      1 dutasteride stays in body for a month, it is hard drug so why take frequently. More importantly its not recommended yet its dangerous
