There would have been a gallery (probably Studio V in Broadcasting House) working on the BBC news special report at the start airing on the BBC News Channel and World News while BBC One was on regional bulletins. On the clock hour they finished their broadcast and the published schedule switched both the domestic and international news channels over to Studio B and their gallery to start their broadcast. Unfortunately, due to an understandable cock-up given the dynamic nature of the programming, some wires appear to have gotten crossed and BBC One then had a minute or so of promos and a network announcement scheduled at the end of the regional news. This is standard so that all the different regions can get back into sync with a return to network programming. As the Network Gallery always takes its lead on timings from BBC One Playout and the network editor, they would have waited for BBC one to opt in rather than start early for the news channel. Knowing this the network directors in SCAR and WN would have chosen to stick with the studio feed rather than throw to an error slide or something, which would have been more visible and resulted in the usual wash of fake outrage from the shit-rags the next day. Rita almost certainly knew she was on camera within a few seconds, though as a professional she'd have been in position for 90s prior to transmission anyway. Conclusion: the Editor should have noticed a mismatch on their schedules earlier, but it's difficult for them as BBC News Channel and World News are in different buildings, so they'd have to be actively looking, it wouldn't have just been an easy lookup for them. Whoever initially prepared the schedules should also have noted this, but as it would have been a news team choice rather than the normal scheduling team, they probably overlooked the issue. A few mistakes there, but overall no real harm done. Mistakes like this happen everywhere in real life, they're just a lot more noticeable on TV. Plus in most real world scenarios you don't have a bunch of nerds like us scouring the airwaves looking for these boo-boos do you! 😂
