      I have a higher arch with a flexible foot and I keep hearing that's not a good combination. I've been suffering with all kinds of for pains for years. I've not been strengthening my legs for years due to the foot pain. When I was stretching morning a night it did get better. I'm back to stretching twice a day because the pain came back in the ball of the foot and outer foot. Can I strengthen without causing more tightness and pain?

      too many cortisone injections over a period of years, for plantar fascitis. it seemed like nothing else relieved the pain and I was unaware of the risks. I think I got at least 10 injections in each foot.

      I have fat pad atrophy, capsulitis( podiatrist thinks) inconclusive MRI. Last year I had a bone spur removed from distal big toe. I've been an avid cyclist my whole life. My right leg is dominant and likely over time overused the right foot. I've done steroid injections, 2 different lasers, taping for a year+, custom orthotics with ridiculously built up met pad that has now caused a tailors bunion. I just bought Altra Via Olympus sneakers that have the widest toe box I've ever seen ( greatness). At this point I'm testing Spenco insoles and trying to reduce the volume of my met pads. Im on my 3rd session of radial Shockwave therapy to be followed up by cryotherapy. Quite a story huh? After that the only options left are stem cell injection, fat pad injections, PRP and exploratory surgery. Yay! At my last podiatrist visit I told her to cut my foot off and give me a blade. We had a good laugh. My podiatrist calls me a unicorn. Shes never seen anything like this. Good times! Thanks for informative videos

      Out of the blue I had stabbing pain in my left heal 2 weeks ago , but I had been drastically I increasing weight lifting which I’d only done a little each week, I’m a marathon runner and been racing off and on for 8 years, I’m 60 now. But my new foot dr just keeps talking about my heel fat pad, but all he did was an Xray. I’m on my feet 12+ hrs a day for over30 years, but I have always cross trained , so, should I get a second opinion? 🤷

    Thanks for the great presentation! I was diagnosed with fat pad atrophy- advised to get better walking shoes, insoles. But no mention of the feet exercises! I am 77, avid exerciser and walker. Definitely will do what you suggest. Thanks again!

    Doc, I'm in my mid-twenties and I've got it in both heels for unknown reasons for 4 months now. Left heel is somewhat soft but with confirmed atrophy and right one doesn't seem atrophied but it has a very thick and inelastic feel to it. If I'm in a very cushioned shoes with good arch support, I can endure walking for about 5 minutes, otherwise I can't go anywhere without crutches. Spending days lying in bed usually eases the symptoms a bit, but they bite back with full force as soon as I start moving around a bit. I've tried almost all conservative and nonconservative therapies without any luck, and was extremely consistent with them. Went to multiple doctors and they don't know what to say to me. Even received PRP injections in my right heel fat pad in hopes of trying to restore a bit of function it had before.
    All medical findings I did so far do not suggest that there is any other medical issue lurking in the background. Only an MRI showed inflammation and oedema in the fat pads.
    My question for you is do you think that it will get at least a bit better through time, or will I continue to be in agony for many years to come or even life? Ever since this started my normal life has been out of the window. It is so severe that I had to ditch everything and go back to my old parents and live with them since I cannot even go grocery shopping for myself. 4 months might not seem like a extremely long time but it sure feels like that when the pain is strong. I've noticed my mental health deteriorating at slow but steady pace and I'm worried I'll even become suicidal in months to come.

      @@nffc3309 No. It did get a bit better on its own but not a lot, e.g. hurts less when resting and I can walk now for like 15 min instead of 5 (not pain free). Unfortunately I found out I have nerve issues there as well, and that makes it way worse. Another thing I found is that I was scammed for PRP as I was given a fake one, and also the first PRP was mixed with depo medrol (cortisone) and injected directly into fat tissue. That injection caused scarring and atrophy as I have a big hole in my heel where the shot was given. I am considering possible fat pad grafting or real PRP with my current doctor. No definitive conclusion was reached yet.

    I had had injuries at the bottom of the right foot, more particularly at the arch of the foot, caused by a blunt tool that I had to press hard being barefooted (which continued only for a couple of months and that was 5years back) though no such outward scar etc. was ever visible.
    Due to this, I experienced burning sensation at the region and for the last few years the foot pad had receded considerably.
    During the last few years I consulted with many a specialist without much of help.
    Now, I have focussed upon management of the pain.
    I am presently 45, may I ever recover ?

    Thanks for the informative video. I’m an older runner, training for and running marathon distances, and I’m 70+ years old. I’ve developed fat pad atrophy in the ball of my right foot under the first metatarsal. Symptom is I get a bad blood blister at this location every time I run over about 5 miles (happening sooner and sooner as problem persists). I’ve tried taping and different shoes with various levels of cushioning and nothing prevents the blister problem. Once I get a blister I think my gait changes to compensate and that causes progressive problems in my right knee which can be debilitating. Running is important to me and I don’t want to quit (already swim and cycle train for triathlon competition). Finally went to a podiatrist and found out about the fat pad atrophy but only treatment offered was expensive orthotics. I think his mindset was if your foot hurts just quit running duh. I suppose the usual patient is overweight diabetic with massive foot problems and my situation probably seems trivial. Thanks for the home treatment tutorial. I’m going to add these exercises into my daily routine and we’ll see what happens. Plus I always thought going barefoot was healthy for the feet and that’s the default mode for me, guess I’ll need to change that habit.

    The doctor put the injection for heel spur wrong,, he injected me in the middle of my foot where the fat is and ..the spur is still in my heel and now my foot is pain and gets big from the medecine in the injection,,can you tell me how I can get the medicine out of my foot???

    My toes hurt too…..it’s like skin on bones……😢. There are doctors that do fillers for fat pad but I don’t see it in my country.

    I got fat pad atrophy from 6 alcohol sclerosing injections administered by a podiatrist and one cortisone injection before that. Now feels like walking on the bones under the ball of the foot. Never had that feeling before the treatment.

    Strange question. Have you heard of this area becoming inflamed with long covid? I suddenly got horrendous pain after covid but wasn't doing any exercise?

    Thanks for you video. I had an injury to both heels / bursitis in both / severe inflammation - both heels for 18 months. I couldn’t find anyone that had a clue how to help me and I no have no fat pad left at all. It hurts and is like rocks all over my heels and forefoot. Heels are the worse. I have tired everything you have suggested apart from surgery / just wondered if there is anything else you can suggest I am so lost and desperate to get some form of life back I am 38 … lost 2 years already xx

    I got mine from walking on concrete the majority of my life. I've had orthotics made and different foot pads. I can't stand to have socks on my feet now. I have purchased sarcony and thinking about Brooks. I use to have callouses the foot Dr. Would trim but since I've retired They have gotten a lot better. I may try the house shoes next. It's just the fat under my feet. My heels are okay.

    A year ago I jumped and landed on my feet. I have had this heel pad pain for over a year now. I have tried everything; X-ray, Ultrasound, MRI, all showing the foot is fine. I visited multiple physics, all declaring PF. Using some of the PF methods, the foot might have become worse. Now it's in a state where I feel that there is permanent damage; the healing has completely stopped. My life has become unbearable at this point and I've been suffering for so long now. I have kept weight off of it for so long now using tape and generally just staying at home for a long time. Do you think I'm finished - I'll have to tape it for the rest of my life?

    i have bad feet pain like on the metatarsals and the heels , i thought it was plantar fasciitis but then my podiatrist said it’s not and gave me coushioned arch insoles, and i still get the pain. it’s so much worse when i stand still and i can’t find the solution or problem..

    Mine are aggravated by doing reformer pilates. I've been wearing metatarsal pads to help cushion my feet during workouts. Still bothering me some but I'm hoping that strengthening my feet and ankles will help. I did have this before doing pilates and I've had flat feet with over pronation and a history of foot problems.

    Hi I fractured my calcaneus about 5 months ago couldn't walk for 4 and a half I can fully weightbare but my heel looks very narrow at the bottom and feels like I'm standing on the bone even when walking with crutches. It's to painful to walk without the crutch. Is this thinness and standing on bone sensation the atrophy you described in the video and would lack of load bearing have caused this in my situation?     40 years old, 103 lbs. I work in an office, but I run, lift, do calisthenics & plyometrics, roller-skate, and dance (e.g., salsa, Latin hustle, ballroom, etc.). The last time I went out dancing, I did 8 hours before calling it quits. The following day in dance class, I had to stop in the middle of the second hour because the edges of my heels felt punctured and the balls of my feet felt sliced. My PCP thinks I have plantar fasciitis, but I’m wondering if maybe it’s issues with my fat pads or both. Videos I’ve seen on plantar fasciitis talk about the bottom-center of the heel and the forefoot. For me, it’s the outer edges of my heels and the crook of my toes that hurt along with the balls of my feet. Could it be I’m experiencing both ailments? Or _is_ that plantar fasciitis?

    years of pain and hundreds of pounds spent and im still suffering at work with my feet killing me to the point where i would gladly chop them off and have wooden stumps instead. i have no pain in the morning and no pains when resting but my word once i start standing after 20 mins or even less my heals feel like someone in chewing the bones .

      Yes, mine feels like the inside of my heel is being cored out with a razor blade with each step or pressure... It's awful awful AWFUL

    Thank you for the informative video! Is the fat pad atrophy permanent change regardless of the cause or can it get reversed and better over time? If the fat pad atrophy is caused by a steroid shot, is there a chance it would get better once the steroid effect goes away?

The answer can be difficult, generally we see inflammation to be the biggest issue, but steroids could potentially cause long term fat pad lose (in our opinion we don't see this unless there is MANY shots)

      It can all really depend on what caused the fat atrophy, was it an injury, stiffness, surgery? It can be a complete issue overall

      Piezogenic papules can be fat coming out the sides, so technically there would be less at the bottom for sure.

      In fact injections can have the opposite effect. In some studies too much cortisone has been associated with loss of fat pad.

    I am not sure what I have is this but the bottom of my feet in the pad behind the toes goes numb every day for about 5-6 hours and then goes away. I have seen arthritis Doctor's (circulation is very good)), Neurologists but not a podiatrist. Have you ever come across this situation. There is no pain just massive numbness. I got the slant board and while it helps stretching it has not relieved the condition.

    So...interesting !! I was telling my mom to use a tennis ball to massage her feet & do resistance workout with a stretch ribbon band which she did for a while & she loved it ,but her physical therapist told her to stop everything she was doing & just do things she taught her instead !! I also noticed she has lost a lot of muscles & fat !! Not only due to her age ,but steroid medication !! The arch of her feet became significantly higher !! So,I was telling her it must had been the steroid !! And her poor circulation !! She's got angina. 👍👍👍💜🥁🐉🎤💞

    I don't know what's causing my heel pain 😭 I've had it for months but in the last week it's gotten so much worse. My doctor sent me for x-rays but her office said the results are not urgent so booked me 3 weeks out. I'm assuming that must mean I don't have a heel spur or fracture. But I also don't think I have plantar fascitis and after watching this I don't think I have fat pad atrophy either. I just want to get back to running but even walking is hard right now.

      @@User-yh1et yes I still have it. I did have a big heel spur and plantar fascitis. Both in my left foot. I stopped working out and being active (and gained weight 👎) and with rest it got better. Saw a podiatrist in December who said I could slowly start working out again and to do stretches. In February when I got back from a long trip I started running again but it slowly got worse and worse til I had to stop again. So disappointing. Then I went to a physiotherapist who said my gluteus medius on my left side was weak causing me to walk improperly with my right foot straight ahead and my left one slightly out, causing my problems. He gave me exercises to strengthen that muscle and stretches to do. He also said I had to work on my core which had gotten weak from not working out. Right away my left calf which had been super tight got better. But I also right away got sick with a bad virus that took me 3 months to get over. So I stopped working out again and didn't do the exercises. Now 3 weeks ago I started working out again. Can't do anything high impact because of my foot but I'm riding my stationary bike and doing weights, and the exercises the physiotherapist gave me. I think my foot is getting better but hard to know since I haven't been doing running yet or walking a lot. I'm going to give it til September of doing these exercises and if my foot isn't better then I'm going to do my last resort which is Shockwave therapy which can help both with the bone spur and plantar fascitis. Hopefully that helps. Good luck!

      Thank you for sharing! I have quite similar situation as yours except heel spur. I was advised to do shock-wave therapy + laser & ultrasound therapy as well. Does shockwave therapy really help with fat pad syndrome? (Will it make the fat pad thinner?). Thanks again.

    I think i have high arches, i go barefoot at home (at the most i wear socks or my fluffy slippers), and im a skinny girl. That might be why im thin at the heels. My feet are really percussive. If i walk outside with thin shoes like flats or fashion sandals, you can hear it through my skeleton like BOM BOM BOM BOM. A loud walk. I think if i built up the ligaments with exercises and did regular stretching, it could help. Its a little bit of a "damn you, you poorly designed body!" moment to think we need orthopedic shoes to wear even indoors or else we get these problems :/
