A couple of points I would like to make. First (and as always), Dr. Chung makes excellent points here. I saw Dr. Chung for the first time on a video consult, after which he became my regular proctologist. I live close enough that I can go to his office. Second, I learned that he takes a very 'regular guy' kind of approach. He does not hide behind grand words and self-aggrandizing doctor-speak. he represents a medical practice mind set that I wish was far more common.
Had a fissurectomy 15 weeks ago. My proctologist said I’m 100% healed now. I’m seeing Dr Chung says soft but my proctologist said you want your stools to be hard like a tootsie roll. Kindly, could you run this by Dr Chung and see how much he laughs or agrees? My doctor is a good surgeon and good doctor but he sees his patients for like 3 mins and leaves. Doesn’t give much info.
@@Jimmy_The_Goat I would say it worked. I would have targeted I didn’t have to get the surgery but the cardizem cream wasn’t working well. The only thing that worries me is that sometimes I still can’t sit for as long or comfortably as I could before which is inconvenient because I drive for a living. I still have to wipe a certain way which is different than before. I still get itchy from time to time. Nothing feels the same which sucks. However, I still feel the surgery was probably necessary and it did ease the pain immediately. Doctor said I’m 100% healed. When I exercise, i.e. play basketball or push ups, I feel most comfortable after that. So maybe I just need to strengthen the area properly. Idk. It worked. I would recommend it. Just make sure you have a great trustworthy doctor. Took about 2 weeks for real relief after the surgery but it was less painful immediately after compared to the unbearable pain I was in prior.
@@theeradic5629 Thing is, my pain and dicsomfort are fairly mild, like 4/10, but it has been a recurring problem for 8 years now. I am really scared of permanent changes to my body, as I am not even 30 yet, so I am really not sure whether I should do this.
Dear Doctor Chung! You do not know how much your videos helped me. I had been suffering from anal fissure for the past 4 months. I am at a stage where it has healed 90%. I feel no pain and blood stains. Just slight discomfort, though. A lot of times, I had severe pain. So much pain for up to 12 hrs every day that sometimes I even got suicidal thoughts. Your videos made me learn about my lifestyle mistakes. I started owning my pain and have taken steps to be healthy. Dr Chung! I can't thank you more. Your videos not only give remedial ideas, they are also a way to get psychological relief from the depressive mental pain one goes through during anal fissures/ hemorrhoids. Thank you so much on behalf of thousands of patients. To the patients, please keep hope. Good days will come. Meanwhile, keep steering your lifestyle towards health. All the best! 🙂
I'm in a similar situation. Almost recovered. Have you been using any ointment? How do you keep poo soft? My proctologist said, I have a soar fissure... No bleeding, no pain when passing stool.. just occasional discomfort.
@@DecentTalkersalt added to a warm water bath can help healing. Washing the area and a little inside with aqueous cream after passing, never ever soap or shower gel. A tiny square of cotton gauze placed over during sleep wicks out the pus.

Thank you for your expertise! This is the first time a Dr. has said not everyone needs to increase their fiber. I kept telling the Dr. it was making me more constipated and they look at me like I’m a liar. Then they say drink more water then😩. Well I’ve solved the problem years ago by listening to my body. My father died of rectal cancer at the young age of 45 so I'm happy I found your channel because I've been struggling. 😢 I already don't have a gallbladder.
I really appreciate your videos. Never had any issues until last year and it’s when I started googling and going on RUclips and found you. It’s been a very embarrassing and frustrating couple of months, even discussing things with my spouse, I never feel comfortable discussing my own medical issues even more so anything having to do with this specific topic. I’ve finally started to make changes in my diet that I feel are sustainable, practicing stress management, and scheduled an appointment with a doctor (although it’s not for months). So until then, I will keep watching your videos and hopefully find relief. Thank you so much.
Dear Dr Chung, I want to first of all thank you for your consultation a few months back that really changed my attitude towards my problems with my fissure. Your channel gives me so much positivity and hope that when I get my flare ups I'll have relief. I admire your transparency and a lot of the things you say hold a lot of truth, particularly the fibre intake being a unique thing to each person. Moreover, I think the kind of fibre is also very different depending on each person. I have found a lot of relief from fruits that get me going a lot more than vegetables do. Fruit is so demonised because of its sugars, but I think people forget how wonderful it actually is for the gut. It's what you said - everyone is completely different and should experiment with their diets. I have found food/poop diaries to be extremely useful!! Again, thank you for talking about a subject that a lot of people suffer in silence with. I hope your channel continues to grow and grow (I'm sure it will), and that you continue to give us not only good information, but pretty dang GOOD VIBES. Have a wonderful day doctor and thank you!!

@@Asiansxsymbol I don’t want you to be frightened because the surgery was a fantastic success. BUT. It was one of the most painful experiences I have ever had. Be prepared. I went on MiraLAX first thing as to not get constipated. It works. Remember that it’s painful BUT you can get through it and each day gets better. 👍🏻👍🏻🙏🏻
I had a colectomy scheduled a few years ago because I had a BM evey 2 weeks and was diagnosed with colonic inertia. I tried just about everything to stop being uncomfortable. I would drink 3-4 liters of water daily and ate my fruits and veggies. Magnesium Citrate didn't work for me. It gave me a lot of gas, made me distended and very uncomfortable. A couple of weeks before my surgery, my friend gave me a bottle of 400mg magnesium oxide capsule and those saved my life, honestly. I now have daily BM. I take about 2-3 capsules daily but now i need to figure out my diet and how to stay regular. There are times where I'll have 4-5 loose stools a day. Or i can go a few days without a BM if I forget to take my magnesium. What it all boils down to is that everyone is different and there isn't a "one size fits all" remedy. Everyone's body chemistry is different and we need to find which formula works for our body's blueprint. But magnesium is definitely a magical remedy for many digestive ailments. It's a shame doctors don't turn to it before prescribing medications.
@@LastBencher-x5q great question. I am not a chemistry major (and I dont even remember anything from the semester I took in college) to actually understand and explain how they are the same (but slightly different). I do understand that they both can be used as an antacid and a laxative.

@@nadiabonnici7788 I tried mag citrate and it made me very uncomfortable "gassy". I drink a lot of water (3-4 liters daily), I love eating fruits and salads. Oh, and try doing some walking or core exercises. That really helps!

As I get older, I've done some self experiments on the ol digestive system to keep the ol stool from hardening and keep it regular. What works for me is making a conscious effort to drink more water. My experience: After 2 years of misery (chronic IBS-C), I ended up with prescription 45ml Lactulose each day, after every other med had failed. Now, it’s like “clockwork”, I can go every day with perfect softness. Stuff works like a miracle. No pain, no side effects, for me. About two hours after taking it, it’s 💩 time. Of course, it works best, when combined with a healthy diet and lots of liquid intake. Also a blessing for the inner hemorrhoids, that were on the verge of prolapsing due to all the prior straining and long sitting sessions 🚽. Now, all is good, and everything can slowly heal. 👍

@@rachangoudapatilnidoni5058 Fairly safe to take, even prescribed to pregnant women, my gastroenterologist said. Just gradually reduce the dose to a point where you can go to the bathroom regularly without constipation. Was told a clear sign of a dosage being too high, is diarrhea. I started with 45ml/day for a while, now 30ml/day still works. In a few weeks trying 15ml. This stuff does not get digested or absorbed by your body. Large colon doesn’t like it, attracts water to get it out, which makes stool soft and then makes you have the urge to go. Only thing to watch long term, is possible imbalance of electrolytes which can be checked by a simple blood test, which I have done twice a year. Definitely less dangerous than those stimulant laxatives. Works for me, hope it works for you. Only side effect, In occasionally have is bloating/gas. Greetings from Ohio, USA, Marco
@@rachangoudapatilnidoni5058 My gastroenterologist said it is fairly safe, so even pregnant women can take it. Your body does not digest it or absorb it. Large colon gets irritated by it, absorbs fluid and softens stool, which then makes you go to the bathroom. A dosage too high manifests in diarrhea. A dosage too low manifests in constipation. Only long term, electrolytes should be checked which a simple blood test can do. I have this checked twice a year. My only occasional side effect is bloating and gas. I noticed with Magnesium Citrate that I had to increase water levels during the day and between meals to stay properly hydrated. Considering it draws water into the intestine it makes a lot of sense. Good tips as always!
Thank you so much for your kindness in sharing this valuable information. I recently had surgery and I must say the constipation was worse than the surgery but your tips helped me make it through.
For me roasted peanuts works like magic. It has to be peanuts with the skin still on there. Fiber supplements do NOT work because they only contain fiber. Two things are needed for you to go: fiber and OIL. Peanuts have both of those things.
A small bowl of bran cereal and almond milk for breakfast and one before bed and a glass of apple juice with each meal, a few yoga techniques like the wind pose for instance, and a massage using an upside down U from the ascending colon to the descending colon works wonders. Sitting with one leg crossed over the other at the knee or higher and switching periodically can also get things moving. As an aside: If you use one of those potty steps to raise your feet, you are doing a modified version of the Yoga wind pose. I have found that raising a single foot, start with left and then lowering it and raising right creates a sort of 'bicycle' twisting action that can get things going. I have a child with autism who has severe constipation, encopresis, etc. Update: Wanted to add one more tip, roll a rolling pin under its own weight (Dont' press down on it!!!) upwards on the ascending colon and downwards on the descending colon side. It helps to breakdown those stubborn clumps that are stuck.
Thank you Dr. Usually people approach surgery after all other methods have failed or after days and weeks of excruciating pain. Metamucil every day. Lots of fluid. Magnesium citrate at bedtime. Low FODMAP diet. Can't tolerate lactose anymore. Going to try and reintroduce beans this week.
I agree with the Metamucil and I also take it. But everyone is different with how much to take each day. I only take 1 heaping tsp with a full glass of water each day and that works for me. Any more than that makes me gassy all day.

Summary Building a routine and sticking to it is crucial for achieving soft stools and promoting healing for anal issues without needing surgery. Highlights 🚽 Building a routine for soft stools is essential for preventing mishaps and promoting faster healing. 🥦 The focus is on finding a diet that works for you, rather than just adding more fiber. 💧 For loose stools, increasing water intake or adding soluble fiber can help bulk up stool. 💊 For hard stools, trying stool softening medications like Colace, mirax, or magnesium citrate can be beneficial. 🩺 Establishing a routine is the first step in finding relief for anal issues and should be followed by seeking specialist help if needed.
I walk 5km (~45min) almost everyday (just before lunch break), don't eat too much proteins, toss in some 30g of nuts mix and drink at least 1.5L of water (unsweetened tea morning and evening + 2 coffees during the day). Not crazy, but it works perfectly for me! I also avoid alcohol outside of events: It becomes a spray mess really quickly :/ I started using a portable bidet (as japanese toilet style is not an option right now, but I plan to buy one) and wet wipes (specially made for hemorroids too) and care to not wipe or just really gently. I got there by myself after seeing a proctologist who immediately told me to go to surgery (with laser, but still...) after having my first (and hopefully last) hemorrhoidal thrombosis. I couldn't resolve to go to surgery after 'just' that (I went through hell for some months though...). Right now, a full year after, and for about ~4 months I am almost free from it (although, I'm starting to think it will never get back to 'normal', I have almost never pain, almost no pain even when I do, minor discomfort for some 2-3h after going to the bathroom) and still feeling it can get better but not sure. Thanks Dr Chung for your friendly advices and for sharing it with everyone needing it out there! :)
Also check tap water if it has minerals and heavy in drinking,i had cracked super dry poop even after taking fiber suppliments,i switched to bottled water and my stool goes back to normal and easy in 2 days....
I wish you would've been around years ago! Maybe my routines will help someone else....I have found the perfect combination for me (on my own with trial and error)- 400mg Magnesium, a probiotic, and 300mg docusate sodium. Things that make things worse for me are too much dairy products especially cheese, stress, vacation, eating too much processed foods, eating out a lot, eating too much fried foods, popcorn, and especially hefty pain meds are extemely constipating. Fiber supplements made things worse for me, but eating fresh veggies and fruits helps me tremendously. A small amount of coffee in the AM seems to help me too. Walking is extremely helpful in getting things moving. I also find that womens hormones play a part in the constipation cycle as well and is something to be aware of. I'm coming from the perspective of suffering for nearly 20 years, finally having surgery, and not wanting things to get bad ever again. I really want the one surgery to last.
Keep.a food diary to single out the culprits. Even lentils bung me up. I take magnesium every night now. But mirilax is my saviour if im having a flair up. Ive been having lots of veg soups and keir with soaked prines first thing.

Magnesium citrate is a game hanger for me! So glad I found it.

@SeraKookit's a stimulant? And puts the majority of people on a high. Maybe you are confusing it with alcohol?
Is milk of magnesia different?
@@LastBencher-x5q great question. I am not a chemistry major (and I dont even remember anything from the semester I took in college) to actually understand and explain how they are the same (but slightly different). I do understand that they both can be used as an antacid and a laxative.
porridge really used to work on me but lately no and even mag citrate is not working, eating prunes work but i have to take about 6 or 7. but i think i will give mag oxide a try cause i don,t like eating fruit alot
@@nadiabonnici7788 I tried mag citrate and it made me very uncomfortable "gassy". I drink a lot of water (3-4 liters daily), I love eating fruits and salads. Oh, and try doing some walking or core exercises. That really helps!

I only had Cdiff for a few weeks and am still traumatized by it so I can only imagine how bad it was after 8 months! I am trying fermented food too and I think it is helping. The food intolerances are what really drive me crazy. I could eat most things before Cdiff and now have to watch everything.
I first went to the Dr with mild abdominal pain. He thought maybe I have kidney stones and prescribed Amoxicillin as a general antibiotic. Well ten days later I was painting the toilet red with straight blood, I thought I was going to die, I wish I would have died. The ER almost had to remove my colon but thankfully not.
I will forever have ptsd from the hell I experienced and the long recovery. I now wash my hands 20 times per day, bleach the bathroom every week, and stay away from all public bathrooms and restaurants. I’m actually crying just thinking about what we’ve been through.

Lots of meat, berries, butter and less veggies works for me. And chew thouroughly! All good now. The less fiber has actually works better for me. And lost weight and feel better with better blood work.
I noticed with Magnesium Citrate that I had to increase water levels during the day and between meals to stay properly hydrated. Considering it draws water into the intestine it makes a lot of sense. Good tips as always!

For me roasted peanuts works like magic. It has to be peanuts with the skin still on there. Fiber supplements do NOT work because they only contain fiber. Two things are needed for you to go: fiber and OIL. Peanuts have both of those things.
After a right hemi-colectomy I was advised towards less fibre. I started a carnivore way of life and I feel fantastic…the toilet is my friend now!
Sure , until it comes back
A small bowl of bran cereal and almond milk for breakfast and one before bed and a glass of apple juice with each meal, a few yoga techniques like the wind pose for instance, and a massage using an upside down U from the ascending colon to the descending colon works wonders.
Which side is the ascending colon side?

I had gotten my poops under control by taking Mira Lax each morning and Metamucil each day for a few weeks. I was going to Mexico for 6 weeks so I packed a supply of each to get me through. I have been here 2 weeks and haven’t taken anything. The food here seems to be keeping me cleaned out.
Is it safe to take both and does it make your poop soft ?
Metamucil every day. Lots of fluid. Magnesium citrate at bedtime. Low FODMAP diet. Can't tolerate lactose anymore. Going to try and reintroduce beans this week.
I agree with the Metamucil and I also take it. But everyone is different with how much to take each day. I only take 1 heaping tsp with a full glass of water each day and that works for me. Any more than that makes me gassy all day.
I take 1 teaspoon of psyllium twice a day in orange juice and works extremely well!
Fiber works for me…I eat so little because I gain weight easily so I know I am not getting all I need in food…I eat healthy a healthy diet and at least one hearty serving of veggie in the evening meal and it works. Dear Dr. You are doing a real wonderful job here, advising fischer, hemorrhoid sufferers.
I am from India. Since covid vaccination my problem of Fischer got worst(I came to know it after 2nd dose) and now the sphincter muscle is so rigid that every morning I get lot of pressure from inside but due to shrinkage of sphincter muscle I can not get rid of poop easily (which is actually soft) so many times I am on toilet seat and trapped in between passing stool process, doesn't understand what to do and I apply force which results in fischer then more swelling, pain, legs pain. It's a terror entering toilets when you have Fischer. Now a fistula has developed. Literally life is not fun anymore. I am constantly examining myself, my diet. I hope someday I will get real relief. And some food Literally helps you in this. Thanks doctor, I may write you a long letter about my this episode. God bless you. 🙏

What product can we buy that qualifies as a teaspoon of "fiber" to take along with meals?
You could try psillium husks. A teaspoon in a glass of water with meals.
I had bowel resection surgery 6 months ago. Have problems with constipation!! Tried many things. I’ll keep trying….thanks
1Tbs olive oil 1 tbs fresh lemon juice on an empty stomach
I'm always constipated. I'm stuck in a hospital bed for the past 2 years. I can't even sit at the side of the bed to eat. I have continued gas. My stomach is louder than my tv. I'm 74 , short and my bed is to high for me to put my feet on the ground. (Thank you medicare) I have to poop in bed lying on my side. Please help me.😢
Thank God
for about 7 years I didn't get hard poo
And that What I do
Drink worm water cup or to when I wake up and worm water before I sleep
Like the first think you drink is water and the last think is water
Water is the best thing for soft poo
Drink water about 2 or 3L
You mean warm water? No-one wants worms! I might try warm water.
Thanks for the information it make doing the #2 easier.
Can grief cause constipation?
Is dulcolax as safe as miralax to take everyday?
Dulcolax will cause dependency...no laxative should be taken regularly for a long time
@@nitikasehgal6331for how long we can take it. Or can we take it until the fissure gets healed completely
scared to death of the deep probe and surgery. IDT my body will do well. It does not like things in. And it is very dirty area to try and keep clean. I am very concerned.
Summary
Building a routine and sticking to it is crucial for achieving soft stools and promoting healing for anal issues without needing surgery.
Highlights
🚽 Building a routine for soft stools is essential for preventing mishaps and promoting faster healing.
Thanks for the summary! Beautifully done.
How r u now anudeep
I walk 5km (~45min) almost everyday (just before lunch break), don't eat too much proteins, toss in some 30g of nuts mix and drink at least 1.5L of water (unsweetened tea morning and evening + 2 coffees during the day).
Not crazy, but it works perfectly for me!
I also avoid alcohol outside of events: It becomes a spray mess really quickly :/
I started using a portable bidet (as japanese toilet style is not an option right now, but I plan to buy one) and wet wipes (specially made for hemorroids too) and care to not wipe or just really gently.
So did you get the surgery?
Thanks for sharing!
I have small internal hammoroid in rectal canal and blood in my stool i am very much worried
Carnivore has helped me greatly in that dept
💯 agree. I have no issues while on carnivore. 😂
Do you poop less?
I’ve been on carnivore for 2 months now, & I’ve always dealt with constipation but now it’s worse. Do you have any suggestions??
@@Becky44-e7g eat more fat and it should help
Absolutely…Went 90% meat - done. Never been more regular. And perfect 4 Gleason score. Eat more meat.
@@Becky44-e7gIncrease your fat intake and also salt too.
that guy is the BEST
Also check tap water if it has minerals and heavy in drinking,i had cracked super dry poop even after taking fiber suppliments,i switched to bottled water and my stool goes back to normal and easy in 2 days....
Never thought about that
Bottled water helps with softening your stools?
Yes
Keep.a food diary to single out the culprits. Even lentils bung me up. I take magnesium every night now. But mirilax is my saviour if im having a flair up. Ive been having lots of veg soups and keir with soaked prines first thing.
How soft? I feel like it doesn’t come out great when it’s soft either. I have pelvic floor dysfunction
Broccoli, and blueberries and probiotics. Works for me. Water and exercise. No red meat. Fiber, protein, vitamins.
So helpful!
What about oxalate?
Can i take potassium citrate instead of magnesium citrate , please?
I want to drink more water. Im 70 and more water means more urinating through the night. Advise?
Just drink loads during the day and warm water morning and night. Then stop drinking 2 hours before you go to bed
can small amounts of magnesium sulfate be used as softener?
I sêem to always get constipation because I dont move as much. I would love to get miralax
Dr. Chung I cannot poop every day to save my life! I WISH I could! I poop every 3-4 days no matter what I do. Apparently everyone has their own pooping schedule according to their own body. I eat fiber and drink lots of water, I even eat foods that help you go on a regular basis like watermelon and pears and even then I go every 3 days. I pooped every day for a few days a long time ago (don’t know how that happened) and It felt AMAZING to poop everyday! My husband poops every day like clockwork I’m jealous! At this point I’m just venting. 😁 Hey it can always be worse right?! Well it actually is if I don’t eat well. 😒
not everyone needs to poop everyday as long as you don't have symptoms. I know many people who are jealous of you and wish they didn't have to poop every single day.
@@yourfriendlyproctologist What! 🤯 I guess we all want what we can’t have.
Avoid fiber
I was using magnesium for years, then I was put on antibiotics for an infection and the pills weren't supposed to be taken with magnesium. I got really constipated and really strained on a bowel movement. I got a fissure and it's been months and am going for a fissurectomy soon. I have been using diltiazem and bought the Pranicura but don't like the menthol in it. (they should have put in lidocaine instead!) I find Desitin works best to relieve the discomfort I also use Recticare cream like you had recommended.I take the magnesium every night and I have had no problems. I also have Colace too. No more hard poops! I eat a lot of fiber in my diet. I am bad at drinking water but am trying to get better at that.I am in my sixties and NEVER had a fissure before this!
