Operator error is never the manufacturers fault. The manufacturer knew exactly how the ride was supposed to be operated and this information was undoubtedly given to the amusement park before they ever put people on the ride. The smiler clearly wasn’t meant to run in any strange or unusual manner the instructions are simply to not ever place 2 trains within the same block zone and the computer followed these instructions perfectly, until a human operator bypassed the computer WITH people onboard.
