You can see how it works with the editor. If you don’t kill the swat guy the door is already unlocked as a “cutscene” door because you killed everyone in the level. If you did kill the swat guy though, he drops keys that you need to grab to open the door instead.

My problem is that sometimes the NPC's spawn in slightly different places or walk around the map, so it can be hard to keep doing the pattern.
The flamethrower bits are the most satisfying.
Also I enjoy controller gaming on my PC.

If you kill the last regular enemy (non fat) while in tony's cutscene trigger the game freaks out and lets you go straight back to the van
