    Your Doctor Thinks You Have High Cholesterol: Here's What to Do - Thomas DeLauer
    HDL, LDL, and the LDL misconception
    So, what about LDL “bad” cholesterol versus HDL “good” cholesterol. Actually, LDL and HDL are not different types of cholesterol, rather they are different types of cholesterol “boats.” You see, cholesterol doesn’t like water, which means it can’t swim freely in the bloodstream. Therefore, your body needs to package cholesterol into boat-like carriers called lipoprotein particles. LDL (which, here, also includes VLDL) stands for Low-Density Lipoprotein and HDL stands for High-Density Lipoprotein. Both LDL and HDL carry the same cholesterol. The only difference is that LDL carries cholesterol from your liver to your other organs, whereas HDL carries cholesterol out of your blood vessel walls back to your liver, and is therefore generally considered “good.”
    It’s also important to recognize that the main function of LDL is not actually to transport cholesterol, but to transport triacylglycerides (TAGs), fats that your cells burn for energy! Dave Feldmen, whose lectures you can watch on RUclips, extends the boat analogy, introduced above. The boat is the Lipoprotein particle, the passengers are the TAGs, and the cholesterol is the life preservers. The life preservers are important, but the main objective of the boat is to transport the passengers (the TAGs) around in the bloodstream.
    But perhaps the most important question, “is LDL actually “bad” cholesterol?” The short answer is, it depends on whether or not LDL has been turned to the dark side by sugar! LDL by itself is fine, good in fact. It’s goes about its business transporting TAGs and cholesterol to organs around the body for energy, cell membranes, and hormone synthesis. But when LDL mixes with too much sugar in the blood, the sugar becomes attached to the LDL in a process called “glycation.” This makes the LDL particle more vulnerable to a damaging process called “oxidation,” and it is this oxidized LDL that is dangerous to our health. Oxidized LDL is also called small dense LDL because glycation and oxidation make LDL smaller (aside: combining processed vegetable oils and n-6 PUFA-rich foods with sugar is the worst because it accelerates the glycation-oxidation process that turns Anakin/Good LDL into Darth Vader/BAD sdLDL).
    Anyways, the reason small dense LDL is bad is that this damaged form of LDL can’t be recognized by LDL receptors and taken back up by our livers. So where is the small dense (oxidized) LDL to go? Well, first it begins to accumulate in the blood (this leads to what’s known as a “high LDL particle count”), but eventually it invades the walls of our blood vessels, leading to the formation of a plaque. Not only is small dense LDL particularly good at getting into our blood vessel walls, but, once there, it is the only kind of LDL that is recognized by the scavenger receptors on blood cells. These white blood cells try to gobble up the small dense LDL and, in the process, turn into sickly “foam cells” that bunch up and clog our blood vessels!
    So, to review, LDL transports fats and cholesterol to cells around the body. When LDL is exposed to too much sugar, it gets damaged (turned to the dark side) and becomes small dense LDL. Small dense LDL can’t be recognized by classical LDL receptors and taken back up by the liver, so it builds up the in the blood and slips into our blood vessel walls. Here is it eaten by white blood cells that poof up into foam cells that clog our arteries.
    It’s common that people on ketogenic diets will have high LDLs and that this will create a conflict with their doctors, most of whom will advise going off the LCHF diet and onto a statin. If this happens to you, ask your doctor for (1) a lipid panel with fractionation to determine if you have high levels of sdLDL and/or (2) for a coronary calcium CT scan (known as a CAC test) or a carotid intima-media thickness CT scan (CIMT test) as functional test to look at whether you have plaques forming.

    My doc tried to put me on pills after I started keto, but I already knew all of this stuff thankfully and just said NO (to their great annoyance). All my other health markers greatly improved and were totally healed so I didn't worry. Now my cholesterol is normal again. Keto even cured my "incurable" blood cancer.

    When my Doc told me my LDL was too high (131) Total (249) HDL(92) ratio of 2.7. I suggested the large/small particle test, even before I saw this video. She declined saying I had a 2% risk for a heart attack. So which is it? My cholesterol is too high or I’m not at risk 😂. I told her if the AMA hadn’t lowered the LDL standard due to pharmaceutical funded studies, we’d still be at the former standards, right? Crickets….. I told her my LDL has NEVER been at 100, even as a leaner, more active teen…. Crickets…. Some Docs do know but HMO’s won’t allow them to comment. When you are leaner and healthier than your Doctor it’s difficult to take some of their advice too seriously, especially when it concerns Keto and cholesterol. Keep up the great work Thomas!

    Dr ordered a panel on me & at the time I was zero calorie fasting on the eighth day. Of course he wanted me back on the statins. I told his nurse to pull his own copy of Guyton’s Medical Physiology off his shelf (every physician has one) & read about the numbers in the fasted state. Lost 70lbs, off a bunch of rx, no cpap, CAC score is 1, A1C is 5. Last bad food choice was a cookie at Christmas. Love your talks, with thanks & respect for all you do!

      Basically if you are zero calorie fasting, you become a fat burner instead of burning the carbs you ingest. The fat on your body is the largest repository of cholesterol. You are breaking that down & it circulates in your blood at higher levels during prolonged fasts. It freaks out your Dr & their “standard of care” screams write for statins & “you could have a stroke or die” mode. I got the lecture & did the research & refused the rx.     Been on Keto 3 years and carnivore 3 months an feel amazing at 61 years old!
    Lost 35 lbs and leaner with more muscle mass than in my twenties!
    No medication and eat clean!
    We must be our own best doctor!
    Two of my sons are doctors an on board due to my push for burning on fat fuel!
    They get little nutritional studies in med school which is a head scratcher!!!
    Many doctors are lazy an in bed with the pharmaceutical industry!
    No money for them if we’re healthy!

      My LDL just came back @ 251. I have been on clean KETO for 8 months and I feel GREAT!! Steaks, salads, greens, cauliflower, nuts, bone broths, Olive oils, coconuts, coconut oil, MCT oils, naturally raised cow butter.
      I have lost the 20lbs needed, have energy for days, and pop out of bed in the morning.
      I have a Coronary calcification scan scheduling appointment tomorrow, and just put in for a Lipid panel with Fractionation.

    Life saving video. I've been on keto for 2 years & my cholesterol has never looked better as far as my HDL and triglycerides occasionally my LDL goes up especially when I'm in more of a fat burning mode. I'm getting so tired of explaining it all to my doctor who always ignores my good numbers and focuses on prescribing me statins that I don't need & which I will NEVER take - I'm going to send her this link.

      The doctor gets kickbacks from statins she prescribe. The establishment needs to milk $$$. The establishment has nothing to do with health.
      It is just a business with lots of laws and lobby behind big corp/$$$. $$$ dictatorship.

      Don't focus so much on cholesterol. Focus on cholesterol oxidation and inflammation! What do you think happens when you cook meat? It oxidizes the cholesterol/fats which in turn alters the function of HDL and LDL in your body making both more atherogenic! Ask your physician to check your inflammatory markers - Interleukin 6,10,17A, SMC- Endothelin, TNF-Alpha, Lp-PLAC2, and a full cholesterol panel called CardioIQ by Quest Diagnostics. Since you have been on KETO for 2 years I would be SHOCKED if your Lp-PLAC2 was not elevated. You can also do a carotid ultrasound checking CIMT. Yes, I am a functional medicine practitioner with nearly 30 years in the health and fitness industry with a specialty in disease prevention and reversal. I am also an exercise physiologist and now teach other professionals across America on this very topic. Good luck!

      @@Total_Body_Fitness_USA Thank you for the detailed info. I did have the chol density test previously with good numbers, the next time, will check the other markers you have suggested. My BP is low and my HR recovery is very fast after HIIT. I've had acarotid screenings in the past 2 yrs each with an excellent report. I've been a low carb, no sugar/flour advocate for the past 30 yrs, but the keto lifestyle has greatly improved my chol numbers and definitely reduced inflammation. God Bless.

      @@cvwatch That's good to hear. Checking interleukin inflammatory markers along with Lp-PLAC2 will assure there is no endothelial inflammation. As I tell all my clients. Do what works for you. In any case, keep in mind that cooking of any meats causes cholesterol/fat oxidation and after ingestion they cause alteration of HDL and LDL, not to mention they are highly carcinogenic. Take 12mg of Astaxanthin 3-4 times per week and eating a lot of greens helps counteract the negative effects.

    The thought process on this is definitely changing in the medical community. Total cholesterol used to be the scary number. At least we are now focusing more on cholesterol ratio which is actually useful as an overall health measure.

    Hi everyone. I want to say that about two years ago I was having a total hard time to loose weight due to my age (51) I was having horrible hot flashes that wouldn’t go away with nothing, not even medical pills and feeling terrible like going down hill even thought I was eating healthy and exercising 4times a week but I knew something was wrong. One day I came upon one of Thomas video and started keto and to my amazing Thomas, I started feeling soo much better and have loss 15lbs and have no more hot flashes now. I look so amazing that I don’t look my age and I’m 53) I say begin in a fasting stage and keto has been my life saver and yes my doctor say my cholesterol is high but I’m not worried Last visit he asked me are you taking your satin and I told him no in he’s face and he said well what your doing that it has gone down 15points and I said I detox my liver once a month and still doing my keto with fasting. He said ok you looking good keep it up.

    I feel this video was made for me!! My doctor is a very petite woman, like 100lbs. I was 198 down to 165 when I saw her last and she hated everything I said I was doing just because my LDL was a little higher than normal. She didn't like intermittent fasting, NOTHING! She told me I didn't need to loose any more weight too. I am 5'3 and still considered overweight and she was just like your fine. It was like how is this 100lb woman fine with me being overweight still??? I would still like to loose another 15lbs. I am very thankful for this video.

    Today had Healthy Heart screen, TC:288 LDL:216.. MY CAC CT SCAN SCORE - 9.5. That's right nine point five, male mid 60s.. all this cholesterol fear mogering is almost criminal. .no...no .. it is criminal! !! I'm very lean, work out regularly building muscle, 6 foot 212lbs, 34 inch waist. Keto 3 years.

    I’m getting my specialized fractionated labs done Tuesday. My doctor is using me as a guinea pig before making judgement on the diet and maybe trying it for herself lol. She even told me if insurance doesn’t cover it she will reimburse me. She is a skeptic but an open minded one.

      Colin Charan-Kumpula I’m 39 and relatively healthy. I am only in the early stages of metabolic syndrome and don’t feel the need for exposure to radiation for something that is more curiosity and research based for her to be a better doctor for other patients who mention wanting to do keto. I’m exposed to enough radiation working as a nurse in the ICU and all the xrays done around me there. If I can’t reverse this metabolic syndrome and it gets worse I would look into the test. My goal is repairing and protecting my future health.

    Expect to pay higher life insurance premiums when they see your overall high cholesterol number. They don't get the relationship between LDL...HDL...and Triglycerats. Very frustrating!!

    Wiser patients make wiser doctors. Asking for the fractionation test is a great idea! It causes the doctor to actually think about things before prescribing stantins.

      Especially if your HDL and other numbers are good. It’s about the whole picture. Ask your doc how much they know about fractionation and hyper-responders. Hopefully you don’t get a blank stare. 🤔 🤷🏼‍♀️

    Im not worried about LDL anymore and I refused to take man made chemicals to alter these numbers which in turn will wreak havoc in other ways in the body, 100 years ago there was no such thing as chronic heart disease and definitely no such thing as statins. Just eat the way God meant for you eat, get rest, and keep stressors to a minimum and the rest will take care of itself...My TC is 390 with high LDL high HDL and Trigs at 100....Im 6,1 177 29" waist 5 % bodyfat and my doctor looks at me if im a sick man

    My doctor wrote a hand written note on my labs and mailed it to me. It said, "I HOPE YOU ARE NOT ON A KETOGENIC DIET!!" Lmao. I hadn't told him because I didn't want to debate during my annual check up. I'm moving to Las Vegas in a few months. Hopefully I can find a Keto friendly doctor there.

      The problem with most people is actually relying on a doctor for their health. Doctors know almost nothing about the human body. Specialist know a little more but even what they do is almost complete guess work at all times and then add in the fact that they have to include all of this information from other subjects they know nothing about like nutrition and it's no wonder so many people die. Take responsibility for your health and don't rely on doctors and you will be much better off.

    Getting a Dr that understands these things is so important! I have Kaiser now and just randomly picked a Dr. I really lucked out and got one who is fully supportive.
    About 10 years ago I had a dr put me on a statin that caused severe insomnia. So he changed me to a different one that caused severe muscle cramping. After that I refused to take any statins.

    Funny thing is decade's ago total cholesterol #s use to be 300 , they dropped it to 150 to 200 depending on what lab you went to .... they weren't making enough $$ on meds! Time for people to educate themselves on the lies perpetrated by the medical industry. This guy is well informed on the truth!👏

    My LDL shot up from 96 to 279 after 6 months on keto. After repeat lipid test and going in circles my doctor he finally ordered a fractionation/particle test at my reques. Awaiting results as we speak !

    I just had my Lipoprotein NMR done 11 days
    ago (5/12/22). I have been following the
    keto diet for 3 years. I am 66, 125 Ibs. 5'7".
    Walk 5 miles a day and do free weights. In the past years of age 45-60 I was very athletic,
    now not as much now but only walking and free weights.
    On no scripts. My BF is 15. BMI: 19.15
    Results came back as follows:
    Cholesterol: 312 (up from 2020 of 259)
    HDL-C: 100
    HDL-P: 31.7
    LDL size: 22.0
    LDL-C: 207 mg/gl
    LDL-P: 1816 nmol/do

    OMG!!! I literally just yesterday received an alert via my Patient portal, that my Cholesterol levels were too high and my doctor wants to speak with me...LOL I knew it was not a situation to be concerned about, because they are FAR too over cautious about Cholesterol levels... What I did not know was about these extra panels that can be done... Thank you Thomas... Been at this since 1994 and always Learning... That is what life is about, being open to Learning.
    As an addendum I am am super lean sprinter and Weight lifter for 30 plus years....

      If something goes wrong with you, blame Thomas and not your doctor. I see, it's becoming a fashion to unnecessarily bash doctors by pseudo doctors on RUclips for misleading people. If you die of heart attack, Thomas should be held responsible.

    Thank God you covered this! I literally just got my bloodwork back and for the first time, my cholesterol wasn't "stellar"-my LDL was elevated for the first time ever, and so were my triglycerides (not sure about that one, but it wasn't too bad she said). So I will cover this video with her the next time! My question is, how do we know if OUR particular body will be able to utilize the cholesterol the right way and with diverse particle distribution? I am nervous that my body may not react the same. Maybe you can cover this in another video, and what to do if the outcome is not favorable?

      You may forget about LDL all together, just do this:
      Divide your Triglycerides through your HDL number.
      The outcome of 2 is great if 6 and over you should investigate it further according the advise in the video.
      Just ask for an advanced Lipid profile / LDL-c less than 100 great between 100 an 129 acceptable / 130/160 borderline

    Great video. Luckily my doc does keto and understood immediately when he looked at my bloods and tested my ketones. I’ve had trouble explaining this to friends who think it is black and white with lipids being raised negatively.

    Thank you Thomas for all your help! I really needed this my doctor was freaking out so I'll have to ask her to do this. You have so inspired me in my keto journey! I decided to help too. I just started my own channel EZKeto! I started with my meatloaf. This is a lot harder than it looks! Any suggestions you could give me would be greatly appropriated. I have lost 64 pounds since I started in Feb. Without you my doctor would have convinced me quit. Thanks for all you do and all your help with my journey to a healthier life!

    When I started keto my doctor asked if I'd be a Guinea pig and come back in 8 months for a full blood work with extra labs. Three months and I've lost almost 40lbs. Curious about my yearly employment physical that will come first.

    Wish I would of known this 20 years ago while on keto..my doctor made me retest and challenged me. I ate oatmeal every day and cleared it, but I got my water weight back and had too many carbs back in my system after losing 60 lbs and going to the gym 6 days a week. They see LDL and freak out.     My doctor knew about fasting and keto and told me my cholesterol probably wasn't a big problem, so he didn't see a need to get a fraxination test. It seems some doctors nowadays are very informed.

    My calcium score is zero and I competed in body building shows doing very low carb diet. LDL was always 250-300 range. But low body fat. An individual who has a high carb diet, could have terrible blood sugars and bad insulin sensitivity causing tons of inflammation. High carb diets cause high blood sugars. Lower carb diets high fat cause a raise in overall Chloestrol. Whatever energy source you choose your body will show via blood work. I went high carbs and the LDL dropped but all the bad went up. Have high cholesterol is fine just don't have high inflammation. Turmeric and Fish oil omega 3s are the way to go.

      Bingo. I got my hsCRP from 5.0 to under 1.0, but my cholesterol went from normal to a little high. I'll trade high cholesterol for low inflammation markers every time.

    Very timely video. I just got my lab results and they were the best I have had yet. I sent the results to my cardiologist and his RN wrote back that they will look to increase my statin since my LDL number was 62 and the new protocol states that it should be below 55. I wrote back stating that I had already stopped taking the statin and the results were due to LCHF diet. My follow up appointment next week should be very interesting.

    My LDL went up 50 points over one year since I have been diagnosis with Celiac and went on a Gluten Free Diet. Have you seen any correlation between GFD and increased LDL?

    I'm glad it's working for ya'll. I started keto Nov 2017 and in March 2018 I had a heart attack that killed 1/3 of my heart. Age 34. I was only trying to lose 15 pounds...I had a normal BMI. 4 weeks in ICU, 12 days on full life support, 1 week on the floor and 2 weeks in rehab. So what went wrong for me?

    Valuable info to take to my doc, but as a 75 year old KETO male with HDL @93, I am reading that number(93) is dangerous(high); but not sure why!. Other lipid panel values: T.ch = 169, Triglycerides =32, VLDL =6, LDL (calc) =70, A1c = 4.9., resting h.r.= 48-52. Regardless, KETO to the end for me.

      When its our time its our time. Just know your levels are much better than the typical person at your age who eats like garbage. You most likely have low inflammation from your diet and you will be healthy til the end!

    Understanding doctors is simple. There are commonalities in any advice driven industry. I'm thinking of a famous money manager who once said about his industry "Never be wrong alone." If a money manager subscribes to "conventional wisdom", and their premise goes wrong, they can say "Well, we didn't see that coming". The outlier that is wrong gets fired. This is how they protect their jobs.

    Yep, just had blood work done and my doctor says I should see his nutritionist. How much you wanna bet she would tell me to go low fat. Not doing it. I can’t tell from my numbers, being a Canadian, whether they compare to anything posted here; but I’m not going to worry about it. Thank you Thomas.

    THANK YOU. I'm doing strict keto and I had solid HDL and Triglyceride levels, but LDL was elevated and the doctor wanted to put me on a statin. I declined. Next time I go in I will requests those lab tests.

    Like, how high? My LDL went up 300 pts! I'm not overweight, my other numbers are great, but wow, that's such a huge difference. Since the last time I had my lipids checked, I quit smoking (6 months) and started Keto/Carnivore.

      Grushpea if you can’t breakdown fat then it’s a little risky. Need to make sure your body can actually break down the fats.

    Thomas you're amazing, I love your videos. I can't tell you how thankful I am for this video! I just got blood work results 2 weeks ago and my A1C has never been better but my LDL and total cholesterol went up & out of reference range. It wasn't enough for my doctor to be worried but it has me scared. I'm going to ask my doctor to order the lipid panel with fractionation to really see what's going on. Thank you again!!!

    I soooo needed to hear this information. I’ve been doing keto cycle almost two months. Had my bio markers recently and my LDL and Cholesterol levels were elevated. Yet my glucose was on point! This made me nervous and almost caused me to quit keto. My doctor emailed me with recommendations to eat a low fat diet or medications may be needed. I’m sticking with a keto lifestyle. Next time I will request the fractionalization test. Where can I read the articles mentioned to further my research? Thank you 🙏🏼

    Good info. However, I’d like to point out one small error: a CT Calcium test does not identify plaque in the arteries. In fact, the scan cannot even detect soft plaque (the unstable kind) in the arteries. What it does detect is calcium-as the name implies. What’s the difference? First, a calcium CT scan is backwards looking-meaning, like A1C results, it shows you what has transpired over time. The presents of hard calcium in the arteries simply states (and in layman’s term) that there was an aggravation on that part of the artery and the body repaired it. Think of a pothole in the road that was refilled with asphalt. People on statins also tend to have a paradoxical presence of calcium in the arteries. In short, presence of calcium in the arteries shouldn’t be worried about too much. Only an angiogram with contrast test can show any blockages in your arteries.

    MY ldl is 144 a1c 5 and i do fasting every day and i am also very active .CT angiogram was normal still my Doctor keep calling for every time my blood work results comes up but sometime inflammation also cause high ldl and plus hypo thyroid also cause high cholesterol

    I’m headed to the doctor’s office for my first appointment since starting Keto...
    Let’s see what he has to say.
    Edit: He was shocked that I had lost over 50 lbs. since my last weigh in. I said, you told me to “have some meaningful weight loss”.
    He said, “Hell yeah, that’s some meaningful weight loss!!” And it was the first time since knowing me that I was not considered ‘morbidly obese’. All my #s improved including cholesterol and LDLs. He cut my BP meds in half, instead of increasing them as expected. Then I told him that my wife and daughter also joined me in Keto and combined we had lost 125 pounds in 3.5 months. I’ve never seen a doctor so Thrilled at a checkup. I can’t wait for my next appointment in 6 months.

    YES! I just got my blood work...and was so excited. My HDL was great...my Triglycerides were low but yes the LDL was high. giving my an overall high. My ratio of overall to HDL was good to! Doc office called to offer drugs (I think they are probably mandated to do that) but I would love to have that LDL dense test. I wonder if Medicare covers?

    Cholesterol Blood work done today. Blood seemed to flow a bit better this time vs thick the last time. Not sure if that has anything to do with doing KETO since September but have fingers crossed & hoping for the best!!!

    I have been borderline diabetic for over 10 years while at the same time My cholesterol has always been very low. I have also lost weight at times with no change in my glucose numbers. I have been intermittent fasting for about 6 months and have lost about 25 pounds. I recently had my blood work done and my fasting glucose level was 79. My LDL was elevated quite a bit. Good thing was that my doctor said, keep doing what you are doing, we will just keep an eye on it. This info on other testing is helpful and I will suggest that when we do my follow up blood work in 6 months.

    I am 64, been Keto for 10 months, lost 85 lbs, off arthritis med and 4 others. Heart disease runs in my family and I am still being treated for hypertension, but when my blood test said high cholesterol I asked my primary if I should have a CAC. He looked at me funny and said" I don't think you need one. Why do you think this is? Too expensive? They have the machine and software.

      I’m thinking of going to get one done on my own because my doc isn’t for keto. Why should I ask her to help me have a test to reinforce my pursuit of a way of eating she doesn’t approve of? The place I found (JohnsHopkins) said insurance doesn’t pay for it and the cost is only $75. Why not? 🤷🏽‍♀️

    ask your doctor for (1) a lipid panel with fractionation to determine if you have high levels of sdLDL and/or (2) for a coronary calcium CT scan (known as a CAC test) or a carotid intima-media thickness CT scan (CIMT test) as functional test to look at whether you have plaques forming.
    Easier said than done.
    I thought you had a video somewhere about when in your fast is best to get the lipid test. I do OMAD and have fasted up to 72 hours. When would be best time to have a lipid test so that my doc doesn't' have a heart attack

      Because there is no peer reviewed RCT in favor of what Mr.Thomas is talking about. Everyone has their own hypothesis. Unless RCT peer reviewed papers say otherwise, it's just a bunch of opinions.

    Wow! This video is a ring right to the finger. I’m been doing sport, exercise or training since I was a kid, different kind during my life: recreational as a child, training as a teenager, performance and endurance during adulthood. Always being thin and taking care of what I eat. Since recent years I adopted an intermittent fasting approach and low carb diet (almost zero sugar) and exercise 5 day a week running 8-9 km per day and doing weights to improve muscle and maintaining it. Several years ago I was diagnosed with high colesterol 😳, somenthing completely shocking for my life style and my overall health even my genetics (my body fat percentage is alway below 17%), the only approach from doctor was to take medicine (statins). I fight and refuse this outcome and stop using them, there should be another answer AN THERE HERE IS! Thank you Thomas!

    You don't mention the RATIO of good to bad cholesterol. My doctor tried to get me on the dreaded pills, but my ratio put me at the very lowest chance for heart disease, far below the minimum level.

    I being in keto for 3 years because I have food allergies. I can't eat carbs or salads because they make me sick. I being eating once for 30 days, just water and minerals during fasting. My Dr just told me that my bad LDL is 299 and good LDL is 89. Should I worry? I'm lean.
