@@Asiansxsymbol You'll need a prescription. Talk to a dermatologist and tell him you want to use dutasteride because finasteride isn't working well enough for you. If you're having troubles then talk to a hair transplant surgeon as they're generally more liberal about prescribing dutasteride.

From what I remember, Merck tested a selective AR-1 inhibitor called MK-386 on stump-tailed macaques and found that it was ineffective against hair loss. So, yeah, it's mostly dutasteride's superior inhibition of AR-2 that gives it additional strength over finasteride.

Hi, what is your dosage? I just did my first dosage today 0.5mg dutasteride, doc also prescribed me 0.5mg daily. Never been on fin , 1,5-2 Norwood and some hairline thinning. But head full of big curly hair. Can't say I'm agressivly balding. .. age 42.... I'm wondering if 3x weekly or even 2x is good enough, or just follows Doc's dosage

0:51 A minor correction here: Hypertrophy is not the same as hyperplasia. In hyperplasia, there is greater cell proliferation than normal. In hypertrophy, individual cells grow in size (this is what happens when the liver "regenerates"). In BPH we have hyperplasia, not hypertrophy.

I've been on dutasteride for 2 years, it filled in my crown really well in the first 18 months. The past 6 months the hair on my frontal scalp has really started to lose density. I've Increased the dose to 1MG daily up from 0.5mg X In the dutasteride study it shown daily 0.5mg to decrease scalp DHT by around 50 percent, & daily 2.5mg to decrease scalp dht by around 80 percent. The results also shown the 2.5 mg to give moderately better results over the 0.5. But of course this then increased side effects, I'm not going to go crazy and take 2.5 daily but I'll stick to 1MG & hopefully gain stability, if not I'm throwing the towel in

To those thinking of reducing Finasteride or Dutastiride doses: I started with Finasteride 1mg every second day. No improvement over 6 months. Instead of increasing Finasteride, I started Dutastiride 0.5mg every second day. Also no improvement over 6 months. For past 4 months I've been taking the normal Dutastiride dose (0.5mg every day) and have seen a huge improvement.
How long were you on DUT for when you took it every second day? Why do you think taking it everyday has made a big difference when it's half life is so long?
I took Dut for six months every second day. I assume taking it every day works better because now I'm taking double as much as when I was taking it every second day?
Almost 4 years on daily Dutasteride. Hairloss totally stopped and the hair has thickened a bit. As for side effects, no decreased libido but very decreased amount of semen fluid. Sometimes there is nothing at all, other times it's normal. Same feeling though. I am adding 2.5mg daily oral Minox now. Have written this before I know. Just to say my anecdotal experience with Dut has been very good.

Have you ever considered a vid for people willing to spend and put on as many topicals for hair loss? Not nuclear but a safe and aggressive protocol. Like min, tret, fin, fluridil, stemoxydine etc. I'm sure a lot of people would love that. The prob is that topicals can hinder eachother in absorption
i was on fin 1mg daily for a year, it only slowed down my hair loss and then i switched to dutasteride 0.5 a few months ago and i went from almost norwood 3 to somewhere between norwood 1 and 2 with thick hairs

Because the first time i went to the docor for minoxidil and Finnasteride they refused to give it to me because my hair looked fine.

The best french youtuber has like 17k subscribers and he can get away with a video where he tells people that minoxidil has an "indirect DHT blocking effect". However, channels bragging about hair transplants are blowing surprisingly. I crossed your channel and MPMD thanks to the algorithm, and without you both I would still be on minoxidil only because in my country the lack of intel is very annoying. I know I am not the only French (or European) following you. Thanks for giving precious insight for free for the young generation. Sup Choom !!! Je suis aussi français et ton message est 100% vrai ! Kévin a littéralement sauvé mes cheveux grâce à la finasteride que je refusais de prendre pendant 9 ans .... J'ai commencé le minoxidil à 20 ans (ça m'a aidé mais n'a pas arrêté la chute des cheveux) le minoxidil a seulement ralenti mon alopécie. Il y a 3 ans j'ai fait une greffe de cheveux (1200 unités folliculaire) qui fut un succès. Je suis passé de Norwood 3 à norwood 2 grâce à la greffe il y a 2 ans. Et depuis 1 an j'ai ajouté finasteride à mon régime (j'ai jamais arrêté le minoxidil bien évidemment car cela fonctionnne) . Et aujourd'hui à 30 ans j'ai presque les mêmes cheveux que j'avais quand j'avais 18 ans !!! (J'ai juste mes golfs légèrement creusé). Donc merci Kévin ! Ps : la finasteride que je prend seulement à 0,5mg tous les deux jours a eu énormément d'effet et la petite surprise c'est que ma libido a augmenté ! Preem comment and happy to answer your questions choom. time of day doesn't really matter because finasteride has a short half life although it is probably best to take it at least a roughly similar time give or take a few hours. Both finasteride and dutasteride can be taken with or without food.
@@haircafekevin I think evening is best to optimise. Since the main testosterone boost is in the morning you'll decrease its conversion to dht. Practically speaking though it doesn't matter.

I don't know about other people taking duta, but I've found that taking it every third day doesn't seem to work for some reason. EOD on the other hand seems to work perfectly, it's the dose I got most of my regrowth at. I switched to every third day twice and both times I had shedding that stopped within a month and a half of returning to EOD.
Hey Kevin, med student here, I just finished my clinical rotation at the department of dermatology. A very knowledgable dermatologist in training told me that there is a shift in the consensus on what causes hairloss. Of course, androgens play a huge roll but he said that the thining of the skin of the scalp area, especially in the frontal and vertex area are what causes dht to miniaturize the hair follicle due to a yet to be elucidated mechanism. He said that this is further backed by research on hair transplantation data which shows that despite transplanting ''dht insensitive'' hairs to the frontal and vertex area, people still experience hairloss of these so called dht immuun hairs. Moreover, the fact that older people have less hair doenst make sense if DHT is the sole reason for hairloss since older people have lower test and therefor lower DHT levels than say a 25 year old male. Therefor, he advices to start with minoxidil in the case of young man and in conjuction adding finasteride. This also explains why some people lose hair despite being on fin for more than a year. What are your thoughts on this? I know you are always mocking the bloodflow theory, however this mechanism is different from the bloodflow theory as it pertains to scalp skin density and traction. Love your content by the way and I am seriously considering Fin due to your eloquent video's and your use of scientific research instead of anecdotal evidence like other YT'ers!
This scalp tension theory is another variation on the blood flow theory, it just doesn't involve blood flow anymore because that aspect has been debunked. I have read the arguments that there are some cases of failure of the "donor dominance" rule during hair transplant, but I think these occurrences are more likely due to problems with transplant technique, because in the vast majority of cases donor dominance is the rule, and hair follicles maintain their DHT sensitivity regardless of where they are transplanted to. As far as older people having more hair loss, androgenic alopecia is a chronic condition, and the more hair cycles that we go through the worse it gets. Testosterone levels don't fall that much in men with aging, and even women with much lower testosterone levels than men can get androgenic alopecia. The problem with androgenic alopecia is not too much testosterone, it is overactive 5AR enzymes and androgen receptor sensitivity in the affected hair follicles. There are a lot of things going on with aging, and almost every medical condition gets worse as we age, and hair loss is no exception. So I still don't buy that there is some "Factor X" related to scalp tension that is the cause of the balding pattern seen in androgenic alopecia, and I think even the dermatologist you quoted would admit that there is a large dose of speculation in these scalp tension theories. That being said, glad you like my videos, and I'm happy someone studying medicine still finds them of interest.

What about combining Finasteride and Dutasteride? I know several doctors who recommend something like 2-3 days a week of Dutasteride 0.5mg and the other days Finsteride 1mg. Personally, Dutasteride has worked better for me

@@arigold418 I am not a doctor, but if you want quick results I would go with a daily dose for a few months at least. Then maybe combine it with Finasteride it just take it fewer days per week. My guess is that 3 takes of dutasteride per week are slightly better than Finasteride every day.
I'm already on Finasteride 1mg/day since a little more than a year, but it doesn't seem to work that well for me as I now realize my hair started to recede from as early as 21 / 22( I'm 27 btw )...So, clearly I need intense DHT suppression therapy. So, will go for 0.5mg Dutasteride weekly now as my dermatologist said. P.S. Researching further if topical Finasteride is any significantly better than oral Finasteride, so that I can finally allocate the treatment for 5AR inhibitors! Also, I'm looking to persuade any experienced knowledgeable dermatalogist to start oral Minoxidil for the best results, as I don't want to start without under the guidance and observation of a dermatologist

Have a question. If scalp dht is inversely correlated with hair counts, not to say it’s not relevant, why is there less of an increase at 24 weeks vs 12 weeks for 2.5 compared to 0.5. Is it possible that scalp dht reducing to just 50% can have almost the same effect as a 79% reduction in scalp dht over a few years since the dht in the hair follicles would be close to 0% for both ? Trying to understand as I posted a theory on your most recent video Just thinking there might be no extra improvement in a longer term window as opposed to the first year

I have successfully reversed my perfect hairline to get MPB. My regiment includes: 1- Broccoli sprouts (5x a day; boiled in Zinc water) 2- Onion Juice (twice a day; apply just before you go on a date and see the tears in the eyes of your loved one) 3- Massaging scalp for greater BludFlow and finishing it off with Red light theraphy.
Kevin, I'm taking 3 times a week dutasteride 0.5 mg, and i'm seeing lots of results, i've been trying to find a study comparing 0.5 mg DUT 3 times a week vs 1mg of finasteride every day.... I'm pretty sure is still much better DUT 3 times a week vs everyday FIN.
None of the studies looked at every other day dosing but the effects on the tissue are long enough for both drugs to be effective when used every day. 0.5 MG of dutasteride of three times per week is probably more potent than even daily finasteride use based on the existing research and because of duasteride's longer half life. They're both great drugs though, totally adequate for treating hair loss.

@@lud6441 Is not worth it for me, 3 times a week is perfect, but i've seen doctors prescribing it once a week. I've recovered lots of hairline with the 3 times a week schedule, but I also use stemoxydine/alfatradiol at daytime and minoxidil/tretinoin at night. This is a war that you must fight from all the possible angles.
The nuance is on point I disagree that you start weak. The goal of treatment is to regrow as much hair as possible. Once you lose it, it’s very hard to resurrect follicles. The goal is to start as early as possible and use the nuclear option. You’ve proven that finasteride at 1mg and dutasteride at 2.5mg has no meaningful difference in side effects, but meaningful difference in efficacy.
Great video. I’ve just added another day of dut to my stack. Fin 1mg on Mon Tue Thurs & Fri. With Dut 1mg on sat, 0.5mg on sun & Wed. Carry that on for another month then add dut another day. Would a month be ok?
Maybe androgenetic alopecia is an evolutionary group-based selection advantage of some kind!? For example, it may have evolved to give young men a fighting chance on the "dating & procreation market"! Think about it: Older accomplished men with a full head of hair would have had an unfair advantage over young men, and androgenetic alopecia proved to be beneficial to our species as a group, maybe in terms of procreation quality or something!? I also liked that study which suggested that because androgenetic alopecia increases the area of skin exposed to sunlight, male pattern baldness may have evolved to increase the production of vitamin D...
I've been on fin for 4 months. I dont see any improvement, however I haven't lost any ground, so the med seems to be working. Its tempting to switch for dutasteride asap, as we know time is of the essence. Kevin, you seem to be more convinced now noticing the might of the drug although I know you dont recommend it because of ethical purposes. Greetings.
It's really up to you. In South Korea and Japan Dutasteride is approved for AGA as the standard treatment. Dut is more effective, but also more expensive.
Sorry if this is a redundant question would you say if someone has used fin 1mg for 2 years and losing ground should they add dut 0.5mg twice a week or a higher dose or higher frequency

I know you said it can take up to 2 years for Finasteride to work. But when would you say is the best time to consider switching to Dutasteride? And if I switch should I already start with 0,5 mg daily or with a smaller dose. How would you handle it?
Yo bet! Since you are kind of getting into the game late I would say it is fine to start dutasteride right now, 0.5 mg daily. If after 6 months you are still losing ground then I'd consider 2.5 mg of dut.
You should also draw attention to the fact that literally more people reported impotance on placebo than on 5mg of fin and 2.5mg of dutasteride. It is kinda funny because it only shows how your mind is in many cases the only reason for side effects. If you take it with the possible side effects in your mind you are much more likely to get them.

I’ve read a lot of anecdotal evidence of it causing thinning for a lot people, but I’m not sure of what mechanism that would be through. If it’s not caused through increasing dht, then taking dut or fin wouldn’t cancel out any Kratom related hair loss I would assume. Are the people claiming Kratom causes hair loss just experiencing regular age related androgenic alopecia and just blaming the Kratom? You bet! The Kratom subject is extremely mixed in terms of research. I'll probably get to it eventually but right now its on the back burner. Definitely an interesting subject though.
I've heard anecdotal stories about kratom causing hair loss. I haven't seen much in the way of actual medical literature, but I've read enough anecdotal reports to think there might be something to it. It likely wouldn't be due to DHT, as it appears to occur in random patches, as opposed to the typical course of MPB. On a side note: please don't take kratom recreationally. I can't tell you how many people I've talked to who thought it was a safe, non-addictive herb, & ended up having a h3ll of a time trying to come off of it, replete with severe physical & mental withdrawals. It's very pernicious in that it could be a while before you realize anything is wrong, but it does eventually sneak up on a person if it's used daily over the long-term. People will often dogpile you if you speak negatively about it in any way, but the reality is that kratom is essentially an opioid in how it affects our brains, & thus, is habit-forming in much the same way that traditional opioids/opiates are.
Hello Kevin, I’ve been taking finasteride for 2 years 3 times a week and switched over to Dutasteride 3 times a week for almost a year now after getting a second hair transplant. I still have diffuse thinning around my crown area. My hair transplant doctor told me before to take Dutasteride daily for a while first then do 3 times a week later on. I did not follow his advice. Would taking it daily now make a difference or would I have reached the same level of efficacy now that its built up in my body for over almost a year of taking it 3 times a week?

finasteride has thickened the fuck out of my hair and I've seen amazing results on it 4 months in but im only 24 so I imagine finasteride 1.25 mg for the next 6-8 years will be enough and then when im 32ish years old I will add dutasreride 3x per week if I need it. but right now I thank God that I found your videos and got on finasteride back in July before I lost more ground.

Yes, this is fairly common. It works, a good choice for people who can't tolerate daily dutasteride use but don't get sides on finasteride. I talk about it in the video.
Finasteride 1mg is not available in my country so I started taking dutasteride 0.5mg (Mon, Wed, Fri). And minoxidil 10% once a day before going to sleep + Biotin 10mg. I am losing hair all over my head not in a MPB pattern. Dont know if something is wrong with me or it's just DHT. Even side hairs which are near the 'ear' area are also very thin. Hopefully the drug will work.

No, I went over this in the video. Testosterone doesn't cause hair loss at a normal physiological level. The studies showed greater hair count growths with dutasteride even though the testosterone was higher.
You may be aware of this. The history of Finasteride/AGA. Merck developed the drug because of the discovery that people with 5AR2D were never affected by BPH -/AGA. They have the same T level production and, (don't "miss") with the 5AR2D (T to DHT conversion deficiency) - significantly higher "scalp T", but again, they never bald.
So for a bodybuilder who cycles/blasts 500mg testosterone enanthate every 3 months then cruises on 150mg test for the other 3 months. Would you recommend dutastaride or finasteride, I am currently on finasteride/Ru58441 and minox/stemoxidyn, I’m thinking to cut more 5A/DHT conversion as I’m blasting test by using Dutastaride and using the RU for the free test in scalp…. What do you think Kevin?
Since 2.5mg dut is superior to 0.5mg dut, one could say that the lower the scalp DHT, the better. I would therefore be curious to see the effects of using 0.5mg dut orally, while also applying a tiny bit of topical finasteride to just the most stubborn zone.
If you can get dutasteride powder you can make your own topical with the concentration you want. In theory topical dut should give you proportionally less systemic dht inhibition to scalp dht inhibition, compared to oral dut. Topical delivery method is superior but also more of a hassle, still worth it imo.

Yo! No, hats cannot cause hair loss. That is unless you wear them really tightly in which case you may give yourself traction alopecia but that should reverse on its own. I may cover alfatradiol again although there isn't a huge amount of data on it. It is an adjunct treatment with mild effectiveness but certainly not a replacement for finasteride.
Does blocking dht reduces collagen synthesis on skin? Or is it possibly opposite of that? One dude commented in another man's RUclips video- "Our models show that the androgen receptor is responsible for 50% of the collagen content in male skin." Fin destroys androgen receptors along with lowering serum DHT, the main hormone that attaches to androgen receptors to synthesize proteins.".. And this guy thinks fenesterid makes the skin worst by lowering collagen.. hey man do have any elaboration/ remarks regarding this?
Let me correct some of this misinformation. Finasteride does not work by destroying androgen receptors, it works by blocking the 5AR enzyme and thus lowering DHT production. So androgen receptors are still there in the skin and are still sensitive to testosterone, the main circulating male hormone. So this whole argument is based on a fallacy that finasteride destroys androgenic receptors, which isn't true. Over all in the body, the main androgen is testosterone and not DHT. DHT affects the prostate, giving men prostatic hypertrophy, the sebaceous glands, contributing to acne, and the hair follicles, given susceptible men baldness. That's why it is a trash hormone after adolescence. There's no evidence taking finasteride makes your skin worse by any effects on collagen only seen in a test-tube.

Hi Kevin. I'm glad that you have made a video about dutasteride. It's such a shame thought that only one country approved this drug to be officialy used to treat AGA. By the way, you said that dutasteride dosn't have generics, but in the country where I live there are generic versions of Avodart and they cost the same as generic finasteride. Oh, and I have some questions: 1. Do you think that dutasteride will be FDA approved for treating AGA in the near future? 2. Why do you think there are many doctors/dermatologists that fearmonger about finasteride/dutasteride? In the country where I live (especially in the eastern part of it, but this occurs very frequently in other parts too) many (if not even most) dermatologist talk how bad finasteride, dutasteride or even minoxidil are or do not even know that they are used for treating AGA. Additionally, they think that some vitamins, shampoos and herbal-based topicals are effective ,,treatment" for hairloss. I mean, shouldn't they be taught about how AGA works and what is effective in treating it and that fin, dut, min and such have a high safety profile? Because now it looks like they receive their ,,knowledge" about AGA from - I dunno - some witch doctors/shamans, sites like propecia help or just some gossips. 3. Can minoxidil be used less frequently after it succesfully regrow hair (and after about - lets's say - one year) just to maintain the results? For example once or twice in a week?
I agree it's a shame it has not been officially approved for AGA, but it doesn't matter too much, since it is available and can be used off label. I don't think anyone will bother to apply for FDA approval now that it is off patent. Regarding fear-mongering, I haven't seen that much among doctors here in the US, but it must vary in different countries and I don't really know why. Maybe the medical educational system is not as good in some countries? I don't know. There are a lot of "alternative medicine" providers here, but they are usually not medical doctors. They are chiropractors or naturopaths, though admittedly there are some high profile real doctors who are quacks like Dr. Oz that make money selling supplements. Regarding minoxidil, most people seem to lose ground if they stop it, so I think you can possibly taper it a little, but once or twice a week may not be enough to maintain its efficacy.
Dutasteride is OTC at any pharmacy where I live. Approved specifically for hairloss or not, doesn't matter much. Maybe it's a good thing, as demand would rise with awareness, increasing prices.

Would be interesting to see if the weather (heat and moisture) would have an impact on the efficacy of dutasteride pills, for people living or travelling in tropical areas for example, as they write it should be stored in a 15°-30°C (59-86°F) range. Sup kevin , i am 19 and i have been slowly thinning since 16 or so and by my 18th birthday my hair was significantly thin so i went to a doctor and started fin and minox , after 8 months i literally grew all my hair back but i started to get cardiovascular sides from the minox so i stopped and the side effects went away. I stopped the minox 3 months ago and i am shedding like crazy despite being on finasteride daily and dut once a week , i lost hair on the temples but i am still much better off than when i first started , do you think this shed is temporary?
It could be a shedding cycle, but could also be some loss from going off minoxidil. Did you have cardiovascular sides on topical minoxidil? Or was it oral minoxidil? The latter is more likely to cause sides. If you were on oral minoxidil and got sides I would try topical if it is ok with your doctor. If it was topical minoxidil, maybe your doctor would allow you to try 2% minoxidil? Whatever you do, make sure your doctor approves because cardiovascular sides are nothing to mess around with.
@@haircafekevin yeah it was the topical minoxidil , the oral version is a big no no for me, i will be going back to the doctor and i will consult him if it is ok to use 2 percent. Cardiovascular health is very serious which is why i was scared when i felt the sides.
No, you are confusing serum DHT with scalp DHT. 0.5 mg of dutasteride suppresses about 50 percent scalp dht. You can try using it once weekly with finasteride on the other days to get an extra bit of dht suppression.

From what I remember, Merck tested a selective AR-1 inhibitor called MK-386 on stump-tailed macaques and found that it was ineffective against hair loss. So, yeah, it's mostly dutasteride's superior inhibition of AR-2 that gives it additional strength over finasteride.

Hi, what is your dosage? I just did my first dosage today 0.5mg dutasteride, doc also prescribed me 0.5mg daily. Never been on fin , 1,5-2 Norwood and some hairline thinning. But head full of big curly hair. Can't say I'm agressivly balding.

0:51 A minor correction here: Hypertrophy is not the same as hyperplasia. In hyperplasia, there is greater cell proliferation than normal. In hypertrophy, individual cells grow in size (this is what happens when the liver "regenerates"). In BPH we have hyperplasia, not hypertrophy.

I've been on dutasteride for 2 years, it filled in my crown really well in the first 18 months. The past 6 months the hair on my frontal scalp has really started to lose density. I've Increased the dose to 1MG daily up from 0.5mg

In the dutasteride study it shown daily 0.5mg to decrease scalp DHT by around 50 percent, & daily 2.5mg to decrease scalp dht by around 80 percent. The results also shown the 2.5 mg to give moderately better results over the 0.5. But of course this then increased side effects, I'm not going to go crazy and take 2.5 daily but I'll stick to 1MG & hopefully gain stability, if not I'm throwing the towel in

To those thinking of reducing Finasteride or Dutastiride doses: I started with Finasteride 1mg every second day. No improvement over 6 months. Instead of increasing Finasteride, I started Dutastiride 0.5mg every second day. Also no improvement over 6 months. For past 4 months I've been taking the normal Dutastiride dose (0.5mg every day) and have seen a huge improvement.
How long were you on DUT for when you took it every second day? Why do you think taking it everyday has made a big difference when it's half life is so long?
I took Dut for six months every second day. I assume taking it every day works better because now I'm taking double as much as when I was taking it every second day?

Almost 4 years on daily Dutasteride. Hairloss totally stopped and the hair has thickened a bit. As for side effects, no decreased libido but very decreased amount of semen fluid. Sometimes there is nothing at all, other times it's normal. Same feeling though. I am adding 2.5mg daily oral Minox now. Have written this before I know. Just to say my anecdotal experience with Dut has been very good.

J'ai commencé le minoxidil à 20 ans (ça m'a aidé mais n'a pas arrêté la chute des cheveux) le minoxidil a seulement ralenti mon alopécie. Il y a 3 ans j'ai fait une greffe de cheveux (1200 unités folliculaire) qui fut un succès. Je suis passé de Norwood 3 à norwood 2 grâce à la greffe il y a 2 ans.
Et depuis 1 an j'ai ajouté finasteride à mon régime (j'ai jamais arrêté le minoxidil bien évidemment car cela fonctionnne) . Et aujourd'hui à 30 ans j'ai presque les mêmes cheveux que j'avais quand j'avais 18 ans !!! (J'ai juste mes golfs légèrement creusé).

time of day doesn't really matter because finasteride has a short half life although it is probably best to take it at least a roughly similar time give or take a few hours.
Both finasteride and dutasteride can be taken with or without food.

I don't know about other people taking duta, but I've found that taking it every third day doesn't seem to work for some reason.
EOD on the other hand seems to work perfectly, it's the dose I got most of my regrowth at. I switched to every third day twice and both times I had shedding that stopped within a month and a half of returning to EOD.
Hey Kevin, med student here, I just finished my clinical rotation at the department of dermatology. A very knowledgable dermatologist in training told me that there is a shift in the consensus on what causes hairloss. Of course, androgens play a huge roll but he said that the thining of the skin of the scalp area, especially in the frontal and vertex area are what causes dht to miniaturize the hair follicle due to a yet to be elucidated mechanism. He said that this is further backed by research on hair transplantation data which shows that despite transplanting ''dht insensitive'' hairs to the frontal and vertex area, people still experience hairloss of these so called dht immuun hairs. Moreover, the fact that older people have less hair doenst make sense if DHT is the sole reason for hairloss since older people have lower test and therefor lower DHT levels than say a 25 year old male. Therefor, he advices to start with minoxidil in the case of young man and in conjuction adding finasteride. This also explains why some people lose hair despite being on fin for more than a year. What are your thoughts on this? I know you are always mocking the bloodflow theory, however this mechanism is different from the bloodflow theory as it pertains to scalp skin density and traction. Love your content by the way and I am seriously considering Fin due to your eloquent video's and your use of scientific research instead of anecdotal evidence like other YT'ers!
This scalp tension theory is another variation on the blood flow theory, it just doesn't involve blood flow anymore because that aspect has been debunked. I have read the arguments that there are some cases of failure of the "donor dominance" rule during hair transplant, but I think these occurrences are more likely due to problems with transplant technique, because in the vast majority of cases donor dominance is the rule, and hair follicles maintain their DHT sensitivity regardless of where they are transplanted to. As far as older people having more hair loss, androgenic alopecia is a chronic condition, and the more hair cycles that we go through the worse it gets. Testosterone levels don't fall that much in men with aging, and even women with much lower testosterone levels than men can get androgenic alopecia. The problem with androgenic alopecia is not too much testosterone, it is overactive 5AR enzymes and androgen receptor sensitivity in the affected hair follicles. There are a lot of things going on with aging, and almost every medical condition gets worse as we age, and hair loss is no exception. So I still don't buy that there is some "Factor X" related to scalp tension that is the cause of the balding pattern seen in androgenic alopecia, and I think even the dermatologist you quoted would admit that there is a large dose of speculation in these scalp tension theories.

What about combining Finasteride and Dutasteride? I know several doctors who recommend something like 2-3 days a week of Dutasteride 0.5mg and the other days Finsteride 1mg.

@@arigold418 I am not a doctor, but if you want quick results I would go with a daily dose for a few months at least. Then maybe combine it with Finasteride it just take it fewer days per week. My guess is that 3 takes of dutasteride per week are slightly better than Finasteride every day.
I'm already on Finasteride 1mg/day since a little more than a year, but it doesn't seem to work that well for me as I now realize my hair started to recede from as early as 21 / 22( I'm 27 btw )...So, clearly I need intense DHT suppression therapy. So, will go for 0.5mg Dutasteride weekly now as my dermatologist said.
P.S. Researching further if topical Finasteride is any significantly better than oral Finasteride, so that I can finally allocate the treatment for 5AR inhibitors!
Also, I'm looking to persuade any experienced knowledgeable dermatalogist to start oral Minoxidil for the best results, as I don't want to start without under the guidance and observation of a dermatologist
Dut is a good choice but remember it can take a while to see results, sometimes over a year.

Have a question. If scalp dht is inversely correlated with hair counts, not to say it’s not relevant, why is there less of an increase at 24 weeks vs 12 weeks for 2.5 compared to 0.5. Is it possible that scalp dht reducing to just 50% can have almost the same effect as a 79% reduction in scalp dht over a few years since the dht in the hair follicles would be close to 0% for both ? Trying to understand as I posted a theory on your most recent video

I have successfully reversed my perfect hairline to get MPB. My regiment includes:
1- Broccoli sprouts (5x a day; boiled in Zinc water)
2- Onion Juice (twice a day; apply just before you go on a date and see the tears in the eyes of your loved one)
3- Massaging scalp for greater BludFlow and finishing it off with Red light theraphy.
If only I used this protocol instead of finasteride I could've been a Norwood 0 by now.

Kevin, I'm taking 3 times a week dutasteride 0.5 mg, and i'm seeing lots of results, i've been trying to find a study comparing 0.5 mg DUT 3 times a week vs 1mg of finasteride every day.... I'm pretty sure is still much better DUT 3 times a week vs everyday FIN.
None of the studies looked at every other day dosing but the effects on the tissue are long enough for both drugs to be effective when used every day. 0.5 MG of dutasteride of three times per week is probably more potent than even daily finasteride use based on the existing research and because of duasteride's longer half life.
They're both great drugs though, totally adequate for treating hair loss.

I've recovered lots of hairline with the 3 times a week schedule, but I also use stemoxydine/alfatradiol at daytime and minoxidil/tretinoin at night. This is a war that you must fight from all the possible angles.

I disagree that you start weak. The goal of treatment is to regrow as much hair as possible. Once you lose it, it’s very hard to resurrect follicles. The goal is to start as early as possible and use the nuclear option. You’ve proven that finasteride at 1mg and dutasteride at 2.5mg has no meaningful difference in side effects, but meaningful difference in efficacy.

For example, it may have evolved to give young men a fighting chance on the "dating & procreation market"! I've been on fin for 4 months. I dont see any improvement, however I haven't lost any ground, so the med seems to be working. Its tempting to switch for dutasteride asap, as we know time is of the essence. It's really up to you. In South Korea and Japan Dutasteride is approved for AGA as the standard treatment. Dut is more effective, but also more expensive.

Either will work. Starting less frequently might be a good way to dip your toes into extra dht inhibition without changing things too much.

Yo bet! Since you are kind of getting into the game late I would say it is fine to start dutasteride right now, 0.5 mg daily. If after 6 months you are still losing ground then I'd consider 2.5 mg of dut.

You should also draw attention to the fact that literally more people reported impotance on placebo than on 5mg of fin and 2.5mg of dutasteride. It is kinda funny because it only shows how your mind is in many cases the only reason for side effects. If you take it with the possible side effects in your mind you are much more likely to get them.

Glad to have you here choom! Nah, you don't have to be that strict. Finasteride has a short half life, only 5-8 hours so just take it daily at roughly the same time give or take a few hours.

You bet! The Kratom subject is extremely mixed in terms of research. I'll probably get to it eventually but right now its on the back burner. Definitely an interesting subject though.
I've heard anecdotal stories about kratom causing hair loss. I haven't seen much in the way of actual medical literature, but I've read enough anecdotal reports to think there might be something to it. It likely wouldn't be due to DHT, as it appears to occur in random patches, as opposed to the typical course of MPB.
On a side note: please don't take kratom recreationally. I can't tell you how many people I've talked to who thought it was a safe, non-addictive herb, & ended up having a h3ll of a time trying to come off of it, replete with severe physical & mental withdrawals. It's very pernicious in that it could be a while before you realize anything is wrong, but it does eventually sneak up on a person if it's used daily over the long-term. People will often dogpile you if you speak negatively about it in any way, but the reality is that kratom is essentially an opioid in how it affects our brains, & thus, is habit-forming in much the same way that traditional opioids/opiates are.
Hello Kevin, I’ve been taking finasteride for 2 years 3 times a week and switched over to Dutasteride 3 times a week for almost a year now after getting a second hair transplant. I still have diffuse thinning around my crown area. My hair transplant doctor told me before to take Dutasteride daily for a while first then do 3 times a week later on. I did not follow his advice. Would taking it daily now make a difference or would I have reached the same level of efficacy now that its built up in my body for over almost a year of taking it 3 times a week?

finasteride has thickened the fuck out of my hair and I've seen amazing results on it 4 months in but im only 24 so I imagine finasteride 1.25 mg for the next 6-8 years will be enough and then when im 32ish years old I will add dutasreride 3x per week if I need it. but right now I thank God that I found your videos and got on finasteride back in July before I lost more ground.

Thank you, yes I have used it and I got comparable results to finasteride. Only difference is dutasteride gave me an increased libido which was annoying. I dropped it years ago.

Yes, this is fairly common. It works, a good choice for people who can't tolerate daily dutasteride use but don't get sides on finasteride. I talk about it in the video.

Finasteride 1mg is not available in my country so I started taking dutasteride 0.5mg (Mon, Wed, Fri). And minoxidil 10% once a day before going to sleep + Biotin 10mg. I am losing hair all over my head not in a MPB pattern. Dont know if something is wrong with me or it's just DHT. Even side hairs which are near the 'ear' area are also very thin. Hopefully the drug will work.
Diffuse thinning, probably chronic telogen affluvium. Finasteride has helped mine.
I also have thinning around my ears and sides, have no ducking clue what it is though

No, I went over this in the video. Testosterone doesn't cause hair loss at a normal physiological level. The studies showed greater hair count growths with dutasteride even though the testosterone was higher.

You may be aware of this. The history of Finasteride/AGA. Merck developed the drug because of the discovery that people with 5AR2D were never affected by BPH -/AGA.
They have the same T level production and, (don't "miss") with the 5AR2D (T to DHT conversion deficiency) - significantly higher "scalp T", but again, they never bald.
So for a bodybuilder who cycles/blasts 500mg testosterone enanthate every 3 months then cruises on 150mg test for the other 3 months. Would you recommend dutastaride or finasteride,

Since 2.5mg dut is superior to 0.5mg dut, one could say that the lower the scalp DHT, the better.
I would therefore be curious to see the effects of using 0.5mg dut orally, while also applying a tiny bit of topical finasteride to just the most stubborn zone.
If you can get dutasteride powder you can make your own topical with the concentration you want. In theory topical dut should give you proportionally less systemic dht inhibition to scalp dht inhibition, compared to oral dut. Topical delivery method is superior but also more of a hassle, still worth it imo.

Yo! No, hats cannot cause hair loss. That is unless you wear them really tightly in which case you may give yourself traction alopecia but that should reverse on its own. I may cover alfatradiol again although there isn't a huge amount of data on it. It is an adjunct treatment with mild effectiveness but certainly not a replacement for finasteride.

Let me correct some of this misinformation. Finasteride does not work by destroying androgen receptors, it works by blocking the 5AR enzyme and thus lowering DHT production. So androgen receptors are still there in the skin and are still sensitive to testosterone, the main circulating male hormone. So this whole argument is based on a fallacy that finasteride destroys androgenic receptors, which isn't true. Over all in the body, the main androgen is testosterone and not DHT. DHT affects the prostate, giving men prostatic hypertrophy, the sebaceous glands, contributing to acne, and the hair follicles, given susceptible men baldness. That's why it is a trash hormone after adolescence. There's no evidence taking finasteride makes your skin worse by any effects on collagen only seen in a test-tube.

Hi Kevin. I'm glad that you have made a video about dutasteride. It's such a shame thought that only one country approved this drug to be officialy used to treat AGA. By the way, you said that dutasteride dosn't have generics, but in the country where I live there are generic versions of Avodart and they cost the same as generic finasteride. Oh, and I have some questions:
1. Do you think that dutasteride will be FDA approved for treating AGA in the near future?
2. Why do you think there are many doctors/dermatologists that fearmonger about finasteride/dutasteride? In the country where I live (especially in the eastern part of it, but this occurs very frequently in other parts too) many (if not even most) dermatologist talk how bad finasteride, dutasteride or even minoxidil are or do not even know that they are used for treating AGA. Additionally, they think that some vitamins, shampoos and herbal-based topicals are effective ,,treatment" for hairloss. I mean, shouldn't they be taught about how AGA works and what is effective in treating it and that fin, dut, min and such have a high safety profile? Because now it looks like they receive their ,,knowledge" about AGA from - I dunno - some witch doctors/shamans, sites like propecia help or just some gossips.
3. Can minoxidil be used less frequently after it succesfully regrow hair (and after about - lets's say - one year) just to maintain the results? For example once or twice in a week?
I agree it's a shame it has not been officially approved for AGA, but it doesn't matter too much, since it is available and can be used off label. I don't think anyone will bother to apply for FDA approval now that it is off patent. Regarding fear-mongering, I haven't seen that much among doctors here in the US, but it must vary in different countries and I don't really know why. Maybe the medical educational system is not as good in some countries? I don't know. There are a lot of "alternative medicine" providers here, but they are usually not medical doctors. They are chiropractors or naturopaths, though admittedly there are some high profile real doctors who are quacks like Dr. Oz that make money selling supplements.
Regarding minoxidil, most people seem to lose ground if they stop it, so I think you can possibly taper it a little, but once or twice a week may not be enough to maintain its efficacy.
Dutasteride is OTC at any pharmacy where I live. Approved specifically for hairloss or not, doesn't matter much. Maybe it's a good thing, as demand would rise with awareness, increasing prices.

Would be interesting to see if the weather (heat and moisture) would have an impact on the efficacy of dutasteride pills, for people living or travelling in tropical areas for example, as they write it should be stored in a 15°-30°C (59-86°F) range. Sup kevin , i am 19 and i have been slowly thinning since 16 or so and by my 18th birthday my hair was significantly thin so i went to a doctor and started fin and minox , after 8 months i literally grew all my hair back but i started to get cardiovascular sides from the minox so i stopped and the side effects went away. I stopped the minox 3 months ago and i am shedding like crazy despite being on finasteride daily and dut once a week , i lost hair on the temples but i am still much better off than when i first started , do you think this shed is temporary?

It could be a shedding cycle, but could also be some loss from going off minoxidil. Did you have cardiovascular sides on topical minoxidil? Or was it oral minoxidil? The latter is more likely to cause sides. If you were on oral minoxidil and got sides I would try topical if it is ok with your doctor. If it was topical minoxidil, maybe your doctor would allow you to try 2% minoxidil? Whatever you do, make sure your doctor approves because cardiovascular sides are nothing to mess around with.
@@haircafekevin yeah it was the topical minoxidil , the oral version is a big no no for me, i will be going back to the doctor and i will consult him if it is ok to use 2 percent. Cardiovascular health is very serious which is why i was scared when i felt the sides.

No, you are confusing serum DHT with scalp DHT. 0.5 mg of dutasteride suppresses about 50 percent scalp dht. You can try using it once weekly with finasteride on the other days to get an extra bit of dht suppression.
