I'm pretty sure it goes both ways. The whole thing is a fear gas hallucination representing Batman's deepest fears. Joker represents Batman after losing control, giving in to his desires, and crossing the line he swore to never cross and thus becoming no better than the villains he's typically up against. The villains represent his friends witnessing him becoming this way, losing their trust in him, and ultimately losing their lives to him. To put it in simple terms, it's a psychological manifestation of Batman's worst case scenario. Particularly noteworthy is that this hallucination doesn't involve Batman being unmasked, which was literally something that was happening in the moment. Batman isn't afraid of being exposed as Bruce Wayne, he's afraid of Batman being exposed as a monster.
