It shows how he's being kept alive, (for some reason) so that he can go through HELL on earth because the government won't put him out of his misery. It's both disgusting and telling of how worthless all of our leaders are. He should've been killed for mercy, but no... they kept him alive.
It's mentioned in the bits used in the song, the doctor believes that he is brain dead and cannot feel. When they find out he's aware they keep him alive out of a misguided moral sensibility.
I watched it with my WW2 Army infantry Grandfather when I was 14 and that was the night he told me about everything he saw about the Liberation of Buchenwald. I will always carry a feeling of honor that my Grandfather felt for whatever reason he could talk to someone about what he saw.

I can't help but notice the endless subscribe animations throughout the video. Subscribe for what ? To see less than 2 minute of low effort "reaction" for one of the most monumental songs + videos ? They should at least admit it that they're really not into this. It's a waste of everyone's time. I saw Metallica live in Denver back in '92. When they performed this song, there was a huge TV screen on each side of the stage playing the movie parts of the video; so awesome!
