Complete your 40 days mandala you will automatically do it correctly at 40th day. Initially you will not be able to do it perfectly as she is saying complete your mandala you will automatically notice the difference. I complete my mandala now I can do exactly how she is performing. so no worries keep practicing.

Viprita Shwasa is supposed to happen at the tip of your nose only, your breath should flutter just at the tip if your nose (not abdomen or chest). But if it isn't happening, don't worry, keep practicing. I'm doing Shambhavi Mahamudra for a while now but still Viprita Shwasa doesn't happen properly. I hope it goes right .

Try to keep it to the tip of the nose. Initially you will feel it in abdomen, then after practicing few months, you will start feeling it in chest. Then after some more practice you will get it on the tip of the nose. Practice is the key.

You don't really have to put it in a sequence. It can be done by itself. But if you are practicing all four pranayamas everyday then after would be good. As this will easily take you towards the meditative state and you can meditate better after this.

From these five elements (components) we have animals, plants and humans, and then from these only we have some great saints and also some thieves. Same way Shambhavi Mahamudra is also created by great people from the components of yoga.
