  • We got to sit down with Dr. Bret Scher again to discuss what the current landscape of research around using the ketogenic diet in the treatment of psychiatric disorders looks like. Dr. Scher is a metabolic health specialist and is also the Director of Metabolic Mind, a nonprofit transforming the study and treatment of mental disorders by exploring the connection between metabolism and brain health.     To anyone out there struggling with medications side effects: do not be afraid to advocate for yourself and explore different options. As a scientist, I agree that there’s a spectrum of evidence one can draw from beyond randomized clinical trials. I myself asked my psychiatrist to try a different medication for my OCD (one with much lower levels of evidence) because of intolerable side effects. It worked and continues to work great.

      I agree that practicing self-advocacy is a good idea and I’m glad you found a med that works for you.
      With that said, it can be somewhat harder for people with psychotic illnesses to be heard and believed by mental health practitioners, even when presenting a tight, well-reasoned argument.
      That’s not to say it’s not possible to initiate change, but it’s an additional barrier when you’ve been labeled an unreliable narrator by the medical system.

      It's not enough to prove it's a long term "cure" but it's enough to see if patients react in a positive way. But there are individuals who've been followed for as long as 3-5 years and were all doing great and in the 5 year case I read about the women stopped taking her prescription meds after 3 years, and 2 years later she told her doctors she was basically "cured" and didn't need check in with them any longer.
      From what I've read the keto diet changes the gut microbiology. It basically feeds the good gut bugs and starves or kills off the bad ones. There are like millions of kinds of gut bugs, but one study I read said they found at least 5-6 specific bugs that all of those they tested who suffered with psychosis had in common and the keto diet got rid of those.
      I think the toughest part of this treatment is getting patients to start and stick with the diet. That takes some effort on their part and sugar probably promotes the bad gut bugs, and that can be addictive.
      Truth is, what we don't know about both psychosis and gut bugs far exceeds what we do know. But we now do know there is a connection between our gut bugs and our brain and psychosis. And that's a pretty huge leap ahead from where we were even just 5 years ago and the keto diet has proved that.

    I'm doing the Keto diet and exercise for my Schizoaffective disorder. It has helped tremendously. The only issue I'm still having is my sleep. It's all over the place. I'm up late, have broken sleep and tend to have a hard time getting up in the morning hours. Other than that, I'm improving. Really grateful for doctors like Dr. Scher who are studying this treatment.

      I’m starting keto too for my schizophrenia. I’ve noticed improvements too, but like you I feel it still takes me a while to wake up in the morning. Is it medication related? Once I wake up, I feel good the rest of the day.

      @caitlinhoey841 I have always had sleep issues, even when not on meds. But they probably don't help. I was hoping that Keto would help with my sleep issues too.

      Hi!! Could you share how you are facing the side effects of the medicine like being super hungry with the keto diet? That is one of the biggest issues I have with my sza partner. Do you also do intermittent fasting? I'm happy to hear you are much better.

    Would be great to have a residence clinic that had all these practices in place where one could stay.
    Especially for those mentally ill who are on the streets desperately ill or those who are not able to manage this therapy on their own.
    And it would seem that would provide amazing data for research because it could be an in-house research project. All details followed carefully by medical professionals using this therapy.

      A doctor in France was able to do just that with 22 or 24 of his long-time, treatment resistant patients. High percentage of really nice improvement, including reducing dosage of meds and eliminating others. If you can’t handle keto right now, try going all carnivore. BBBE - Beef, Bacon, Butter and Eggs.

    Hi Lauren, I use the Keto diet along with intermittent fasting (18/6) to control my epilepsy and I am 2 years seizure free. As I am still on meds I can't say for certain it's the diet, but at the same time, I can't see it being a coincidence that I am 2 years seizure free. I'm no doctor just my 2 cents. I hope you're good.

    I really appreciate you delving deeply into this. My brain seems to feel better to me with protein and saturated fats, and, being elderly, I have to monitor my heart health numbers and artery/plaque conditions. Thank you for doing the work, and also for doing such a high quality of presentatation of it.

      These are the criteria for metabolic syndrome. Get these in order and you will be well on your way there. Keto is quite good for all of them:
      Abdominal obesity. This means having a waist circumference of more than 35 inches for women and more than 40 inches for men. An increased waist circumference is the form of obesity most strongly tied to metabolic syndrome.
      High blood pressure of 130/80 mm Hg (millimeters of mercury) or higher. Normal blood pressure is defined as less than 120 mm Hg for systolic pressure (the top number), and less than 80 mm Hg for diastolic pressure (the bottom number). High blood pressure is strongly tied to obesity. It is often found in people with insulin resistance.
      Impaired fasting blood glucose. This means a level equal to or greater than 100 mg/dL
      High triglyceride levels of more than 150 mg/dL. Triglycerides are a type of fat in the blood.
      Low HDL (good) cholesterol. Less than 40 mg/dL for men and less than 50 mg/dL for women is considered low.

It's just that it may help whose with metabolic dysfunction even more. Hope that helps!

    Wow people really are struggling with this keto trial I can’t believe how much the comments have turned negative or skeptical and how many less comments there are from people who have schizophrenia spectrum illnesses saying they feel supported by your videos. I hope people with schizophrenia that so valued this channel aren’t feeling alienated by the content. I feel like that but I thought it was just me it feels like Lauren has left us behind and gone off on a very unique and unreachable treatment method abd lifestyle for many of us right now in the midst of psychotic illness and it makes sense but it also sucks

    Would it be possible for you to talk to someone who doesn't have ties to keto to explore the possible risks and give a more balanced perspective? Whilst the experts you are speaking to are certainly well qualified, they are also biased as they have skin in the game. The studies you linked look promising, but the evidence is not strong enough yet to safely recommend this to viewers. I just worry about dogmatism in the keto-sphere. Thank you.

      Agree they are all keto practitioners someone who isn’t studying or working for a ketogenic therapy clinic would be helpful and more balanced lots of doctors use the ketogenic diet for epilepsy and type 2 diabetes but not for everything so any doctor particularly a paediatrician as they use it more often just not for unproven “currently not yet approved reasons” those doctors aren’t anti keto they use it it’s well known not specialised but they also aren’t basing their whole career on it being a possible miracle cure either

    Even well funded drug trials take decades. KarXT, a potiential wonder medicine for Schitzophrenia and related disorders, has taken almost 20 years between concept and availability. Perhaps it will be available in 2025 or 2026.

    Cool I'm on day two now. Not sure if i'm doing it right but I feel like I did a good job. My partner wasn't so supportive but he will cope. I have Psychosis NOS, and i take Abilify. My hope is to go off meds completely, which is possible.

    I happened to see two new books at the library that directly talk about diet as a treatment for mental illness. They are "Change Your Diet, Change Your Mind" by Georgia Ede, MD who is a metabolic psychiatrist and developed the first and only medically accredited Ketogenic Diets for Mental Health Clinician Training Program (seems like you would want to interview her), and "Calm Your Mind With Food" by Uma Naidoo which is more about anxiety and the gut-brain connection. I'm not sure if it talks much about keto.

    so im 16 and living in wv and because of the healthcare problems in wv it was hard for me to get help for my schizoaffective disorder mainly because of my early age. and so ive been looking for new ways to improve my mental and physical wellbeing. what would you recommend for my age range regarding a ketogenic diet?

    But it is true, i think most of my illness comes from the meds (for epilepsy) i still scare to let completly my medicaments because i really can d!e😅 because my seizures are unconsious and very short but strong and that deteriores my brain everytime i have a seizure. I only have 3 seizures from adult time, with what i got diagnosed and my brain is very bad😅. One of the 3 seizures i had came from trying to stop taking my medication. And with a strong medication like for epilepsy and i am sure for schizo too, the others medications don't help at all, like CBD, even clonazepam do nothing to my sleep😅 only ezopiclone but this drug is very strong and deteriorate the brain also (from experience). I think the worse symptoms you could have in general, comes from lack of sleep. And i mean you for all of us😅. Sicnce 4 years i can not sleep and i have a lot of health problems now, maybe all started with meds but it got stronger with the sleepless nights.

    2x 0.5g vitamin C daily significantly improved insulin resistance in type 2. It didn't change a1c, I guess that still depends on carb intake. But on a keto diet, you don't have that problem so vit c might do as well on insulin resistance as metformin.
    EDIT: well scurvy in guinea pigs causes low glucose tolerance and very low insulin secretion, so maybe quite a bit of type 2 is vitamin C deficiency?

      That's interesting, I believe Dr. Richard Johnson has spoken about this that vit c blocks fructose metabolism. Vit c and water around carb consumption could be a good idea. Another interesting treatment is Allulose. Allulose blocks the absorption of glucose so if you take it right before a sweet potato or something your glucose will be much lower than if you hadn't taken it. He's an expert on insulin resistance and recommends the keto diet as a treatment as well

    You will lose fat, but the thing to watch out for is losing muscle. I lost 50 pounds in a few months before I realized a lot was muscle. I wish you the best of luck.

    I wish there was a way to get more research funding into nutritional and other therapies besides pharmaceutical ones. Wouldn't insurance companies benefit from prescribing patients exercise and better nutrition than paying loads of money on prescribing psychiatric drugs? Why don't they fund these types of studies?

    What if you have mental illness and diabetes type 2? I worry I'll get ketoacidosis if I try this diet.
    The other issue is who would ever be covered by insurance to supervise me if I did try this keto diet?

      keto is great treatment for type 2 diabetes. American diabetic association says keto/low carb diets are legitimate tools to treat T2D

SUGAR is a poison for some of us with mental issues… from Cutting out sugar all the way to Carnivore almost EVERYONE improves in some way… even if it helps with antipsychotic medication weight gain… some may binge on fruit and are reversed doing a high fat keto diet. Thank you for sharing!!!!

      My comment may have come out wrong…. What I mean is the spectrum from cutting out added sugar to paleo low carb medical keto and carnivore… even if all symptoms are not completely gone it still helps when you get rid of the sugar… unfortunately the psych ward feeds processed fruit cups and sugary milk. I don’t see how any of that is controversial? Hope that clears that up ❤

    I am not on full keto diet, but on part, and thanks to you, i have tried it and i feel a lot better. More I think of my psychosis, the more I think I was possessed by demons. Honestly, I cant find different explanation.

      If you are possessed by demons then an exorcism would work better than psychiatric medicines. But psychiatric medicines work better. That is why most churches won't do exorcisms any more.

      God bless you always and I will pray for you. I did not miss the medicine even once in 15 months. My doctor did not tell me about this mental illness in the beginning. So I searched medicine and saw that I have schizophrenia.Many Md psychiat tell schizophrenia treatment is 2-3 years??? I started from 9 mg and Now I'm on 5mg,I was also take 4mg but only 10 days which makes me feel much better. after 10 days I shifted on 5 mg which doctor prescribed

    The association between metabolic dysfunction and mental health is really strong. There was a study of children at age 10 who if they were insulin resistant were 5 times more likely to have been diagnosed with or at high risk for Schizophrenia by age 18.
    We should not be allowing our kids to be metabolically unhealthy we are putting them and our selves at risk!

      There’s also an association between insulin resistance and childhood trauma. I understand your line of thinking and am open to the idea of metabolic intervention, but I also don’t want us to be locating the entirety of the problem within the biology of the child when we should also be looking to relational stressors in their environments.

      Of course, but there are metabolic treatments that can be effective even if the damage is coming from something like trauma. Just because diet can treat metabolic damage doesn't mean that the previous diet caused it. Same for exercise, you might be metabolically and exercise can correct that, but it wasn't necessarily the case that not exercising is what made you sick. If that makes sense@@80islandia

    Lambert demonstrated the effectiveness of valproic acid in France in 1968, this study that lead to its widespread use in France to treat bipolar. What has been forgotten and lost to history is that Lambert's patients had "epileptic insanity".

      @@Catlily5 Yes, although lithium is thought to be slightly proconvulsant depending on the brain region.
      But yes, for the most part there is a clear link.People don't seem to be able to see the significance of the connection. In the 1990s the second messenger effects of anticonvulsants were all the rage and anything with the slightest effect on epilepsy was tried out. Now it's true that most of these agents didn't work but many probably did do a little bit of good, just not enough to pass RCT.
      The theory is sound enough, although it's not nearly enough evidence to say something works. It is however enough to bring something like keto well and truly out of the fad diet realm. If we took all these other anticonvulsants seriously, why shouldn't we take keto seriously?

    I loved the video!! It is so great to here that more of these studies are going on, and that most of them are having such positive outcomes. I know I have only been following your channel for a short time. and that I don't normally comment. but I did want to point out a correlation that I was seeing in Your Blinking during the video. You only seem to blink a lot when you are Listening. or auditorily processing information. When you are talking, or verbally processing information. your blinking REALLY slows down. You know the old trick where you sit in a chair. pat your head, rub your tummy, and make a circle with your foot. and one of them normally goes the wrong way. But if you are listening to music, it activates other connections in the neurons and most people can do it then. Well I am just wondering if it would work the same. that if you had music going in the background. or headphones on quietly. WHILE listening to someone else talk. if you would blink less. I did this years ago. As my father would always call me a Sissy for blinking all the time. before I was diagnosed. and it seemed to help. Just a thought. Thank you for all of your hard work. and videos. and for being our Ginny Pig. HAHA in this process. I am looking at changing to the Keto diet myself. and admittedly, selfishly, have been waiting to see how you are doing with it. but now i think I am defiantly going to try it.     Im really concerned with the risk of diabetes with keto. I saw my BG go up the entire 2 months I was doing keto. And elevated glucose increases glycation end products. Its definitely not the most healthy diet and has risks for many. But I understand the use for specific conditions.

      Are you sure you were doing keto? Blood glucose in all the RCTs are very stable and drop to low healthy levels. Even the American Diabetes association now recognizes low carb and keto diets as an effective way to treat diabetes

      There are literally hundreds of thousands of people on ketogenic diet to specifically lower their blood sugar (myself and my wife included). I'm not sure under what mechanism your blood glucose would rise when eating so few carbs.

    It’s like enough talking about it of course it works! There just need to be more effective treatments in psychiatry targeting mitochondria. Psychiatry is so behind in doing much of anything. Jogging is great for negative symptoms but if you want to go out and have a life, a career it takes up time And energy on the patients behalf. I mean, develop more effective treatments! Move the needle for god sakes

      Because she is having tarditive dyskinesia 😊 this comes from antipsychotics. i really hate this channel since december 2023 - it is all about the keto diet, which is complete nonsense.
      For me personally, my body runs on carbohydrates - I have done a gene test. I tried keto/low carb with 0 effect. I just had sleepless nights. I also tried to eat high protein, also with 0 effect.
      Guess what, the dna gene test suggests, that I personally need:
      - 54 % carbohydrates
      - 28 % fat
      - 18 % protein
      I am gonna stick with that. Beware that the carbohydrates have to be complex and not like sugar, white bread etc.
      If you don‘t want joy in your life, start with the keto diet. 🎉

    Has the carnivore diet been tried? My son refuses to take his meds. He was taking in too much sugar. Now that that has been greatly reduced, he's finally sleeping better and lost 20 pounds the first month.
