Dr. Malik thank you for this excellent presentation. I have had elevated SHBG ever since my allogeneic peripheral blood HSCT for the AML leukemia I was diagnosed with in March 2014. The oncologists/hematologists didn't seem to know, or didn't want to explain to me, the finer details of SHBG. Thank you for the description of the complex balances that perhaps reflect the conditions that affect SHBG.
Please don't misread my comment, the SHBG level is very high in my case after the SCT (has nothing to do with AML leukemia), I was told by one oncologist/hematologist that it was my liver causing the higher SHBG levels, a PA endocrinologist said that it was due to the chronic GVHD, it might be a gene expression effect. I don't know but this presentation added to my understanding of this situation with me and SHBG biochemistry. I have found that there are a multitude of gene expressions, body biochemical expressions not well understood. It seems at this time and my observations, that although the HSCT to cure blood cancers has been going on for the past 45 years at City of Hope in Duarte CA. The "medical industry" still doesn't have a sure, beneficial for the patient, method to "tame" the chronic GVHD. What I have experienced and seen is that they discover a certain biochemical pathway that participates in allowing chronic GVHD, then they go through the clinical trials to bring this "molecule" to market. The blocking of a specific biochemical pathway I would surmise might cause undesired effects in the system as a whole and disrupts the homeostasis of the blood components. Thus the side effects are numerous, the medicine might increase the possible survival and mitigate chronic GVHD, but at a cost of "quality of life". The side effects of these drugs can open up the possibility of opportunistic infections, and in parallel lower the blood components like red blood cells, platelets, and white blood cells. Personally I think that there should be extensive research (clinical trials) using umbilical cord derived mesenchymal stem cells (uMSC) and uMSC exosomes to develop consistent dosing regimes and protocols. I have no clue what mine is like that for. None. If it were NAFLD surely I'd be dying or dead (also I think I'd have more than 7 lbs tops of visceral fat overall) I do know I've been emasculated and unhappy since puberty and beyond and I've always felt low T/high cortisol even though my cortisol is usually the level of a yoga master (really it's more on the lower side) and my total t seems like it can be/stay okay but my free T is a disaster... I'm going to have to look up GVHD and learn more... It might explain the persistent subclinical anemia/fatigue/permanent gym like muscle fatigue/low grade DOMS (without gym mind you) as well. I pretty much have impaired gym performance because of it bigtime.

I loose my 5 years due to vitamin D deficiency, I consult with multiple doctors because I was suffering severe fatigue, mode swing, unable to learn new things but no one ask me for Vitamin D test. Last month one of Doctor advised me for D test and it was 17 only, testosterone was at 190 and B12 was also at 230. I am 41, after this I start using D+K (5000 IU D3 and 180 mcg K2)along with omega 3( 1100 EPA 1100 DHA) and believe me my life start changing, now I can study continuously. No Fatigue, no weakness, I can continuously work. Major changes in erection level also, memory is also improving.
49 Year Old Male with typical low T symptoms. My GP told me that he would only be concerned if my Testosterone levels were practically zero. I had to push HARD to get bloodwork, he did not want to do it. When the results came, my Total T was actually pretty good. I used the numbers provided to calculate my own Free T, which was at around 1.6%. My SHBG is in the upper end of the normal range, the GP classes it as normal. because technically it is, but I know that's the reason for my issues. I have manged to significantly improve my Total T over an 18 month period by supplementing Magnesium, Zinc, Boron and Copper. I use individual products for each, well researched and am pleased with those improvements. My total T is now much better than it was when I was in my early thirties. I hope that increasing my Protein levels might help out on the SHBG and thus the Free T.
As long as your values are within "normal" range, doctors won't care. But normal doesn't necessarily mean optimal. Sometimes you just have to be your own doctor and find what works for you.

I'm in my upper 40s and ever since I started taking vitamin D3+K2, zinc, and magnesium about 5 years, I feel great! I just got my bloodwork from the doc and all of my levels are optimum. Before that I was obese, low energy, and no sex drive but now that has changed. Also my PSA levels are normal too! Now I have to make sure I don't get stimulated during the day so I don't have an embarrassing reaction LOL. Also I eat a lot of crucericurous vegetables too which is supposed to suppress estrogen and lower SHBG which I didn't know until today! Thanks for the video as I now have confirmation that I was doing the right thing all along 🙂
wow thats gerat to hear who mentioned the dosage to you? Do you just take it as it says on the bottle or were you advised by an professional? The recommodation is often too low and basically for ppl who are not in need for it but we all know as older as we get we do need more to feel fit: I would love to hear back from you!
@@6sinnecoach I have done a lot of research over the years and found that 5000UI of vitamin D3 is optimal at least for me as I am darker skinned and live in a higher latitude. My last blood test showed me at 37ng/ml! I take this along with 100mcg K2, 30mg chelated zinc, and 400mg of magnesium. I take every morning except I take the magnesium at night to help me sleep.

When I've stopped drinking beer & started weight training i gained muscle fast . Even now in my 50s. Still build thick mass. Especially my back & legs.
Why can’t geriatric physicians explain testosterone as well as you just did. I am 67 years old and I’ve had LT for a long time. I have been treated off and on for the problem for at least 10 years now and have never had it explained to be as well as you have done in this video. I now have some more things to talk to my doctor about. Thank you.

I think the increased protein and fiber actually decrease the overall need for insulin levels. Fiber can actually stabilize blood sugars requiring less SHBG to manage the body. The net testosterone effect is more important than the insulin levels. Type 2 diabetes has significantly high insulin levels and the body's effectiveness and it is more or less insulin resistance. For every hormone there is a receptor and it is just as important to understand the relationship between the receptors as it is the hormones because the importance is not high levels of testosterone but the connection between testosterone and the receptors that it connects with so it is more important to not have high insulin levels because they make the connection more difficult to happen. The fiber and protein may actually make the testosterone more efficient than just high levels. The key is efficient connections and high levels are easy to detect but the receptors and connections are not and why a lot of diseases that are unknown at present have not the proper treatment because it is a disease of the receptors more so than a disease of the hormones.
Thanks Doc!!!!! I am currently on a high dosage of vitamin D (once a week) due to low levels of this and testosterone in my blood work. I was taking a over the counter vitamin D supplement everyday. I'm hoping this works. I'm also taking zinc and working out more. I work long hours at work so I see the sun when I travel home for about 15 minutes and milk doesn't agree with me except for the unwanted ability to make music from my rear. Trying to increase my testosterone so I can have good work out sessions and a healthy sex life . Again thanks your the greatest and you are always on point when it comes to educating your audience!!! Hey Doc could you make a video talking about reverse kegels? I think that there is a lot of information about Kegels, but reverse kegeling is often neglected, and from what I read, it's crucial to do both in order to achieve a balanced pelvic floor.
Age can bring irregularity or particularly constipation. Get that taken care of and sleep improves. Improved sleep brings libido back up. Just anecdotal\ of course.
My total t 708, free t 9.6, dhea 323, shbg 69.8, estradiol 36.5… zero symptoms but my dr said its good shbg higher which is contrary to the internet lol.
Just to be clear, testosterone doesn't increase in the early mornings exactly. It increases basically right before and during the wake cycle. So if a person is a night owl when they go to bed, which is the morning, they won't have an increased amount of testosterone at that time of day.
Yes, mine is abnormally high! I just started boron today, which is supposed to help. (I'm female, total T is high normal and free is at the bottom of normal. I also have estrogen dominance. I definitely feel like my T is low.)
@@MsJacob24 I haven't had another blood test yet but I didn't feel it doing anything. It could be that another form of boron would absorb better. I also tried tongkat ali (herb) which felt like it was helping.
I have a very complicated one Im 38, my free test came back well over the max, my shbg came back well under the minimum... now i cant put on muscle at all really, i feel like i have many of the symptoms of low testosterone.... My triglycerides are over the max just about and 2 of my i think white blood cells are above the limit... akso ive never taken steroids in my life
Hello Doctor I think that nothing helps to increase actual testosterone. While you could try various supplements and similar things, they will only give you marginal results at best. I don't want to discourage anyone, but for most people, it's not worth the trouble unless you're using a drug or pure hormones. I recommend that you consider this combination of basic amino acids: N-acetylcysteine (NAC) and Glycine. While this won't directly increase testosterone, it will help balance it by providing the building blocks to produce Glutathione (GSH). NAC and Glycine should be taken at a 1:1 ratio between 600 mg to 1200 mg. Raising Glutathione with precursors is more beneficial than taking straight Glutathione due to the various levels in each tissue type. Taking NAC with Glycine will provide endurance, strength, and energy, and improve general health in all conditions. Glycine is combined to supplement the mitochondrial dysfunction when NAC is taken by itself. GlyNAC can instantly improve energy levels and reduce inflammatory factors. By raising Glutathione, you can see a significant reduction in reactive oxygen species (ROS) as well as other factors. Moreover, we have seen a significant decrease in autism markers after a 3 to 6-month period at a 600-1200 mg once-daily dose in those who have autism. Since GlyNAC corrects Glutathione imbalance, we also see tissue regenerative effects in all tissue types. Lastly, N-acetylcysteine with Glycine may prove beneficial for certain people with ED. Could you take a look into this?
Obesity decreasing HSBG? Studies say the opposite. Also, low carb diet increase SHBG; therefore a ketogenic diet even though might increase total testosterone, this will be unavailable because of the increasing in SHGB. Maybe you should review the video?
For some reason, my testosterone is quite a bit higher now than it was in my 30s. Last reading was 420 as a 37 year old, vs 755 as a 43 year old. Im not supplementing or anything, no therapy. However, i feel like crap. Is something else going on?

Morphology, FSH, LH, Testosterone, Abumin, SHBG, Prolactin and Estradiol (estrogen). Knowing Albumin, SHBG and Testosterone levels will allow to calculate Free Testosterone and Bioavailable Testosterone levels using online calculator. It's more accurate than just Free test done separately and less expensive. You can add TSH, FT3 & FT4 to check for any Thyroid issues. Worth checking cholesterol (LDL, HDL and total) as they are testosterone precursors. Last but not least: PSA (prostate). TRT doesn't cause prostate cancer, but might not be recommended if there are any existing prostate issues.
The answer to every question is yes. However, I would not touch Finasteride. In rare cases it causes mood and sexual health problems that do not stop even when you stop the medication. It’s rare, but worth considering. But yes, TRT will worsen hairloss if you are prone to it and Finasteride will protect you from that. If you are not very prone to hairloss, dandruff shampoo can work as well (that’s what I use, totally reverses the effects of TRT on my hair).
So I'm still a little confused. Is it better to have lower or higher SHBG? SHBG binds free testosterone, but at one point I thought I heard that increased SHBG causes increased testosterone.
Still unclear on which exact test to ask for? If you get a testosterone test, it will typically just measure total and free( and free is not accurate unless self calculated?) Also what about the dangers of getting test treatment?

Heard of it ? Tried boron, tried fish oil, tried diindolemethane... STILL stuck with upper double to mid TRIPLE digit SHBG so even with a quad digit testosterone total... my free test is... terrible often. Every single symptom you describe is me BUT of course... And it doesn't help my "mental" (there is no such thing as a MIND btw physicians worth their salt acknowledge this that there is only a brain and that we are not just chemical receptors walking around) libido is through the roof from never having any masculinity or lovins in my life but physically it's nearly absent, like it was broken by my late teens and IDK why... :( Well I do... but what can you do since men still need HCG with external test to create ejaculate. Could it also be that SHBG goes down in correlation with high BMI and Hugh protein intake because men who train hard with weights eat more protein and tend to have a higher BMI due to carrying more lean muscle?
I feel like shit below 11-12.5 ng/dl. When I'm even at 7 ng/dl free I'm... just miserably destroyed... Like seems like 14ish ng/dl is my biological ideal... and that my body/total test are desperately trying to attain homeostasis and *can't*.
The best protein is animal meat and fat. It is what our bodies are made of. There is a reason there odds no such thing as animal fat or protein unless it’s made through heart and chemical reaction. See it this way. You’re out in the wild. You need fat and protein. You catch a fish out a wild pig, as long as it’s clean, you can even eat it raw. Plants, you need a lab. Animal meat is also the best way to get nutrients while plants have defenses like oxalates and lennins that bind to their nutrients causing problems to our health

You failed to distinguish between plant based proteins and soy protein .. soy has high levels of estrogen.. plus phytates tha bind with essential minerals like calcium magnesium zinc and iron.. it causes your body to excrete them and not absorb them
Dear Dr. Malik, I try to be appropriate; it occurs to me that there could be many in my position who want to know more about how the male mind is influenced by hormones. My thoughts easily wanders when I hear you explain how low testosterone may cause higher mortality, I am 77 and have had Chemo series 2018 and 2020, and my 6 functions have declined of course, so now I am in the danger zone. Lately I began to be depressed in the mornings, and today I slept 12 hours or even more! After getting up I tried to understand the functions, - and then began to watch Dr. Malik's Urology but the thoughts wander and I think of how nice you are etc.etc. in stead of what you say.

I had low T, I started taking boron, and my T shot through the roof.
There might not be many studies on it, but for my personal case, it works.

Don't be a Boron. Your T is normal. There is no advantage to putting minerals in your pee. Save the money, invest and live better.

I definitely feel something after taking boron especially when it comes to libido.
I think the increased protein and fiber actually decrease the overall need for insulin levels. Fiber can actually stabilize blood sugars requiring less SHBG to manage the body. The net testosterone effect is more important than the insulin levels. Type 2 diabetes has significantly high insulin levels and the body's effectiveness and it is more or less insulin resistance. For every hormone there is a receptor and it is just as important to understand the relationship between the receptors as it is the hormones because the importance is not high levels of testosterone but the connection between testosterone and the receptors that it connects with so it is more important to not have high insulin levels because they make the connection more difficult to happen. The fiber and protein may actually make the testosterone more efficient than just high levels. The key is efficient connections and high levels are easy to detect but the receptors and connections are not and why a lot of diseases that are unknown at present have not the proper treatment because it is a disease of the receptors more so than a disease of the hormones.
Thanks Doc!!!!! I am currently on a high dosage of vitamin D (once a week) due to low levels of this and testosterone in my blood work. I was taking a over the counter vitamin D supplement everyday. I'm hoping this works. I'm also taking zinc and working out more. I work long hours at work so I see the sun when I travel home for about 15 minutes and milk doesn't agree with me except for the unwanted ability to make music from my rear. Trying to increase my testosterone so I can have good work out sessions and a healthy sex life . Again thanks your the greatest and you are always on point when it comes to educating your audience!!! You need magnesium for Vit D to work.
Why? Literally zero evidence that supplementing vitamin D raises testosterone. Plenty on evidence however on excessive vitamin D toxicity.

Hey doc, could you do a video on androgen receptor sensitivity? I’ve heard mumblings that it may have more impact on low/high T symptoms than the actual test levels

I know that the epilepsy drugs have a negative impact on hormones. I don't have epilepsy but I take valproic acid.
age 44 my SHBG is high (60) & prolactin high all thyroid levels normal. Free T bordeline low. Tried boron & tonkat ali but still can't get it lower.
Of course I'm an older woman so I'm not the target of this - or almost any other - message about testosterone. But thanks for what I was able to learn from your presentation. I do suggest using pictures to help folks get the concepts a little easier.

Hey Doc could you make a video talking about reverse kegels? I think that there is a lot of information about Kegels, but reverse kegeling is often neglected, and from what I read, it's crucial to do both in order to achieve a balanced pelvic floor.
Isn't it just the opposite of a kegel? You relax the muscles used in a kegel?

Age can bring irregularity or particularly constipation. Get that taken care of and sleep improves. Improved sleep brings libido back up. Just anecdotal\ of course.

My total t 708, free t 9.6, dhea 323, shbg 69.8, estradiol 36.5… zero symptoms but my dr said its good shbg higher which is contrary to the internet lol.
Your Testosterone fluctuates naturally by up to 40%. Every day.
Bear this in mind when considering your T levels.

Just to be clear, testosterone doesn't increase in the early mornings exactly. It increases basically right before and during the wake cycle. So if a person is a night owl when they go to bed, which is the morning, they won't have an increased amount of testosterone at that time of day.
Yes, mine is abnormally high! I just started boron today, which is supposed to help. (I'm female, total T is high normal and free is at the bottom of normal. I also have estrogen dominance. I definitely feel like my T is low.)

@@MsJacob24 I haven't had another blood test yet but I didn't feel it doing anything. It could be that another form of boron would absorb better. I also tried tongkat ali (herb) which felt like it was helping.

Im 38, my free test came back well over the max, my shbg came back well under the minimum... now i cant put on muscle at all really, i feel like i have many of the symptoms of low testosterone....
My triglycerides are over the max just about and 2 of my i think white blood cells are above the limit... akso ive never taken steroids in my life

I think that nothing helps to increase actual testosterone. While you could try various supplements and similar things, they will only give you marginal results at best. I don't want to discourage anyone, but for most people, it's not worth the trouble unless you're using a drug or pure hormones.
I recommend that you consider this combination of basic amino acids: N-acetylcysteine (NAC) and Glycine. While this won't directly increase testosterone, it will help balance it by providing the building blocks to produce Glutathione (GSH). NAC and Glycine should be taken at a 1:1 ratio between 600 mg to 1200 mg. Raising Glutathione with precursors is more beneficial than taking straight Glutathione due to the various levels in each tissue type.
Taking NAC with Glycine will provide endurance, strength, and energy, and improve general health in all conditions. Glycine is combined to supplement the mitochondrial dysfunction when NAC is taken by itself.
GlyNAC can instantly improve energy levels and reduce inflammatory factors. By raising Glutathione, you can see a significant reduction in reactive oxygen species (ROS) as well as other factors. Moreover, we have seen a significant decrease in autism markers after a 3 to 6-month period at a 600-1200 mg once-daily dose in those who have autism. Since GlyNAC corrects Glutathione imbalance, we also see tissue regenerative effects in all tissue types. Lastly, N-acetylcysteine with Glycine may prove beneficial for certain people with ED. Could you take a look into this?
Obesity decreasing HSBG? Studies say the opposite. Also, low carb diet increase SHBG; therefore a ketogenic diet even though might increase total testosterone, this will be unavailable because of the increasing in SHGB. Maybe you should review the video?

For some reason, my testosterone is quite a bit higher now than it was in my 30s. Last reading was 420 as a 37 year old, vs 755 as a 43 year old. Im not supplementing or anything, no therapy. However, i feel like crap. Is something else going on?

What’s the best overall bloodwork to do to see all the results?
Morphology, FSH, LH, Testosterone, Abumin, SHBG, Prolactin and Estradiol (estrogen). Knowing Albumin, SHBG and Testosterone levels will allow to calculate Free Testosterone and Bioavailable Testosterone levels using online calculator. It's more accurate than just Free test done separately and less expensive.
You can add TSH, FT3 & FT4 to check for any Thyroid issues. Worth checking cholesterol (LDL, HDL and total) as they are testosterone precursors. Last but not least: PSA (prostate). TRT doesn't cause prostate cancer, but might not be recommended if there are any existing prostate issues.

Does TRT cause hair loss and can Finasteride help, and can you take finasteride when you are on TRT?
The answer to every question is yes. However, I would not touch Finasteride. In rare cases it causes mood and sexual health problems that do not stop even when you stop the medication. It’s rare, but worth considering. But yes, TRT will worsen hairloss if you are prone to it and Finasteride will protect you from that. If you are not very prone to hairloss, dandruff shampoo can work as well (that’s what I use, totally reverses the effects of TRT on my hair).

Lower SHBG = Higher Free Testosterone. Serum Testosterone Needs to be within the normal parameters for it to have a noticeable effect…

STILL stuck with upper double to mid TRIPLE digit SHBG so even with a quad digit testosterone total... my free test is... terrible often.

Dr. would you be interested in my experience with gout arthritis and testosterone. These words are never uttered in the same sentence at the rheumatologist office but I promise you there is a connection. If testosterone doesn't play a part in gout why isn't it common in women? Dear Dr. Malik, I try to be appropriate; it occurs to me that there could be many in my position who want to know more about how the male mind is influenced by hormones. My thoughts easily wanders when I hear you explain how low testosterone may cause higher mortality, I am 77 and have had Chemo series 2018 and 2020, and my 6 functions have declined of course, so now I am in the danger zone. Lately I began to be depressed in the mornings, and today I slept 12 hours or even more! After getting up I tried to understand the functions, - and then began to watch Dr. Malik's Urology but the thoughts wander and I think of how nice you are etc.etc. in stead of what you say.
