      I recently had a finger broken. After I took off my splint after over a month, my left pinky finger does not straighten and I feel pressure on my palm below the knuckle and pressure between the left ring finger and pinky also. The ring finger is also somewhat hard to move but just not back as far. Middle finger slightly has a tightness issue but goes away after stretching it. I also did have a very tight and sensitive wrist from being in the splint. I could not move my wrist at first. A few days if stretching and massaging helped but still have some wrist issue and forearm tenseness. I love your videos! How do I become an ASTR provider!?

      I’m unable to make a fist after distal radius fracture and plate and screw surgery, also had carpal tunnel release and tenolysis (surgery to remove adhesions from flexor tendon) I still cannot make a fist my fingers just get stuck .. click my channel to see my video where I show this.. anyways do you think you can help me?     I got trigger finger on my right hand around age 60. It happened after using a hatchet for hours chopping on a pine stump. Google says that many people that get trigger finger are in some phase of diabetes. It does run in my family. After seeing this video i used household items like a wooden spoon to message scar tissue on hand and finger that was getting stuck. It worked to get finger working normal again, no more stuck finger in the morning. It was very painful to make the finger open up at first. After some online research i started using (tumeric with curcumin capsules) and, black pepper with meals. The black pepper helps the body to absorb the curcumin/tumeric. I believe that has helped. Thanks for the video. Do research online on trigger finger, the drawings of how the finger works help explain what is going on to make the finger to get stuck. Thanks.

      @TijiaCab I'm glad you aren't suffering from trigger finger anymore! The reasons why patients have trigger finger isn't straight forward, unfortunately. Many have high levels of inflammation and the majority have underlying health issues going on that they are unaware of including severe vitamin, mineral, nutritional deficits and/or hormone imbalances. Many healthcare providers are not trained to know what to look for or how to catch these deficiencies. My trigger finger patients benefit greatly from home treatment with our patient tool kit and hand program and some will need to set up a Telehealth Evaluation to review labs and do our nutrition based assessment to determine the root issues going on.     I did myself on my left trigger thumb and it does work. I was follow same procedure for last two weeks. Now my is thumb recovered 95% and work with less pain, I had a three month before and planning for Cortisone shot soon.     I also have the same patient case before never heard of this at that time but what I did was basically the same thing you've done on the video. I never had any fancy instrument like that so basically, I had to do it all by my hand. One thing for sure I learned that *each patient has a different presentation of trigger finger and these techniques doesn't really GUARANTEE their full recovery.* As a practicing PT, we also have to know if the patient could really benefit from us by these techniques we teach them, rather than *wasting their money, TIME and effort* to come each treatment and just for *NOTHING*

    I had a trigger finger (it was my right thumb) woke up one morning and I couldn't bend it. It became very painful in my palm and very hot, I used to freeze a bottle of water and have my hand on that at night. Went to my doctor and I had an x-ray/scan and they injected cortisone into the base of my thumb. It was a couple of weeks before it came right. Perfect now.

      @@snailnoodle I opted for surgery instead of the shot because I did not want steroid shot.
      Took 6 weeks to recover with 98% fonctions back. Now100% back. Prior to surgery, the doctor said 98% with surgery and 60% with the shot. I did not take the shot.
      I wish I had astr option but this is the first time I heard.
      Update: I am glad I went the surgery route.

      Change doctor if the idiot is not hearing you. Find a good orthopédiv surgeon who has done the same surgery before.
      Surgery is simple and you get 98% of functionality back within 6 weeks.

      So your comment was 5 years ago.
      Did that ‘treatment last’?? I’d be surprised. Generally the only times these manipulative techniques are successful are when the inflammation in the tendon sheath is due to a traumatic injury on the palmer side of the hand pretty much directly to the actual tendon sheath. Time & treatment can reduce the swelling, & the trigger MAY go away. Other than that, you either live with it and its symptoms, ranging from annoying when it kicks up, to extremely painful, & permanent locking… or you resort to surgery. It may sound ignorant but I am SOOO LOOKING FORWARD to my 6 th trigger finger release. I’ve never had a reoccurrence in any of the previous 5 fingers I’ve had surgically repaired over the last 18 years.

    You say his trigger finger is severe.... Mine is ten times worse than that! There is no way I can open my finger at all and if I move it with my other hand to open it, the pain is so bad I think I am going to faint. I am going to see if I can do this massage to myself and see if I can get it better... I have been in excruciating pain with this for a few months now... I can never open it on my own... I have to do it with my other hand and endure terrible pain... this is so bad for my work. I got it overnight by taking an over the counter liquid medication for heartburn... As I watched the video I did the same thing to myself. I used a pencil eraser to work on the portion below the finger itself on the hand. I used the heel of my other hand up my inner arm. It was very painful but by the end I could open my finger on it's own... painfully, but I could do it... This video is very helpful as I have been in agonizing pain for about 3 months with this. Thank you so much for showing how to do this.

    It's insane and kinda sad to see ppl spend two years to recover from trigger fingers. I used to have that several years ago, but luckily i found a minor surgery that help me get rid of trigger fingers once and for all. I hope this treatment can be known by ppl suffering from it. It really works and doesn't have any significant side effect. It's been almost four years and I've been weight lifting since the time I recovered from it

      @DerivativeCovariant In the past ten years of practice, I've never spoken with a patient who has been healed with surgery. Typically, they come to us because the surgery/surgeries haven't worked. So I'm glad that you are an exception to that. More than 80% of the trigger finger patients I treat have nutritional deficiencies, vitamin/mineral deficiencies, hormone imbalances and underlying health issues. I oftentimes have to do functional medicine with those patients to review their labs and do a nutritional analysis to find out where they are having issues. The manual therapy part of ASTR is also very effective for naturally breaking down scar tissue and releasing fascia that has built up from repetitive motion, poor body mechanics, etc.

    Hi Dr Joseph . Thanks for your video. I have trigger finger and I had it for over two years. Had an injection but it came back after just one month. The injection was very painful. I m impress with the treatment and was wondering if it's a permanent fix or does it need a follow up . Also, are you promoting the specific course for therapist only?     Trigger release is done do to the tendon thickening and typically a nodule growing. That nodule is solid and is what causes the popping or catching sensation. I fail to see how a massage is going to remove the nodule.

      I agree 100%… as I’ve had 5 FIVE triggers released, with #6 looming. No kind of massage or manipulation can eradicate the underlying condition that causes TF. My hand surgeon recommended a few different types of OT manipulation & massage, when I was first diagnosed & even the massage therapist said ‘you know this is only being done because your insurance won’t pay for the surgery until other options have been exhausted, right? I can tell you’re going to need surgery, & I’m willing to bet this won’t be your only finger’ She was right.

    Isn't the cameraman or at least the narrator,...the doctor? sure sounds like he is. That's why a student is doing the treatment. So that he can explain and ask questions...and point out how easy it was to learn his "AST" tequnique.

      Yes, I was the narrator, I did not realized that my voice was loader than the chiropractor and the physical therapy. I was trying to explain what is going on, so the audience get a good history of the condition and how fast doctors can learn ASTR treatment and implemented in their practice the next day after taking the class

    I've got left hand middle finger lock also; from learning guitar. I've had issues with this hand several times in my life and a couple with the right hand. As recent as 2018 I had the same trigger finger as now and it was excruciating, would lock up at least a dozen times a day. Fool that I am never used a splint, but daily nighttime poultices of cayenne pepper and castor oil mixture wrapped around my fingers would eliminate the inflammation and it finally went away about five months. Now, I'm going through the same torture only two months after taking up guitar; sometimes I can exercise it away just immediately before playing but more often exercise and cayenne/castor oil poultice is not good enough loosening to play. I'm resigned to forget guitar for a few months until this goes away now that Summer approaches and warm weather helps this issue.
    What was in that gallon jug? I'd like to know. ciao.

    Wow, I was unaware there were other therapies for trigger finger. My HMO starts with cortisone shots before slicing the sheath open to allow the inflamed knot in the tendon to glide freely. The pain I feel when my finger snaps open is excruciating. One of six shots worked the other five took surgery. Four more to go. I work with motorized tools and inflammation is a problem in my hands. I need to learn more about these none invasive therapies thanks for the vids

      I’m like you, have had 5 successful surgical releases. 6th surgery is going to be in a few months most likely. Fortunately, according to the MRI’s, my hand surgeon surgeon believes it won’t occur in either of my thumbs or pinkies! Those structures are anatomically different. I did have to have CMC LRTI basal joint Trapeziectomy and mini tightrope surgery on my right thumb, all due to severe osteoarthritis… but the only thing that ever worked for my trigger fingers, was surgery.
      BTW, the recovery from that thumb ‘joint replacement’ surgery was a Moth***ker!

      @@Dr.Jacobs Thank you for the reply doc! I actually cured my thumb by following your approach in this video within days of writing this! I had had the problem for a long time already and it was getting worse and worse. When i saw this i started searching my upperarm for which tendon would be associated with the thumb, and did so quickly because when i started rubbing around i found a tight line that was sore to the touch. I massaged it for a few days and like magic the thumb was good as new and I'm proud to say is nimbly writing this very message. Thank you so much.

    Dr. Jacobs, do you do anything to reduce the reoccurrence of swelling within the synovial sheath around the flexor tendon following this treatment to trigger fingers? I typically make a low profile anti-trigger ring that my patients will wear for a few weeks to prevent the return of triggering(I am an OT, hand therapist) I have had fairly good results with this approach, but I have to admit I haven’t had THIS dramatic reduction in triggering with manual tx in only one session! I am curious if you kept up with this patient/student/PT receiving the treatment to see if he had any recurrence in triggering, or how many sessions he required. Thanks!

    Do the instruments you use scrape the muscle and scar tissue, similar to Ga Sha? About ten years ago I was scheduled for release of my trigger thumb with a plastic surgeon. During this time I visited my rheumatologist and told him about my thumb. He volunteered to give me a cortisone injection and I agreed. I have had several cortisone injections over the years in many different joints and areas of soft tissue. The injection that I had in my thumb was done with such precision that the pain vanished - and has never returned. I was thrilled to be able to cancel the surgery.

      Hi!! I have trigger finger in both my thumbs for 8 months now. I'm undergoing physical therapy at the moment. My rheumatologist told me that if the therapy doesn't work, he will inject me with cortisone. I know that that is not good if you have pre diabetes and I do. So what I want to know, if your taking of cortisone a regular thing now, like a maintenance that you're gonna take for the rest of your life? If it's not, then I may take a chance to have the cortisone for 1 or more times. If it worked for you, it might work for me too.

    Hello sir, this video really seems like great treatment. I'm suffering from mallet finger. 2 months ago, I hitted with a basketball in my ring finger of left hand due to which it bend and feels no pain. Right now, it's in an angulation of 30 degrees and it's look ugly and it's not bending fully from the joint too. I'm kind of feel pain while I apply little pressure on finger due to any work.     Had trigger finger on both middle fingers from running a weed eater for 30 years , could not open and close hand three times without locking up. Started taking a spoonful of turmeric every morning , fingers rarely lock anymore and pain from arthritis in knees, back and ankles have gone!

    Hi Dr. was bruising through some videos and came across this one, can these exercises be used with someone who has a fractured wrist. I fell on the 28 of October last year, my hand was in cast for 6 weeks and the cast came off three weeks ago on Monday, I'm doing therapy at the moment and was just wondering if these exercises can help to get the fingers going again

    My right forearm muscles got tight from "mousing" at a desk for 12 hours a day and gripping gym weights. As a result, my index finger became stiff and painful. Periodically massaging the forearm muscles connected to my index finger helped a lot.

    I used the edge of an old flip phone with some vaseline hand lotion on a clients finger and he felt no pain 4 weeks later from 1 treatment. There seems to be something to IASTM even if research doesnt always support it

    Hello Dr Jocobs. I am amazed to see the results of the patient with trigger finger (T.F.). I suffer trigger middle fingers on both hands for over7 years. My GP in England injected Cortisone on both fingers and worked for a few months and then T.F. returned. My GP injected the second time but did not work. I had physio, acupuncture and Chinese medicine and they did NOT work. My T. F. are not too bad, they do not give me pain, only discomfort when I open my fingers, Most of the time, they can open but with a "click" Some days are better than others. Do you think the method that the chiropractor uses in the video can help? Can I do it at home to myself? Thanks

    In late 2014 (or maybe 2015) i cut the inside of the knuckle of my pinky (between middle and proximal bones) and cut the tendon that goes to the distal bone. I didnt think much of it at the time, so i didnt go to the emergency room so i cleaned it and put some gauze over it. About 6 months later i developed trigger finger in my pinky and had to repeatedly pull my finger straight to release it. Went on for a few months.. today my trigger fingers gone and i am unable to bend the distal knuckle on my pinky. I recently went to an orthopedic to get the exact reason for why that is (thats when i found out i sliced the tendon cleanly and the scar tissue is preventing it from being pulled back through the tendon sleeve). When i think about it, it makes my pinky hurt. Trigger finger was painful

    I have trigger thumb but after viewing all (except this) the information I thought I might have to resort to some stronger physio or even surgery on it. It is just the sheath problem similar to Carpal tunnel etc.. but after seeing this video I'm impressed that physio can clear it up. At the moment it's not complaining but I've manipulated it myself (before watching the video) which may have alleviated some of it but I can still feel the pain at the bottom in the bone joint area. I'll have to manipulate further along the wrist and arm now after this. Impressive.

    I suffer carpal tunnel in my left arm (I am right handed) and I also had trigger thumb in that hand. I went for pt up until the pandemic hit but the pt did not really know what to do for the finger, so i got the cortisone shot. It went away after this but i still had the carpal tunnel problem, After maybe 7 mos , i lost sensation in my middle and index fingers of that hand; I had the cortisone shots again; the relief the second time for the numb middle digit was not significant in that it did not releive the numbness. I got used to having no feeling in my left hand digits now. Now, recently, i started getting the swelling and pain in my right thumb. it feels like it's about to start getting stuck. I am wondering if the underlying cause is systemic or if it is connected to spinal problems caused by sitting too much since the pandemic lockdowns changed how we work. I have aches all over my arms and neck.

      I am so sorry that you have chronic trigger finger and carpal tunnel. Typically trigger finger and carpal tunnel syndrome pain caused by scar tissue, fascia restrictions and inflammation.       Larry Nevel Yes I've had this done from my husband who is a chiropractor. The thing you have to continue self care working on flexors and over power the extensor .One good exercise for this is rubbing it every time you get in the car on the wheel. You rub finger on the wheel of the car. I had a severe dog bite Hand becoming serverly infected. Had to have surgery done. Also take lecithin abut 1000 mil a day . It helps cell inside.

      I have seen worse, but the lack of ROM on extending his finger was pretty notable. Regardless, the treatment was very effective. I have done very similar things in my office, and it gets the job done without the need for an invasive surgery

Patient did not suffer from pain as it is pretty early on, but overall, just wanted to leave the comment that not all variations can be treated this way... sadly... >_

      Dr. David Berdan, DC my brother just started waking up to his pinky and ring finger on his dominant hand curled up and locked. Can’t open them unless ran under hot water for a couple seconds.
      He recently started working as a cat detailer, vacuuming for 8 ish hours a day.
      Is this going to need surgery? What should he do? Get a new job? Switch vacuuming hands ? Helpppp

      @@ThisIsSparta300 He would need to be examined by someone to find out one way or another. There are too many variable to properly diagnose without an examination. I'd suggest looking into health professionals in your area who specialize in diagnosing conditions in the extremities (namely hands in this case). Getting a 2nd or more opinions is always a good idea.

      @@gab31282 because the patient is forcing his hand to open to "show" that it worked. You can tell it's forced because the the 'pop' each time he opens his hand after the AST treatment. This is easy to do because his trigger finger wasn't that severe to begin with. Great acting though! Almost had me fooled too the first time I watched it. I have trigger finger in both hands and I'm still skeptical. $499 for those tools? That's nuts.
