I can only personally speak to this, but patient education is one of the most rewarding and meaningful things I do. However, the most recent pandemic has taught me that the majority of people have a belief system and seek affirmation of said belief system on the internet, which is extremely easy to do with the amount of misinformation available now compared to 20 years ago. Another concerning trend is that many people are closed minded and truly can’t allow themselves to hear an opposite opinion. The most dangerous phenomenon I’m seeing online now is distrust of the medical community in general. It’s saddening of what a small percentage of people sought my medical opinion during an infectious outbreak, and chose to formulate their own belief largely based upon their political affiliation. I’d love to be alive in 100 years to see how history documents these behaviors. I guess that since I’ve always been in science (chemistry undergrad at UNC, MD at Duke, internship/redisency/fellowship at MUSC) that I assumed the objective nature of science rather than the subjective/intuitive feelings and opinions was accepted as fact by all. I have since learned otherwise, and in some ways feel like civilization has regressed to times when scientists were persecuted for objectively proving commonly held subjective beliefs to be wrong, such as the earth is round, it actually revolves around the sun rather than the inverse, cold weather doesn’t cause URI/common colds etc.

I was amazed how many women were expressing desire and passion until i realized we're all older..its Good Morning America! We have more hormones than the younger generations - I kid you not.

I dramatically reduced my cortisol, and the high blood pressure that underpinned it, with the following steps:
    1. Stop drinking alcohol. It's both a depressant and a neurotoxin. You don't need it to 'manage' everyday stress. It causes the very stress you are trying to manage!
    2. Do some mild exercise in the morning. Walking to work instead of driving gets your heart going and exposes you to natural light, which has a direct bearing on the quality of your sleep.
    3. Eat a plant rich diet. There is a well established link between your gut microbiome and your mood and the best way to feed the biome is to eat a lot of probiotics and prebiotics. This does *not* mean a lifetime of salads or veganism. I eat plenty of white meats, including oily fish which is essential for brain health, and the occasional steak.
    4. Exercise at least three times a week with intensity. While your morning and afternoon walks will keep you moving, you need a strength and endurance regime to get fit. No exceptions.
    5. Walk / clean / move for twenty minutes after every big meal. This improves digestion, nutrient absorbtion, and blood sugar regulation. It also kills your afternoon crash. Try it.
    6. Reduce caffeine consumption. You cannot reach a state of deep sleep if you consume caffeine 12 hours before bed. Why? Because the body produces neuroadrenaline - a form of adrenaline that circulates in the brain - that prevents you from having non-REM sleep. Alcohol has a similar effect. Both destroy your deep sleep cycles and you need them for proper hormonal balance.
The above steps have quite literally changed my life. I look better, feel better, and am better. No medications, no over-the-counter remedies, no supplements.

Smoking big doinks out in Amish is a good way to manage cortisol levels.
