Dr. Melfi was the one woman that Tony wanted and couldn't have. That's probably why he was so attracted to her, because she didn't fall at his feet like other women did. She also understood him better than anyone else, even Carmella. It makes sense that Tony would fall in love with her when he confides in her and opens himself up to her all the time.

Here’s my opinion on Tony: He definitely wants what he can’t have, but this was not one of those moments. Early on, my thought has always been that Melfi is the woman Tony wanted to end up with, a beautiful, smart, independent go getter who was willing to challenge Tony and make him a better man. However, I think he thinks that Carmela is the woman he deserves to be with. Not saying Carm isn’t a good woman but their relationship was always strained, and they don’t seem to have much spark. It’s evident throughout the entire series that he is deeply insecure and I am sure he thinks Carm is the best he deserves
I don't think Tony would have left Carmela for Melfi though. I think Tony definitely saw something 'different' in Melfi. At this time he was depressed, and she was being that sympathetic ear he needed, after all its her job. I'm sure this is pretty common, where the opposite sex/patient, thinks they're in love with their therapist. Especially at the time when his mother and wife weren't giving him the sympathy he wanted. But they're people that live with him day in and day out, and they aren't being paid to help with Tony's depression.. It didn't hurt she's very attractive, which Tony will come calling to that. As he can't keep his eyes or hands off any woman that's attractive to him. I don't really think he was ever in love with Melfi. I think he was very attracted to her. Reality is, he didn't even know the real Melfi. Dr. Melfi vs Jennifer Melfi are two separate people. People at their job are different personality wise than in their personal life. Especially a shrink. That is what Melfi was trying to explain to him, twice, when he made advances at her. And she's right. You can tell this is probably not the first time a patient has done this to her.
A) Tony wants the good woman but would never change and he has to be smoking crackhead if he thinks Melfi will sit around while he bangs side whores B) Carmela might have turned out differently if she’d met a better man. How we make idiot decisions young and end up locked in well into old age is a constant theme
