In my mind I always viewed it that Carmella regretted telling Tony about her feelings for Furio, but in the moment she was desperate for Tony to understand what she had always felt about Tony's affairs.
It's why Furio had to take action. He kills Tony and faces the fallout (I doubt he lives long with plenty of people gunning for him to avenge killing a boss and get some legitimacy points) or bails out. If he commited they would be caught sooner or later, someone would spot them or Tony would find out on his own, he's dysfunctional, but not stupid. Or Carmella would flap her tongue and let it slip.
It's often forgotten what a great actress Edie Falco is. When she confesses to Tony her desires for Furio, I think it's one of the best scenes in the entire series. In fact, they're both at the top of their game.
Furio would either have to kill Tony or flee with Carmella to Italy. Either way, he’d be miserable forever once he found out that Carmella would hate a lesser life and would nag all the time. She wants luxury and no effort.
I would think in a real situation like that, if he had somehow gotten to Italy with Carmela, even his own family would disown him. Stealing the boss' wife? That's got to be considered not only extremely dishonorable, but could cause trouble, business and personal. A family like the one depicted here would be ashamed of his actions.
After Tony’s death Carmela returns to Italy with Furio. She drives a Fiat, wears a tiny gold crucifix and lives in a nice villa by the sea. She later gives birth to her third child, Furio Jr..
