  • Emergency Rear Door Breaks Off Hitting RH Tail - 3:48 Coming in Hot 5:40 Aftermath - 7:55
    AAIB Report- www.gov.uk/aaib-reports/aaib-...
    Extra Footage: • Damage & Follow up | G...
    Extract from AAIB Report:
"The investigation found the safety clip for the internal emergency operating handle of the baggage door was not correctly installed, so instead of holding the handle closed it held it in a slightly open position. During the flight, it seems most likely that this handle moved sufficiently towards the open position to disengage the shoot bolts from the door frame allowing the door to open.
    Full Report; assets.publishing.service.gov...
Unedited GoPro footage from cockpit view from our second flight of the day, my wife and I returning from Charlton Park to our home base Fairoaks. 15 minutes into the flight the Baggage hatch latch opens and door breaks away from the piano hinge, it some how catches on the leading edge of the horizontal stabiliser. The loose metal door managed to wedge itself between the elevator fork and the main surface restricting free movement of flight controls.
After the initial impact of the door hitting you can see the AC pitched itself into a negative attitude, we were only at 2100ft on the QNH & started to roll slightly to the right, after a second I regained partial control (accepted 300ftpm level decent, maximum endurance reduced to just less than 4 minutes) to then have a uncontrolled full deflection of the ailerons both right and left. With direct line of sight of the damage from the P1 seat and the old RAF grass strip at Membury visual I did not hesitate to declare a Mayday with Brize Radar and proceeded to line up for R13.
After trimming out everything I dared on the downwind leg the yoke continued to buffer, turning base felt ok but turning final the AC just didn't want to behaviour and I had to fight the AC back onto the centre line, over the motorway & all the way to the ground. With the debris on the wing, it felt like it was trying to stall the tail, having heard other pilot stories I was more concerned after falling out the sky rather than running out of field, if we got there. POH states 96Kt with the door open, I can't imagine an improvised airbrake on the tail improved the performance, coming in hot, 100kts was my target over the threshold. Gear down, couple of hard bounces and roll out onto second field.
Mooney's have control rods rather than cables connecting the flight controls and an articulating tail trim so although in the video the aircraft seems responsive it needed huge asymmetric inputs to hold level, the warping on the right side elevator surface, popped rivets, twisted linkages on stabiliser and sheered metal on the empennage just shows how much force was being applied on the flight controls.
    Wind from 090 - 7Kt
    No significant weather
    OAT - 27 '

Absolutely. I witnessed an emergency landing similar to this, however, the pilot (worried he was running out of field) tried to turn the plan to the right while still carrying a lot of speed and the result was catastrophic. I don't agree fully. He was waaay too fast when landing. If there had been a normal gras strip, he would have overshot the end by far. As my teacher always said: End every flight with a spot landing/ engine failure training - if possible. It’s so important to reduce the energy/speed enough before touching down for various reasons (length of the field, energy when colliding with obsticals, reduce the risk of bouncing (!)). But yes: I don't know if I‘d done better. ;)

I think he was focused on the place he had spotted to land and he was anxious to land there (which is understandable) as fast as possible disgarding the rest. In other words he preferred a chosen target to a stabilized landing in open country. Somehow he was also lucky but he didn't panic and that's very good ... Anyway I was not in his shoes and fortunately everybody was safe, so great job.

At some point I started flying to Asia a lot and I developed a severe fear of flying for some reason, maybe I watched too many of those "air incident" shows. When you know too much about the principles of flight and realize how many things can go wrong, that can happen. I finally got over my fears and am back to visiting Asia regularly and I always keep what chuck said in mind, I never complain about a flight as long as I can walk away from the landing ;)

Please be sure to teach your students that no matter the emergency they should always manage speed and the startle effect, minimize the turns in the descent, prepare the airplane for the off airport landing, try to prevent crossing fields across a road because that is the easiest way to hit something, always maintain airplane control, turns in the descent can lead to loss of control.Once on the ground, kill the engines and secure the mixture. once stopped do not taxi the airplane. Stop shut down and get away from the airplane as fast as possible. Always keep an escape route and plan in mind no matter where you fly or how short your flight may be. Happy landings guys.

Watching this video over a few times, I would say he handles everything exactly as pilots (from USA) are trained to deal with a mayday situation. He evaluated the situation, spotted a place to set down, and then adapted to the situation as needed and flew the airplane (that is the one some people forget to do). I recived a couple hours of power-on wing stall spin recovery training. The speed that you can become upside down is astonishing given the right circumstances. In this case that could have happend if the pilot would have stopped flying the airplane for even a few seconds. I give his performance a 10/10. It could serve as a traning video for how to deal with an emergency.

I know I'm really picking on him a lot. As others have said, in the end he was alive and with a generally intact aircraft. For that he has my respect.
      Review the facts though:
Entered a 45 degree steep turn within seconds after (possibly) loosing a flight control
(almost) "hit the sea of loos" while landing an an airport his landing was so bad. (Was that a drainage ditch he bunny hopped on rollout?!?)
Didn't put the gear down until 50ish feet off the ground on short final
I count 14-15 bounces on landing (Depending on what you count). It usually takes less than three to collapse a nose gear.
His priorities on landing were cussing while dismounting the GoPro while taxiing at 20 Knots.
Any or all of these SHOULD have killed him. He was lucky... not good.
I've personally had a birdstrike and a cabin door open in flight. I know what it's like to hear a "BANG!" and have the airplane jolted. It's not all that hard to stay calm and not kill yourself. FLY THE AIRPLANE.

  • @christopherfranz1547
    @christopherfranz1547 4 года назад +592

Some of the comments here seem to be missing some crucial information from the video.
There was a baggage door that separated from the airframe and was *literally* wrapped around the horizontal stabilizer. Clearly the stabilizer was damaged as well as severely contaminated (think about how the air is supposed to flow over the tail and then think about how the air must have been flowing around the tail with a whole baggage door stuck to the leading edge, and part of the exposed leading edge bent downwards).
Not being party to this whole event, I (and most of us) have no idea how the control of the aircraft was impacted by the event - it seems highly plausible to me that the pilot did not feel like the aircraft was in good flying condition or controllable in the way it should be.
I was taught that you do NOT want to change the configuration of the aircraft if important surfaces are contaminated with ice (or in this case an ENTIRE baggage door) more than necessary. This means limiting flaps. I also understand why the pilot may have decided to not reduce speed too much - in addition to a bunch of other weird aerodynamics, I would have been concerned about a potential for a tail stall if I looked back and saw what he saw. I honestly have no idea what would happen if half of the horizontal stabilizer stalls, and I wouldn't want to find out the hard way.
Think before you comment on these things. There are a lot of factors that could have been at play here. I am certain that there are areas that could have been improved upon, and will be valuable as learning experience, but the Monday-morning quarterbacking is a bit over the top. This was not a simple baggage door flapping in the breeze; while I will probably never know for sure, there is every possibility that there were very serious control issues that made a quick emergency landing seem like a prudent choice.
Especially given that nobody was hurt, and the aircraft was successfully landed, I'm not interested in sitting back with my feet on the desk and talking about how I'd have done it better. I'm glad that Steve Le Van decided to post this; it is a great example of a very unusual type of emergency that I think a lot of us could learn from (and good motivation to make sure that that latch is on our checklist). Thank you!

      @akiko009 3 года назад +26

I believe this is not so much about discussing how to have done it better, but picking apart how the pilot responded, what clear mistakes he made (there is *always* something), and then to try to learn from it to help decision making if something similar ever happens to oneself. I think I would've done some things differently, and made other decisions, but at the end of the day everyone walking away unhurt is the key accomplishment and he nailed that one. Extra points for landing a Mooney on an uneven surface at higher than normal speed, because of the rubber shock "absorbers" that sometimes feel more like shock multipliers.

He should have done better on decision making, at least not calling mayday literally 3 seconds after finding out what happens. We were trained not to call mayday before running through the DECIDE model (or FORDEC for some company) except losing both engines at low altitude. We even do it with red LAND ASAP, it took just half a minute, but usually give you way better outcome. I am glad he landed uninjured, but in my opinion, he took a risky approach in handling the situation.

A steep turn before he's even checked controllability, extending flaps over max flap speed, approaching 30 knots too fast (a buffer was a good idea, but not that much), turning final @ 100' AGL, very nearly a gear-up landing and then putting his head in the cockpit in the flare. For a lot of the time, he really wasn't in control of the situation and really got quite lucky.
Great that he got it down and both walked away and he's to be congratulated for that, but the rule is "aviate, navigate and communicate" and he put them in the wrong order. I'm sure he'll learn from it and hopefully other pilots will also.

Even though they walked away from that one, the emergency landing was mismanaged in many ways. Speed management, they pilot had power so he could have reduced speed and pitch for his best glide speed and bring that puppy down smoother. He had enough distance to stop in the field he chose. His high speed made him overshoot field and land long with multiple bounces and the list goes on. In that airplane he bounced 15 times that is a clear indication of airspeed mismanagement. He did not have the head to do his emergency checklist because he was not mentally prepared for an emergency. He dropped the landing gear late and he did it at around 80kts.

    • @flipadavis
      @flipadavis 3 года назад +27

"With the debris on the wing, it felt like it was trying to stall the tail, having heard other pilot stories I was more concerned after falling out the sky rather than running out of field, if we got there. POH states 96Kt with the door open, I can't imagine an improvised airbrake on the tail improved the performance, coming in hot, 100kts was my target over the threshold."
"With direct line of sight of the damage from the P1 seat and the old RAF grass strip at Membury visual I did not hesitate to declare a Mayday with Brize Radar and proceeded to line up for R13."
So he knew this was an old airfield with lots of room and knew that his control surfaces were compromised so he chose the extra speed intentionally.

Perfection with the landing sequence isn't a requirement in an emergency landing. Getting the plane and occupants on the ground safely is the ONLY mission that counts. I'm confident this pilot knew his plane's flight characteristics better than any of the armchair pilots advising him to come in slower, let the landing gear down earlier, cut the flaps, or whatever other tips are being offered. He was also the only one with his hands on the controls who could feel what the plane was doing. As far as I can see, he didn't miss doing a single thing that ultimately mattered at all. Applause should be what we are hearing in these posts, not criticism. Some of these critics probably also posted negative reviews on Sullenberger's Hudson River landing too!

    Phase: En route
    Nature: Private
Departure airport: Charlton Park Airfield, Malmsbury, Wiltshire
    Destination airport: Fairoaks Airport (EGTF)
    Investigating agency: AAIB
    Narrative:
The aircraft was in level flight and had been flying for approximately 15 minutes when the baggage door opened and detached. It struck the right tail plane and remained wrapped round its leading edge near its tip. This caused the pilot control difficulties and increased drag. The pilot declared a MAYDAY and made a successful emergency landing at Membury Airfield.
The investigation found the safety clip for the internal emergency operating handle of the baggage door was not correctly installed, so instead of holding the handle closed it held it in a slightly open position. During the flight, it seems most likely that this handle moved sufficiently towards the open position to disengage the shoot bolts from the door frame allowing the door to open.
It could not be determined when the safety clip was incorrectly installed or why it had not been noticed.
Damage sustained to airframe: Per the above AAIB report "Damage to baggage door, rear fuselage and tail plane"

I was waiting to get to the end and find out it was staged. When I realized this was real I nearly pissed my pants. Good job staying alive

well,....... stay tuned. People have done wackier things for notoriety. This almost reminds me of a few of the sailing "distress: videos on here. There was nothing near death about this. The airplane did not lose any measure of controlability during all of this. But what makes my antennas go up was their looking back there before anything happened and the lack of running an emergency checklist and a few other things like not using better landing locations, and finally, would you care at all about the camera during a real emergency like this? That would not make the list of concerns. Just puts the antennas up as the MO and technique are similar.

I love the copilot's attitude. That's a rare quality and she was controlling the pilot's mood. He finally broke down and laughed with her, but kept his cool. I didn't see any signs of panic? As to the pilot, it's so easy to criticize him hiding behind those keyboards. I thought he did an excellent job. All this BS about being to hot and no flaps etc.. He was the one with the yoke in his hand. Only he will know how that Mooney flies with a piece of door hanging on the HS. And assuming the door wasn't latched properly isn't appropriate either. Properly closed latches fail folks, it's happened to me.

Panicking can get you killed - stay calm and work out your options - even if it's at 300mph. He played the best card he could given what he had available to him and that landing was dope as. I find it hard to imagine a keyboard warrior would do the same - they'd most likely jump out the seat and start scrabbling around for a parachute...

Great job. You both lived. I think the situation was handled masterfully given the damage to the horizontal stabilizer.
Look at the stab pictures on mooneyspace before you judge (1 cm separation from empennage and twisted airframe!). Control must have been intense
