I've seen some places using GTPAL more often, its uncommon in my part of the world but its growing in popularity. I dont have much experience with it myself. The P could have different meanings from what I understand, depends what method you are using so its hard to comment.
If T stands for term and twins are one pregnant, what is done when one baby dies and ind lives? Is it not gravid is pregnany, but each individual baby is a para? I think you are wrong.

Each baby is not a para. A Para is an emptying of the uterus. A delivery. Period.
