I think Ross was more in shock than cognitive dissonance

You can be in shock and act to alleviate the dissonance at the same time.

This simplifies it. It can be a very bad situation and the person that has this issue completely justifies and has no guilt about their actions.

They don't feel bad about repeatedly breaking engagement agreements because it's never their fault, in their own mind

Exactly stated. Reminds me of the time I presented evidence to my sister that her idol Hillary has been and remains to be a horrible human being.

He founded the theory. He didn't founded cognitive dissonance as a concept or coin the words. This is not a minor discrepancy.

Is there a personality disorder of being chronically dissonant?

      Yes, Narcisstic Personality Disorder.

It's a very common symptom in Cluster B personality disorders. Someone with such a personality disorder experiences constant conflict between their believes and actions, which causes cognitive dissonance
