LOL, I’m 81 and will be having this surgery pretty soon. I made the mistake of watching actual procedures and not understanding, now that this fellow did such a detailed job of explaining the area from three directions, I need to go back and watch the actual procedures again. Thanks for simplifying and making this so clear, so even a layman can understand it a little.
I went to the testing and all I feared was getting erection somehow. Am a dude and I had the hernia at groin near the penis. They was a female who was doing the testing, it was embarrassing I was doing squats and sit ups while I was naked.
after reading articles and med-surg books online for the past 2 hours for a case study... you are the only person who has let me fully understand the difference between direct . indirect inguinal hernia I love your illustrations, it makes more sense especially since I am a visual person thank you
It helped me great much to show in these pictures. I didn't know what teachers at our med school were talking about since my first year Anatomy classes. This 15 minutes solved it all. Thank you.
i get 18/20 for a written test. but i dont have any clue about it. i just memorize all things. now i understand hernia in details. good effort, dude! thanks so much
Thanks....Just diagnosed with indirect on my right side... 52 year old white male with history of manual work... was recently constipated(was this a cause or a symptom? maybe I had already ruptured?) but not chronically, just one day, and was not "pushing" like a madman... this is the first explanation I've fully understood... so these "deep" and "superficial" rings(either end of the inguinal canal) are analogous to the rubber grommets that allow electrical wires/cables(spermatic cord) from your car engine bay (abdomen)to the interior(scrotum via inguinal canal) without letting water(intestines) through... and mine have perished lol... here in the UK a mesh repair seems the only viable option. My one is not sore and lying down goes away itself, and been out 7-8 miles with the dog each day... just got to avoid straining and heavy lifting but getting it done privately asap as the NHS is f**ked with this covid 19 hoax....
Armando, what an amazing demo and explanation of Inguinal Hernia, which is what I was just told I have. I have a consult this Tuesday with a referral from my GP's office, so I've been doing my research on it so I am more informed on what it is and so I can ask pertinent and well formulated questions, should I have them! THANK YOU so much for this post!!!
I had previously watched your various videos, although I am now recovering from a left direct and indirect inguinal hernia open repair, and I am back to your channel. Fit 62-year-old female. Thank you for this anatomical understanding. Excellent presentation!
Outstanding and thank you. As a psychiatrist who graduated med school many years ago, this was so helpful for my brain, just to understand the specific anatomy. I’m seeing a surgeon in a week and likely will need repair of my weaknesses
So... Male, caucasian, smoker with a constant cough and chronic constipation... Yeah, that's me. Was diagnosed with an inguinal hernia a few hours ago, though never told what kind, just that it would be an extremely good idea to speak with the surgeon I was referred to asap to discuss getting it taken care of. Specifically in regards to its interaction with the spermatic cord, I think it's an indirect. Fascinating description of the issue at hand and quite useful in regards to explaining it better than the ER doc did. Didn't think it was a medical emergency, but ended up going to the local clinic's immediate care unit since my doc is taking a vacation this week, then was transferred to hospital ER for ultrasound after doc assumed I may be dealing with the strangest case of testicular torsion ever and no lab techs were on staff that late in the day at the clinic. No pain, just discomfort and a testicle withdrawn toward the abdomen. Anyway, thanks again for the rundown. Makes it much easier for the layman to understand when drawn out like that.
Can you give further details of your treatment please? As i am suffering from a similar problem. There is no clinical evidence of hernia but i do have discomfort and pain at times. Ultrasound suggested that i do have hernia but it is on preliminary stage and my surgeon adviced not to get operated now. Its been around 1 and half year. I am very confused as i am afraid of its link with my sexual function.
Very nice explanation and sketches. Just amazed at how the medical community can figure this stuff out. I had my surgery done 6/24/2024 and my surgeon said he found a direct and indirect hernia in me on the right side. So, I guess you can have both hernias simultaneously. This explanation is by far the best I found (though had to watch it a few times to understand it as I've had no medical training). Thanks to Armando for doing this! You really broke it down nicely for the laymans' community.
I thank you so much for this video. I never understood the difference between direct and indirect hernias in detail. Next week it will be my last exam in med school- so it was time to finally understand! :D thank you.
One of the rare times, when a topic is completely understood by just watching one video! 🫡 Thank you for such great explanation that even after 8 years of upload, this video is still teaching people!
This is the most amazing video on hernias I've seen. Believe me. I'm preparing for my residency exams and during medschool I never got to fully understand this as well as I do now. Your videos are awesome and really, really helpful. :)
Excellent! Thank you for such a concise explanation and demonstration. "Direct" consultation scheduled. Stressing over it, but not as much, thanks to you. Peace.
Amazing explanation and diagrams. Thank you for the education. Just had a double inguinal hernia repair and although my surgeon explained the process, it was still confusing. Your video clarified many things
Made perfect sense to me. Very pleased to have found such a well explained video. I have 2 Inguinal Groin Hernias myself , one on each side of the Groin ( not yet classified into their Direct and Indirect categories by the Consultant ) but at least now I have proper knowledge where I can understand what my Consultant team are talking about. From my viewpoint of both Hernias, the Left side certainly looks like a Direct Hernia, while the Right side, found Yesterday, March 2020, could well be an Indirect hernia. All this actually caused by long term use (20+ years) of Morphine which in turn caused the Chronic Medical Constipation.
I have a small inguinal hernia (right side; direct I am told), to be repaired laparoscopically tomorrow (TEP). My surgeons has done dozens of these procedures, sometimes several per week. I feel more confident after this groin anatomy lecture.
How did it go? I don’t know if I have one or not bu like it’s weird! Like it will hurt for a couple of days and then will be gone for like 2 months and some of the pain is in the right testicle while some is in the lower pubic area! It’s a feeling like heavy or pressure. Laying down is always a relief. Can you let me know if this is what you had! It’s just weird it comes back after like 2 months. No pain when coughing, pooping, farting, etc.
Hi! Thanks so much for making this topic clear. Do you post a PDF of the final product (the notes and illustrations) that you have after you make the video?
I have a minor direct hernia and it does not hurt or bother me at all.. nor has it increased in any way in 4 years when I first discovered it... is it worth getting surgery?
It will get worse at some point. I use to wear a truss, stopped wearing it for two years. Now all of sudden the hernia is awakened and causing severe issue. It is in my scrotum now. Hernias almost always will get worse no matter what. My great grandfather died because of one.
@@jaredontv I've read that small asymptomatic inguinal hernias are better to be handled with "watchful waiting". But you still have to check with your doctor. I know the reply is a year later, but just in case somebody is wondering.

The direct inguinal hernia does not go through the superficial inguinal ring, otherwise it too would follow the spermatic cord into the scrotum. A direct hernia passes medially to the inferior epigastric artery and then pushes against the posterior wall of the inguinal canal.
No, the direct Herna DOES pass through the superficial inguinal ring but the spermatic canal is too small for it to go all the way to the scrotum. So it bulges into another direction.
