I Have a Hernia! (NOW WHAT?)

  • Hernias are one of the most common injuries suffered by men, with over 25% of all men likely to suffer one in their lifetime. That said, when you consider the increased risk in populations who lift weights, the numbers become even more staggering. The question is, what happens if you get a hernia? Are your days of lifting weights over? In this video, I’m going to answer the most common questions about hernias and tell you what to do to fix a hernia if you get one (if you even need to).

First, it helps to discuss a little bit about what a hernia even is. There are many different types of hernias but the one that is most commonly seen is called an inguinal hernia, which occurs in the inguinal canal or groin.
At birth, you are born with a weakened tissue in the lower abdominal wall due to the opening that never fully closes called the inguinal ring. When intra-abdominal pressure rises, such as though coughing, sneezing, or lifting heavy weights, the contents of the abdomen can be pressed against this hole causing it to gradually break down over time.
At the point that the hole is large enough, some of the internal structures, like the small intestine, can be pushed through the hole while still under the skin of course which can cause a dull or sharp ache in the groin area.
Anyone that suffers a hernia when lifting will likely see a visible bulge in the area just to the right or left of the pubic area. The question then becomes whether or not you should have hernia surgery to fix the problem.
If you were to opt for surgery there are two types of repairs that are performed. The first is one that isn’t often done anymore due to its increased failure rate. This is called a high tension repair. It involved sewing up the hole from the outside in. The sutures often break however as soon as IAP increases in the abdomen through any of the activities listed above.
On the other hand, the mesh surgery is something much more commonly done these days. It is called a low tension repair because the hole is never attempted to be closed and the tension on the tissues in the area remains low. Instead, a large piece of mesh is implanted on the inner abdomen that covers the hole and prevents the contents from pushing through the hole. Even if the hole becomes bigger over time, the mesh is large enough to cover the area and prevent aggravating the symptoms again.
I had two hernias in my life.
The first was repaired laparoscopically with a mesh and has been holding up well for the last 10 years.
The second occurred while demonstrating an exercise I was instructing you not to do, the one arm dumbbell row, back in 2013. I have lived with this hernia ever since and have opted to take a watch and wait approach. This is something that many choose to do as well but is it the right thing to do?
If you choose not to get surgery you have to understand the risks and what type of things you need to be on the lookout for. First, you should try and avoid exercises like the one armed dumbbell row. The offloaded arm combined with the position of the knee up on the bench and the back leg behind you creates increased tension in the area while allowing gravity a better opportunity to push the inner abdominal contents down.
A tripod stance on the exercise fixes many of the limitations that come with the performance of the exercise done this way.
The other thing to be mindful of is the use of exercises that greatly increase intraabdominal pressure (IAP) like the deadlift, squat or even leg press. I’m not saying that you have to forego these exercises if you have a hernia. Not at all. In fact, I think that would be a greater mistake in the long run if you did. You simply have to understand that these exercises require greater bracing which increases IAP and places more inner to outer pressure that can increase risk of further herniation.
Lowering the weight on the exercises a bit while wearing a belt can help to mitigate some of these stresses. The belt gives your abdomen something to press into while helping to keep the abdominal contents centralized. The lighter weights will decrease the increases in IAP that substantially heavy weights demand. You may explore alternative bracing techniques as well that decrease the need for IAP a bit by including a focus on isometric stabilization of the rectus abdominis and transverse abdominis.

As a General Surgeon , that have examined and operated on hernias before , I would like to say this is one of the best videos I have seen that explain this kind of pathology and will definitely use it as a reference for my patients. Just remember if you have a hernia , please be aware of the red flags like vomiting (because of intestinal obstruction by the hernia), changes in skin colour like redness , fever , and severe pain , please refer to a doctor and don’t wait , and always remember a hernia can be prevented by good constructive Trainning regiment that helps build your abdominal muscles and core strength. Train safely , train well ✋🏼.

Thanks @Motaz Jalloun for this comment. Do you see often patients having pain on the side of the repair but not in the inguinal canal where the repair was done but higher up, at the level of the bellybutton? I had the laparoscopic repair 4 weeks ago and still feel pain, not in the area of the repair, but higher in the abdomen. Any advice? Many thanks 🙏🏻

In 1996 I had an inguinal hernia in my left groin. Whilst awaiting surgery, I also developed a femoral hernia on the same side. When standing in the shower, I was literally having to cup my hand in my groin to stop either dropping out. Frquently I would have to push them back in. The pain was dull, like a serious ball ache when you've copped one in the nuts, but it was constant.
Eventually i suffered a strangulated hernia, I collpased on the ground in agony and my entire body was trembling, I was rushed to hospital in an ambulance, and passed out from pain.
As a former Rugby player, I've broken fingers, broken ribs, dislocated my shoulder, broken my nose, broken my ankle, and done the ACL. As well as concussions.
Believe me when i say, a Strangulated hernia is the most painful thing you can imagine.
When the consultant (surgeon) spoke to me, he said it was a standard procedure and they would be using a mesh. After surgery however, he said it had been far more complicated and took them an hour longer than expected.
It was something I hoped I'd never have to go through again.
Twelve years later i suffered an inguinal hernia in my right groin, but this time i did not wait and got it done immediately.
Anyone who has a hernia, including you JEFF CAVALIER FROM ATHLEAN-X I would urge you to just go and get it sorted, don't mess around, because if that thing pops and strangulates it will be the biggest regret of your life not sorting it when you had the chance.

Plus hernia is mostly related to abdominal walls so try strengthening the abdominal walls by including core exercises 3-4 times a week.

I will tell you right now, I have my second hernia for about 8 months now. Two things: First, it is up to genetics. If you have that inguinal area weak, it will probably happen at least one in a lifetime. Next, it would never happen if I did dragon flags how they should be done (squizing your butt). So inform yourself about the exercises you do, especially the ones that you intuitively know are maybe a little dangerous for hernia (like deadlifts) and only do them with the best possible form. But I want you to know, even if you get it it is not a big deal at all, just have to watch out a little. Cheers

I’m on Day 3 of recovery. I’m already completely off pain medicine. Don’t be scared, friends.
I’m 43, in good health but nothing special.
I had a right side inguinal hernia mesh repair - and during my examination my surgeon detected a weakness on the left too, so he meshed that preemptively. All done in one operation, laparoscopically. My right hernia was slightly higher up compared to the average inguinal, I’m told, but I can’t say whether that really effected my comfort after the operation.
Day 1 is rough - but not unbearable. No need to be worried. I didn’t even need the opioid painkillers. I stuck to a Tylenol + Aleve combo.
Icing is a must. I did 20-30mins on/off for the first 2 days. Helped a lot. Remember to ice your balls too - they swell. I iced in bed right before sleeping, but didn’t do it again until the morning. I had help, but I kept ice packs in a small cooler by the couch, and in another small cooler by the bed for convenience anyway.
Also, get up and walk around the room for a few of the 20mins phases when you’re not icing yourself - walking sooner rather than later has speeded up my recovery significantly. Seriously, do it Day 1.
Diet is super important: you really, really don’t want to get constipated from the painkillers. So for 2 days BEFORE the operation, I ate vegan/vegetarian meals with high fiber.
The afternoon/evening after my surgery, I only drank plain chicken broth (just liquid) with a little fiber powder in it, with salt and herbs, and a handful of saltine crackers. Felt SO GOOD after the pre-surgery fasting. Also has a nice dose of protein in it, which helps.
Day 2, I upgraded to lentil soup with crackers and some peanut butter on wholewheat toast. Felt AMAZING.
I read someone’s advice not to over-hydrate immediately after surgery until you’ve peed. Apparently there’s a risk of the bladder not waking up immediately after surgery, so by drinking too much too fast, they needed to go to the ER to get a catheter. So I did drink some water in the recovery room, but was cautious at home until I peed the first time.
I started taking stool softeners the evening of the operation, and took them again in the morning and evening of Day 2. I also drank a cup of Smooth Move tea each day.
Day 1, I lay down propped up when I wasn’t up and walking. Day 2, I made myself to sit in a regular chair to get myself ready to sit on the toilet.
I had my first bowel movement in the evening of on Day 2, without giving myself an enema, thankfully. However, I had some home ones on hand just in case. Likewise, I had Milk of Magnesia on standby, but so far haven’t needed it.
In short: hydration + lentil soup + stool softeners is a winner.
I’m on Day 3 now. Trying not to get cocky, but I managed to shower fairly comfortably this morning and I’m off pain meds completely.
A couple of other handy bits:
I was very lucky: my surgeon gave me ‘mobile blood clot prevention’ things that are basically compression socks that have a little motor that squeezes your calf every minute or so. I wore them while I was sleeping and I think they’ve been a good addition. But even if you don’t get those, regular compression socks will also do the trick, no doubt.
Also, after my intubation my throat was quite sore and I often had to cough, which is very painful. A throat numbing spray was a great help.
If you have people taking care of you, make sure they take Covid tests and don’t have a regular cough/cold - you definitely don’t want to be coughing or sneezing during your recovery.
    And that’s everything so far!
I was so grateful for the accounts and advice I found in places like this, so this is me thanking all you strangers and paying it forward. Truly, don’t be scared - it’s been nowhere near as bad as I feared - you can do it, friends!
    Power up!!

After the abuse i gave my body at times because of severe anxiety- alcohol, bad diet, i find it amazing after ditching, and getting back on track- that people generally think i am years younger than i am!

I finally got the mesh surgery this morning . The day before my surgery, I did a 30 minute run, 20 minutes on the rowing machine , followed by 45 minutes of leg weights. I opted for the laparoscopic procedure. Afterwards it felt like being kicked by a horse. Despite that I was up and walking around within 4 hours. No shortness of breath, or major fatigue. I’m REALLY glad I got this surgery while being in good shape. That fitness day prior really helped.

Good video, very informative. Around July of 1969 I was a 19 year old kid in Vietnam serving with the 3rd Security Police Squadron at Bien Hoa. One night we had a rocket attack I joined a bunch of guys who were assigned to load ammo onto a flatbed truck and take it out to the east perimeter. We were to join the troops already there and beef up the defenses. I had been drinking and perhaps because my judgement was off, I lifted a 120 lbs. box of M-60 machinegun ammo by myself. I probably weighed 145 lbs. at the time. We got on the truck and as we were being driven out to the east perimeter I felt a burning sensation in my lower abdomen, and I knew what it was. We were being taken out to fight and I had just given myself a hernia.
For a long time, I kept it to myself. I knew I needed surgery, but I did not want to have it in Vietnam as I knew it would be no frills. I was right. My guts were constantly spilling out through the tear in my muscle wall, and I was constantly having to push them back in. Pretty soon all my fatigue pants had a stain in the same place from my dirty fingers pushing my guts back in. I finally went to the dispensary to see a doctor. I knew I could not carry another man if I had to. After a brief examination, the doctor consigned me to the hospital at Long Binh. Being there was like paradise. I was in an air-conditioned Quonset hut with 50 or so other guys. Most had been wounded but many had suffered varies injuries like me. I learned that there are 100 ways to get burned in combat. Maybe more. Many guys were in bad shape and sometimes I felt like all I had was a scratch.
I was only at Long Bihn for two days then I and 25 or so other guys were flown by C-130 to Vung Tau. There were two helicopter pads where wounded were brought in and triaged. Our huts were not air conditioned, but it was still a great life with nothing to do but lay around, chit chat with other guys, read, and nap. After about a week a doctor came to my bed and examined me. He told me to report to a certain place in a couple of hours to be shaved and that I would have surgery the next morning. The medic who shaved me cut me 14 times. I counted them. The next morning, I had surgery, and I was out of it pretty much the whole day. In the evening I stopped a nurse and told her I needed to urinate. She said "Oh, well go ahead." Thousands of guys had come through this hospital. Many badly wounded, many died there. There was no sympathy to be wasted on a guy who merely had a hernia. The latrine was a long way off and I relieved myself against a sandbag wall. I was in a cold sweat from the pain and feared I was going to pass out but made in back to my bed.
About a week later I and several other guys were flown to Cam Ranh Bay for recovery. Again, I was surrounded by guys recovering from various wounds, burns and injuries. A week later I was flown back to Bien Hoa and went back on duty that night. I was assigned to patrol and a staff Seargent and I got into a fight with a couple of drunk Army troops. Two weeks after hernia surgery is not a good time to fight but my NCO treated me as if I had never had surgery. Now at age 74, my hernia has opened up again. Not much, just enough to be annoying. I'm waiting to see if it gets worse.

Hey, Jeff... I have been catching many of your videos. I am over 55. I have an inguinal hernia. I did not expect to hear that you did, too. I have a long history of being into fitness (33+ years) and I have developed many bad habits. However, I have learned much from your videos. Thank goodness for folks like you. I recently was going back to the one arm rows, as they were a favorite until I just now watched this video. Although the rowing was not the initial cause, it was a nasty sneezing/coughing spell two years ago when the burning sensation began. One important thing you pointed out (that I didn't know) is that inguinal canals don't close up as they exist as a passageway for veins and arteries and seminal vessels through the lower abdominal wall, that makes many at risk for hernias. I have read that there are some physical therapy techniques and exercises utilizing strengthening of the pelvic floor group. Do you have any information to share about these exercises?

I had an inguinal hernia at the age of 29. Definitely due to my line of work, which required me to lift 50-100lbs off the floor in awkward fashion many times a day. It was horrible. Had it for roughly 4 months before I received surgery. Got the mesh net. Before I had the surgery, it was about the size of a golf ball sticking out. The surgery was brutal. Bed ridden for about a month, slow moving a month after. The pain meds cause you to become extremely constipated, but due to the nature of the surgery that constipation leads to the most excruciating pain I have ever felt. No pain med could ever suffice. I had to stop taking the meds. On top of that for the first week you can barely sit up let alone sit on a toilet. Also, had some sort of shot in my spine on the day of my surgery to numb the pain but ever since have had severe back pain now. This was 2 years ago. The scar tissue left from the mesh net feels like there is a pen stuck in my lower abdomen lol not fun. Needless to say I don’t fuck around with excessive lifting anymore.

When I first started doing core training, I watched Rocky 3 for inspiration on how to train because I wanted abs like Clubber Lang. This dude would do a set of sit-ups and finish by twisting side to side aggressively. I tried that very thing and got a hernia above my groin region, left of my belly button. Never got surgery for it as it reduced in discomfort and size on its own gradually, but I can still feel and recall exactly how I got it. Moral of the story - Don’t try and attempt to train like a celebrity in a movie when you first start training. Lots of time it just bs for the camera and it can hurt you if you aren’t entirely sure how to train properly.

Age 36 . Electively opted for a bilateral inguinal hernia surgery done 3 days ago by the laparoscopic mesh repair TEP method. Right side 2cm and left side was 1.8 cm.
Things I considered and asked before the surgery:
    1. Must definitely opt a good surgeon with good years of experience doing laparoscopic surgeries.
    2. Ask what quality of mesh is being used, recurrence rates and complications associated with it.
    3. Ask what experiences has the doctor had with other patients.
    4. What experience does the doctor have dealing with complications from the mesh?
    My recovery process till now:
At Hospital -Day- 0 (night after the surgery) : Since it was performed under general anesthesia , it sort of wears off in a couple of hours and the pain kicks in. Pain was bearable but I couldn't get any sleep the whole night, I was not able to lie on my left side which is my common sleeping position.
At Hospital- Day-1 : Pain had subsided slightly. They did give me pain killers so it wasn't much to go through. Could get out of bed & take a short walk. Will feel a sting while you cough or sneeze, recommend you to hold a soft pillow and brace it against your abdomen to soothen the pain.
You might get your airways irritated due to the way anesthesia was given, the nurse came in with nebulizers to clear your lungs twice a day. Had a catheter inserted, not a good feeling. Was concerned about the pain when someone tries to remove it the next day.
Discharged from the hospital in the evening Day-2 : Pain had subsided a lot. Nurses continued to give me painkillers. Catheter was removed, no pain but a slight burning sensation for a second. I was able to get up and pee, experienced no pain while urinating. There will be slight discomfort because of the inserted catheter. I was constantly told to be up on my feet and take short walks. Left home at 5PM , make sure you get someone to drive you home, I don't think I could have driven home myself. Watch those speed bumps and ask the person to be cautious while driving you home, initial weeks are the most crucial.
Day-3 : At home: Slept well. Minimal pain. Walking around in the house normally doing regular chores.
Complications are less than 5% . 1-5% = 95% HOPE! 95% Positivity. I had severe preoperative stress because this was my first main surgery ,I was very nervous and I was training since I was 18 years old, I did not take a weeks break from the gym, felt totally downhearted.
Looking forward to complete 3 months of rehab and get back to the gym and start from baseline.
God Bless! We Fall We Rise

Glad I saw this video, I'm 300 pounds and started working out and feel pressure around that area and thought that can't be good. So I think I watched this video just in time to not hurt myself. Thank you and keep making great content.

I kept putting off the repair for 10 years.. I thought I couldn’t exercise with it. Then finally got it repaired last year, I’m at my lowest weight in 5 years &’ most fit ever in my life. I wish I could have seen this video earlier.

Hey Jeff, not a regular viewer but found this video cause I got one last week and realised yesterday what it was (thought maybe it was a muscle spasm then it went away when I lay down). I’m calling the hospital tomorrow to get an appointment. I live in Japan so the language barrier makes this even scarier. Good to see that this is not life ending if treated with respect.

What ended up happening? Did you need surgery? I had the same exact thing happen last night.
