That's not even a mask underneath, it's just his face. I'm not sure why it's his face, but any time we've seen him in prison or whatever, he always looks like that. And it's just never really addressed... (My personal theory is that he mutilated himself so that he could better mold different facial structures around his skin. Since originally, he wore people-masks.)
The problem with this series is that a guy swinging a bat can be terrifying. I don't think there's a villain I don't fear except stiltman and that's only because I haven't seen many stories with him.
Hi, Australian here and I must say, Kangaroo is absolutely terrifying. Been in Australia for 18 years and kangaroos are very very very good at hopping in front of your car and getting hit, so be careful around Kangaroo as he could easily hop in front of your car and cause lots of damage.

The Thousand would be not only be scary in real life, but absolutely horrifying. Imagine sleeping in your bed and this thing crawls into your mouth and takes control of your everything (body, mind, soul). I just realised that it also has abilities similar to Chameleon; posing as someone trusting, someone you know, someone you love.
I was gonna comment that Prowler is not actually a villain (he reformed after his first appearance in the main continuity), buuuuut his Ultimate counterpart *is* a villain and a major figure in Miles' life, so I guess he still counts^^' Regarding villains you could try... I don't remember, did you do Jackal? Or what about the Tarantula?
Drowning is one of the worst ways to go, and the second guy is all water, which also means you can't physically hurt him. Hell yeah he is terrifying! Also picture this: you're drinking water one moment, and the next thing you know your lungs explode or you get gored from the inside. Scary stuff man.

If you run out of villains do alternate universe Spider-Man villains they are super scary like Spider-Man 2099 or Spider-Man noir their villains are terrifying
