    ITear seems like something that should cost 150$ ONE TIME ONLY!! Since dry eye is most of the time for life, they should be ashamed to charge people since their product cant cure the problem.

    I just found your videos. They are great! Thank you so much! I was diagnosed with MGD last year when my eyes blew up outta nowhere. I had lipiflo (sp?). Useless! Then I was given a sample of Tyrvaya. I was like a crack addict, trying to get my hands on more and more of it because the local ophthalmologist could not get my insurance to cover it. I finally had to get an appointment with a renowned corneal specialist at Wills Eye Hospital. I was prescribed Tyrvaya ($89/mo w insurance), restasis ($10 month), and meibo ($0 from
    manufacturer). I was doing fine (except my right eye would tear for hours every morning) since December 2023, but yesterday my right eye blew up! I went to UAB’s eye hosp ER, and they offered no help. Said it’s just a flare up. So I researched and found the itear100. It was easier getting a script (online) for this device than for Tyrvaya. So, I ordered it and now I’m
    eagerly awaiting its arrival.

    America is crazy that anything can cost $500 for medication. Nothing really more than $30-$50 in Australia. Most drugs less than $15.

    Tyrvaya is the only dry eye Med that has worked for me. I get it for $10 through the company and it’s been great. Highly recommend

      Hello. My husband has been on Xiidra for a couple years now. It helps him. He just got prescribed this nasal spray. Should he try the nasal spray without the Xiidra being that both is almost a thousand dollars a month & wouldn’t it be nice if he could just use 1 or the other?? Also would it help to both though? He already had had ipl done, Lipiflow done, & he does have the Punctal plugs.

    Sorry am i missing something, but if all the Itear100 does is vibrate at a specific frequency range, then why shouldnt I just make my own device that vibrates at the same frequency instead of paying a monthly fee for something that doesnt even actually need to be refilled?

      Yes, it's a scam. Fortunately this appears to be just a sonic toothbrush in a custom case, so you don't even have to build anything. You can just buy any cheap sonic toothbrush. (Frequency of itear100 is 270hz which is pretty much identical to sonic toothbrushes)

      @@DrEyeGuy I'm on Tyrvaya and love it. Tip: put your finger under your nose/across your top row of teeth, and press. That should stop the sneezing. I sneezed when I first started Tyrvaya, but I don't sneeze now. I'm not sure why.

      A lot of dry eye patients indicated that they had improvement in their sinus symptoms so the company performed and just completed a trial for sinusitis showing results comparable to currently marketed products. It is currently going through the regulatory process for this indication . Stay tuned.

      @@DrEyeGuy I will continue to use Tyrvaya until I receive Nulids. If the device proves to work better than a prescribed chemical, I will stop using Tyrvaya. Sorry for the delayed response. You are very helpful with your advise about dry eye disease.

    Is the itear just basically a vibrating device? Couldn't you hypothetically just use anything that vibrates to stimulate that nerve/just massage that area with your fingers to stimulate it? I get it won't be as intense perhaps but maybe that's better for those who are prone to light sensitivity and migraines? I just massaged that area on my nose myself and my nose def feels tingley like a pre-sneeze sensation and I feel like some tension in my head has been relieved a little. Can't speak about eye watery-ness because I used eye drops right before 😅 but I'm gonna keep trying and see if it helps!

      Might recommend you talk to iTEAR100 company. there is a usability trial with intensity matching to patient individual sensitivity. no office visits required. virtual appointments. goal is to allow the nerve stimulation without illiciting effects like this.

    Is Tyrvaya good for MGD? How does it directly help the meibomian glands? Does it thin out the oil or increase oil production?

    Do you have any other at home ideas that you think could work instead of buying this?? It makes no sense that the company would charge a monthly fee! This is literally a vibrator.
    Do you think there is long term likelihood of your body getting used to this and it not working at all?

      Excellent questions! I actually did a video about my experiment with a vibrating toothbrush, using it to stimulate the trigeminal nerve. If you haven't checked out that video yet, it was actually pretty surprising, as I had pretty good results from it. Also, yes, I do think it's possible for the body to get use to it, or at least the treatment might become less effective. I'm not sure what the percentages are, or the likelihood of that happening is.

      "vibrator" is a broad term. Vibration variables include frequency, force, amplitude which are coupled with the material and the interface with the skin to create a pressure on the skin in a specific repetition rate. A jackhammer is a "vibrator" too but you wouldn't apply it to your body. Different nerves have different responses to various frequencies and forces. The external nasal nerve might not respond the same way and to the same frequency as other nerves. the iTEAR100 was optimized over 12 prototype cycles to stimulate the external nasal nerve and very little collateral skin or bone around it. The retractable tip turned out to be a key safety feature.

      its a prescription product under CFR 801.109 as the Dr. states. we are legally required to turn it off at 30 days and your Doctor can re prescribe it. This all requires overhead and management of the prescription which is what you are paying for. The FDA RX label means that the company is held to high standards as far as ensuring the vibration parameters match the clinical data.

    I use Tyrvaya, but my even with my Medicare Advantage, it costs $170 bucks from my pharmacy. Frankly, I think the toothbrush works better (certainly as good as) Tyrvaya. The iTear wants me to jump through too many hoops to keep using it. I can buy the Sonicare once and use it forever.

      @@DrEyeGuy Yes, for a few minutes. Best relief is drops, heat and blinking exercises, but massage helps if I am not blinking enough. I really wonder about the longterm effect of irritating a nerve though. Thanks for the great channel.

    I don't really understand the recharge fee on the iTear 100. To me, an ongoing fee for just a simple vibrating device is a major downside.

    I have the nu lids device and feel it is just "ok". But once I heard about this new itear100 I want to try it. But, I started putting my nu lids device on the side of my nose like you did with the toothbrush and I feel like it may help a little? But I am not sure, since I do not have the tear device. Am I crazy to try this? And or what do you think of the idea. Thank you.

    How is this different from using a silicon vibrating derma cleanser on the face? They can pretty easily target that area and work at various intensities rather than a toothbrush?

    Would the long term use of artificial stimulation of the trigeminal nerve potentially lead to down regulation of natural stimulation ? And In theory would stimulation over time potentially lead to onset of chronic trigeminal neuralgia ?

      Excellent questions! I feel like this type of question can be applied to many new dry eye treatments. With repeated IPL treatment, does that have any longterm bad side effects? We don't know yet unfortunately.

      In several years of data and thousands of treatments, this has not be seen. It is alway difficult to prove a negative just like we can't prove it doesn't cause cancer or heart disease but the probability is at least very low

    MY INSURANCE WOULD NOT COVER THE TYRVAYA. IT WOULD BE $600 DOLLARS. I JUST TRIED TO GET IT TODAY. I DON'T EVEN KNOW HOW TO GET THE ITEAR100 MY PHARMACY DIDN'T SAY ANYTHING ABOUT THIS BUT IT IS ON MY DOCTOR'S PRINT OUT THAT I RECEIVED TODAY.

      The tyrvaya manufacturer has a program where your cost will be $0 or $10 per month, if your commercial insurance doesn't cover it. If you have medicare (like me) or medicaid, you can submit an appeal if you have tried drugs like restasis, but it didn't work. Your eye doctor has to provide a note saying you've tried restasis, or other drug that didn't work. I was approved promptly.

    None of these videos seem to disclose the Psychiactric side effects of the medication which is very disturbing.
    Psychiatric
    Very common (10% or more): Insomnia (up to 19%), abnormal dreams (up to 13%), depressed mood disorders and disturbances (up to 11%), irritability (up to 11%)
    Common (1% to 10%): Agitation, anxiety, depressed mood, depression, hostility, middle insomnia, nervousness, nightmare, restlessness, sleep disorder, tension
    I'm not saying don't try the medication, but I feel people should be informed before deciding to.

      Awesome comment! These side-effects were found in chantix (same medication as tyrvaya). The initial studies of tryvaya (lower dose than chantix) didn't find these side-effects. But this comment is great because it is good to know. Sometimes these details aren't found out until the medication is prescribed to a much larger population of people than just the trial studies.       I am using Tryvaya, got a sample from my Ophthalmologist. I am experiencing insomnia, and anxiety. None of this was mentioned as a side effect in the literature that came with the medication.
      It is helping a bit with tear production, as long as I hit the right spot in my nose.
      I may opt for the external stimulation device due to the side effects with Tyrvaya. The insomnia is a big problem.

      @@jeanneschlumbohm1138 I'm sorry you are experiencing those issues. I opted not to use my Tryvaya as to avoid the side effects. There isn't enough literature or tests out there, and I don't want to be a guinea pig for it. I already have anxiety and am nervous that it could increase it. Definitely look into the iTear. I'm consiering it. It's just a pain to get a hold of because they require a prescription for it, so it's a whole process.

      Thank you for bringing this up. I had to stop Tyrvaya as well due to (I'm not sure of the phrase off of the top of my head) a general "numbing" of any feelings. This even led to temporary erectile dysfunction. It went immediately into the trash can and a week later I was back to normal.

    I've been using tyrvana for a few months and see no difference. But, when I use it (even when I don't sneeze) my nose runs and I'm concerned the med is running out of my nose! Anyone else have this experience or know of a solution?

    Hey Dr. I'm 35 now, a few years ago my Schirmer test was a 2 and 3, people always think I'm completely stoned because my eyes are often glazed and red...Would you recommend these options over NuLids? (seems easier and more cost effective). And is it worth it to shell out the money for Lipiflow before starting one of these maintenance options?? Thank you

      Hard to say... I am using Nulids right now and I love it. Lipiflow, or IPL are great options, but I always recommend at home treatments like these with or without IPL or lipiflow.

      Don't waste your money on Lipiflow. It only lasted for a couple of weeks. I paid $900 and it was a waster of money. Save your money.

    Got it today, my doctor said not to sniff it once I spray it but I don’t see anything on the instructions saying you can’t sniff it. Anyone knows?

      Correct, the idea is you want the medication to hit the inside of the nose. Also if you sniff and inhale then you could get a lot of it down the throat and cause some irritation.

      Excellent Question. It can have lasting effects, but I think it depends on the person. Hopefully with time we learn more about the long term effectiveness.

    I'm wondering, since this drug stimulates the trigeminal nerve, will this drug work for people who have tried other methods to stimulate that nerve that have failed? I have tried both TrueTear as well as your toothbrush method and they have both caused me to feel like something is watering or something is happening but the relief is short lived and never really made a difference for me. Are you seeing Tyrvaya work for people who haven't had luck with previous methods that stimulate the nerve?
    Also, are you seeing people build up a tolerance? I tried your toothbrush method and after a few days I seem to build up a tolerance where I get less of a reaction, but coming back to it after a few weeks / months I feel a reaction again, even though it doesn't provide much long term relief. I would suspect since this drug works on the same nerve it may also work well initially until a tolerance is built up.

      Yes, all methods seem to build up a tolerance to it, but it still works at building up the baseline tears, with less of a reactive tear response, if that makes sense. If you have no reaction with Itear, then likely you won't have a reaction with tyrvaya.

Thanks for confirming my suspicions about Tryvaya, I'm not going to hold out hope that it's some miracle cure for me given my past experiences in trying to stimulate the trigemenal nerve.

      Really need a 2-3 week trial with consistent usage to know if neurostimulation is working. there are immediate effects and then changes in basal tear production which require more time. The toothbrush was not designed to stimulate the nerve but might stimulate the nerve in a gross way, like a punch in the face might. Because the force it's applying is less specific and much larger over a larger area without a retractable tip like iTEAR, the tearing effect might go away quickly and you will be left with a lot of nose pain.

But it's all fun and games until someone loses an eye. Random people on internet giving medical device is frightening.

    Does this work well for clogged meibomian glands? I don't have a problem producing tears but my oil is very thick and my glands are totally clogged.

      Research does show that it can help the meibomian glands.... I don't know to what extent they would help clogged glands though. I would likely be reaching for other specific treatment for that.

    My first question is do I have to see my priority Dr to be referred to a dry eyes Dr? And second question I have is I have medical and my insurance gives me trouble getting drops. Do you know if medi cal will cover any of these treatments if I can't afford out of pocket monthly costs?

      You can just go to an eye doc that works with dry eyes.... at least I'm pretty sure u less your insurance is really picky about that. I don't know about medi cal specifically, but it general it's commercial insurance that has been better about reducing it down to a low copay. But you could check the tyrvaya website and see if you can get help that way. With Itear, insurance isn't involved, so it's all cash pay.

      @@DrEyeGuy I have medicare drug insurance. My insurance didn't cover tyrvaya, as it was not on their formulary (that did include restasis). I appealed, and my tyrvaya is now covered.

      @@DrEyeGuy thank you. I got dry eyes, map dot dystrophy and glaucoma so a lot going on. I been trying to use another treatment than alternative tears and can not find another approach. Thank you for your videos they are very informative.
