    Hi I just got my t slim a little over 2 weeks now I love it I've not felt this good in 2yrs last year I was in and out of the hospital alot in mid October I was in ICU for 4 days with dka I didn't know how bad I was till the fourth day when I was getting ready to go home. Lucky for me a nurse I had I knew I coached her daughter in softball and she was one of my daughters friends. This nurse told me about the dangers of dka the next week I meet with my endo doctor and we decided to go with the tslim best decision I've made still learning but really I've been sick for 2and a half years doctors don't know why but I get nauseous and start throwing up to the point I dyhright and my blood sugar levels go through the roof like 400 plus. Long story short we decided to do a insulin pump diet change bc I lost 130 lbs in 10 months. So in the short time I've been on the pump I'm eating 2 meals a day with 3 to 4 snacks. Like I said earlier I've not felt this good in almost 3 years.

I'm a type 1 also..... LADA! I call it type 1 for older people! We have Control IQ here in the US but can't use it until after all my training. Thanks for sharing!

    Michelle i Am in the U S and I just got my control IQ and you will love it as it helps with the highs also and I love the fact that I generally get to sleep all night all the time once I corrected some of my settings. So far I have only had it a week. They do suggest you download it when you are getting ready to change your infusion.

    I remember when my A1C was so bad. It was around 14 or so... I felt really bad because of it, and now because of Control-IQ, it’s now 7! The device is amazing and worth it, but the only problem is when the device dies. When it does, the pump actually makes you change your pump and sensor. It gives you a “Welcome” screen and everything. The pump is worth it overall in my opinion :)

    Hey Michelle!! I just recently discovered all your videos since I'm in doubt whether to choose the t:slim X2 pump or the omnipod dash. I'm currently wearing the omnipod, but the new feature of connecting the Dexcom G6 to the t:slim really makes me curious. However, since I'm only used to a pump without a tube I do not know what to do... Do you think the t:slim pump connection with the G6 is greater than the fact I will be connected to my diabetes literally?? I would love to hear from you 😊

  • @worthybookjourney3231
    @worthybookjourney3231 4 года назад

    You are exactly right about basal stopping some lows but not all. If it is too much of a decline towards low, it doesn't work. And as soon as you make the least bit of a turnaround, it will come back on even if you are still low. If you drop again at all, it turns insulin back off. That is their attempt to keep you from rebounding I hear. But sometimes, it can keep you low longer if it turns on too soon. Also, the G6 loses contact with the pump. My son's lost contact for an hour already today. If it loses contact, you get alarms, and Basal IQ is not connected for however long. And they say laying on the G6 (as in sleeping) will cause it to lose contact. Ours does it plenty of other times as well. So I would rather have it than not, but it is far from perfect. Control IQ has a similar set of issues. If you go low and treat the low you may start rising rapidly due to the treatment. Control IQ sees that as going high and will increase basal rates. But you can go in and turn off Control IQ until you get things straightened out and then resume Control IQ to counter that issue.

    • @MichelleLord
      @MichelleLord  4 года назад

      I agree - all this tech we have is far from perfect, but it certainly helps :). I've been watching out for the connection issues you were previously warning me about, and I haven't experienced many yet...I've been keeping my pump on the same side of my body as my Dexcom. I lost connection once when it was on the opposite side. I also turned off my alarm on my t:slim for connection loss (we have too many alarms as it is as T1Ds 😝)

    Hi Michelle! Just one question, do you think that you woke up at 216 because Basal IQ suspended your insulin delivery?
    I'm switching to your Insulin Pump (T:Slim X2) because of my regular lows, but I'm really afraid of having high sugar levels... You know, we all wanna have perfect levels and even though I'm having great levels lately and not as many lows as before using Freestyle and now, Dexcom G6, I'm still afraid of having high sugar levels because of this new pump Basal IQ...
    Thanks for your channel!! You're doing great!! Cheers from Spain!!

    Like you said basaliq is helpful in some situations not all of them, when your blood glucose goes straight down (over bolus for example), it will not help much. Like in the add : Basaliq decreases frequency and severity of hypoglycemic episodes. I like it , next to dexcom, insulin pump helps to keep my blood glucose to stay longer in range which i turn helps with keeping my life healthier. I’m been loving Basal IQ for overnight lows I’ve been having the best nights and remain stable at about 4.5! The only issue I’m having is when I’m working: I work in a restaurant and I always have had issues with getting bad lows at work lows that usually stay for a few hours! My diabetes team suggested that I turn off all basal doses while at work to help prevent the lows but since I’m constantly in movement I still remain low and with Basal IQ on it’s great cause it stops insuline delivery automatically however whenever I finish work and get home and take off my pump to shower I spike really high because I’ve been off insuline for up to a few hours thanks to basal IQ and my constant lows! It’s been a struggle but I do still enjoy this new update!

    • @MichelleLord
      @MichelleLord  4 года назад

      Hmmm that is tricky and definitely a downside of Basal IQ. There is always the option to turn off Basal IQ if it is suspending your insulin too much? This technology is far from perfect, but I'm glad you are still enjoying the update overall 💞

    It's helped me stay in range more because I use fiasp and have feared taking a full bolus sometimes because the drop is so steep. Now it drops quickly and then eases me perfectly in range so I'm not scared of taking a full bolus.

    I hate the auto mode in my 670G I don't bother with it. I just have the smart guard that disables insulin if I drop below 70. Anything else I feel is better done by hand, the CGM is OK but does not measure BG and there can be a big delay in treatment. If these pumps used actual BG they would be so much better.

    You need to watch the calibration deviation of your Dexcom. For example if you’re Dexcom is continually measuring interstitial fluid 20 mg/dL blow your actual BG then you need to be alert to your Basal-IQ. It may turn off your insulin when you don’t want it to and your actual BG starts increasing because you’re not getting enough insulin. You may want to turn off your Basal-IQ off temporarily or decide to manually calibrate your Dexcom every 24 hours instead of having it automatically calibrate.

    • @MichelleLord
      @MichelleLord  4 года назад

      Good point!! Basal IQ relies on Dexcom accuracy. This is definitely something to watch out for.

    Great to hear bIQ is working for you. I tried it and it just wouldn't work for me. It stopped the basal but I still had hypos and I then went high later even if I didn't treat the hypos much, in the end over the time I was testing my time in range dropped to half what it was without bIQ. I have delayed insulin absorption so I think this means it just wont work for me. Tandem support worked with me but in the end agreed that I should turn it off. Fingers crossed for Control IQ when it arrives in the UK!!

      Control IQ works great for highs and lows and if your settings are correct it works like you do not have to even have a pump it just does everything. The settings are the main problem when it is not right.

    • @jonrogers3760

      @@gastrogal5353 Sounds like you've got a challenge on your hands with the gastroparesis. That sounds like the opposite problem to the one I deal with but I totally relate to the feeling of if only I didn't have to eat. I basically bolus and then watch the CGM trace until it falls - which can take anything up to 8 hours - before I eat. Then as the insulin gradually continues to absorb I tend to get a series of dips and hypos for another few hours - then I need to eat again and the cycle repeats!
      I've also tried the medtronic 670 but the CGM on that was useless for me, didn't keep calibration, lots of gaps in the data and sensor errors and generally just a very poor experience. I've been using Dexcom for the best part of a decade from G4 on and it has always been far better than either medtronic or libre when my HCPs have got me to try other things. Last time I tested medtronic CGM I wore both medtronic and G6 at the same time, absolutely no comparison in me. I'd say it's definitely worth giving G6 a try...
      Happy to chat in more detail if you want but don't want to hijack Michelle's video comments, let me know what's best for you.

    I'm in Canada and my 9yrs old was diagnosed last year. She is doing MDI with Libre but we would like to have her on a pump. Unfortunately insurance has told us they will only cover $700/lifetime for a pump but will cover the monthly accessories. My understanding is that if we were in the US $700 would cover the omnipod PDM but here in Canada all the pumps are $6000-$7000. So far our insurance will only cover the Libre and not the Dexcom. We're in NB and medicare doesn't cover any of the costs and neither will the "NB Pump Program". It's incredibly frustrating! Omnipod has also said that they won't sell us pods if we purchase a PDM second hand. I know you've been living with T1D for many years in Canada but in Ontario, do you have any insights for me?

    • @johnedwards5575
      @johnedwards5575 4 года назад

      DCARA06 that’s incredibly infuriating. I’ve heard similar frustration with provincial programs in Manitoba. I’m MDI for 34 years, since I was 9, and though lived in BC until recently, I hadn’t explored pumps there. In Ontario now it seems Libre is covered if over 65 ( cheapest at Costco or Walmart if you pay out of pocket) and pump coverage is available once every five years I believe. Through private insurance (Sunlife and Manulife) I have full coverage for Dexcom, Fiasp and 80% of Tresiba. From what I understand of pumps, they still require a fair bit of tweaking. I’ve posed this question to lots of T1s and educators the last six months: if you had only one to choose: Dexcom or pump? Universal answer has been better benefit with the Dexcom, and for me, it has given phenomenal control I wish I had for decades. In fact, I’ll double down saying it is far more accurate and helpful than Libre ever was for me for the two years I tried. Any option to switch insurance plans?

    • @DCARA06
      @DCARA06 4 года назад

      @@johnedwards5575 Thank you. We are with SunLife after switching from Blue Cross. Right now we have 0% coverage for a CGM but 100% for Libre. I do want to switch to Dexcom if possible to get coverage but I will miss being able to scan her while she is swimming without needing to worry about Bluetooth connectivity and that she doesn't need to have her own cell phone, With Dexcom she would have to carry a cell phone and have a data plan, at 9yrs old that wasn't something I would normally get for her. She's not yet mature enough to keep track of devices especially while outside playing, swimming, etc. let alone to be withing Bluetooth range. BUT I would LOVE LOVE LOVE to see her blood sugar while she is at school, the sitters, a friends, etc.

    • @johnedwards5575
      @johnedwards5575 4 года назад

      DCARA06 I wonder if there is something that works about as good as Dexcom. I’ve heard of something called Miao Miao that sits over the Libre to make it more like a CGM... using Bluetooth. But then, if say she had a cheaper iPod, any available Wi-Fi would be able to pass along sugar readings. I’m not certain a full fledged cellphone would be needed. I’d also highlight if she has some aware friends, and adults, that goes a long way to ensure she’s doing okay. Thinking back to my time as a youngster with diabetes, I had some candy in my pocket, kids and parents knew... only times I packed a blood meter (that took 60 seconds to blot and wait for results) and needles with a vial of insulin was if I was going out for a meal. My concern now after scarily going through it, for any T1s are severe overnight lows when sleeping. Miao Miao might work, until the Libre2 is available in Canada.

    • @DCARA06
      @DCARA06 4 года назад +1

      @@johnedwards5575 Yes, you're right. I have been thinking about the Miao Miao and probably should try it. I don't know anyone in Canada who has but it's worth a try. We just got an update from our employer saying they are going to try a pilot program that will cover the Dexcom G6 $2000/year which is less than half I think & currently they pay more for her Libre so we might just stay with Libre.
      Thank you for your help. :)

    • @johnedwards5575
      @johnedwards5575 4 года назад

      DCARA06 Best wishes that it works out. You probably want to confirm if Miao Miao would work with just an iPod however. In theory it should work, but sometimes this stuff has awkward hoops to jump through. For Dexcom cost, if you had a phone, then cost would be sensors plus a new transmitter every 3 months. With transmitters at $295 a shot, it’s pricey. However... there is a $1200 saving per year if you go for the subscription. 9 sensors and 1 transmitter sent to you every three months for $299 a month. For the cheapest Libre prices? Walmart and Costco. $90 a sensor. Just some numbers to keep in mind.
