      I just learned about your channel. Thanks for keeping things real.
      Please do a video about insulin resistance. Most skinny fat people are IR and when they try to cut carbs they end up hypoglycemic. So it becomes unsustainable. Even if they try to eat fat for energy.
      I changed my physique doing the same you explain, but I can not do the cutting part because I pass out if I reduce carbs too much.
      Trying to supplement to fight insulin resistance and eat good fats and even use ketones. But it is very complicated.
      Also. It seems I reached a plato in weight lifting. It looks like my frame can't go over it. May be doing something wrong. IDK.

      It depends, if they're doing it at low doses and his levels are not supraphysiological it's not that big of a deal, if he's blasting TRT then sure, it probably is the main cause of the transformation.

    ~95% of the time, the doctor isn't the problem. primary care doc here... in general, doctors working for major health systems (i.e. Kaiser, Providence, Ascension, etc) are given 15 min appointments for follow up appointments (meaning anything besides initial appointment) and 30min (sometimes up to 60 min) for initial appointment to establish care. These appointment times do not allow for meaningful discussions on fully optimizing a patient's health. Majority of the time, we have no say in these time allotments, we just show up to work and deal with these awful circumstances. For those that care, please complain to your insurance companies and health systems- they are the ones who have the power to implement adequate appointment times.

      This is very true, insurance companies have positioned themselves as blameless and put doctors as the face of everything wrong with healthcare. Then comes along a charlatan like Gary Brecka “human biologist” 😂 (as well as so many other health gurus) that just trash the medical field. It’s a really old tactic that’s worked forever because there is never a shortage of suckers. It’s really unfortunate for doctors like yourself to deal with this type of shit but it irritates me so much how not enough people call out these fake doctors such as Mr. Human Biologist Gary

      Thank you for clarifying how doctors are working for health systems that present the business model doctors have to plug into.. which is why doctors aren’t able to get into a deep dive on one’s health
      Just 10 mins and here try this ssri cya in a few wks seem like the norm..
      How do we as patients do better for ourselves? Can we find a doctor that works private practice? An MD not handcuffed by Kaiser of a large healthcare system? What kind of doctor can we look for that will look at the sources of illness versus treat the symptoms?

      @UCel7DNTir4CHFy7_FpfNeyQ you definitely can find a doctor that works private practice. This would probably have to be through your insurance though or else you’d have to pay out of pocket (again this is another thing doctors get blamed for but it’s insurance companies that make this so tedious).
      I can say all good doctors want to treat the illness and not just the symptoms even though it may not always seem that way. Health gurus and “Wellness” people love to use this as a talking point but it’s not true.
      It’s one thing for doctors not to have time to do a deep dive in your overall health but it’s another to purposely not treat the illness. What makes you feel that way?

      @@user-ve9wm8xs7z unfortunately good primary care that includes enough time to collaborate with patients is often limited to private practice which are often not covered by insurance. If you have the money, a direct primary care practice is the way to go. Unlimited time with your doctor for a monthly fee (usually 75-100 per month). It's sad that this kind of care is not available to people of all income levels but sadly that's what happens when healthcare is delivered through a capitalist lens. If you can't afford a direct primary care membership then I'd recommend coming to your appointments with a list of your health priorities (diet, exercise, mental health, etc). That'll at least help focus the little time you have with your doc on the things that are important to you. Best of luck to you

      Huh? WTF are you talking about? His T levels aren't a big deal because they have controlled them. They are uninteresting to review because an injection is keeping it steady and in the healthy range. It's not going to be drastically affected by his diet and exercise regime.

    Residency trained pharmacist, we got a TON of patient counseling practice in school and during my residency programs (basically pharmacy is knowing medications and either advising a physician or a patient). Working at the VA and trying to help people with heart failure, they don't want to walk or move or go to the bathroom, so they get worse. Trying to help people with diabetes, they don't want to count calories or be remotely active, so they get worse. Spent 1 hour at a time with 7 people a day trying to educate and prepare them to take care of themselves and to facilitate results now, learn their medicine; they just don't do it.
    I can't do patient counseling anymore from seeing people come in for yearly checkups, still smoking, still complaining about being able to breathe or move around their house, gaining weight, blaming their pills... this even happens in private hospitals too. Now I compound at a local shop, basically only end up seeing people that need specialty meds and they are much more receptive than the general populace.
    It's not that doctors or nurses don't have the time or don't like you; it really just gets awkward to keep people's attention and rapport up to convince to change the way they are LIVING. Lifestyle change is paramount to the habits that have been created without any conversation or forethought. What order you get ready in the morning, how you leave the house, the way you drive, how you interact with people. It's beyond hard to change, let alone convince somebody that they've been doing it wrong and need to change. Despite what any contrarian can say, everybody wants a magic pill / silver bullet.

      The people you're describing sound pretty much like my dad. The amount of times he's been advices by doctors, dieticians, his kids, etc. and him making almost no effort to actually do it is ridiculous. Sure he would try it for a few weeks but then he'd quit because "it's not helping". Like mf you been eating and living like crap for YEARS and now you expect to turn it all around in a few weeks?
      And the fad diets.... Ugh.

      So much this!! Nurse here. Patients get so defensive about lifestyle modification, but are very quick to blame "all these pills" for them feeling poorly. Side effects exist, medication interactions exist, but VERY few people actually want to put in the work to be able to come off their meds safely.

      Every doctor should be prepared to never abandon the patient, but in college you learn just not to abandon the illness. When the patient does not adhere to treatments for the disease, it is as if the doctor is no longer able to monitor them. The doctor who was trained under the illusory concept of having power over the disease and the patient's actions is condemned to feel like a failure and frustrated at various points in the career. However, the doctor who seeks knowledge about "caring" with the same commitment and dedication that he takes to “cure" is a human being in permanent fulfillment.

      I totally see where you are coming from. I work in physical therapy. However, there is a huge disconnect between doctor visits and patients truly grasping what it's really going to take to make the necessary lifestyle changes needed to live beyond the next 5 months. I've always been a huge advocate of preventative health care, which is essentially what proper diet and exercise is. But for patients looking to get their numbers under control, from weight to triglyceride levels, what does this actually look like in an exercise and diet program? I honestly think that exercise scientists who have the capability of prescribing specific programs based on need should be part of hospital and doctor office staff. I believe the disconnect between doctor visits and the needed lifestyle changes could disappear with prescribed programs. Hell, make a partnership between doctor offices and fitness centers. Incentivise the whole process. But, it's hard to know how and to whom to make such suggestions.

      TRT is testosterone replacement therapy. Injecting or consuming testosterone to maintain and boost testosterone levels in the normal male range. Testosterone decreases slowly after a certain age (30s)

      people like you are idiots. He's over 50, so testosterone replacement is LITERALLY needed for like 70% of men over 45. Some as early as in their 30s. The current state of our food+work environment literally kills testosterone, and men are at lowest testosterone levels in history. Thats literally number 1 cause of everything bad AND good. So no shit if his test levels were low he should take replacement therapy, doesn't mean he's wanting to be "jacked". He just wants to look like a normal HEALTHY 50 year old guy, not a couch potato with no energy. Hater idiots.

      What do you think a man in his 50s can change for low T? There is literally nothing a man can do beyond TRT if it is too low. Diet and exercise will have minimal impact. Just taking TRT isn't going to get anyone jacked on its own. He still had to hit the gym and watch his diet to get results.

      @@DadeMurphie TRT wasn't even mentioned in the initial comment. There was a whole section in the video about doctors prescribing pills for everything, because it's unlikely people will just get their shit together on their own. Sure, when getting jacked in your 50s is your goal, there's not really a way around TRT. But losing 35 pounds and generally leading a healthier lifestyle is 100% on the table without any supplement at all.

    As a healthcare worker I agree with what you said about pills. Most doctors don't like putting people on pills and would rather it only be a temporary measure or not done at all. However, trying to convince the 270 lb 30% body fat patient that he needs to exercise more, takes a lot more work than can be done in a 15 minute doctor's appointment

      the biggest problem with pills is a pill's quick effect.. which in turn pospones any lifestyle changes you may be considering at a given time creating a bad habit. anytime i've been on meds even for depression a few years ago, a depression that caused me physical pain which i found really weird at the time, i took anti-depressants only until the point i was able to manage mentally and then quit, was told it would take me at least 8 months i quit them within a month never been on again. pills do their job but are only a support tool not the magical solution. whats even worse is your body eventually becomes resistant to medication my gran ended up at a point where almost no painkillers work

      @@somethingandahalfYeah, that’s a terrible idea. SSRIs can create massive problems if dropped quickly. It takes much longer for the withdrawal and rebound effects to go away than it takes for the meds to have full effect.
      At least if you’ve been on them for a while.

      Oddly, the GLP-1 / GIP "pills" motivate people to change their lifestyles. It's because you lose so much weight on these drugs that you have to exercise and do resistance training in order to avoid losing a lot of muscle mass along with the fat.

      I went to the doctor because I had a mysterious cough that had lasted a month. Ruled out a ton of stuff, and he landed on it being mild acid reflux - and wanted to put me on a heavy duty antacid for a month. I was like "okay, sure, but... is there any diet changes I should make? why did this start happening out of nowhere?" and my doc was like, "oh! that's a great question!". He was super excited to give me advice after I asked, but it honestly sounded like he was so used to people not even bothering to wonder about lifestyle changes that he was taken aback

      people dont need to be doctors to understand things they learn about. Most, if not all, family doctors know nothing about proper human diet for example. If they arent taught it in school, which the are not, then they wont know.

      Actually it's really not hard for an ordinary person to get a 100K loan, problem is banks expect you to pay it back on time so the difference is the willingness to experiment.
      But that's what business venture capital is for.

      I’m on medication for narcolepsy.
      I’m always grateful to medical professionals who don’t go to medication first, but understand it is sometime necessary.
      Good doctors are sometimes hard to find, be a good one, and you’ll never have a problem finding patients.

      Exactly. Patients don't do things like lower calories and get exercise in. The average person does not know what the 4 macros are and definitely not how many calories they have. In a 20 minute consultation you can't teach them what they should be learning in health class. Even then everyone has their own misguided beliefs like they can eat all the fruit they want and lose weight. Or that walking around the block everyday will burn off the large pizza that they eat to themselves. People want quick fixes and get mad at doctors when they tell them how to lose a pound a week.

    So I'm in the Army going on 7 years now, when I go to the field for training and I'm sleeping outside sometimes for a month at a time. On the ground I really have noticed that "field sleep" is much better quality then sleeping in the bed at home. Now I'm not saying it's more comfortable cause sometime it'll be hot as shit and hard to go to sleep or we'll end up on a hill and wake curled up like a dead spider in bottom of my sleeping bag. Buuuuuut tho my back is in incredible pain and its a bitch to go to sleep..... I wake up and feel amazing and rejuvenated every time even 2 hours of sleep has me feeling incredible, I'm not the only person that feels this way most folks in the military can attest that "feild sleep" is amazing whether it's because they work the dog shit out of you and you just get better sleep because of it or there is some actual truth behind this magnetic electro feilds mombo jumbo shit.

      Eating in caloric surplus makes you fat. Being fat and eating a lot of highly processed seed oils and added sugars make you fucking extra inflamed. All inflammation markers go through the roof a lot.more than what would just fat cause

      @@MuppetsSh0w I can't think of any food that had a majority of calories from fat 50k years ago. Certainly not nuts, seeds, berries etc, and game meat doesn't have a lot of fat. It would need to be something like 30%+ fat to have a majority, and game meat isn't anywhere close to that. So unless you think that people hunted A5 Wagyu cows or something there's no chance that people got a majority of their calories from fat.

    My primary doctor handles meds and refers me to other doctors who are in lifestyle changes. Most people he sees are not interested in changing over time, just quick fixes. The magic of the pill

      Yeah, most people dont want to make any effort to improve their health. Taking pills is easier. They work for the short term sure but they only fix the symptoms and not the underlying problem so taking the easy way will bite one in the rear later in life as even more health problems bunch up.

    I love this series. Would be really interested to see you critique some of the workouts Eddie Hall and Brian Shaw do. I know strength vs hypertrophy requires different approaches (especially at their level) but would be interesting to dive into some of those specific differences in technique vs what they do that's good/bad for anybody.     I stumbled across your channel by accident one day. I was skeptical and thought watching one of your reviews of workouts was going to be just another "expert" with some correct and bullshit takes to try to hype up your own self image. I couldn't have been more wrong. I not only only love the content you put out but I really appreciate your approach and they way you deliver your information. Even with the awkward jokes at times which fits my awkward humor perfectly. I've shared your video with other trainers I know that are well more versed than me and they are equally as impressed and in agreement. Quite frankly, I never heard of you before and I couldn't understand why but you said best yourself, it's the face for sure.       @@pricklycats Pills are useful, but you cannot say that they are not bad in the long term. To fix a problem that you have right now (which in my opinion has to be a very big problem for it to require a pill), they work fine, and if you use them for some type of treatment that's also fine. But taking pills all the time for minor problems like a headache or stuff like that.. That's not needed AT ALL, unless the headache is extremely strong and does not let you live your life normally.
      In other words, my point is just that if it's needed and completely necessary, taking a pill (or a number of pills in short-term) is fine. But taking pills all the time for ANY problem that you have that could just be fixed in a healthier way given more time.. That's just a waste of health (and also money, lol)

      I think that studies on just efficacy of drugs miss that the side effects are sometimes as awful as what they're treating and it scares people. Also, just because we have access to research these days doesn't mean we know how to read or understand it.
      It makes me wonder if being rich and having access to "alternative medicine" is bad for your health - if you are low or middle class you'll just get what the doctor gives you, which is probably better in the long run.

    Thank you for being an advocate for good doctors. We try to advocate lifestyle changes and prevention all the time. The people just won't put in the effort or just want a quicker solution. I literally have to beg my dad to eat more vegetables or take his fiber.
