  • This is an example of a 12 hour shift in Endoscopy/GI as an anesthesiologist assistant. It is more of what a typical day would be like instead of my other videos. You can appreciate the routine more in this: Setup, induction, case management, emergence, return drugs, repeat for the next case. I go into the medications for each patient a little more as well.

    I've been looking into going to school to be an anesthesia assistant here in Canada. The route for school here is a bit different, you have to be an RRT or a critical care RN and then apply to the programs. I love your videos and you are the only person I have found who vlogs their life as an anesthesia assistant! Thank you sooo much for showing us your life!! You're videos make me want to reach this goal even more! Keep it up :)

      @@haldol4all first of all I love your username lmao I think it's extremely competitive. About two or three years ago I was looking at CV perfusion there and I stumbled upon a reddit forum about Michener and the application process for all programs seems intense. It's either Michener or I think there's a school in BC but I don't want to move all the way out there! I'm hoping to get into Toronto General at some point and maybe that will make the admission process a bit easier since they are partner institutions. Good luck to you, friend!!

    I’m seriously considering Anesthesiologist Assistant. I’m a nurse but I’m in Florida and with my school situation it may actually be a shorter path to become an AA over a CRNA (talked to the advisor and everything) it’s very interesting

    I went to st mary hospital in korat Thailand 🇹🇭. I had a fantastic experience. So nice doctors and scub nurses. Consultant on Monday all done by Tuesday mid day. Definitely do it if you have pain like i did for over three yesrs in my side back.

    I love your videos. I have medical conditions that require frequent sedations with the mix of Propofol and Fentanyl but I also need GA’s often too. So the team I have includes an anaesthetist. Because of the frequency of procedures or surgeries I usually have the same 2 anaesthetists my surgeon uses for me so thankfully we are familiar with each other and he already knows my history with GA’s and is aware that I don’t come out of them quite affected. I never used to have any issues waking up or straight after surgery but because I’ve become extremely underweight and malnourished that I now and it takes everything out of me. I also have POTS and extremely low bp so they always place a catheter for a few days because I’m a falls risk. My low bp is so bad that I can’t even walk to the toilet without fainting. Being an Anaesthetist is SOOOOOOO involved and complex. I knew to a degree that it takes a highly educated person to be able to do what is needed. You would have to be 100% prepared, not just for the drugs used for a GA but have drugs at hand ready if a complication arises. I didn’t realise that prior to surgery you not only need to get the anaesthetic drugs but also drugs anticipating that you will need to use them. Obviously being good at mathematics is a HUGE must in your specialty to get the right dose needed for each individual. Your videos are very educating and showing us how involved your job is and showing how vital and important the part you play in the OR. Ultimately you are keeping someone alive. Keep making these videos, it gives so much insight. I always like seeing BEHIND THE SCENES, especially if your showing what happens to me during surgery.

    I had a very bad reaction to succinylcholine. I was stiff and rigid for almost 3 days post-procedure and couldn't even bend to sit down. I'm surprised my heart didn't stop beating since my entire muscles in my body were virtually paralyzed (severe myalgia).

      tell anesthesia to give you a pre-fasiculating dose if they have to use SUX next time to prevent myalgia. Source: I'm a CRNA

    Hey Sally! I see you kind of rotate around to different specialties at your hospital. Is that standard for most AAs or just your hospital? I think the diversity would be so interesting

    Hi I’m about to to have endoscopy procedure in couple days. Just a quick question if you can help, what’s a light sedation and are you awake and remember things during the procedure? And what kind of medicines are gonna be used on me ? I heard fentanyl I’m so nervous, can I do without it? And are they allow endoscopy without sedation ?

    What bolus dose do you give for EGD? I find EGD's to be the most challenging anesthetic to give because even if the patient is asleep they still cough once the endoscope is inserted.

    Certified Registered Nurse Anesthetists (CRNA) are Doctoral Educated and trained and you can work anywhere in the US and many other countries. You can also practice independently and bill insurance companies directly in many states. If you are an RN I would advise you to pursue the CRNA route.

    As an AA, do you ever do cases yourself or are they always under the anesthesiologist?

      AA’s work under the supervision of an anesthesiologist. They are not with you the whole time because they’re supervising 2-4 rooms at a time. Only during induction, and sometimes extubation .

    Hello,
    What state are you working in ? I ask this because I see AA can only work in limited states which sucks because those are not places I want to relocate to . Do you know if they just hire you under a different name title like Anesthsia Technologist or something? I just don't understand that if you went to the schooling for AA you should be able to work anywhere but all I see is CRNAs . Did you ever take any nursing classes as well ? Im just really curious about which state you work in and your schooling. I enjoyed the video . Thanks in advance!

      @@integrachic2953 my friend died of a drug overdose and when I had my endoscopy they said they never seen anyone this frightened like I was. Now I'm waiting to see a surgeon for the next surgery.

    who gives cricoid for your patient when you don't have any assistants? do you change your circuits and soda lime for each and every of your cases? do you use sugammadex for all of your patients for reversal(if aminosteroidal NDMR is used)?

      Hi.. Nurse Anesthetist here in Germany. We do cricoid when assisting Dx during difficult to see glottis.. normally for us, circuits are only changed at the end of shift. Standard for OPs from the neck up, we do change the circuits for the longer ones as the patient’s head will be farther from the anesthesia machine. Soda lime is changed when 1/3 turns purple or if CO2 level on the circuit itself is 3 up.. sugammadex is also rarely used as muscular blocking agents are adapted to length of procedures and usually wears off before OP ends.. in some cases where it is used throughout the OP say for GI or hip replacement(where surgeon wanted very relaxed muscles)then either sugammadex or neostigmin+atropine are used.

    Do patients breath on their own when sedated with propofol IV for a 3-10 mins upper endoscopy for GERD?

      Patients can stop breathing with propofol, that's why a provider trained in general anesthesia is required to administer the drug. There is a sweet spot where the patient is breathing but does not respond to painful stimuli. This is called "titrating to effect".

      No. You have to move to another state to work. Either move or become a CRNA (nursing school instead of premed courses, 2 years working as a nurse usually in the ICU then CRNA school). Hopefully in the future more states will open up to AA's

    Hey! im about to be a 1st year biomedical science college student and were just looking at other options besides med school. so im comparing Dentistry vs Anesthesia assistant and one factor is the price I know dental students come out 400k plus debt with averaging 150k wish how much is aa school on average and average pay big factor since I have very small scholarship funds

      Depends on if you stay in state or not. I'm in my 1st year of AA school but it looks like I will end up with ~140k in student loans

      Hey man! So you are spot on in your debt differential and pay assessment. Two more cons for dentistry. It is 2 years longer of a training path and the field itself is becoming over saturated in many areas that are not the boonies. Starting pay for a associate dentist and AA is roughly the same. Associate dentist probably makes 150-165k and new grad AAs make 150-180k depending on location. An experienced AA probably edges out a experienced dentist associate considering your pay doesn’t increase with experience in dentistry as a employee typically in the way that it does for AAs. Veteran AAs can expect W-2 compensation base’s in the 180-230k range, with most pay scales topping out at 200k. Dental specialists can make more as associates and business partners ranging anywhere from 250k (orthodontistry and perodontistry corporate) to 500k+ (OMFS, endodontisry. But the catch there is that the vast majority of a dental class will not have the grades to specialize. And many residencies for these specialties are 2-4 years long and even cost MONEY to go to them. So if you want to specialize in dentistry to make big bucks, you’ll spend even more time in training and most likely much more debt. There are many periodontist, orthodontists and endodontists in 600-700k of student debt..... Now the last big question, do General practitioners in dentistry that own their own practices make more than CAAs? Yes but it is complicated. I’ve anecdotally heard that practice owners in dentistry make around 250k. Buuut the asterisk are that it is very stressful to run your own business, no longer being a 9 to 5 job. People can call you for concerns at weird times in the day, worrying about overhead, staffing issues, insurance companies, facility maintenance etc. not only that, but as a business owner you’ll get no benefits. Lastly, to buy a existing practice or build your own you’re likely looking at increasing your debt by 700k-1 million dollars. There are dental practice owners that make 350k+ but they hustle and do a lot of specialty procedures in a in need rural area typically. Don’t expect it. Most dentists end up as dental associates making as much as new grad AAs with twice the debt and 2 years more of schooling. And if you really want that 250k dentist owner Moneu, you can get there as a AA. Either go to ruralish area where the starting is 180k and work 10 hours OT (you’d probably would’ve put that extra 10 hours of work a week anyway when you’re running a practice) or wait till you’re experienced mid career with a 200k base. Then 1.4-1.5x base per hour of OT work can easily take you there. I had a similar dilemma, and in my opinion AA is the better financial move.
