Optimal Thyroid Lab Tests vs "Normal" (What Your Thyroid Lab Tests Should Look Like)

  • The big difference between optimal thyroid lab tests and normal thyroid lab tests. If your doctor says your thyroid labs are "normal" then you need to watch this video.
    Do you have the symptoms of hypothyroidism? Have you been told that your thyroid lab tests are normal? If so, this video is for you.
    The reason for this discrepancy has to do with how doctors look at thyroid lab tests and the results of those tests.
    Doctors are trained to look at a very broad reference range which includes people who are both healthy and unhealthy.
    A better way to look at your thyroid lab tests is by narrowing down the reference range to what I call "optimal" levels.
    You can use these optimal levels to tell if your thyroid is ACTUALLY working correctly.
    In addition, you'll also find that there are far more thyroid lab tests in my recommended set when compared to the TSH and maybe free T4 that your doctor might order.
    By looking at all of these thyroid lab tests (TSH, free t3, free t4, total t3, reverse T3, thyroglobulin antibody, and anti TPO antibody) you will get a much better picture of thyroid function.

      Hi dr. Childs,
      I'm from the Netherlands have had graves disease since I was twelve and had my entire thyroid removed last year at twenty years old. All my doctors have always only checked my Ft4 and TSH. I am kind of left on my one to figure out these specifics; I have a question about the dosage to reach the optimal range. Would you say that this is indeed a matter of very small changes in the dosis? Right now I'm taking steps of 6,25mcg, so would you say that's about right?

      My TSH is 6.86. I meet with my endo. Next week. I had a partial thyroidectomy last August.

      I'm always told that my blood labs are normal even though I have all the symptoms of hypothyroidism it's so bad that I have zero energy, losing my hair, gained weight and can't loose it and slew of other symptoms that they ignore all because my bloodwork says it's normal and they offer me no treatment at all. I have high corisole and low iron and polycystic ovarian syndrom that I read thyriod can cause they ignore all of this. So what is your suggestions on getting help especially when I'm low income?

      @@jolenejohnson7320 Eat liver. It’s cheap and very good for you. In addition, get some iodine in your diet. Kelp capsules or shellfish.

      Hi Dr. Childs,
      I just got my thyroid labs, and the doctor said that they are normal. However, I just had a horrible week prior to getting tested with a racing heart, blood pressure spikes, palpitations, and chest sensations. I was using Halodine everyday, twice a day due to traveling, and after I returned home, it took a week of not using it for my symptoms to subside. Halodine is Povidone-Iodine nose swabs.
      Here are my lab results, they use the same ranges as you:
      FT3 2.9
      FT4 1.34
      TSH 1.070
      SHBG 161.0
      Thyroxine Binding Globulin 24
      (TPO) Ab.

    I was always in the so called normal range, suffered for two years. Finally, I was able to see a specialist after my TSH level hit 12. They had to see me since it was very obvious. In the meantime, I suffered tremendous hair loss, lost more than half my hair and fatigue which still disturbs me. Yet, my endocrinologist says: I can’t understand the correlation between fatigue and hypothyroidism. 🤦‍♀️

    This is one of the tightest or strictest optimal range of lab values I have seen, but the reasoning makes sense. It's easy to see how "mainstream" care totally misses the boat when it comes to thyroid health.

    Thanks to Dr Childs and his videos I educated myself enough to know that two GPs had no idea about how to diagnose a thyroid issue especially as my TSH was “normal” - I insisted on and paid for the additional tests and found the problem myself (low T3, low T4, high RT3...) which finally explained why I’ve been feeling so terrible - just need a solution now 🤔

      Selenium, iodine and reduce stress. Detox from bromide and fluoride. Add salt loading to minimize detox symptoms. And add mineral salt to your diet (which should be free of processed junk)

    This was an EXCELLENT video. Thank you for this breakdown. I was just diagnosed with TSH of 6.4, but had none of the other tests. I'm 58 and feel great, not aware of any symptoms I'm supposed to be having, so I've not agreed to taking Synthroid yet. I want to educate myself first and have my blood tested again in 6 weeks. Now I realize that my fingers are generally cold (but it is winter) and each time my temperature is taken (time of Covid), I'm running around 96-97 degrees. I'm scouring through your videos and your channel has been a blessing to me.

      Any doctor who knows what they are doing will tell you to be off your meds for 18 to 24 hours BEFORE another thyroid test in order to see how much of the supplemented hormones are left over after the initial surge, not the surge itself.

    This is the best clear description of thyroid blood tests! So what to do if TPO is less than 8 and THYROGLOBULIN is less than 1? Less than 8 is not bad but not good either. How to lower?
    Btw, I watched your video on NP thyroid. Another great description. I was using WP and Naturethroid! Now maybe NP. I started using nutrimeds but TSH went up to 4.5.. no good for me!

    We measure them differently in other parts of the world so it’s difficult to tell using the units you provide.
    My lab uses a Free T3 range of 2-5.8 pmol/L
    Free T4 is 9-19 pmol/L
    TSH is 0.32-4.00 mIU/L
    I regularly (once a month or more frequently) get those 3 done (have a standing order), but haven’t had the others done in years.
    Optimal for me seems to have changed over the years of Graves. It used to be higher, but I feel better with a Free T3 around 4-5 and a Free T4 of 12-15.

    My reverse T3 is at 15 in optimal range, however my free T3 is 2.9 with a total T3 of 139. My free T4 has stayed at 1.1 for the past year and my TSH hovers from .7, .8, and 1.7. I haven’t been tested for thyroid antibodies since I just got my primary to order the T3 labs.

    Thank you Dr. Childs..my sister & I both are very symptomatic & have a very strong family history of thyroid disease. My tsh is 3.2 hers is 4.3 so our dr says everything is fine. This video makes me realize that everything is not necessarily fine.

    My wife’s serum t4 is 12.9 ug/dl. Normal range on the report says up to 13.3 is good. Converted to ng/dl is 12,900! That must be total t4? Tsh 0.764 and t3 uptake 29%. I gave the doc the list of tests recommended this is all we got back so far… not in for thyroid issues just dealing with anxiety and panic attacks and didn’t want to leave any stone unturned!

    My doctor orders different ones at different times. I think due to the lab that they are using at the time. This time the lab was doing TSH3G their range .3-4.2 my result was 7 Free T4 their range .9-1.7 my result was 1.0 and Total T3 their range 0.8 -2.0 my result was .6 . Looking at your video made me realize that no wander I'm feeling the way I am. I've told the doctor before that I need all the Thyroid tests done but she's only done it one time.

    Hi Dr, Great explanation, thank you. just subscribed. My Tsh is 1.19/ Anti TPO is 15IU/ Free T3 is 2.8pg/ Free T4 IS 1.52ng/ Total T3 is 58ng (low) Total T4 is 2.69ug. Do these labs warrant treatment? the endo i saw said my labs were normal. Thanks in advance for your help.

    Dr. Childs you couldn’t be more right! Every time I ask to get my Thyroid levels checked they will only complete the TSH and the Free T4 labs. I recently got tested and my TSH is in the optimal range at .65 but my Free T4 is only 1.0. My doctor of course says that my labs are within normal range but I’m still not feeling optimal. I maintain a healthy diet, exercise 5 days a week, do my best to get good sleep and take Thyroid support supplements. With that, do you think by switching the time I take my levothyroxine from AM to PM it could help to increase my Free T4 levels? If so, will it take 4-6 weeks to notice the change? Thank you!!

    My functional medicine doctor wanted me to have optimum level of free T3, 4.0. I already felt good 3.1. Talked me into Armor 60mg, then 90mg. Progressively, I got sick until it was Bed Couch Bed Couch every day. Racing heart. Shaky. Whole body pulsated. Constant headache. Exhausted. After I had to call an ambulance, BP 198/108, pulse 108, I quit. Then he tells me it was antibodies from what I are the night before. They attacked my thyroid causing a release of thyroid tissue adding thyroid hormone in the Armor I was already taking. The last thing I ate, I normally eat with no problem, so I felt he was grasping at straws. I had been tested for all the components of gluten, cross-tested, 120 food test, and what I had eaten was listed as a safe food. So, he says the test was wrong. Please, think hard before getting on something as serious as thyroid meds. Get a second opinion if you feel pushed or threatened with horrible consequences if you don't comply. My Dr dangled the carrot of optimum health in front of me. Didn't happen. Actually, the opposite.

      A free T3 level of 4.0 would not cause hyperthyroid symptoms, most people do not even feel good until it is at 4.5-4.9. Other issues would have been going on in the body to cause that reaction. Autoimmunity (like he mentioned), upping the medication to fast, and others. Before having been diagnosed with hypothyroidism, your thyroid hormone would have been higher than 3.1. A part of optimizing your health is experimenting and figuring out what is going on.

    Dr Childs, according to Stop The Thyroid Madness, FT4 above midrange will convert into rT3, I’m surprised you recommend a FT4 that high. I assume you don’t see in your practice an elevation of rT3 when FT4 goes above midrange?

    So I was diagnosed Hashimotos 20+ years ago. I am wanting to wean off my levothyroxine (150mg daily) and see if my thyroid is functioning again after changing diet to keto/ketovore 5years ago and losing 30kgs.
    Would I be better off getting this comprehensive blood tests done BEFORE or AFTER I come off meds (or both) ? TIA.

    My tsh is 0.88ish consistently past few months. It actually never looked abnormal before I started meds. Common. However my T3, T4 did changed since 2021. And mine are not falling into the right category however both doctors are pretty happy with the results now.. that is where is gets confusing.. But I actually feel pretty good after 5 months after being diagnosed. My Rt3 12.9. But nothing to compare it to since it wasn't tested before

    Hi DR. Childs, just looked at the last lab work done in May 2022. The THS was at 0.627, and
    T4 Free was at 1.13 these are the only tests that where order and have been since I started on treatment for hypothyroidism. Unfortunately my doctor retired and have to started with a new doctor fresh out of med school. I have labs done next month, I had requested a LDL particle size test. The nurse called me back to say that they spoke with the Mayo Clinic and was told that they don't do it. Have a feeling that if they will not do the test for that will not do the full test for a thyroid issue.

    I had medullary thyroid cancer with hurthle cell, they caught early and i had a total thyroidectomy with the radioiodine ablation, feeling horrible but dr says numbers are good, asked him to test my t3 and he said that was ridiculous, then he said my t4 was a little low so he upped me from 137mcg to 150mcg and said if i had more t4, i would produce more t3.....

    This tsh testing has to change. Ive been miserable for 12 yrs had my thyroid tested 4 times and not one Dr. Said i was out of range. So i never thougt my problems were from thyroid. Recently i tested again and my TSH was 10.

    Please create a video, talking much slower btw about things pertaining to the thyroid for us people who have no thyroid’s and no working parathyroids. I have Graves Disease. I go up & down too often. It’s been 39 years. I take Synthroid, but I am interested in animal thyroid hormone. And I’m a laryngectomee, plus other health issues. Thanks! Sue

    My holistic dr diagnosed me with having hashimotos 8 years ago, I have moved and been to many other doctors that say I am fine, including an endo, but she does say I have antibodies, and I feel like a wet rotting dish rag most Days and show all of the symptoms, HELP!!!!!!!!!!!!!!!!!!!!!!!!!     My GP in Australia said he could be criminally negligent for prescribing Thyroxine medication to me because my TSH is only 4.08 mIU/L 😥

    Can you have a huge change on the TSH value within a month ?
    Twice at 7 and twice I went down to 1 without meds but just with diet.
    I have noticed that when my TSH decreased I decreased my eosinophilis while my neutrophils and hemoglobulin went above the raccomend limita.

    All my labs turned out normal not optimal, except for my T3 Toal. That was low at 58. I’m 45 and very lean, low body fat and flat stomach with abs and toned. I have a lot of natural energy, and wonder if 58!even matters being low since i have more energy than average person and young daughters, and don’t rely on coffee just drink green tea

    The problem is that the US still uses the old Imperial system, and we use the decimal system (pmol/L) for frre T4 and T3). You are silent on the units you use, so they are absolutely meaningless to me. I don't know what to convert to in order to compare.

    Is it normal for a patient to feel best with their Free T3 at the top 10% of the range? I feel best with my thyroid hormone in the top 20-10% of the range, maybe even a little bit higher. My resting heart rate is the same and blood pressure still sits on the lower end of the range.

      Are you taking Biotin (a B vitamin ) supplement? Biotin (even in low doses ) interferes with blood diagnostic tests leading to both false negatives & false positives - So stop taking biotin for at least 2 days before a thyroid test to avoid misleading results on TSH, T4 & T3.

    I was diagnosed with hashimotos we 22 yrs ago. The doctor who had me have blood test checked tsh and thyroid antibodies, my antibodies level was 6700, I started off on 50mg thyroxine. But now a different doctor has put me on 150mg 4 days a week and 200mg 3days a week. Every time I have bloody test check levels I ask for her to have the t3 free t3 free t4 and all I get is we dont check for those. But I see from your list theres 7 different levels should be checked. Maybe time I ask around for different doctor check all these levels. I guess why would my thyroid antibodies be 6700 back 22 yrs ago. From australia

      5 days is ideal but I think 3 will do if you are not on a sky high dose of biotin.

    hi doctor my daughter is 35 years old she have 2 kids 16 and 10 3 months ago we found out she is pregnant she didn't plan it but it happened and she have thyroid problems not sure what is exactly all i know is she have nodules and she check them ever one year but also we found out she have something called long TQ interval and now am so afraid that something will happen to her if she decide to have the baby what do you think she is putting in a tough spot including me with her decision to keep the baby or not i only want to know if it safe for her condition to have the baby and i am not sure why i picked you to ask this :/ i think its bcz my answer to her is to go see her heart doctor again to make sure if its ok but she keep saying her doctor gave a list of medication that she shouldn't take and told her that her problem is very mild so i am confused of what to do now and scared for her thank you

      With my last pregnancy when I was 33 years old I'm 39 now. They discovered I had hyperthyroidism with thyroid nodules. It was horrible towards the end of the pregnancy. Keep her lifted up in prayer I don't think she needs to get rid of her baby however you definitely want to look into natural supplements and make sure she's with a really good doctor that understands her needs. After I had my son the hyperthyroidism stayed and now it's worse than ever.😭 I sincerely pray that she recovers and attacks this early on on so she won't have lifelong problems. God bless!!

      Free t4 1.3 ng/dl
      Free t3 3.8 pg/mL
      Total t3 99 ng/dl
      TSH 0.07 mlU/L
      What do you think about those results?

    Been on Eltroxin 2 months, my TSH has fallen from 11.7 to 5.7 but my FT4 has gone from 12 to 14...? Should the increase in T4 be a concern?

      Wish to know the answer for this.Kindly specify the units and range of T4 like for example ng/ dl pmol/l etc.,
      Also while searching on internet about ranges ,some research based information on internet says ft4 on the within the range (high normal) and even with euthyroid ( tsh in normal range)is a risk factor for solitary tumours and heart comditions.And high total t4 levela destroy metabolic pathways etc.I am aware that googling throwss up lot of information part of whicu can be unfiltered untrue etc. And internet information.not fully valuable and is never a substitute for real.medical
      Professional.etc
      So i dont fully take such information.
      However ,any one giving possible insights on this willbe appreciated

      He said in another video that increasing T4 is the goal insofar its role is to be converted in T3 so that would increase the chance that you would increase T3 as well but bear in mind that an increase in T4 could turn in an increas of REV T3 rather than T3 which is something you wouldn't want (che one of his videos about T3 vs REV T3).

      physiological signs indicating reverse t3 is getting reduced and total t3 (ft3)is increasingly getting converted from total t4.
      I mean how do one know if more t4 means is converted more to t3 and not to more reverse t3.
      Are there any general physiological signs or indications which would indicate increase in t3 ,decrease in reverse t 3.,when total t4 is high in the range.
      I know that blood tests would be the one to show the ncrease in t3 (which would indicate more t4 conversion) and reverse t3 getting down...but want to know if there are any physiological symptoms other than blood tests.

      @@anonymozanonymouz9323 for a precise answer go check is video about REV T3 vs T3. In a nutshell, having high REV T3 conversion (rather than T3) could be due to your life style i.e. what you eat (especially gluten), what you think (your mindset is crucial to manage stress especially if you work a lot), what you do (daily min 40 mins of excercise at moderate intense is fundamental to contribute to T3 conversion).

      Free t4 can give you an idea of how much is in reserve and, potentially, how well you are converting T4 into T3.

    I kept blood work from years ago, before I came down with Graves that required my RAI. My TSH always hovered around 1.5; should I assume that is the optimal for ME and should I aspire to achieve it or should I go with your recommendation of 0.5-1?
