The game's 5 original endings look and sound better than ever. New screens and music have been ended after achieving any ending.
Simplified ways to get each ending:
Red ending: kill every monster you see. Take red and blue pills frequently. Eat rats and bad food. Tell the man in blue you know him. Drink water mixed with plant food on purpose. Give Hank blue pills. Talk to Sleepy Cat. Keep your mental health as lousy as possible. The ending screen music will be different after a red ending.
Blue ending: kill most monsters you see. Take blue pills frequently. Eat poor-or neutral-quality food. Grab the gun before talking to Chie. Tell the man in blue you know him. Give Hank blue pills. Keep your mental health okay, but not great.
Green ending: kill few or no monsters. Take no more than a few green pills. Eat good, cooked food. Talk to every NPC. Care for Chuck the plant. Use batteries to read books, play video games, and listen to music. Give Hank health tonics. Deny the man in blue's existence. Take good care of your mental health.
White ending: ACHIEVE A GREEN ENDING ON A PREVIOUS PLAYTHROUGH. Kill few or no monsters. Take only one green pill. Max out relationships with The Director and the adopted cat. Give Hank at least 9 health tonics. Offer the Sleepy Cat plush to each NPC (Chie, Kenny, Benzido, The Director, Hank, The Man with a Box on His Head, and the adopted cat) to receive Ultimate Dish items. Eat only good food, including 1-2 Ultimate Dishes. Drink water and coffee daily. Use batteries to read books, play video games, and listen to music. Take very good care of your mental health. Enter the middle hospital corridor instead of continuing to the left.
Yellow ending: ACHIEVE A WHITE ENDING ON A PREVIOUS PLAYTHROUGH. Reach Chie's apartment and hand her the Sleepy Cat plush instead of the purple doll.

Correct. SLS is upscaled to a 4:3 presentation though, so I wanted to compile the updated endings together (plus the addition of the new scene after beating the game).
I have this weird issue where I have perfect S star mental health but I still can’t the white ending
