  • While many people aim to return to sports after suffering a serious injury, others want to exercise. Lifting weights is a common desire I hear from my patients. In this Ask Dr. Geier video, I answer the question of a reader who wants to return to the gym as quickly as possible after you dislocate your shoulder.

I just dislocated my right shoulder, and I’m in a sling right now. I want to get into working out again, but I don’t know how long I should wait.
SURGICAL TREATMENT FOR MANY SHOULDER DISLOCATIONS
A shoulder dislocation can be a difficult injury, especially in a young, active person. There is a high incidence of recurrent injuries, meaning that you can suffer repeated shoulder dislocations. That is a key reason orthopedic surgeons often treat these injuries by arthroscopic surgery to reattach the labrum and tighten the capsule.
REHAB AND RETURN TO SPORTS AND EXERCISE AFTER YOU DISLOCATE YOUR SHOULDER
If you decide to try to avoid surgery, physical therapy can be helpful to regain strength and function of that arm. It’s hard to know how much it decreases your chance of ultimately needing surgery, but it might help you lift weights and train in your sport quicker. Also, wearing a sling can help with discomfort and swelling for a period of time.
In this video, I discuss the initial treatment of a shoulder dislocation, including the use of a sling, going to physical therapy and seeing an orthopedic surgeon to discuss surgical options.

I dislocated my shoulder 3 months ago. I'm 19 years old and my lifestyle is very active (running, football, surfing, skiing,...) I heard different opinions but the most common suggestion was that I should wait if it is going to happen again and after that going on surgery. The biggest problem is that after such pain I can easily black out. And the problem is sports in water, what if I black out there??? So I'm thinking about surgery right now. Will after surgery be any consequences and if what kind? Thank you for an answer!

Trust me on this one.. I see some guys saying they do pullovers and stuff after the dislocation... DON'T F*** AROUND...
I was very much into bodybuilding, but i dislocated my shoulder while wrestling with my friend. After that my life changed. The last 5 years i had 4 dislocations. The second one happened after 4 weeks in a sling, and 2 weeks of therapy on a lat machine with only 30 kg of weights ( i was pulling around 90 - 100 kg before the dislocation ). The last 2 dislocations happened while i was sleeping, so trust me don't do exercises like pullover or behind the head shoulder press or pull ups or any type of exercises where you isolate your shoulder . Shoulders are a small group of muscles like the biceps or triceps, and it doesn't need much weights ( keep it simple ).
I had my arthroscopic surgery 10 days ago, and i have a long rehab in front of me. The best thing that you can do is swimming , and take your time with that. Don't rush into lifting weights, because you can hurt yourself, not only the shoulder, because the weights could eventually fall on your head or something.
I don't want you to go thru this like i am right now, because sport was my life, and now am very much depressed because of it .
    Take care of yourself!

sorry to hear about what happened to you man. i'm in the same boat too. i dislocated my shoulder 4 times in the past 5 years. Each time over something with less force. Ive gained a lot of weight and fat too. I got depressed after every recovery period following a surgery i tried to get back into working out and second i saw results my shoulder popped out. My last dislocation occurred almost 2 years ago (4th one). my capsule is loose and i do have some bone loss. I decided to not have orthoscopic surgery because the though of even going through that pain or experience makes me uncontrollably nervous.
I am on the waitlist for surgery which most likely will be next summer but again i'm not sure if i even want to do it. I have started going to the gym. Doing deadlifts, bicep/tricep curls, leg presses, machine rows. Every time i do an exercise i keep my elbows close to my body (almost touching) and try to never raise my hand above my head. I use only machines as well. So i guess there are ways you can avoid surgery but i am still at high risk of another dislocation. Im just worried even after surgery i wont be able to do the things I love like boxing, basketball, mma since i'm worried it might still pop out. So i stopped those things all together and accepted my life will be completely different now.
What made you decide on getting surgery?

Took me 3 months rehab before starting swimming to ease in to working out again after 5 months. I had a lot of experience weight training before my dislocation so my shoulder awareness has always been on point and I took it as slow as needed, including rotator cuff work and general shoulder warmups before every session even before my dislocation. So I'm hoping the freak accident I had won't happen again, so far so good.

After so many dislocations (15-20 in a 3 year span), I started to feel my shoulder randomly gliding forward out of the socket, specially when working out. Since then, I've become very self aware when it comes to the positioning of my shoulder. I was forced to learn about the mechanics of my shoulder in order to be always in a safe position. I also started adding rotator cuff work in every single workout. My shoulder will never be the same, but I'm trying my best not to be through this injury ever again in my life. I feel significantly safer now than I did before, but, as I said, the risk is always gonna be there, and not necessarily low.

When you read this comment, I knew it was too late for a suggestion. I'm typing with 1 hand, just got second shoulder dislocation after multiple small shoulder dislocations from age 17-23.
At the age of 24, I have to go to the hospital first time for shoulder dislocation but not surgery. 6 months later, I started to go to the gym and got second shoulder dislocation after 7 months of lifting even though I did warm up carefully. My suggestion is that do not move your hand for 1 month after the first dislocation and do not lift heavy above your head, do not straighten your hand when pushing above your head. It's fucking hard that you can not work hard in the gym to gain muscle. WHENEVER YOU FEEL PAIN IN YOUR CHEST, OR SHOULDER WHILE LIFTING, PLEASE STOP IMMEDIATELY. It's a sign of the next shoulder dislocation and it'll take several months for recovery. You should stop it and take a rest for a few weeks or anytime which I also do not know then come back to the gym when your shoulder doesn't hurt

I’ve had 3 surgeries on my right shoulder as a result of many shoulder dislocations. The first two failed and the third one has held up (so far). The surgeries were over the course of about 4 years from age 16-20 and I am 24 now. The doctor is 100% right; physical therapy and rehab are key. I did a very minuscule amount after the first two surgeries and I paid the price. After the third surgery I made sure to put in the work. Over the past 3 years or so I’ve been able to lift pretty heavy with weights. I recommend not doing anything that puts major strain on your shoulder (flys, bench, dips, pullovers, etc.) i say this because about a year ago my other shoulder dislocated and I believe it was because it was overcompensating for the lack of strength on the other shoulder. Shoulders are a tough part of the body to maintain if you do not practice strict routines and rules. Whatever you do, be safe.

Just wanted to give my own story to help spread information to others. I dislocated my non-dominant shoulder about 2 years ago getting thrown in judo. I was younger than 20 at that time. It was pretty depressing and tough at that time but after a certain period you get over it and it can actually be character development because you learn to do other things you would not have done before. I did physio and then went back into the gym after a while and have not re-dislocated since - I think I am one of the luckier ones since I do not feel a lot of instability. I do not practise judo anymore because I don't want to risk it but still occasionally did boxing and BJJ casually.

Im 19 yrs and back in 2021 I had my 3rd Dislocation in 3 years. The first 2 dislocations the MRI said all fine and I continued to lift weight and never had a Problem. But as i got bigger and the weights heavier, i got more and more a feeling of instability and it sucked to hold myself back with certain excersices. Then it popped out again as I was pushin a box at work. I ended up having a surgery (Bankart Repair and the made the capsule smaller) and im really happy with it. Im in week 9 post surgery and altough im still not having Full ROM, my shoulder feels much more stable and the pain was gone after a few days. Im in the lucky postion to had the surgery really fast and the doc how made it is an expert with shoulder instability. But PT is everything guys! I love the gym more than my life and it is really depressing (havent hit the gym properly since 3months and will not until 4-6 months post surger) to get smaller but still its worth. Just train legs and try to stay fit and healthy to speed up your recovery! I think it that way: Maybe no i lost a lot of gains but in 1 year im maybe even bigger than I would be without surgery (cause instability was holding me back from giving 100percent). Stay safe my fellow gymbros! Wishing you all a speedy recovery and god bless you

I have dislocated my shoulder 3 times total split between my left twice within the same week and my right once. I never quit exercising for more than a week give or take a couple days, but I always eased back into. After I dislocated my shoulder all three times I focused on minimal stress on my shoulders, but I heat packs, ice, and mobility work. This trio along with a recovery based nutrition plan has allowed me to recover within no time and feel relatively normal.

But definitely should see a PT and continue with the at home workouts afterwards to keep ur shoulder strong

There's not much you can do and will most likely look a little smaller (not drastically). The good news is when you do get back to the gym it will be easier to regain the muscle so it's not like you'll be starting from scratch. I'd also say make sure you keep at maintenance caloric intake, drink plenty of water and consume a lot of protein. I'm on the same boat, 20 and have to wait 6 weeks until I get back, it sucks.

I dislocated my shoulder for the first time in 2017, when my arm was pushed 90 degrees behind my back, from a vertical starting position, while intercepting a basketball in mid air. After that, I gained about 95% of ROM and had no instability issues, I lifted weights and did lots of push ups for many years. Now in 2024, after a short break from gym, it dislocated while I was rolling over in bed.
The fact it came out during something so mundane is very scary as well, considering the actual pressure I used to put on the shoulder during gym sessions! 72hrs after this new dislocation, im not in huge pain and started pendulum swing exercises, which actually loosen things up and relieve pain.
I do plan to return to my usual gym routine but ill be starting with lower weights and building up very slowly, I suspect stopping gym may have contributed to this injury, as the muscles that stabilise my shoulder would have become weakend and the soft tissue damage from lifting heavy found an opportunity to take revenge! 😂

hi Dr. Geier, I'm a 54 year old fit male. I dislocated my shoulder 9 weeks ago in a skiing accident, I can't say for sure how far my arm was forced behind me, I had just planted the ski pole in the ground really hard, and then my ski boot popped out of the binding. It caused me to lose my balance and I was thrown to my left and down while my hand still stayed attached to the top of the pole.
I'm doing therapy everyday [ just the exercises the PT] gave me to do. Plus I'm still doing sit ups and some leg exercises. My question is:::
My range of motion is improving very well, at the end of the session for that day, I will grab a 3lb disc and try to raise it up with my left arm semi straight out. It is very difficult to do and kinda painful, only a few seconds. Plus there are about 4 movements that were very painful and hard to do in the beginning from day one and are still painful and hard to do today. [Using left arm] 1-washing top rightside of scalp 2-washing the rightside of the neck 3-putting hand on lower back 4-wipping my butt. And then out of the blue I will get a big jolt of pain for a few seconds or so. Is this normal am I expecting to much to quick. I'm going for an MRI on 5-12-18 so I'm hoping for the best and it just needs time.

I've been bodybuilding for many years, so for me, I'm very concerned about not being able to perform at peak efficiency so I'm going to do alot of rehab, slow stretching, and eventually back into weight resistance. I go back to my orthopedic surgeon this Monday to get an evaluation after the 2 weeks since the dislocation occurred. The doctor said, it is very rare that it gets back by itself. After checking the bones he gave me a wig, which i should wear a week. After one day i started to move my shoulder an do light rotatorcuff exercises und lateral raises front and back... because I knew that, if my muscles and rotatorcuffs are strengtened, my shoulder would rest in its proper place. Today i went in the gym. I did light weight training. But deep shoulder stretch movements like the stretch after a pullup, I had to avoid.

I thought it would fix itself...well it didn't.

35 years old. Dislocated right shoulder 4 weeks ago. Have been doing physio, and pampering it. Am able to do chin ups and pull ups and push ups now, although only about 20% of what I could do before until it gets tired and sore. Trying to be patient. I dislocated my shoulder on 2nd of December 2019, I went back in the gym 2nd January 2020. I am starting extremely slowly, I used to bench 220lbs for reps (x10) but now I’m changing my routine to focus on lower weights and higher reps and lower rest as I don’t have the power or flexibility I used to.
Holding my arm at a right angle is tough still with any weights which mean squats and any presses are out of the window for now, so just work out with more machines which put less stress on the shoulder.

I had a fighting injury and my shoulder dislocated after it dislocated I had it relocated after a hour, they put me in a sling and i went home and slept with it and had it on for a couple days. After I took my sling off and started doing calm exercises like slowly moving my arm in circles I would make sure not to lift my arm up to much, but I do boxing and jiujitsu and ever since I couldn’t go back to training in till my shoulder was healed enough to work with. But after 3-4 months of exercising and resting it I went back to training, Currently a year later my shoulder feels good, during the process of my shoulder healing, I use a 5 pound dumbbell and do full ranges of boxing moves and doing stability exercises and I would also use a resistance band. I could spar but I’m still in the process of trying to get my shoulder to the best condition it could be in but other then my arm feels great without no surgery or physical therapy and I can spar with no problem I always make sure to warm my shoulder up before I do spar but I got my shoulder checked out recently and the doctor said I’m fine but I asked for physical therapy just to be 100% and just hoping it won’t be a problem later in the future as I continue to train.

Since the day I dislocated it I didn’t sit down and be scared I started working out my shoulder very light exercises to avoid my shoulder from getting stock. I remember it was very painful at nights to sleep. The first week or two let me tell you I couldn’t sleep a bit I had sleep straight up on my computer chair because laying down was super annoying and painful. Anyways I’m now back to the gym and been lifting very carefully and thank god the shoulder has been keeping up pretty well. I don’t feel all that comfortable to do whatever move I want with this shoulder but I’m hoping it gets a lot as the time goes by.

I've just had surgery after a dislocated shoulder fracture, did a superman into a tree after coming off whilst downhill mountain biking through woods. 4 hours surgery, bone fragments removed, a reduction in the humerus head and a metal plate, surgeon said I'd basically crushed it. Just took me a year to get from 14 stone at 31%body fat at 5'8" to 10 stone 12 at 12 % body fat, both my aim and achieved within last month. It has been really hard work and finally happy with my body a beachbod😁, and then this! What can I do to keep the fat off? Surgery was 5 days ago and can't walk any more than a few hundred metres without turning blue and nearly collapsing. I've been told it will take up to 3 months healing then another 3 months physio to get to the point of being able to do everyday normal things and it's highly unlikely I'll be able to raise my hand above my head again due to the severity.
Will I ever be able to go back to the gym? I'm 44 btw.
Absolute agony but the gas and air was good stuff 😁👍

Ive dislocated my shoulder a total of 5 times in less than 2 years. From experience my shoulder gets easily dislocated each time. Right after dislocating and popping it back in the shoulders will sore a couple of days and limit mobility. Right after that pain goes away, bench press and shoulder press wouldn't really hurt (at least for me) and could lift quite heavy. 225 lbs bench without shoulder pain. Im considering a surgery tho since im 22

As of by the time i was about to get my operation done, ive dislocated close to 15 times. I lost count really 😅. Now im 5 months into recovery and im feeling much better and undergoing physiotherapy. Nothing close to benching or anything heavy but this is already good enough for me. I had my labrum torn and had bone loss on both parts, the humerus and the socket itself being 8% and 11% or wear. They inserted a total of 3 pop socket for that and my condition was not the worst but pretty bad at that time as the drs told

    • @P0rQpine
      @P0rQpine 3 года назад +1

@@Rodristotle i dislocated somewhere around 15 times. That only means my shoulder got loose every and each time. First experience was during a muay thai fight i had while throwing a punch. The next couple of times after was a lot from basketball while rebounding and had people contesting from the back, all out swim stroke during freestyle, and crossfit. For heavy lifting like bench press and shoulder press it wont hurt at all really, in my case. It shouldn't be too as the shoulder is not rotated and moved in a vulnerable position. After those dislocation, id say it took me maybe 4 days at least to feel comfortable lifting weights again. What was quite tough ironically is the barbell squat as i have to externally rotate my shoulders to grab the bar. Anything with external rotation is very risky. Enough torque will pop it out. Hope this helps. Cheers

How have you managed with this? I dislocated my shoulder 2 weeks ago and have been depressed since. I was lifting 5-6 days a week. Injured playing basketball :/ should of just went to the gym to lift that day lol any advice helps.

Antony Almendarez hey just gotta let it heal up for now. All the tendons are weak and you should do physical therapy. That’s the mistake I made and that’s what caused me to dislocate it an additional 2 more times lol. Once from playin ball. In the gym I would just lift easy for now. But I lift somewhat heavy I know I can do more but my shoulder won’t let me

Kabuto Nice! Yea I went to the orthopedic doctor and he has me resting for 2 weeks before starting PT for 4 weeks then back for reassessment. Starting to feel better each day. Definitely resting and trying not to aggregate it or pop it back out lol. Yea man my basketball days are DONE! 2 ACL tears on the same knee, 1 broken finger and now the shoulder! I can’t even watch it on Tv gets me so mad lol. But thanks for the reply, you have no idea how uplifting your message is. What’s been the most challenging as far as your workouts since the injury? How long ago did you originally dislocate it?

Antony Almendarez Yeah man, I miss b-ball a lot. I was still playing after 3 dislocations in my right shoulder but then I fucked my knee up bad playin b-ball and had to have surgery and I lost a lot of cartilage so I’m done with ball. Yea I miss competing and giving it my all but I gotta focus on what’s important. At least I can still train lol

Antony Almendarez and shot man I disclosed first time like 5-6 years. And two more times after that. Most recent one was about 2 years ago. My shoulder won’t be the same again. But like bench press, incline (bench or bar) is what keeps me back from lifting the weight I want. But like I kill it with shoulder workouts for some reason😂 ironic huh? But I genetically have big shoulders so it’s okay and I think I built enough muscle in that shoulder to the point where it won’t pop out again. Like I said PT is very important. I wish I did that

I am 20 years old i got sholder injury and then i take Ayurveda medicines since 3 months... My doctor recommend me not to sleep by putting you hands at back of your head and i continued 3 months... My pain is gone but i feel little difficult when i take more weight to my sholder exercise... I feel that my sholder is going to dislocate now when i do exercise... I think that gradually increaseing the weight can overcome this problem...

If you are below 21 try avoiding anything that can dislocate your shoulder again... It takes atleast 6 months(in my case) to get over pain completely and your shoulder will feel strong enough... Don't get excited by this bcz u can easily dislocate your shoulder again... If you want to workout in the gym, go for light weight and slowly increase it with time

Would like to know what your opinion is on my shoulder injury 9/24. I fell and probably partially dislocated my left shoulder. Saw urgent care got a sling and have been resting it. I also saw orthopedic doc on that friday. Range of motion came back fairly quickly. still feels weak and have minor pain in anterior delt area and also at base of scapula . Beside lifting I also participate in Bow hunting and Archery in general. What would be a good time and exercises to try and get strength back, or is it to soon. I have been doing the instructed rotator cuff excersises with the band I got from my orthopedic doc.
    any advise would be appreciated.
