Dr. David Geier hello I’m 42 I have a meniscus tear and it also has arthritis dr told me that later I will need knee replacement. My question is would it be best to do the knee replacement in my 40’s rather than wait and prolong the problem?

10 years ago I had a tear in my left knee. It took six months to diagnose. I even had an MRI done some 1 to 2 months after the initial injury. The MRI report said I had no tear. After 6 months of complaining to my Internist she finally did more manual testing and sent me in for another MRI and I was diagnosed with a Grade 1 medial meniscus tear. Before that I had been doing rigorous Physical Therapy with a tear in my knee and I wholeheartedly believe that tear was much larger than Grade 1. My physical therapist (he was a really good one) we kept up the rigorous exercises for another six months and my tear HEALED! Ten years later I have another possible tear in my knee and I am doing the same thing I did 10 years ago. You can heal with no surgery it just takes a longer time to recover, but at least no surgery!

      Thats bogus
Meniscus repair is the thing to do
Otherwise most tears wont heal fast
      And may worsen significantly

I am 31 years old and I train in Brazilian Jiu Jitsu, which heavily focuses on joint-locks. After a pop during training and my knee locking up, an MRI determined I had a bucket handle tear in the medial meniscus of my left knee.
Although there is a longer recovery time and the possibility of failure, I chose the repair method over a partial meniscectomy to reduce the risk of developing arthritis.
I had the surgery in July 2016 using fast-fix sutures. I was off work and in a leg brace for 2 months. I experienced a lot of muscle atrophy. I went through a strict rehab regime of physiotherapy and progressed to weight training. I also used supplements to assist the healing of the meniscus.
I returned to training at a light capacity in December 2016 and am now training more intense at what I believe to be 90%. I have no pain, but feel the operated knee is not as strong as my other knee just yet.
Overall I believe the surgery was a great success though and am glad I went for the repair option.

    THANK YOU DOCTOR GEIER!
THESE QUESTIONS GO OUT TO ANYONE WHO CAN ANSWER THESE AND I HOPE EVERYONE HAS A SPEEDY AND GOOD RECOVERY.
10 weeks after your meniscectomy were you able to bend your leg all the way back as far as your right leg bends back? Or are you only able to bend it about 90°? And on bending it about 90° did you feel pressure in the knee? Did you still have some swelling of the knee? Were you easily able to walk up and down steps or did you still have a limited range that made it difficult?
I am in my mid 60s and had a partial medial bucket handle meniscectomy. My surgeon showed me the pics. There's no wear or damage to my cartilage. He showed how smooth and undamaged it is. And I was walking around with the bucket handle meniscus tear for 5 months until the surgery! And my job is a physical one. I'm just very lucky.
This is my situation. I'm trying to get an idea of all of this through searching online different resources.
    I'm going through PT now.

I tore the Lateral meniscus completely off and it locked my knee. I am 56 but active and my knee was in good shape. My doctor decided to attempt a repair and used to anchors to reattach it . I was in a straight brace for 6 weeks and then fitted with a smaller limiting brace for 6 weeks along with therapy I'm happy to say I'm pain free and just have some stiffness 11 months later. I was told by my Dr. that 90% of surgeon's probably would not have attempted the repair due to my age but he saw the knee had no arthritis and felt is was worth a try. I'm very thankful he took my health and future first instead of the simple way out. He told me if he would have just removed the meniscus I most likely would need a knee replacement in 5 years. Guess I was lucky to have a surgeon with a conscience.

I am confident if after every meniscus repair you have 3-4 PRP injections or even better, Stem cells, they will provide the stimulation and growth factors to 'glue' the meniscus together, especially as it is being held together by stitches, these cells will fill in the gaps of the meniscus, even in the white zone.

I had pain in both knees, the right being the worst due to arthritis. I had knee replacement surgery which was very successful. The left eventually had to be treated for a meniscus repair. Fast forward 3 mos. post repair. I have not been able to bear much weight without significant pain and the pain is constant. Steroid injection was ineffective. In three days i will have the Hyaluronic acid injection. Next step in my opinion is knee replacement. I am 69 and have enjoyed good health, but this has gone on too long with marginal improvement. I enjoyed your video very much and now I feel more educated about a repair.

I am a 53 year old woman who was active every day, playing competitive recreational hockey and tennis, and alternating working out with (weights and resistance) with yoga. Playing tennis with an unbraced knee after tearing my ACL led to a tear to my left medial meniscus, which my orthopedic surgeon sutured via arthroscopy. I wore a full leg brace, locked, for a month post-surgery, and unlocked for two more weeks after that. I am now 16 weeks down the road from that surgery, have been diligently working with a physical therapist twice weekly, and doing PT exercises at home daily. I began easing back into biking and skating this past month, and I find that when I am active, my knee feels great--I notice very little to no stiffness or discomfort. At night in bed, however, I am experiencing achiness in my knee and sometimes a burning or sharp pain in my left inner knee. Also, after driving my car for more than 5 minutes, sitting at the table for meals, relaxing watching tv or reading, or sitting at my desk working, my knee is extremely stiff and walking gives me a sharp pain in my left inner knee. I am really anxious now that my repair has failed, although I have been extremely compliant in terms of activity. I don't know whether I should back off biking and skating (which make my kneed feel good), or keep doing what I'm doing. I'm supposed to see the surgeon again in four weeks, but wonder whether I should be consulting him now about this discomfort.

Recovering from a medial meniscus repair surgery right now. The surgeon had significant difficulty with getting the sutures to stay set. She ended up putting some sort of anchor plate on to the tibia below the meniscus. While drilling to install the sutures, she released some bone marrow into the joint. She said this was a good thing as it would aid in inflammation and healing.

I am a 25 year old very active guy who is obsessed with football. I got a surgical repair for a bucket handle meniscus tear on the medial side of my right knee on 14 Feb 2023, so it's been effectively one year.
The surgery was a success, however, I am still having significant discomfort in my knee. I am beginning to lose hope. I haven't played football in over a year, have been very diligent with my PT (apart from 2 months toward the end of 2023) and I'm just seeing such slow progress.
I saw my surgeon 8 months after the initial surgery when I started to feel significant discomfort and swelling after going on a short hike. I got another MRI which confirm that he can see the repair and reassured me that it hasn't separated and "looks like it's still healing". I'm very concerned about whether is is actually healing, and whether I actually need to get the torn portion removed, but I'm obviously very skeptical about arthritis in the future.
Any advice would be much appreciated.

Hi! I just had meniscus tear/repair surgery 3 days ago. I wish I would have seen this video beforehand! I am hoping it heals and stays healed. The surgery and these past 3 days have been so miserable. I am in so much pain.
I had a question for those of you who have been through this surgery: When did you start trying to bend your knee. Doctor gave me a brace that only bends slightly at the knee, so I've been keeping that in almost all the time. Also icing it a lot, but elevating hurts the back of my knee so haven't really been doing that. My doctor also told me I need to go 6 weeks without bearing any weight on it. I'm on crutches and seriously can't imagine using them for 5 more weeks!!!!
Thanks in advance for your replies!!!

I had arthroscopic surgery on my right knee to repair a the medial meniscus which had a horizontal tear of the body and posterior horn. Surgery went well and I had little postoperative pain. Surgeon trimmed out the torn section and also removed a floating body in the knee. All was well for the first week until I met with my surgeon at 1 week after surgery. After that pain started to increase and my ability to walk was impacted. I do have slight swelling at the incision points. Feel pain when weight bearing and at 3 weeks I am still feeling pain. From listening to video I think I am still in the healing mode and perhaps I need to be patient and hope the pain will subside down the road.

Partial, tear of ACL/PCL and bucket handle tear of lateral meniscus with large dislodged fragment at intercondylar notch. Horizontal tear in posterior horn of medial meniscus. Will see orthopaedic doctor in 2 weeks to discuss possible surgery. Told to start bearing weight normally today (7 weeks after ORIF) by rehab specialist although original orthopaedic surgeon only said up to 50%. Been using bike machine at the gym and I think it helps. Knee feels relatively strong standing normally. I am a pilot and I need my knee to be close to 100% as much as possible. I had meniscal repair with radiation therapy to the knee following surgery. This was over 5 yrs ago . Today I’m constantly dealing with bakers cyst and knee bursitis off and on . This is very painful and it’s distracting, making it hard to handle everyday tasks . I just had jones fracture repair on my foot and so my left leg was getting all the pressure- which is the leg tht was repaired previously. Could my recent injury be the cause of my issues , or could I hve possibly torn something in my knee , again. It’s very frustrating and painful. The ultrasound procedure is excruciating 😣 I’m afraid to go bck to er because of it . Can you tell me what May be the problem from my description? It would be greatly appreciated -if possible . Dr, Last year I suffered a meniscus tear in combination with a 50% ACL tear. Two weeks Before surgery my Dr. said he would remove 80% of my posterior horn and not address my ACL. The morning of surgery he told me that he would try to repair the meniscus rather than remove. My surgery took 1 hour and 40 minutes. When I woke up I was told that the Dr. had tried to sew the meniscus but was unable, so he removed the 80% as he had first discussed. Shouldn’t a good surgeon know what is amenable to repair before doing so? Also, after surgery I was very slow to recover and after 12 weeks I hyperextended the knee resulting in a complete meniscus tear. I’m not sure how to best proceed. TKR I hope, so as to end anymore frivolous sufferings. Do surgeons typically operate this way?

Failure rate is not only high on repair but also on older adults when you go into clean up the torn meniscus your actually inviting premature arthritis of the knee and if the Meniscus is torn and there's already presence of arthritis in the knee it would be better to just try and rehab it and live on anti inflammatory meds

I disagree living on those meds. I did that over 3 years. My doc made me stop. I have pain in both knees and both hips. I live in pain. I had meniscus repair surgery 15 mo ago. My knee felt much better, but the opposite knee started killing me, along with the hip on that side. I was told it was ITBand Syndrome. I was in PT a long time! But, dumb things like quilting or running errands cause pretty bad pain. Going back to my knee surgeon. Not sure what to do. I know my meniscus is torn again. My hopes were to be able to walk 4 mi. daily. Nope, I had a garage sale and my watch counts 10,100 steps. I was on Tylenol and Aleve a week. Last year I cooked the Thanksgiving dinner. I could barely move that evening. Getting very depressed. Not sure if I should have the surgery again.

I advise anyone with meniscus tear to hold off on surgery and try PT first. I had the surgery and regret having it. It was the most painful surgery while healing I’ve ever had even compared to a broken bone by my knee with wiring. Still have pain since March 1st, 2024. Going for PT but now they are telling me my knee is shot and full of arthritis & suggesting total knee replacement. Going to avoid that as long as possible and get a second opinion out of this area. I live in Florida in “the land of the nearly dead.” Doctors love to do knee and hip replacements here. It’s all about the money. Really ticks me off.

Sometimes what is tough is knowing if the retear is just a whole new tear or the failed repair, usually because the mechanism which fails a repair would
Also prob cause a new tear anyway. I’m going on for a second surgery, after a fail/ new tear. If the meniscus looks mildly saveable, we decided to try to repair again. It is worth it. Will def add stem cell to the recovery this time

Hi I have a Medial Meniscal tear on the 2 Aug and the surgeon said was quite significate. I was doing ok with Mobic med but started having severe reaction to the meds with stomach pain and shortness of breath to name a few. I was told to stop taking the meds, which I did for 8 days as the stomach pain was quite severe. I then started with Nurephen for 2 weeks as I was worried about getting stomach pains again. But my knee was slowing getting worse with pain and stiffness, even though I was doing exercises. I'm now taking Naproxen for the past 2 days and the pain in the knee is slowing decreasing along with the swelling and stiffness. My question is how long will it take to return to normal duties as an operator where I walk a lot of stairs and distances. I'm currently on office duties but cant see myself return to normal duties any time soon.

I’m 51 and underwent a bucket handle tear repair 12 weeks ago. I had been doing well until today. I fear I tore the repair. The tear had been in the knee for more than 12 years and when I woke up from surgery the doctor said “we tried to repair it”. The frustrating thing is I’ve been doing very well in PT and have not had any problems. The thing I was doing today was nothing crazy but I can’t straighten my knee and weight bearing is agonizing. Hoping I didn’t tear it apart but fear that’s exactly what happened.

It’s been a years since the surgery and I’m doing pretty good. Training Jiu Jitsu regularly and keeping up with the kids. I didn’t tear it. I just aggravated it. Slowed down my recovery a few weeks and reminded me to take it more slowly. Right now the only issues I have are a little stiffness from inflammation when I overdo it. All things considered I’m doing really well.

Were you weight bearing after you hurt it at 12 weeks? Because I can’t put weight on mine. And all I was doing was standing in water working my leg back and forth, and it popped. Can’t straighten it nor walk on it. I’m pretty freaked out about it rn.

      @AmerijamAcres 2 года назад +1

@@RyanMrRice I hurt it in Jiu Jitsu class. I was only doing mobility and when I applied some pressure I felt it. I was unable to weight bear afterwards and it hurt pretty bad for a few days. I first hurt it on a Thursday. That Monday I went into the surgeon to have it looked at. I spent two weeks not weight bearing after that and it got better. It was odd because I was not really doing much and for it to hurt that much had me freaked out. If I were you I’d just ice it take some anti inflammatory medicine and stay off it for a few days. Watch your diet too. Stay off the carbs and sugar. High carb intake is very inflammatory. You’ll be surprised how much better everything feels when you get into a solid anti inflammatory diet.

I recently had a repair of my meniscus and I'm 2 weeks into recovery and had a fall. A lot of weight went onto it and it felt like my knee stretched even though it was in a brace. My surgeon is taking a look at it tomorrow since I have been in great discomfort since. My question is that does anyone think if I do have it repaired that if I return to soccer (which is how I tore it) will it be too much stress (I am 18 and my surgeon said I have pretty good blood flow to the area, dont know if that has a lot of significance or not)

    • @irondisciple2984
      @irondisciple2984 6 лет назад

Moser Gaming And Tech if soccer injured it then get healed and do exercises with body weight and then weights stretch daily increase the muscle size in your legs so hams, quads, glutes, calves inner thigh, and back for a strong posterior chain. And to make your muscles do and carry all the work and weight and avoid your joints to get reinjured. Those quick sprints to turn to your left or right might reinjure it since its a twisting motion for the meniscus and its probably how you injured it or from falling or stoping all of a sudden or awkwardly. But i suggest come back to playing soccer after you are healed and with stronger and muscular legs. If you get reinjured then its time to throw the towel and do another sport that isint so demanding on the knees like soccer. Hope this helps. Somewhat.

I tore my medial and lateral menisci in June 2018 and underwent a partial meniscectomy in December 2018. The pain was burning and excruciating behind my knee and down the entire length of my leg into the ankles prior to surgery. After two months of physical therapy and a corticosteroid injection my knee felt much improved; however, it still remained in a "weakened" state with slight swelling, soreness, and stiffness. I was released back to work in May 2019 but three weeks later, re-tore my meniscus. While the debridement helped to alleviate the burning pain, it left my knee in a vulnerable state with now bone-on-bone pain and accelerated osteoarthritis as well. I am 61 years of age.

I’ve done more damage and personally I don’t want any of it taken out because I don’t want arthritis…..I have polyarthalgia that’s bad enough…..I’d sacrifice my leg if I had to because I don’t want anymore pain than needed……I know that’s selfish but it’s true…..my leg has bothered me for years and I’m getting incredibly tired…… plus the leg in question is taller than the other one so a lot of pressure is on the hurting leg and I don’t like it…..if the meniscus can’t be fixed I’d ask if we can’t get rid of the leg all together because I don’t want a part of me that’s damaged beyond repair and left partially broken in my body…..is it selfish yes but I don’t care I hurt…..and no pain meds have helped me at all…..

Hi, had an athriscopy on my right knee, was healing well I believe, that's what the doctor said anyway at the 6 week mark. Then I walked too much and I've had lots of swelling and a little stiffness of the knee...BUT NO PAIN? Has my surgery failed or is having no pain a good sign?

Hello, I had a meniscus tear repair one month ago, i am feeling better day after day, but I still feel the leg is not stable, maybe because I still didn't get back my leg muscles? I am trying to do some exercises at home, but its still getting bigger then the other sometimes and I feel it is tight, I cant open or close the knee very fast.

I have an ACL and MCL tear and after 6 months of physio I still have swelling, shooting pain and lack of full range movement in my knee. This is somewhat due to also having a fracture above my ankle and having and ORIF operation due to the same incident and not knowing about my knee until the cast came off nearly 3 months ago. I was wondering whether or not to continue with physio or have an op to repair them. I am not an athlete but I am quite active and only 30 years of age.

After An ACL/MCL Surgery My Leg Stopped Bending At Certain Point As The Doctor Said I Have ArthroFibrosis Which Needs An Another Suregry To Take Out That Scar Tissue Which Is Stopping The Leg From Getting The Range of Motion .. Is It Normal After ACL surgery ? Plus One Can Come Back To Sports After This Arthrofibrosis Manipulation ? Cheers And Regards

I don't know the statistics off the top of my head, but it does happen, and a second surgery to clean out scar tissue is often needed. If patients get full motion and strength back, they should be able to return to sports.

I’m 64. I had meniscus repair 15 mo. ago. I seemed to heal, but it’s back. Learning to quilt, started bad pain, then a garage sale did me in. I can’t walk much or I’m in pain. Mainly behind my knee…I double over in pain. I have to hold my leg with both hands to get in my car. Then a few days later, it’s better. Cooking a big meal is very painful. That knee is popping a lot, too. Not much swelling. Curious to see what knee doc says Wed. Good luck to YOU! My cousin had to have his meniscus repaired 3 times. He’s in his 70s. Ugh!

I had surgery 6 weeks ago on my left knee.. I tore my lateral meniscus and as i'm quite young,18, I got a full repair of the meniscus with sutures .. I have zero pain and I think I'll be back jogging in about 1-2 weeks .. even though it's a meniscus tear my doctor said he also drilled holes in my knee to cause bleeding in the knee so it would heal better(as you mentioned) , does that increase success rate? Anyway my aim is to be back on the 17th march(st. Patrick's day) , exactly 3 months after surgery, hopefully anyway ⏱⚽️

I had meniscus repair. Then 2 months healing time. Then ACL reconstruction. After a year, pain observed in meniscus area. Again another surgery planned to cut the meniscus. But during surgery, my surgeon opined that it could heal. So instead menisectomy, repair was done again. Now after 6 years of that i still face pain and swelling in meniscus area but not very often. MRI revealed the there still exists grade 3 tear and cartilage erosion. I am 32 and i wonder what should i do now!

    Dr Geier,
I am 34, I have a regular 9 to 5 job where I am on my feet constantly and I play competitive soccer. I have a complex tear of my meniscus (posterior horn, extending into meniscal root). I was given the recommendation of having this tear "repaired". The recovery time is not ideal and after understanding to a degree that there is a chance of tearing it again I am not confident that having it repaired is whats best. I can't afford the time to take 5 to 6 months off of work, its out of the question let alone not having the ability play soccer. I am confident in the ortho surgeons ability to repair the tear however I don't have peace of mind knowing that it could possibly tear again and having to have two surgeries instead of just one is a little overwhelming. It has already been hard enough dealing with this one tear. I am curious as to whether or not my age has a factor in the recovery time of a repair. Does it make sense to repair it at this age, or just have it removed all together? Thanks!

    • @antonia3690
      @antonia3690 7 лет назад +1

      Hi Ken
if you dont get it repaired it will turn into a 'bucket handle 'tear and the chances of saving it will become much more difficult.
You could also try getting 3-4 PRP injections as they could stimulate healing of the menisus depending on how thick the tear is. - I would try this option first before surgery- good luck

My 17 year old son had meniscus repair surgery in Oct 2021. After months of healing and physical therapy he was released to play soccer again in Mar 2022. He promptly fractured his tibia (on the same leg) the first week of soccer practice. He had another 5 weeks of no weight on that leg. Another MRI was done to see the extent of the fracture and we were told that the meniscus had healed well. After he was released from the fracture, he began playing basketball. He is experiencing some pain and swelling in the knee. Is this normal? Will he experience pain and swelling from now on?
