  • What is reverse T3? Reverse T3, or rT3, is one of the most important thyroid lab tests. Learn more about how reverse T3 impacts your thyroid.

    Things that cause you to increase rT3 levels are illness, starvation and excessive cortisol (stress). rT3 slows down metabolism and its theorized to aid in survival. Interestingly hibernating bears have high rT3 levels.

    Yup I asked my doctor for a full thyroid panel and she conveniently forgot to put reverse t3 on my orders (even though I asked for it specifically).

    Thank you so much for all your info vids, suffering for nearly 17 yrs with incompetent ego driven Endos who WON'T do these tests leaving patients to navigate this misery losing their health & vitality & quality of life, is so upsetting & dangerous. Please always do more. Also when a competent Endocrinologist isn't available, what other specialists can be consulted on, ENT's? Internists? Who would also know about adrenal insufficiency?

    Number 1 problem is an iron issue. Number 2 is a cortisol issue.

    I always had trouble losing weight and I blamed my self only to find recently that my reverse T3 was extremely high. Medicare in Australia would not pay for this test. I had to pay for it myself. What's the big deal? It's so important.

    This may be a dumb question... But if T4 is the inactive form and your body needs to convert it.... Why don't doctors just routinely give T3 to patients?

    Just found out my rT3 is high and my doctor prescribed Cytomel. Once started, does it mean I’ll need to continue life long with thyroid meds? I also have inflammation in the gut/yeast overgrowth so I’m also on anti fungal meds too.

    Dr. Child's. I've ordered all the above mentioned tests except for RT3. This is not a very common test conducted in our city and plus its expensive too. Any other way I can get this checked. Pls advice

    What are the values of normal range rT3? How about deiodinase not working well?

      labcorp for rt3 is 9 to 25 but ignore that,,anything above 15 is a problem

    Amazing explanation, thank you! My doctor ran all these thyroid tests and I did show up with borderline high reverse T3. All other thyroid tests TSH, free T3 and T4 were normal. But now I understand that it’s not good my reverse t 3 is high. I was doing some intermittent fasting and often exercise 4 times a week intensely. I’m not on any medications. Do tend to stress a lot too but eat a very clean and organic low carb diet. Any suggestions to bring this down?

      Correction as YT is not allowing me to edit currently but TSH, T3, T4 and free T3 were all normal and borderline high reverse T3 is what my labs have been coming back with over the past couple of years. They haven’t suggested I start any medicine. More mediation and yoga. I see an integrative doctor btw.

    How do I get an internist that understands this. My GP clearly, does NOT! So please inform what questions to ask to get the right internist or endocrinologist.

      do what i did forget regular drs and call the chiropractors and naturopaths and tell the office your looking for a dr that understands this make him tell you on the phone how this works,,if hes not willing move on its cost you nothing,,i found several that way but wanted a fortune to fix it,,so i did it myself,,if money is not an issue call the holtorf medical group they do telemedicine and specialize in this,,they will fix you

    In the UK GP's only test for TSH & T4, Occasionally they will test for anti bodies. The reason being, the treatment is the same .

    Where does TPO come into the picture? I was diagnosed with Hashimoto's off of a high TPO. My TSH, T3, RT3, T4, free T4 were all normal at the time.

      From my understanding, TPO indicates how inflamed your thyroid is, so if it high then its equals more inflammation and the lower the number the better. I have read that diet is big on helping reduce inflammation and zinc and selenium can help as well. My TPO was around 300 and my doctor said that the AIP diet will help get that number down. Hope this helps!

    I asked my doctor about reverse T3 one time and he said they no longer look at it. He said they used to look at it, but it’s been deemed needless now. Doctors seem to feel like it provides no relevant or worthwhile information nowadays.

      I would switch doctors immediately to one who actually cares about their patients. I had a discussion when I first went to my Primary Physician. You need to be strong and tell your doctor what you want and if they are unwilling, switch doctors.
      My preference is a DO rather than an MD.

      @@jonkicker1278: My doctor was open to me trying a combination of Synthroid and T3. Took a looong time to find proper dosing of each, and the T3 did nothing about all the weird symptoms low thyroid patients have. I also had hair loss from it. I was on the combo for about a year and just recently went back to Synthroid only. I had taken it for 43 years before trying the T3. As far as switching doctors, there’s no one to switch to. They are all the same. I was pretty much my OWN doctor when I was trying the T3. I knew more about it than the doctor did. The problem with even “functional medicine” doctors is that many of them are weirdos. They might be more open to trying new things, but they certainly have their idiosyncrasies, too. Let’s face it, most doctors suck nowadays. The system doesn’t allow them to really get to know patients, like in the old days. Insurance companies don’t allow them to do things that aren’t considered “standard” medicine. There are a few good doctors out there, but they tend to be the ones that don’t take ANY insurance. That leaves me out.

      @@pocopico7409 taking t4 and t3 when you have high rt3 will do nothing until the rt3 is gone,,t4 must be eliminated for that to happen

    Does a high level of rT3 also DECREASE the level of TSH, giving a false impression that a person has hyperthyroidism?

    My TSH is 16.9, T4 is 17.8, and T3 3.8. Never got tested for reverse T3. Cholesterol high, weight gain and exhausted. Would T3 be better option, as been on Levethyrodin for 13 years?

      I am not a doctor, just a person who read a lot about thyroid issues. Let me clarify what you are saying. What is "Levethyrodine"? I can't find this medicine on google, maybe you spelled it wrong? There are Levothyroxine or Thyrodin. Both treat _opposite_ diseases hypo- or hyperthyroidism respectively. You have high TSH which usually means you have hypothyroid, but at the same time your T4 is way too high for someone with hypothyroid. Are you 100% sure that those results are correct? Can you check one more time and tell me the measurements ( like μg/dL or ng/dL) that are used in your results? Also check if "Levethyrodin" is the correct name for the medicine you use.

    Is it possible to have everything in the "normal" range on a lab test but really high rT3? Since most doctors don't check rT3 they could say no thyroid problems when there really are problems 🤷‍♂️

    If someone has hypothyroidism symptoms but has TSH 1.09 mUI/L, Free T4: 13 pmol/L and Free T3: 4.6 pmol/L, what could this mean?

    My doctor won’t because she said it’s only for patients that were hospitalized for illness... flat out refused to order it for me. How can I order it myself?

    Hi my RT3 is at 24 the normal range scale was 10-24. I don’t have a decent endo and I had to demand the RT3 lab. The levothyroxine has been putting weight on me and I’m now stuck. Have the full lab report but nobody i trust to tell me what next or what meds etc. Can you assist?

    Hi, just had all my labs done- TSH 0.015. Ft4 1.84. T3 4.34 and reverseT3 22.3 - I do take both synthroid and cydomel everyday ,cuz had my thyroid removed feel my medication needs tweeked abit? But my doctor doesn't want to cuz I'm feeling fairly good??? Just wondering what how you would ajust my meds according to my labs.

    PLEASE provide the optimal levels of rT3 Free T3 and Free T4. I appreciate the explanation you provided, but it isn't helpful without knowing optimal levels. This is my biggest frustration. You talk about why physicians don't order rT3 but if they don't know what they're looking at or for it makes NO difference. I can look at my test results, get online and find out optimal levels, but so what. How do all of these test results paint a picture. If rT3 is high and Free T3 is low and Free T4 is off the charts what does that mean? Do I need a medication adjustment or what? Paint the picture, please.

    I had radioactive iodine treatment 26 years ago , taking L-throxin 0,1 mg .I go for blood test twice a year but only TsH .Over the years I developed so many symptoms of low thyroid but doctor by looking my TsH telling me I'm fine. It is difficult to find good endocrinologists here. Can you tell me if I need to check other thyroid hormones.

    What about Leptin Resistance? Doesn't it turn off the ability for the liver to convert T4 to T3, because of high TNF and HS-CRP? Basically disabling the Thyroid? And at this point reducing production of Vitamine D as well?

    Can you discuss combo t4+t3 therapy and how it is being administered? I know t3 (-synthetic) does not have to be taken in the morning on an empty stomach; so do you just pick a time for the t3 according to your energy needs?

    I would love to find an endocrinologist (because I also have type1 diabetes) who is in the know with this and is progressive about treating hypothyroid patients - my endo is strictly worried about the TSH and free t4- annoying and especially when I was finally feeling great after convincing her to let me try adding cytomel, but because my TSH was only .03 she decreased my synthroid dose by another 25 mcg- now I feel like crap and my weight is going back up and my blood sugars have been erratic since the decrease- so incredibly frustrating- she is leaving practice so I do need a new endo anyways- do you have any sort of list of doctors across the country who are inline with how you treated your patients?

      From my understanding, stress management is huge since stress = high cortisol which is considered the "stress hormone" which gets produced with inflammation, overt exercise or any type of stress to the body, which could lead to thyroid imbalances. It's more of helping balance the thyroid gland than it is finding ways to directly target Reverse T3. I'm not sure you can directly target T3 but I may be wrong.
      Balancing your thyroid gland is how you target you T3

      I don't know the number, but my brother, my sister, my niece, and I all have Hashimoto's. I have 1 brother that doesn't have it.

    Hello doctor. Thank you for educating us. I just moved to Yuma, AZ. I was tested in TX and my results are: TSH, Tird Generation : value 1.190. T4(thyroxine): 6.2
    Free T3: 2.9
    Follicle stim hormone: 31.2
    Estradiol: 53.7
    Testosterone: 204
    I am takin NP Thyroid (Armour) 60 mg tablet.
    Please help me to understand this lab test and if the medication dosis is correct. Thank you VERY much. GM
