I share the difference between open and closed-glottis pushing, why you would want to do one or the other, and how to push effectively to help protect you and your baby.

I had my daughter 8 months ago and I saw all of your video since the day after I found out I was pregnant, and am here to say THANK YOU ! I went in there feeling excited and prepared to have a natural delivery I got there at 4 cm I got complements from my nurses and I was told this was the most beautiful birth my main nurse has ever seen she has been a L&D nurse for 20 years ! So thank you so much for providing me guidance during those COVID times that all the birthing classes were canceled. God bless you so much you are doing amazing work here and it is so much appreciated!!💗

Yes!! Thank you for this! With my firstborn I was told to hold my breath and push, and after he was born my chest hurt so bad that they thought I might have a blood clot. I didn't, it was just from the pressure of pushing like that for two hours. Even after going home from the hospital, my chest hurt for weeks, especially every time I was standing.

26+ years as an L&D nurse and my experience is that closed glottis pushing is FAR more effective! You can feel the baby's head moving much more productively, UNTIL it's crowning, then open glottis or breathing as he/she crowns is great for protecting the perineum. I think there's a balance.... it's not all or nothing. I also volunteered in a women's clinic in the Philippines (NO epidurals, no oxytocin during labor) and this is how they do it successfully....so, relying on Mom's instincts. This is how all their babies delivered. Active pushing, then breathing through the crowning.

This video is great! I practiced Hypnobirthing with my second pregnancy and I would practice this breathing/pushing every time I went to the bathroom. During one “practice” my water broke LOL. When it came time to push, babes was out in 4 pushes!!!

There are three main parts in the push, whether open glottis or closed. To move baby down you need the pneumatic pressure from the chest, abdominal pressure, and the downward press from the pelvic floor. When your holding your breath its definitely easier to maintain the pressure in the chest but without the other two muscle groups working, its all in vain and strain. The most challenging part is getting women to be aware of the muscle groups in the pelvic floor, isolate them, and control them at will. For me, practicing simple pelvic floor breathing helped a lot.
Personally I hold my breath to push. It seems quicker and more powerful. I’m not too concerned about tearing (3 babies, 1 stitch). And I’m only holding a push for 5-7 seconds or so. Maybe I’m used to much longer holds in yoga practice. It also seems more natural for my body to hold.

Hi Sarah, what if I do the breathing down or laboring down technique during this pushing state? I've been practicing it when I'm constipate & it really helps. Like reallllyyyyy helps! Thanks to you 😆
Anyway, is it ok to do that while pushing or do I have to actively push the baby out by really pushing instead of breathing down? Thanks love! 🧡

Can you make a video on calcified placenta? If you've had experience with a preterm mature placenta. In my case, I'm healthy and baby is too. Everything within normal parameters. I am sure my several winter infections (including covid) made the placenta mature too quickly (there are studies that suggest it). Anyway, there are interesting studies I found in regards to consuming beta carotene (I've been drinking carrot juice) and antioxidants etc. that could help. I'm in Spain and they are pretty relaxed with the monitoring. Sounds like USA would pay closer attention. Thanks!

With my first I did the counted pushing because I was in hospital (non medicated birth) and tore and needed stitches. With my next 5 babies I had them with midwives and didn’t hold my breath, breathing while pushing (blowing out the candle) and haven’t had tearing with any of those births.

I did the breathing down technique with my first and at one point my midwife (who was amazing-not complaining at all but just curious) told me I had done well breathing down but needed to actively start working on pushing however it wasn't until two hours later that I legitimately felt the urge to push (then it was about 4 pushes and boom all done, open breathing for those last few I couldn't physically hold my breath so yay for doing this unknowingly) can you speak to whether it makes a difference to actively push before we "feel it" vs waiting until we get the urge? 16wks with my second and I plan on all natural again but would love to nail down my breathing/pushing technique with confidence.

Can you please make a video about VAGINAL twin birth tips?? I am so frustrated by everyone telling me they would never attempt it. I had a successful vaginal birth already with a singleton so my midwives are going to let me attempt to push with the twins if they aren’t premies or in distress.

This is what I got so mad at my midwife for I naturally open glotus push and my midwife kept yelling at me to hold my breath and push I did it once just to prove it made no difference to me the only thing that happened was when I pushed like that because it was a less controlled push I tore and felt it
Afterwards she said that it's rare to naturally be able to push like that but after 25 years of constipation ya naturally get it haha

Hi Sarah love your videos! I have a question for the next Coffee and Questions; do patients always get vaginal catheraters and how does that work for vaginal deliveries? The idea sounds alittle scary to me ! Lots of love from Canada!

    • @n9netwiztidclownz

When I was cathed for my births, it was always taken out immediately after emptying my bladder, not left in during the birth. After I had my son, I had difficulty getting out of bed and the nurse did another catheter but once again - it wasn’t left in. Hope this helps ease the idea of having one. :)

I just had a baby and I ended up getting an epidural . The doctor told me to hold my breathe and push…. I couldn’t feel anything and I couldn’t tell how to push. I kept just spitting at the staff . I definitely popped blood vessels in the face and they had to put oxygen on me. I couldn’t push they ended up needing to use suction on the baby’s head . I ended up tearing pretty bad in the vaginal wall and not my posterior vaginal wall is at the entrance still 3 months later . I need pelvic floor therapy but man I was so upset with how this birth went and I tried my best to prepare with all i could think of

Hi Sarah.I don’t like needles so my biggest fears of labor and delivery is getting an IV tearing and having to get stitches. What exactly is the process of getting the IV?How can I prevent tearing? And God for bid if I do tear what exactly is the process of getting stitches?

    • @mamaraya8818

Have you heard of pereanal massage? That may help. Also, relaxed muscles are less likely to tear so just try to relax and let life happen. I gave birth to my youngest at home on my own (unintentionally) just let my body do its thing once I realized my ride wouldn't make it in time plus getting to the hospital. She was perfect and healthy, I didn't tear and it was so peaceful it just being me and her when she came into the world and not surrounded by medical personnel. Paramedics had a fun story to tell haha. Good luck! Happy laboring ☺

And I didn't see your question about the IV. They'll likely place it somewhere you can still be mobile i.e. your hand. Unless you want an epidural that goes in the back. If you go natural and need stitches they will inject you with a local anesthetic down there and stitch you back up. I've had stitches after an unmedicated birth and you're so preoccupied with just having given birth to your beautiful baby that you really don't notice what they're doing. I nursed while they did my stitches. You'll be great and so will your baby.
