    The right quadrutus lumborum, or QL, is a lower abdominal muscle. It is often tight when you experience a lateral pelvic tilt. Sometimes the QL itself is painful, but sometimes the tilt is causing pain elsewhere, particularly in the SI joints. I had this experience for a long time. The lateral pelvic tilt didn't resolve until pelvic repositioning.
    There are many pelvic repositioning techniques. This is just one of them.

    I pulled my QL at 23 and I'm 29 and lived with the tilt and it's gradually gotten tighter and so many chiropractors I visit say "nah the QL won't do that" and literally this man explained my entire problem verbatim....

    I've had this problem off and on for over 30 years. I think too soft of a mattress (sleeping on back in a dip) aggravates it. I can usually walk 75% of it off but takes 5-6 days to get back in place. Great video!!!

    Wow it makes so much sense. I’m the opposite, my left is higher than my right probably because of my injury. I just “really” watched myself walk in front of a mirror. And I saw everything you were saying! Thanks

      Hello Rebecca, I have the same problem because of herniated disc or si joint injury and my pelvis is higher on left side and twisted, my right gluteus mediums is smaller and my right leg is weaker. Right leg is unstable and shaky during running.
      This has develop over ten years.
      Could you please tell me if any exercise or treatment helped you.

    I had an issue with my left QL when I was 22, so it s been 20 years that I had been lived with it, also brings pelvic tilt, finaly with the right technic of training of my QL has been fixed, also pelvic tilt fixed as well.. Thank you for the video and information.

      Guys get a tennis ball and try to play it with your feet every with a small amount of pressure it's kind of massage but when the ball under your foot, explore some ankle, knee and hip movements, when you feel pain don't go forward more and keep explore the hip around that pain

    OMG - Like your blog I have had this issue for years. If I do a QL stretch as prescribed by physio the issue can get even worse (as it is today). I have the exact problem you lateral pelvic tilt and my right QL is always switched on. No one has been able to resolve this. I will try some exercises today. You could be my life changer - I live in hope

    Hi Neal hoping you see this message, ive watched alot of your videos.
    Ive suffered with left sided pain from my back down to my quad, mainly my left QL and left TFL.
    I feel like my left side is always turned on and because of this I had many trigger points and my right side is weak because its been switched off for so long, is this possible ?
    Also Ive been doing the 90/90 with my feet on the wall, when I take my right foot off the wall my left groin cramps up really bad and also my left QL cramps up to the point I can only hold 1-2 breathes and I have to stop.

    Thanks you so much for the video. I would get these on my left ql after squatting with heavy weight and no belt 315+, back would spams and leave with a wonky stance for a few days an pain. These stance and breathing exercises really helped. Now I just have to fix my technique working out so it doesn’t happen again

    One Day i Was Jogging and Felt Pain in the Front of my Hip on the right side... Eventually i noticed when i stood on it i tilted... IM just now finding out what i have and my left slightly tilts ... What Should i do what Type of doctor i Really Wana fix this

    Do you think putting an insole in opposite side of your hip hike will help?
    I tried this and it immediately activated all my weak muscles and relaxed the overactive ones

    It's still not clear what I need to do I have a "short" leg and my opposite knee is in pain from overcompensation. I was so flexible, dou g the splits until ine pose that made me stuck since then on. No exercise could readjust and balance my pelvis and sacrum.

    Can sitting with 1 leg higher then the other cause this. Like driving for multiple hours out of the week with 1 leg at a 90/90 while the other one is on the pedal ?

    Wow I wish I saw this video years ago but grateful🤙.
    Currently have right Achilles tendonitis and plantar fasciitis and in a cast. I believe it’s from years of lateral pelvic tilt which no Rothman doctors have ever said to me… I want to try this but I am on crutches and scooter and my tilt has been worse bc of this… visually you can see how my eyes jaws shoulders hips and legs are all uneven. My rib cage is so out of place too! I actually want others to see my case to help any one younger dealing with this!
    I am in New Jersey if anyone can help!

    The best explanation and link of QL and pelvis and linking to gait and stance. How it affects running and posterior chain is never explained and this is the start. Sidelined me from running uphill for years and extremely frustrating since no one connects textbook functions with actual movements. Just wish I knew what I do to cause it so often. Running uphill and being a triathlete might not help

    what if your left QL is tight? My left side is side bent and my left side hurts. My left leg is shorter and the illium posteriorly rotated. My right illium is anteriorly rotated. I keep doing the 90-90 but it feels like it is making it worse. Thanks for your advice

      The left QL can be tight, but for a different reason than the right QL. I would recommend watching my video about "why you aren't a right AIC", because many people think their right illium is forward rather than the left (but in six years of doing PRI I've never seen it through testing). Try searching for an "all-fours" PRI breathing technique on RUclips. Some people can't do a 90-90 without guidance (position and breathing is very specific).

      You have to reposition your pelvis through a PRI exercise. Something like this:

      If you are in the United States, there are lots of PRI trained physical therapists that would know exactly what I am talking about. You could check the Postural Restoration Institute website and go to the "Find a Provider Page".

      @@naima99 you can do the same exercise. You don't want to do your right leg. The underlying pelvic pattern is the same no matter what side is higher.

      It's the left AIC pattern that leads to the muscle imbalance. Some imbalance is normal. But when that imbalance gets too great, that's when you end up in a tilt. Another way to look at it is that the left AIC pattern is normal, but when the normal becomes too extreme, the imbalances become too great, and the tilt results.

    Great video Neil , if the right side is High as you mention in the video how does this fit in with the Left AIC , I thought in a left AIC we will see the left side higher ??

      From my experience, either side can seem high from a visual perspective. Underneath you'll still find the Left AIC pattern and usually accompanied with a right BC pattern. The right BC will internally rotate the right ribs and thus cause over activity of the right QL due to this rib position.

    This is probably common with people who had acl surgery or another leg surgery where they had to be on crutches for an extended period of time. Or even favoring the good leg too often. I had acl reconstruction on the left and afterwards I developed a lateral pelvic tilt (higher) on the right.

    Neal, I need this work. I have right pretty severe pelvic tilt. My rt side of body in mirror is higher than left. I’ve had Pilates, PT, MFR, stretches, a pain stimulator implanted lumbar spine. I have knife stabbing pain while walking. It has impacted my life big time. Pain is with walking and it’s only rt sided. It feels like dagger going through QL area. Rib cage, iliac crest, hip and knee pain. I’ve listened to your utube videos and you’ve described me. I’ve tried it all and it hasn’t worked. I daily stretch and try to strengthen. Please comment.

    My QL is painful, inflamed, feels tight when stretching it. I also have inflammation on the top of my hips and especially on my lower floating rib. All of this is worse on the right side. Can a tight QL pull on the lower floating rib so tightly as to cause it, or the tissue wrapped around the rib, to become very inflamed?

      You can certainly try. I don't think it will resolve anything long term, since I am talking about a neuro-muscular pattern that involves your entire body, not just the QL. The QL may be "holding" the pelvis into a tilt, and it may be painful, but it isn't the culprit. It's just a victim of a body that can't shift its center of mass over to the left.

    I was walking and leaning to the left and my left ql turned on. I felt the left back rubs shift down in the first time in like forever. It’s like my center of gravity is off to the right and I don’t know

      Definitely don't want to feel the left QL. It means you are in extension (left AIC pattern).

    I don't know if my pain is caused by this or something else because I have similar symptoms to diabetes (neuropathy in feet) without actually having diabetes. Calves are constantly twitching and I get bad hip pain. I probably have nerve damage in my feet by now. I've been told my left back muscles were tight. However I've had pain down both legs. My shoulders are sore no matter how I sleep (can't sleep on my back because my lower left part of my back gets severely irritated). I've lost the ability to accurately report symptoms anymore because whatever this affliction is has screwed with my head so much and destroyed my life for over 4 years. I've been told by every doctor and specialist under the sun that there's nothing wrong with me. It's been so long I've had to write down everything I've tried because I'll forget most of it when someone asks. The fact that I can even work at all is a miracle.
    God I'm just so tired and my body wants to die even if I don't want to. And I can't even see anyone because of this modern plague we're going through.

    I have a high right hip, low right shoulder and my right thigh is more forward than my left thigh. Is that a left rotated pelvis?

    I think I have Rt. Lateral pelvic tilt. I have super tight QL and psoas on the Rt. But... I have Lt CAM impingement and Lt Labrum tear in hip plus as an added bonus a thumb nail size of OA on my Lt femur. I am in my early 40s. I am trying to hold off on surgery until I'm at least 50. How can I fix this? It seems anytime my psoas and QL get super tight my hip starts acting up.

    Wow. I have a twisted pelvis due to a year or so of not being able to walk properly on my left side due to a knee injury. As such my right side has been dominant and left become weak. I have strengthened my glute med significantly and largely fixed the imbalance, but the pain has still been present. Watching this made me realise that I never properly relax in left stance and allow my entire body to bear weight, probably due to the knee injury. Since, I have been conciously ensuring I am swaying to my left side, allowing my ribs and hips to get closer together and exhale and WOW the difference has been crazy, right sided lower back pain significantly reduced! Amazing video, thanks so much.

      Nothing really changes because the underlying patterns are the same. There might just be extra compensation going on. I'd advise to seek help from a Postural Restoration Institute credentialed provider.

    My left side hip is higher ,my left side rib cage is going out side and my left shoulder is high as compare to my right shoulder. Also my left feet is pronating and i am facing pain in my left side of the neck.

      It's not the "norm" but it does happen. I've worked with many clients who have higher right shoulders. Mine still tends to be higher.

    So what s the fix. I don t get it.
    My left pelvic is tilted 10mm up and backwards. And my left QL is a pain nightmare. Sitting for over 30 minutes is impossible for me.

    My right hip seems higher, if I lift my knees to a 90 degree angle with my heels even on the ground my right knee is higher, my ribs seem to stick out on the left side and I'm having lower back pain on the left side, the left QL feels tight. Is this weird, would this fix help me? I originally hurt my back at the gym while doing barbell rows a few years ago... Thanks.

      What you describe is perfectly normal in a lateral tilt. Your left rib flare is normal in the left AIC right BC pattern (there are videos about both patterns), and a left QL is almost always tight because of the anterior tilt of the left pelvis. The exercise demonstrated can definitely help, but most people will need more than one exercise. But this is a good one to start with.

      ok, much thanks for the response. so if I'm doing the exercise with my feet on the wall, it is actually the right foot I lift off to use the left hamstring? I guess I'm just a little confused what actually means left and what right... :) can you recommend any other exercises/muscles I should work on?

      Yes, you use the left foot/hamstring. You don't use the right leg. Using the right leg would reinforce the pattern we are trying to break.

    Interesting. I will try it out. Is massage or chiropractic treatment anything that would help to solve this issue? (in combination with the fix you mentioned in the video)

      Honestly, I don't know much about chiropractic treatment. I am a massage therapist, however. Massage could help by inhibiting the overactive right QL, but since this is a neuro-muscular movement issue, if you don't teach your brain a new skill (moving into true left stance) it will have no reason to break it's pattern of right-sided overuse.
      What is often not understood (and admittedly this is complicated in the beginning) is that this is a pattern of right sided overuse. The only way to stop right sided overuse is by re-establishing the pattern that it has lost: the left sided pattern (left stance). We need to have both a right sided pattern and a left sided pattern. If we only have the right sided pattern, we end up in pain.

      @@TheKaurajuoma You're welcome. This is just one exercise. For some people that's all they need. Other people have more extensive patterning (especially in the upper body that contributes to the tilt) and need more exercises.

    Hi doc, I used to sit on a fat wallet under my right glute and used to workout in gym in a very bad form,i started having problem with my shoulder being uneven (right is a lil higher and backward and left is lower and forward). My left side of the body is stronger so have a lil more muscles on it. After searching a lot i figured out I have a lateral pelvis tilt with right hip smaller(i used to sit with a wallet on this) and left being high and more muscular. I see videos saying hips which are higher have glutas medius weak,but that part is a lot bigger than the left one. My chests are also in different shape and size bcs of it. I am literally very very depressed seeing that,whenever i wear any t shirt or pants,it feels weird,loose from one side and tight from the other. I will be grateful if you could help me out with this. I feel like I can never have a good body.please help.

    Neal, had knee replacement 1 yr ago. Pain on my right side as you said exactly. Went to Chiro and he said my left leg is shorter than the right ( Knee Replaced). Wants to put a heel lift in my left shoe to correct pelvic tilt.. Your thoughts

      Hello Vito. Does he think it's a "true" leg length discrepancy? For example, does he think you were born with it, or does he think it happened as a compensation for the position of your pelvis? It is quite common in the left AIC pattern (pelvis oriented to the right) for a left leg to appear "shorter" due to the position of the pelvis. When you reposition the pelvis into a neutral position, that difference goes away because the leg no longer has to compensate for the rightward oriented pelvis. If you do use a lift in your shoe, I think it's better to use a full insole, rather than just a heel life. If you only lift the heel, it could alter the natural biomechanics of the foot and cause unbalanced calf muscle activity. But that's just my thoughts in general without knowing the specifics of your case.

      @@NealHallinan Neal, it was from the knee replacement. The Ortho Doc straitened my bowed leg thus making it about 1/4 to 1/2 in. shorter. The Doc said it should not effect my gate. Mywife always yells at me to stop tilting to the left. My left shoulder is down. The Chiro said what is happening is my hip is pushing up thus shortening the muscles connected to rib. When he puts his finger on my rib I can feel it. When I tilt to the right thus stretching that side in my golf swing, I really feel it. What can I do to bring it to a stable position? PS. I have been sleeping on my left side since the surgery. Perhaps this has an effect on my right side being all crunched up?

    In the side that pelvis is up(left) can be created in the foot high arch? And if can it be what can i do for that? I have tightness 4 years on my right side of sternocleidomastoid. The physios doesnt sold the problem im tired to tried many things. Im very active person and very athletic i have healthy life i eat healthy food. Im playing tennis often and i dont know if this make problem worse.

      Your left QL is "on" because of the anteriorly rotated left side of the pelvis. It puts your lower back into an extended state.

      Daniel Hackett was ur left side of ur back looking flatter than the right ? Strengthen ur psoas on that side and the right side stretch the psoas and stretch ur hamstrings. This helped me soooooooooo much and I hope it helps you. Also, do the Thomas test to figure out which psoas/hip flexor is tighter. Most likely, the right will be tighter than left.

    My situation at the moment with the pelvic tilt and scoliosis, is that my Right side of my back is bigger than the Left. Also, my Right Hip is higher than my Left Hip, but my Right Leg is longer than my Left Leg. Does anyone know about this problem and know how I can fix this? Been dealing with this problem for years now and I'm only 19. (btw, i used to be overweight as a kid and always stood on my left leg when I was standing in one position.)

      @@naima99 I doubt you have a weak left psoas. Have you tried the "right sidelying left adductor" exercise I posted a while ago? And have you tried my "All 4s Left ZOA exercise". I'd try that, also. Sometimes people can't expand their left posterior ribcage and this prevents their left hip from achieving optimal positioning.

    I have been suffering 10 years from neck and back tension an discomfort and tingling and burning sensation and pain the whole left side of my body due to a bad sitting habits in work car dialy sittings even this issue affeted my digestive system causing constipation and lots gases and my bladder too
    I tried all stretching exercises in RUclips everyday I do sessions trying to target the trigger points and I feel ok but the issue comes back after sometime
    After watching my body and my pelvis when I stand and letting my body loosen I noticed my letf side shifts automatically to the right side even I feel tension in my left leg when I walk I tried to correct my pelvis to the left with some exercises but the results are not good enough for me
    After watching this video I focused on the basic idea which is shifting the weight from the left pelvis to the right pelvis (in my situation) and breathing and I really felt so much better within 5 minutes much better than all the exercises I have done
    I felt my body relaxed fully and felt some bowl movement and gases to be released
    I’m really so happy for this

    The interesting and scary thing is that the QL refers pain, into the abdominal and RH Flank, and it also mimics appendicitis, which is well noted in Myofascial books.
    Leading to a whole host if unnecessary examinations and not to mention pain.
    As well as Gluteal pain and mimics sciatica.

      As a traditional massage therapist I can tell you- it takes more than one treatment combined with activating the dormant muscle tissues, relaxing the dominant tissues and resetting the nervous system for everthing to release and re- orient the entire body. It takes time and a therapist who knows where to start what to follow with and where to strenghten where and what to turn off plus deal with any pathologies that may be contributing to this issue.
      In a nut shell this vid is correct.

    I think that it is exactly the other way around: If you stand on your right leg, the left QL has to tense up more so than the right side and vice versa. This is also how it also works in my body. Also the way you illustrate how to walk/stand looks totally instable and unnatural. Am I the only one who thinks that way? Especially if you are somebody who walked around like this for years, I doubt that you can reverse this by simply fixing how you walk. There will be physical changes (fascia, ligaments etc) that are adapted. So you HAVE to stretch that tissue to allow a behavioral change to have effect.

      It depends on the person. A rotated pelvis is just a pelvis that is oriented in one direction (usually to the right). All PRI exercises are designed to fix that situation and orient the pelvis to the left.

    You had me at ribs! I dislocated two ribs and have had difficulty jogging since that happened. I dislocated two ribs on the left side and it has made my right leg feel longer and tight in the hips. I’m going to see if this helps. I miss jogging.

    Question about a PRI training. I think this could be the option I’ve been looking for.
    I have lateral tilted hips (I believe it started after a car accident I had in adolescence but not positive), a “crunchy” left knee (I don’t know from what exactly, but it started within a year from the car accident), and a bad right ankle (happened a few years later from a sprained ankle and I never dealt with it 😬).
    Would PRI practitioners deal with the knee and ankle issues as we work on the hip issue? They seem interconnected in that they could either cause or worsen the other.
    Or do they need to be addressed separately? And if so, should one be addressed first?

      Yes, everything is related. The underlying asymmetries are the same regardless of how you present visually.

      What I describe in the video is what causes a lateral pelvic tilt. Overactive muscles (usually a right QL or left psoas) that can't turn off due to the position of the pelvis.

      The massage can help, but massage doesn't teach your brain anything. Postural Restoration teaches your brain to re-establish a lost movement pattern. Since our brain controls all movement, and a lateral tilt is the result of improper movement, we need to teach the brain a new skill, and that skill is to effectively transfer our weight to the left side of our body with the appropriate muscles.
      Massage is good for helping reduce overall bodily tension, but it won't resolve a neuro-muscular movement problem, which is what a lateral pelvic tilt actually is.

      I was always told to release the overworking but then you have to retention the underworking muscles in order to make any neuroplastic changes. Knowing muscle sequencing and trying to self-diagnose for me is the difficult thing. Your videos are giving me some great focus with the idea behind ZOA.

There are many exercises utilized to "break the pattern", one of which is the 90/90 exercise that you can find all over youtube.

      @@davsss6406 same thing, PRI exercises made my face more symmetrical. We use Pterygoid and temporalis muscles to shift temporal and sphenoid bones. Those two bones are the cranial bones we have most control over via muscular contraction.
