I've had something similar happen to me, basically Rev01 gives you an extra life after achieving 50000 points but only if the region is set to JP. My guess is something got overwritten to the RAM address it saves the next threshhold and gives you a life prematurely.
I'm curious if it affects all Japanese Mega Drive systems or just certain board revisions. I know VA0 for example is different from any other Mega Drive/Genesis released outside Japan.

Actually I tested this on emulation. I set the country to Japan and yes I am receiving the life counter glitch and some random person emulated this when recording a gameplay of the rom hack got the green hill glitch so no it's not a hardware issue.
im honestly kinda sad to see this rom hack not talked about more and shocked to not see any new .exe stuff based on this. for me this was the best sonic.exe thing to come out back in the day as it was a really cool concept, with a great new spice to gameplay. some of the new stuff people have done like eyx and omt/olr are great, but its really missing the gameplay in my horror game. and even today i think this rom hack and sonic.exe spirits of hell are the best .exe content there is. i hope to see more like an ordinary sonic rom hack in the future (if i knew how to mod sonic 3 air or make rom hacks i would do it myself)
