Heartily thanks Respected Dr Bhandari Saheb for prompt reply. Sir, your videos are very informative and beneficial. Great service to humanity/mankind. Sir , At which stage of of PKD 1 (and patients age) tolvaptan should be started (although with nephrologist advice). What are the side effects and how to mitigate side effects. I shall be highly obliged, if you would kindly reply. Regards
Tolivaptan in ADPKD has been used long-term without adverse effects. Its more effective if started early.Safe for long-term use. Effects and side effects of tolivaptan is polyuria and nocturia , hypernatremia, liver injury.so it is important to have good hydration in the morning. Liver function test should be done every two weeks for one month then monthly for 18 month then every three month lifelong. This drug may need to be stopped temporarily when patient is having fluid loss because of diarrhea vomiting, and if there is deranged liver function test. Blood test need to be monitored is liver function, test, uric acid, sodium and kidney function test.

Respected Doctor Saheb, Namaskar. Whether any new drug is expected soon for PKD 1 treatment (other than Tolvaptan). Whether gene therapy is also expected for PKD treatment in future (time span 4-5 years). Regards.
Greetings of the day At present only approved drug to slow progression of PCKD is tolvaptan. You rightly said research is ongoing on many steps in including gene therapy. Actually PCKD is slowly progressing disease so to know the effects on needs many years to see the benefit of medication. Right now Control of BP, Adequate hydration and low sodium diet and tolvaptan is main therapy.
