    Is Vitamin A more toxic than isotretinoin at high doses? I read that Vitamin A accumulates in fat stores. Doesn't isotretinoin do the same thing?

      Both Vitamin A and retinoids are accumulated in the liver and fat. However, it takes many months or years of treatment at very high doses for this to occur.

      @@Anon-nv7bp You may be right, but there have been no studies comparing the two. Both are fat soluble and therefore could accumulate in equal amounts in the liver.

    I used Actuaten in 2020 roughly 40mg varied for 6months from memory. Now after 4 yrs I am getting slight acne, one cystic acne and small papulates. What would be a good range IU for low level acne maintenance?

      Good question, It usually depends on thew person. Studies have shown that somewhere between 20,000 to 50,000 units may be beneficial for acne. Much higher doses have also been used in the past.

      It causes increased pressure intracranially, not hydrocephalus, which is a different condition.

    after how many days should the lips start to chop to know it’s working? eg if i start taking it today and in 5 days there are no side affects, should i upper the dose or just take it for longer?

      Usually each dose is taken for a week or two before we increase it further.

      Vitamin A is a fat soluble vitamin and accumulates in the body. It is usually used for 4 to 6 months, and then a break is recommended.

      Vitamin A is a fat soluble vitamin an accumulates in the body. Long term treatment is not recommended bit short periods of 3 to 4 moments are usually considered safe.

    How much vitamin a would i need to take per day as a dosage after an isotretinoin therapy (10mg to 20mg a day) against seborrheic eczema?

      It varies according to each person. Our usually requirement of vitamin A is about 1000 to 3000 micrograms/ml (IU). For it to mimic isotretinoin, doses of about 10 to 20,000 units are probably required. Some of the studies have used as high a dose at 100,000! I usually tell my patients to base the dose dependant on the side effects. If they start to get a bit of dryness in the skin and lips, its probably working at that dose.

      It's quite a high dose, so breaks would be helpful. A three to six month break between courses are worthwhile.

      In a young person with no other medical issues, it seems quite safe. No tests are required in a healthy person.

    Hello doctor sir, my dermatologist prescribed me 20mg of accutane per day for 2 months.
    Im taking dutasteride 0.5mg once a week and finasteride 6 days a week 1 mg ..is there any risk of interactions

      Looking at the literature, there is no significant interactions noted between isotretinoin and finasteride/dutasteride.

      Vitamin A can be even if isotretinoin has been used in the past. It is a fat soluble vitamin so it cannot be used long term. Usually it is used for 4 to 6 months at a time.

      Vitamin A can be considered a natural substitute for acitretin or isotretinoin, both of which have been used to treat HS. There are quite a few treatment options for HS. They are described in this comprehensive video.

      Hi, in response to your queries,
      1. Yes, slim people can get HS too. As mentioned in the video, the bacterial flora can change in any person and follicular keratinisation that occludes the pores can also happen in thin people too. Those with higher weights are more prone to it. The analogy is that non smokers can get lung cancers too, not just smokers even though the latter are much more likely to.
      2. The condition seems to localise near the folds of the skin as mentioned in the video, due to occlusion and moisture. It is unusual to get HS on the body and arms, though it can in severe cases. As long as most of the skin in the armpit or groin is removed, recurrences in the scar are not that common.
      3. Only oral zinc has shown benefit in HS, not topical. There are no convincing studies of topical zinc for HS. @@philippaswallow5206

      The studies that were reviewed used pure vitamin A, not the palmitate or beta carotene. They measured it in IU and very high doses of pure vitamin A were taken. The usual recommended daily allowance is about 3,000 to 4,000 IU, but the studies used 10 times that dose.

      There are no studies comparing the dosages of Vitamin A and Accutane. The studies on Vitamin A suggest that a dose of between 50,000 to 100,000 IU/ day may be needed to help in acne.

    Is 40,000 IU vit A tablet safe to consume for the skin or will it cause liver damage or is even upto 50,000 IU safe doctor ??

      There ahve been studies where doses of more than 100,000 units have bene used. I tend to ask my patients to use a dose that causes a bit of chapping of the lips but if there are any other side effects or concerns, a lower dose will be required.

      The dose of vitamin A would be dependant on the side effects and is variable in each person. As long as a dose helps the skin and causes no side effect, that would be the correct dosage.

      Vitamin A is a vitamin that we all need. As long as excessively high doses are not taken for prolonged periods, it is a safe supplement.

    Hello sir i have a keratosis bumps on my Arms, shoulders and it even spreaded to wrists i been using salicylic body wash and lactic acid body lotion but still no any improvements should i try taking vitamin A capsules and see if it may work i am feeling so frustated and my self esstem is getting low please help me out sir i need your Guidance

      In the UK, we tend to start a dose of 10mg a day for 4 to 8 weeks, then go up to 20mg a day for 4 tom 8 weeks. The brand used here is called Roaccutane, but there will be numerous other brands available all over the world. @@dilipdas1920

      There is no specific recommendation as most studies were done in the 1960s and 70s. Generally, a dose above 25,000 IU may be needed. No safety data is available either. None of the studies revealed any serious side effect.

      It is different for each person and you can only try different doses to see what suits you the best. Also, inflammatory conditions can be controlled but cure is difficult to predict. 50000 is quite a high dose, so it will be beneficial for most.

    I was taking isotretinoin 20mg for 3 to 4month . After quitting the medicine my acne came back again within 7to 10 days , what should I do now I don’t want take that medicine again because later I want to have fmly planing I am a married girl .
    What else can I do to cure pimples, I am a yoga and fitness instructor I don’t know why all this is happening to me I am very fit i pay lot of attention to physical exercise. My doctor gave me isotertinon , angelic acids cream for 3 to 4month .

      Acne tends to be a chronic infammatory condition and there is no cure to date. Isotretinoin sometimes switches off the skin inflammation but in some, it tends to be persistent. Both Isotretinoin and vitamin A cannot be used when pregnancy is considered. Metformin is an option that can be considered along with dietary factors. Here are a couple of videos that explain this:

      The use of Vitamin A for prolonged periods of time can be detrimental to health. In most studies, high doses of even 100,000 units a day were used for about 3 to 4 months without any log term effects.

      2400mcg is about 8000 IU, which is a fairly good dose. As mentioned in the videos, much higher doses of between 25,000 to 100,00 have been used before. By using a dose closer to 25,000IU, it may have a more efficacious response. If there are side effects, then we may need to reduce the dose accordingly.

    I’m just anxious because I consumed 50,000 IU vit A a day tablets / vitamins for 6 months by myself . My Lab works seems fine but the internet has a different perspective on vit A and scares me!

      Ideally, most studies have used high doses for 3 to 4 months. After that, it may be useful to have. break of a couple of months before using further treatment if required.
      The use of Vitamin D and intermittent fasting are also helpful for acne. Here are videos on them

      Generally, Isotretinoin is the standard treatment so most doctors would prefer it. Vitamin A is useful only when we have no access to isotretinoin

    Sir i am taking accutane 20 mg alternative day it will work on me or not?? I am 40kg.At first my demetrologist prescribe me 20 mg daily After 1 month i got rare aide affects that was diagnosed Pyogenic granulomas (PG). I have to stop isotretinoin for 2 weeks for that disease. After that also my demetrologist recommended me not to take isotretinoin because sode affects was occurs. After some discussion with him he prescribed 20 mg alternative day to me My acne was too much improved on 1 month while i was taking 20 mg daily.
    Sir isotretinoin is safe for me or not?If yes how much time i have to take isotretinoin 20 mg alternative day..

    What about beta carotene instead of Vitamin A? The body is only producing as much vitamin A you need?And a overdose should not be possible.

      Beta carotene is converted to vitamin A in the body. So excessive amounts can cause toxicity.

      Usually, isotretinoin induced hair loss is temporary and will regrow over the next few months.

    I have amild acne since 2017 I tried all things you can imagine anti biotics and the best brand skin care i spent lot of money in the best center
    5 years without results I'm so tired about my mild persistent acne. People when they saw my acne theh told me thats normal but for me it's not normal I'm 30 year-old and I can't see my face with pimples like a teenager in this age
    My dermato tell me to strat accutane 30 mg for 6 months in the end of September she told me it's 's the best solution for the mild acne persistent
    My question : can take vitamins A 5000 ui with zinc picolinate 50 mg and cod liver oil an alternative of accutane can I have I good result?

      Yes, vitamin A is worth a try but the dose may need to be a bit higher as mentioned in the video. Zinc could also help. Here is a video that explains how dietary changes like zinc could help.

      I am not sure of the helpfulness of cod liver oil for acne but it may help psoriasis and I have done a video on it.
      Another option worth trying is oral Spironolactone as it is useful in acne occurring in post teen women. Here is a video on it

      30 mg of accutane is a low dose, its recommended to take at least 1mg per kg of body weight

      It is very difficult to cure skin conditions, especially inflammatory skin issues. We can control them. Sometimes a course of medication may switch off the inflammatory changes and it may not recur, but it is difficult to predict.

If sunscreens leaves white cast then it’s big no no for me. So please suggest a good sunscreen which is easily available in India. & not too expensive I mean under 800 rupees.

      The best sunscreen for you would be a tinted sunscreen. I have done a video on it. Here is some information. I am not sure of the exact brands available in India but I think Brinton phramaceuticals in India make one.
