  • After several desperate attempts to keep his old mentor alive, Dr Halstead realises he needs to let her pass to relieve her pain.
    Season 2 Episode 18 "Lesson Learned"
    Dr. Halstead makes quick and difficult decisions when tending to a patient who was also his mentor in medical school. Dr. Rhodes performs a risky surgery on a young boy as an alternative to amputating his arm. Meanwhile, Dr. Charles and Dr. Reese tend to a patient who denies having suicidal tendencies. Also, Maggie takes an aggressive route when training a new nursing student and Dr. Reese continues to mourn the loss Jason Wheeler.

    Everyone complaining about Dr Manning saying "I'll break her ribs": come on guys. Her character is a doctor and a good one at that, it's not like she doesn't know that breaking ribs while doing CPR is fine. The whole point of showing her hesitation, and April's hesitation too (if you notice), is to present the argument that was the primary focus of this clip: does quality of life matter more than life itself? Especially for someone that old, is it really worth it? Tough question with no correct answer.

      A doctor doesn't have the right to decide who gets to live and die, no matter the situation. That becomes the responsibility of the patient and their family.

      No it's because she's old, and if you're elderly and your ribs break it's harder to repair, so you'd be constantly in pain.

    2:24 for those who are complaining about Dr. Manning asking Halstead about breaking a rib, that's the reason why she asked for it first. There's so many reason why she had to ask for permission, she's just concern about the old lady's condition to get worse (or live in worst) also, she didn't totally reject. I understand her.

      Yeah, but it's unprofessional un uncalled for. She's a doctor, she could know that cpr can result in broken ribs and that the life of a patient matters more than whether or not they have a broken rib. Ribs can heal, you can't bring someone back from the dead.

      @@joelvr101 The question here is mire whether or not you should revive someone, breaking their ribs in the process, diminishing their quality of life if they do not have much chance of living longer than a few weeks or something like that. It's an ethics question and that was why she asked him

    The thing is about breaking her ribs is that she’s extremely old. So she won’t be able to move for a while etc. the question here was if QoL is more important

    It's not the fact that the ribs were broken, it was the age and how she was coping. A few broken ribs might not be as bad as us, but for her, it's basically death. It was the right decision at this time.

    NO ONE refuses to do compressions because they'll break a rib. That's literally one of the FIRST things they tell you when you learn how to do CPR; Even if you break ribs, keep doing compressions. If you are resorting to CPR, they are already on deaths door, a few broken ribs doesn't matter.

      @Spidey Girl She clearly said she was worried about breaking ribs. That was her reason for not doing CPR. It's just an error in the script and they didn't run it by whoever they consult on the medical side of things. I just find it particularly annoying because it's a really simple thing and there are so many people that know how to do CPR properly and breaking ribs is not a valid reason to not do CPR.

      @Spidey Girl That would be something to discuss with the next of kin about whether she has/wants a DNR or if she doesn't want life support. In this case though, there appears to be no next of kin available to make that decision so the doctors have to treat her like she wants to be saved regardless of the means they have to go to to keep her alive.

      I think it was more the patient would rather die peacefully than have more pain and suffer the whole time. She would have died anyway and having a machine expanded your diaphragm when you have broken ribs would hurt like hell. The doctor was thinking about what the patient wanted not what the grieving friend was insisting on. This is why doctors aren’t allowed to treat family. Keeping someone alive becomes their first priority, even if it isn’t in the patients best interest.

    im not a trained doctor, only trained in first aid; but yall who said no one would refuse to do cpr in fear of breaking ribs? she’s an old woman. her ribs will be harder to repair and she’ll just suffer more.

      QoL isn't something you discuss while your patient is literally flatlining in the same room. You resuscitate, unless given a DNR order. That is a decision only given to the patient or anyone they deem fit to decide in their stead.

    I am not impressed with the female doctor sorry. You can break my ribs so long you can guarantee that I have a second chance (yes i can die from a punctured organ due to a broken rib but) To not start compression because you don't want to break my rib uhhmmm

    Dr manning didn’t refuse to giving cpr she just hesitated because she thinks of how her bones may not heal because of her age and broken ribs are extremely painful. She was concerned for her well being afterwards

    The reason why nat had issues with cpr is because if she does it right, it'll break her patient's ribs. Who knows how many they would have broken since the patient is so fragile.
    Patient:
    Intubated (meaning the machine was doing the breathing for her)
    Kidney Failure (Leads to many different complications including heart conditions, medication complications, blood pH fluctuations)
    broken Ribs ( in loads of pain most likely cannot do surgery since the patient is so weak, she wont be able to handle it, most of the time you dont do surgery you just have to let it heal, with the patient's age and physical state it will take a very very long time to become healthy, by that time she will pass away from other health conditions)
    Shes bed ridden (pressure injury leading to skin necrosis, muscle atrophy (muscle degeneration which can happen as fast as 1 week bed bound))
    Of course quality of life will be a concern.
    Especially in palliative care, quality of life is a big concern. Thus all the pain medications and the rest.
    People say they want to live but other people just want rest after so much fighting. Sometimes its just ignorance, naivety and the lack of knowledge about how the body works.

      That's not what that sentence means. Some doctors treat a disease, but without really considering the well being of the patient. I've personally seen this in operation rooms, where the surgeon does an amazing job, but the rest of the body comes out a wreck because of it. And then there are doctors that put the patients mental state and well being before anything else, and that's what the sentence means.

      Because ICU would make a really boring TV show. Most of the time patients are just intubated for long periods of time, and have routine checkups until something in their condition changes.

    often when older already sick people break bones it can be game over for them. so i can see dr manning’s point as it may be counter productive to revive her and then have her never recover from the breakages.

    To anyone who ever has to do CPR on somebody one day (I hope you won’t have to), breaking a rib(s) is completely normal. It’s recommended push down 5cm (2 inches) when doing compressions, plus most times it will be elderly people with more fragile bones. So don’t hold back because of possibly breaking ribs; after hopefully surviving a heart attack, a broken rib should be the least of their concerns.

    I think the point of Dr. Manning's comment was to the futility of CPR in the patient's situation. Although I agree, in this case, advanced directives or the subsequent decision maker would have to clarify what would occur in such a situation, however, I think Dr. Manning's comment is to contract against Dr. Halsted's actions. It's a hard choice between but sometimes ensuring a peaceful death is more kind to the patient than life while subject to various treatments that do not cure and only prolong the suffering- for instance, CPR and the subsequent pain from broken bones. I think that's why Dr. Manning made that comment.

      If they put her on dialysis, she still wouldn’t have survived on her own!
      She would be bed ridden and she would have had to have a tracheostomy as she wouldn’t be able to breathe on her own!
      They treated the patient and not the disease! They considered her quality of life and knew that if she survived, she wouldn’t have wanted to live like that!

      The other nurse said how in her first year of OR nursing, she wanted to quit because she struggled so much with it! That lady who they were treating helped her not to give up!
      She meant a lot of Dr Halstead and that nurse (I don’t know her name)
      They give guidance and that kind of thing!

    You all keep saying that it’s bad that she was afraid of breaking her ribs, but you have to ask yourself, would she be able to survive it when her ribs would be broken? She is old, and not really strong anymore so is it worth it to save a life like that if she wouldn’t be able to get better?
