  • “Thank you Dr. David. I have been diagnosed with bone-on-bone osteoarthritis but due to CoViD, invasive procedures such as my scheduled knee replacement was canceled. However, I'm thankful when you said that alternatives are available like knee exercises. If not much to ask, could you show us proper strengthening exercises for my knees so that I can improve with my mobility, for as now I only depend with the help of my rollator. Looking forward to your kind reply. Thank you.”

"Dr David....you've been a blessing for me with your advice on cartilage repair and avoiding knee surgery. The OS tells me I'm bone on bone and I need knee replacements and I'm trying to avoid this. Watching your videos with the appropriate exercises but really worried about damaging my knees further. I'm 67 and regular with 10mins bicycle at home and pilates 3 times a week and a bit of walking. Walking is painful after 15mins. What is right for me to do? Sorry to bug you about this.”

Treating bone-on-bone knee arthritis with exercise to avoid knee surgery is the dream of most people. And the reality is that it is attainable for most people.
We've seen that more than 9 out of 10 cases of knee arthritis we've treated here in our clinic get better without having to get surgery.
We've also had many people commenting on our videos saying they felt relief from their knee arthritis by using our exercises and following our advice.
So what exercises should you be doing?
    How many reps?
    How many days per week?
Are those the only exercises I need to do?

    For future reference:
    1. Low resistance high repetition (like cycling) but use glute muscles not quads
    2. Strengthen your glutes, and get constant control of them
    3. Stretching with muscle activation 33:00. Use your muscles to your advantage to get full range of motion if you have knee stiffness

I have been told I need a knee replacement from arthritis. Recently my knee was swollen and I had trouble bending it. I have had an ACL repair on my other knee so I’ve done many PT sessions. Yes they focus on quads and hamstrings. So I went back to my exercises. This was making it worse. I did one session of your glute exercises and my pain is gone and I can bend my knee more. It totally helped me!! Thank you so much in just one session

My wife has had seven operations on her left knee for replacement. She was prescribed medication to fight the infection/rejections. The medication affected her kidneys, to the point she was put on dialysis. Before the last operation, I refused to doctor that prescribed the bad medication to be used again. So far things are working out. That doctor is in Oklahoma City and claimed that he sees 50 patients each day. Thankfully another discovered the problem and took my wife off the medication. I had to find out that I couldn't get the glutes properly fired until I fixed the pelvic floor (I hurt my back at age 32), and then they all follow. I'm still on the journey. - I'm 75. I can now even jog gently At 65 I thought I was on the way to being an invalid. Finding those glutes has been a lifesaver and I've still a LOT of work to do!!

On the subject of orthotics, I can’t imagine walking without my knee sleeves, for me it definitely helps, I think my knees would be much worse without them. What I’m surprised that you didn’t mention, was how important it is to wear the proper footwear (especially flip flops- I think for preventing injuries), that’s how I got my heel spurs, from wearing my shoes long after the soles wore out! You’re amazing 🤩, though, I just subscribed to you.

So glad I found you!. I have had 2 meniscus surgeries and now my Dr surgeon tells me I'm bone on bone and need a full knee replacement. I do not want to do this surgery since I have MS. I've been going to physio for 4 yrs and now I'm trying fix my knee seeing a chiropractor. I am going to give myself a year to try and fix this to avoid surgery. And yes it's osteoarthritis I have in my knee.

I also have a meniscus tear from a fall and it has taken me 3 years to find the right physiotherapist n chiripractist. Didn't know they have such diverse approaches. One kept printing out exercise routines, not understanding how to push on despite the pain, as advised 'no pain,no gain'... I had to stop as they didnt improve the inflammations. I went to different ones but didnt improve until I saw Dr David's videos. His advices are to strengthen the glutes and always exercise enough before it hurts. Something I had to experiment and adjust as inflammation easily hinders progress. Dr David rightly suggested, 'low resistance, high frequency' rather than 'no pain,no gain'. Thanks to Dr David I gain much more strength now to walk without the walker. Another physical therapist's videos that I check out is Will Harlow, HT Physio over-50s Specialist. I am so blessed to have Dr David's and Will's professional expertise otherwise, according to my doctors, surgeries are often thwarted as the better option. I'm writing from Australia.

Just discovered you. You have a life long fan if you stay logical like this! I am still unhappy with the "physical therapy" I got last year for not being able to walk more than a few steps without bad pain. I have psoriatic arthritis BUT like many people, I have more than 1 kind of arthritis. I was a competitive skater and still have very, very strong quads and old injuries. Has anyONE EVER paid any attention to that? No. Everyone goes straight for PsA as the sole diagnosis and strengthening quads as the sole non-med treatment (but straight to drugs first).
I already feel some relief, albeit temporary at this stage, just doing the few glute stretches you are showing.
I will have to look for hand and shoulder exercises, plus more on this rest idea. You probably already know from the skating history that my philosophy is "Oh. You fell? Ok. Get up and do it again."
I'm doing everything seated right now.
Ready to buy a motorhome and come stay there while you run me through everything.

      Hey Lindaray,
Thanks for sharing your story. Being a competitive skater must have been so much fun. yes I can see how you have been over working your quads. When that happens your body wants to turn them on all the time. So the idea is getting those to relax and increase the dependence your gluts for activities will be most beneficial for you!! We have great RV spots by the mountains.

To do with descending the stairs, I have for at least 8 years now, walked down the stairs sideways, it seems to be easier on my joints & less pain, I climb the stairs that way too, but I focus more on the descension compared to ascension, but I agree that I need to work on my glutes (I thought mb, I just didn’t want to do the wrong exercises), now I know. This video was AWESOME 👏 I’m glad I came across it 😃 Thanks ❤

Subscribed, this is much more useful than the physio. He had me doing knee drivers step up on the first step (which would really set my knee off). I have seen your video on going up stairs which I will try to implement in that exercise. Then it was crab walks with a light resistance band on the feet (knee not overly fond of that exercise either but it doesn't hurt as much), knee bridges which I initially found easy then he told me I had to extend the left leg - those are just too hard and I don't think I'm balancing my weight in the right place and finally at the end I had to do what he called divers which are the same as a single leg deadlift without a weight. This week it was so exhausting doing some of them (the bridges) that they have been slipping my mind! Today I started doing the knee drives, which I had to space out because the knee started hurting. I am on 3 sets of 10. (3 sets for everything to varying numbers) I didn't do anything else because the knee never stopped hurting. Hopefully the pain will be gone by tomorrow. I was told by my doctor when he prescribed osteoarthritis that I needed to push through the pain otherwise my knee would seize up.

Hey Ok Write, sorry to hear that you're dealing with this :-(
We see it too much sadly. We completely, 100% disagree that you need to push through the pain. The only time to push through pain is if you've just had surgery and you NEED to get more motion. But if that is not the case, then you should only do exercises that feel good on your knees. If all you have is osteoarthrtis, then you've got to start with training your glutes and letting your knees calm down gradually. Here are all our knee arthritis videos:

I'm really into home workouts and now working on my butt muscles to reduce using my thigh muscles. My x rays show not really bone on bone, the cartilage is getting worn down because of the stress I put on it (another story). But they both creak and grate on each other. I watched just one other video from you, and I'm going to try and walk outside just a short distance tomorrow for a start. We'll see what happens after that. Thank you. I have found the information provided very useful, I am just at the early stages of Knee pain. I want to get ahead of it and not wait for the pain and other symptoms to increase. I have been watching a few other videos from other therapists and you are right, they do emphasize strengthening the quads more than they do the qlutes. Fortunately, I have been exercising for years and my quads are strong. However, I have lost a lot of weight recently and I noticed that I have lost some muscles from my glutes, so although my glutes are still fairly strong, I will continue to strengthen my glutes so I do not end up having to rely on pain medications as I don't like these medications for all sorts of reasons.

Dr. David, you are genius, a great instructor and my saver. Everything you said, the suggestions, and the exercises really came from your heart, the years of skill and experiences. They are making sense and really work. It has been 3 months that I pulled the door close and accidentally hit my Rt toes. Due to the sever pain on my toes up to my top ankle, I quickly pull my Rt. leg and slammed my foot down to the floor too fast and realized that it was not the right time and the right angle, from that day my knee was so swollen burning and painful when walking. I have been searching for the suggestions and solutions through in the internet: I did rest, use cold compression, NSIAD votaren gel, and exercise. The swollen, burning and pain is slowly a little bit less but still flared after a short walking. I felt that it is too long now and my problem might just be the improper alignment of my knee joint that I still could not overcome. After been watching so many info in the internet, THIS CLIP OF YOUR IS THE BEST ON TOP OF ALL. I already followed your channel. Thank you so much sir. and May God bless you for what you are doing!!!
