I cannot tell if you had ever seen, or even heard of, the film 'Fantastic Voyage' (1966), which involved a team of specialists reduced to microscopic size to perform exceptional brain surgery. It was very well remembered, and doubtless informed this episode. The idea of shrinking down and going inside someone had even been used previously in Doctor Who: Fourth Doctor in 'The Invisible Enemy' (1977). The Doctor is projected into his own brain (don't ask) with Leela, his companion. A spark goes by in the background. Leela: "What was that?" Doctor: "Oh, just a passing thought."
i realised with the 50th anniversary special you missed the 3 50th anniversary shorts (the most important one if the night of the doctor) and the 5 ish doctors reboot (which was also made for the 50th with previous doctors and is fantastic
Listen harder is just "git gud" for comprehension. Funnily enough: That is the solution. With repeated listening and attention: Comprehension gets easier.

i love how capaldis doctor is so alien, inhuman and sometimes even a bit inhumane
I hate it when people talk about the Scottish accent like that - the problem is not his - its yourse: just listen harder.
Might be even worse for 13 who has a Northern accent and a pretty strong one at that. Luckily I'm Northern myself lol
oop...
