    I - Introduce & identify
    S - Situation
    B - Background
    A - Assessment
    R - Request
    Use of Tense
    1. Using the present simple (Do, make, come…)
    2. Using the past simple (did, made,came..)
    3. Using the present perfect(has done, has made,has come..)
    4. Using the present continuous (is doing, is making, is coming)
    Use of Present simple tense
    The purpose of using simple present is to report the unchanging facts that is given in the case note
    To report unchanging facts
    Information is true in the present and is not subject to change any time soon.
    Where we will use the present simple sentence
    We use the present simple sentence at the beginning of the letter, when you report the patient’s name age and status
    This particular part in the beginning of the letter we have to use the the present simple sentence
    Eg: The patient is 29 years old. She is divorced and has two dependent children
    The patient is allergic to penicillin
    When something happens on a regular basis (frequently and happens now)
    Eg: Mr James experiences chest tightness X ( Mr James experiences chest tightness on a regular basis
    For details such as
    Lifestyle choices, the present tense is appropriate
    Eg: Despite previous advice, Mr James is still smoking 20 cigarettes a day
    To describe the patients current social Background
    Eg: Mr James smokes 20 cigarettes and drinks 2 bottles of wine daily.
    Ms Jessica is overweight and has a BMI of 27.8
    Mr John lives in Lake Park with his 70- year -old wife, Mary
    To describe anything that occurred during hospitalisation that is still continuing
    Mr James (still) needs assistance with dressing, toileting and transferring.
    Ms Jessica consumes a low-calorie diet in order to maintain her weight
    Present simple
    Repeated action
    I eat lunch every day
    Mr james experiences chest tightness
    Syntax
    He likes ice cream (subject verb and object)
    She goes to work everyday
    They always eat lunch together
    Use of present progressive
    Using the present continuous eg is doing ,is making ,is coming
    Have/has+been+ing form of the verb
    He is walking
    Use of present progressive tense
    Past Now future
    Continuous
    Eg:
    She is walking
    He is running
    Using present progressive
    Focus : on the present action without relating it to the past
    Use: referral/discharge letters
    Purpose: To communicate a piece of information which has occurred recently
    Something new that doesn’t occur before in contrast to the present perfect which the medical practitioner will have to respond to or take into account in an active manner the reader whom you are writing the letter will have to consider it very seriously and take necessary action that you need to tell them you have to take necessary action because when you use person progressive says the readers attention is advised here or requested here
    Eg:The patient condition is deteriorating or
    The patient is refusing to take her medication
    Skill: contrast
    Communication: the situation in the present is different from the situation in the past
    In the past the patient’s condition was not deteriorating but now it is
    In the past the patient was not refusing to take her medication ,now she is
    When describing an event currently happening
    Mr james is experiencing chest tightness experiencing Right now currently experiencing
    Using present perfect
    Using the present perfect Eg: has done ,has made ,has come
    Used to connect a past action or state to the present
    Used in the elements of the situation
    Although the action or state began in the past ,they are linked to the present
    The intentions
    To link a past event to the present
    Do not use specific time phrase such as two weeks ago or yesterday
    Where to use ? to present
    More recent treatment or advice the patient has received or
    More recent actions or attitudes which have been associated with the patient
    Example I have prescribed the patient propranolol to help control his anxiety
    Instead can write
    The patient has responded well to treatment
    Clues in the case notes time phrases
    For and since
    These prepositions can be used to link past and present actions
    Eg:
    The patient has received massage therapy for the last 5 months
    (the patient received them in the past and receives them up to now)
    For details which describe unresolved situations such as
    Improvement or deterioration in thev patients’s condition the present perfect tesnse is appropriate
    Eg: since his last visit mr johns pain level have reduced from an 8 to 4
    To describe ongoing chronic conditions in the patients medical background
    Eg: Mr james has had hypertension for 10 years and diabetes sine 2000
    Has and have issue
    I & You use have
    He,she it use has
    We you and they use have
    Use of simple past tense
    Useful in referral/discharge letter
    To provide background information (which has been specifically linked to a past time period)
    The patient first presented two months ago with a bowel complaint.Now she displayed symptoms consistent with IBS.
    Clues in the case notes(time phrases)
    Two months ago
    Last week
    The previous month
    Yesterday
    Use of past tense
    When describing an event in the past which is resolved
    Mr james experience chest tightmness =experienced in the past
    The baby has arrived and there have been no complications from the birth
    Use of past tense -With diagnosed
    Mrs catherine was diagnosed with pneumonia and treated with antibiotics prior to discharge last week = pneumonia is now resolved
    When the situation is ongoing but is stable (diabetes that is well managed with medications)
    Mrs Catherine was diagnosed with diabetes in 2018 for which she takes regular medications
    She was commenced on metformin
    3 units of packed blood cells were transfused
    Use of past tense v/s case notes
    For details such as:
    (Which were accurate at the time of the visit, the past tense is appropriate)
    On examination the abdomen was tender on palpation

    Teaching by a certified teacher is our luck and we are fortunate to have our mentor fr. Saleen joseph whose strategies are simple to understand but very complex in presentation which will attract aspjrants to grab opportunity. Its not easy to put wholeself in the task and suffer for the sake of us. Lets not forget to see share and sybscribe. Many platforms are providing classes but they are not at the level of fathers strategies. May god bless him to make some more videos so many can benefit without spending any money

    USE OF TENSE.
    Present simple tense - S+V+O
    To report unchanging facts.
    Likely to be true for a long time.
    Eg: patient's profile.
    Adverbs of frequency:
    Always
    Often
    Sometimes
    Seldom
    Never
    PRESENT CONTINUOUS
    Is, am ,are + ING form.
    Use: referral / discharge letters
    Purpose to communicate a piece of information which has occurred recently.
    Communication: the situation in the present is different from the situation in the past.
    PRESENT PERFECT
    Information which links the past to the present.
    Time phrases: FOR and SINCE.
    SIMPLE PAST:
    To provide background information.
    Something action begins in the past and finished in the past.
    Clues in the case notes: time
    phrases:
    2 months ago
    last week
    in the previous month
    yesterday.
    To express test results /vital signs also.

    Syntax :
    Eg:.He likes ice-cream = simple present = SVO = subject ,verb ,object
    He = third person singular
    First person singular= I ,plural = we
    Second person singular = you ,plural = you
    Third person singular= he ,she ,it ,plural = they
    The Simple Present Tense

    To communicate a piece of information which has accrued recently
    ❤ something new that wasn't occurring before
    ( In contrast to the present perfect)
    ❤ which the medical practitioner will have to respond to or take into account in an active manner .
    Examples:
    ❤the patient's condition is deteriorating or
    ❤the patient is refusing to take her medication
    Skill:.contrast
    Communication:
    The situation in the present is different from the situation in the past
    In the past, the patient's condition was not deteriorating but now it is
    ❤provides some sort of distinction from / potential Contrast to a previous situation
    ❤disassociation from the past is what makes it unique in its relevance to the present .
    1.when describing an event currently happening
    Mr James is experiencing chest tightness
    Experiencing Right Now
    Currently experiencing
    ❤ USING PRESENT PERFECT
    3. Using the present perfect
    Eg: has done ,has made ,has come
    *Used to connect a past action. Or state to the present.
    * Used in the elements of the situation
    Although the action or state began in the past ,they are linked to the present
    The intention:
    *To link a past event to the present.
    * To show it's continued relevance to the current moment
    NB:
    Donot use a specific time phrase such as "two weeks ago " or yesterday
    This is because the intention of the writer is not to communicate this information as a background
    Where to use ?
    To present
    More recent treatment or advice the patient has received ,or
    More recent actions or attitudes which have been associated with the patient
    (As a result of their relevance to the present ,may not carry a specific time marker )
    Example : I've prescribed the patient propranolol to help control his anxiety .the patient has responded well to the treatment

    To communicate that there has been no change in terms of relevancy from the past to the present in these cases
    ❤ using present perfect:
    Clues in the case notes = time phrases ."for " and "since "

    For details which describe unresolved situations such as :

    ❤Using articles :
    ❤An MP = M = em P
    A member of parliament
    An university
    A university= correct
    A hotel= correct
    An hotel = correct
    ❤an historical monument= correct
    ❤a historical monument =correct
    Using present perfect with diagnosed
    1. When the condition is unstable
    Mr James has been diagnosed with asthma and prescribed an inhaler.He will return to the Outpatient clinic next month for a review of the inhaler's effectiveness
    (Asthma treatment is still being finalised.If the inhaler doesn't prove effective,then a different treatment will be considered
    2. When the condition will be news to the reader.
    (When the reader already knows the patient and so you are telling them something new )
    Mr James was admitted to hospital 3 days ago.He has been diagnosed with unstable angina and requires your follow up care
    (The diagnoses is news to the reader and how they will be involved going forward)
    Use of present perfect with diagnosed
    1. When the condition is unstable
    Mary has been diagnosed with asthma and prescribed an inhaler.she will return to the Outpatient clinic next month for a review of the inhaler's effectiveness
    Use past simple tense = used time phrases
    Two months ago .

    My learning experience from this video.
    We can use simple present tense for something that is unchanging or likely to be true for a long time in the patient case note. (eg.patient profile)
    use past simple for background information set specifically in the past (eg. background to patient's condition)
    Use present perfect for information which links the past to the present.
    Use the present continues for the current information, which requires a present reaction or response and is likely to be different from the past.

    From this session, i understood the different tenses which is essential in oet letter writing.... it is cristal clear with the examples and situations you explained..... also, my ever most confusion was the use of has/ have... u explained it clearly along with use of different ARTICLES in oet letter..... do and don't in grammar.. 🌹🌹Thank you father for explaining the entire grammar which i need to use appropriately.... it was very useful.. 💐💐👌🙏🙏 also i recommend to watch the video with a piece of paper and pen to catch all the things in order to make it more beneficial...... 🙏

      .clues in the case note - for, since
      .describe unresolved situation - improvement, deterioration
      .on going chronic conditions
