    Father wonderful session I never knew I could attenmpt writing in oet which was thought to be impossible but with your strategy nothing is difficult.     1) How to choose RELEVANT case notes?
    Ask yourself 2questions
    🌹Is the case note is relevant to the current situation?
    YES/NO
    🌹Will this case note affect how the reader continues care for the patient?
    YES/ NO
    If answe to both question NO= NOT RELEVANT
    If answer to either question - YES= RELEVANT (include in your letter )
    2) What to do with the aspe🙏of past medical history that are either normal or not very important??
    ILLUSTRATION
    *No smoker
    *No known allergies
    *No further hair loss
    *No current medication
    *No significant medical history
    *No surgical history
    *No reported drug use
    *No pregnancies
    SKILL: SUMMARISE

    SENTENCE STRUCTURE
    🌹SIMPLE SENTENCE
    1)She is a regular smoker
    2) She drinks moderately on weekends.
    🌹COMPOUND SENTENCE
    She is a regular smoker and drinks moderately on weekends.
    CASE NOTES
    1) weight :86kg, height :157 cm
    2)Peterson concerned about weight gain of 8kg over last 12months
    SENTENCE
    She has gained 8kg over past 12months and currently weight 86kg.
    SYNONYMS
    Post operative = recovering from/ after

    USE OF PAST TENSE
    🍬When describing an event in the past which is resolved.
    Eg: Mary gave birth five weeks ago.
    🍬with diagnosed
    Eg: Mary was diagnosed with pneumonia and treated with antibiotics prior to discharge last week. ( pneumonia is now resolved )
    🍬when the situation is ongoing but is stable.
    Eg: Mary was diagnosed with diabetes in 2008,for which she takes regular medication
    USING PRESENT PERFECT
    🌹Use of present perfect with diagnosed.
    🍬When the condition is unstable.
    Eg: Mary has been diagnosed with asthma and prescribed an inhaler. She will return to the outpatient clinic next month for a rewiew of the inhaler's effectiveness. (Treatment is going on- not resolved)
    🍬When the condition will be news to the reader
    🌹when the reader already know the patient and so you are telling them something new.

    habitual action/ routine action that continue to the present // we can use simple present tense eg- He is a regular smoker .
    in order to emphasise the duration and still continues //we can use present perfect progressive - He has been smoking one packet of cigarettes per day for the past 10 years.
