      Hi, I am 55. My reading glasses are 2.75 and my long distant vision is just starting to go. I cannot get used to bifocals and it is a pain to cut hair and need my readers and take them off to look at my client.
      If I have this done for reading, will my long distant vision improve too? TY

    Alex, I am so impressed with your knowledge and in your in depth reviews and the education you provide on the pros and cons of the different IOLs. Are you still planning on doing a review of the Technis Synergy lens? I am very interested on knowing what your thoughts are about it. I am curious what the contrast performance is like in that lens.       Luis and Hector Rivera, I had the synergy lens implanted in both eyes in November 2021, and I am very disappointed with the distance vision. The clarity and contrast is not there. Very disappointed!!! I should have gone with a regular distance correction lens and wear glasses for reading. By this time next year, I will replace them with a regular lens. All the hype these new lenses are given is just that, hype!!! Plus, the night time glare and star burst activity is horrible!!! Save your money and go with regular lens.

    I had Alcon Panoptix multifocals implanted.
    8 months later I couldn't wait to have them removed.
    Neuroadaptation didn't take long but my sensitivity and contrast were so poor that I wouldn't drive at night because too much information was missing. Brown holes in shadowed areas during the day and poor sensitivity and contrast at night.
    Had Alcon AcrySoft toric monofocal lenses implanted and it was like I was 30 years younger!
    I still have some residual refractive error that is corrected with distance glasses and readers.
    Overall I am grateful I have better vision than with the cataracts.
    However, I think patients should be presented with more detailed product information so they can avoid a I'll lens exchange.

      I am sorry for your experience. But in majority of cases such problems are related to poor preoperative diagnostic (ocular surface, cornea aberration profile, back corneal surface...) or not optimal residual refraction including residual astigmatism

      @@LoveSunnyDaze Won't the same surgeon do an exchange for you?
      Mine replaced the multifocals with aspheric monofocals. You'll need reading glasses and perhaps glasses for distance or to correct for astigmatism, if you have it, but I think you will like the outcome overall. One thing that I had not anticipated but did encounter is your focal point will vary between day and night because your pupil plays a significant role in the way a natural lens and eye adapt to distance and focusing.

    I want to share my experience:
    I do have Panoptix trifocals in both eyes. 20/10, yes the exceptional twenty/ten, vision for distance, 20/20 intermediate (60cm-120cm) and 20/12.5 near (30-60cm) in normal light conditions. Before operation I had 20/20 vision with contact lenses. And needed 2.0+ add for reading. This is bifocal vision.
    Although the lens itself is yellow it is less yellow then a normal 50 year old eye. (I am just past 50 years old). Colors look natural. White paper looks white in normal daylight.
    All these so called lab tests do not give the real picture. Because the human brain does a lot to enhance the picture. Even how you see the colors. The brain even turns the picture as it is on the retina upside down.
    You are right that the calculations done upfront and the position of the lens in the eye are very important. My left I eye is plano. Right Eye is +0.25 Sphere with -0.5 CYL.
    So the outcome in my case is much better then any defocus curve you find on the internet. There are slight halo's at night around strong light sources which you get used to and they got less visible in time (but still exist). I do not dislike them. I can read fine in dim light conditions expect very small print (text size of 1mm or smaller) are better visible with a lamp at night (In daylight no problem). The smallest print I could find was 0.3mm character size and I can read that just fine.
    So if you have a very good surgeon and no other eye issues than cataract or you normally needed glasses/lenses and you want to live without the need of glasses and you are older then 50, these panoptix IOL's are great. I even see better now than most kids with healthy eye's. And better then I have seen ever before as long as I can remember.
    I had vision tests all my life never scored 20/10. Best corrected vision in my younger years was 20/17 just a little below 20/16. Later it was 20/20 with lenses/glasses.
    I think al these measurements you find on the internet are averages. Better find a very good surgeon with a good track record and a good hospital with good equipment and good pre-tests. A lens can be perfect but if the surgeon is not the result will be bad.

      And the last statement is absolutely correct - the calculation and surgeon's skills play a significant role in outcomes.

      I am going to have a cataract surgery next month, only left eye. Still don’t know which lens I should use. My surgeon recommended the vivity or Panoptix, but he said Panoptix may cause more trouble in future. I don’t have any other condition except left eye cataract. I am only early forty. Could you please advise me?

      @@PL-yo7qx good question.. first of all, the IOL selection criteria should be based on your visual needs first, your expectation about use of glasses and visual compromises.
      so, if you tell me what do you want (how do you use your vision) I could advice something. It is your life and your visual needs, and it is first criteria in IOL selection, next is your eye conditions and medical limitations.

    For a young person what would be the right choice, a vivity or a trifocal panoptix? Even if you need to wear reading glasses but have good distance vision?

    I had the PanOptix implant in both my eyes Aug. 2 Left eye and Aug. 16 Right eye 2021, 7 plus months ago. I'm one of the those persons that is having problems. My far and intermediate is really really bad and starburst/hallo's is also really bad when driving at night. Blurred and shadows around letters. My right eye is the worst of the two. I have dry eye real bad I though. I Just went back to my Doctor and he did a complete exam and he sad my dry is not that bad??? And that I'm one of them that is not satisfy with my vision. So he want's to replace my right lens. I'm not sure the lens he said but I think it was Vivity, he said that I will not be able to see close. But by leaving my left eye along that I would be able to read with it??? I'm not sure I want two difference lens in my eyes. He also said that my Starburst/hallo's would go away??? Because my right eye is the dominate eye???

      @@kickstand2438 I had exchange surgery on my right eye on May 16, 2022. It did not go well. Exchange PanOptix lens for Vivity lens. 5/16/22 Surgery should taken 15 minutes took 3 to 4 times that. Causing Trauma to my eye. Gave me two pills for the pressure on my eye.
      Next morning he had to burp my eye because of pressure on the eye. Pressure was 59 right eye 10 left eye.
      Doctor damaged my zonules 3 to 6.The zonules are the tiny thread-like fibers that hold the eye’s lens firmly in place. So with that damage the Doctor could not center the Vivity lens and none I have a shadow on the right side of my eye. I had appointment with my MD, July 12, 2022 was for the shadow on the right side of my eye. He said that the shadow was from the Vivity lens not being center. That I was seeing the lens and it might not go away. With the damage to my eye I have blurred vision and I'm told by another doctor that my vision will not change. I have an appointment with a MD at Baylor College of Medicine on July 24, 2022 to get another option.
      So that's the short story. My option is do not let any doctor go into your eyes a second time unless he know's what he is doing and stick with one lens, like the panoptix etc. My left eye is good and no problems. But with my right eye being the dominate eye it take over my vision and it's is troubling. It's has been almost 3 months and no change.

    I have been watched these videos and found them very helpful. Can anyone help me understand how the rings work in the Panoptix lens. The centre bit of the lens is presumably for distance viewing in both eyes? Are the next few rings for intermediate vision and the outermost rings for near vision?
    I have heard there are lenses where the centre part is for distance viewing in one eye and for near viewing in the other eye - this doesn't seem like somethink I would like.
    I would love to have this PanOptix lens but would like to know that both eyes will be seeing the same rather than one eye being dominant for distance viewing and the other dominant for near viewing.

      modern trifocal lenses do not have the design you mentioned like "ring A = distance B", all the rings work in complicated combination to create aditional focal points, and it is quite complicated to explain in short comment.
      if you have same lens bilaterally (both eyes) both eyes will have same vision

      The bad part of the Panoptix multifocal lens is about 10 to 12% of the light entering an eye is not even usable. The remaining 88 to 90% of usable light is divided between near and distance vision.
      If you were always in a very bright environment the PanOptix mf wouldn't be so bad, even with the rings, halos and multiple images because the high ambient light would negate them. But in low light sensitivity and contrast are very poor to tje point of handicapping an individual (me).

    Would you please explain about material of this lens? I heard they produce glistening effect in long term which might cause visual problem later in life. I also heard they can develop diamond eye after surgry. Thank you

      There is 2 basic materials for an IOLs: hydrophobic and hydrophilic.
      Shortly: some hydrophobic (particularly Alcon AcrySof have glistening issue), some others not (all Johnson&Johnson, probably Alcon Clareon but not for sure...)
      Hydrophilic materials tend to have more secondary cataract than hydrophobic, but these lenses (hydrophilic) are generally cheaper.

    Thanks for these videos. I am 58 and was scheduled to get Panoptix in both eyes today, but delayed 2 weeks so i could get the Clareon version of Panoptix instead of Acrosoft. I understand the measurements are very important to a good outcome. Does the spere measurement taken at the exam (glasses prescription) even factor in, or is it all based on measurements of the eye. I wear multifocal contacts today with great success and I dont have cataracts. I had to take them out for 6 days prior to the exam because they said it changes the shape of the cornea.

    I had panoptix in left eye. I'm 35. Up close in bright lights it still focuses in and out . I can see it teying to focus. Is this common? Bad contrast sensitivity up close compared to me good right eye

    Unusual question: I live far up north i Norway and I am going to implant panoptix trifocal. Will my brain adapt faster if I do the surgery while it is dark most of the day and I get exposed to a lot of night vision. Or is it best to do it during the summer time when it is light almost the whole day, so that I have neuro adapted before fall and darkness kicks in.

      Good question. I am not aware about any recommendation like that.
      I suggest to go with the surgery when it is comfortable for you, and then if you are concerned about contrast sensitivity, you may try RevitalVision technology, it works well.

      there is different IOL at all, as for now light adjustable does not have any presbyopic solution, so there is a choice of trifocal panoptix (freedom of glasses) vs monofocal IOL (use of glasses for sure)

      @@iol-adviser Or Light adjustable lens followed by a presbyopia correcting LASIK treatment. If some one does not want a trifocal. But I do not know how that works out in real life.

      @@JohnJohnson-zq9rg RxSight Light Adjustable Lens - is a simple monofocal IOL, with no ability to give you freedom of glasses. The main idea is that it eliminates risk of refractive error.

    Do rayner rao600c monofocal lenses have good visual acuity, good UV protection? the biometric calculation of Diopter for far vision has to be positive, what usual value?

      I had cataract surgery with an aspherical monofocal lens, it took 4 months, I only had the right eye due to trauma, and I'm having a lot of dizziness when I try to look for something to move and have to focus on things, then the dizziness, the image in the corner nasal the images are wavy a little and there is light refraction at the edges of the lens, I put an aspherical rayner rao600c, I got a degree variation that was -0.5 and now flat, but it repeated this variation after 45 days, when I took the last degree, it was -0.5 then flat and always -0.75 astigmatism. It's hard to give an opinion from a distance, what could it be? In the post-operative period, I followed the guidance, but I watched a lot of TV in these 30 days and I had to take some walks. Could it be the quality of the lens? I'm a layman on the subject, The ophthalmus was not mine but another indication, not much was said about lenses, if I hadn't put on a zeiss at lara or a vivity alcon. What could the distance really be? Can I change the lens? I'm having the view of the first image attached, an expanded view, without visual comfort, even with the multifocal glasses, dizziness and vision with little life! vision only 50cm with reasonable sharpness. The lens is an aspherical rayner rayone rao600c, hydrophilic 26% water in its composition, I have eye pressure, a little loss in the corneal epithelium, I am also having watery eye sensation and pain in the unoperated eye with good distance vision.
      I had to get my right eye done, the left one can still see well with +2.00 diopter, it's good for a distance I just need reading glasses, that's what's saving me. is there a way to change the lens? If he had more guidance he would have placed a vivity of alcon or AT lara zeiss (repentance). complementary observation: I have recurrent uveitis, the lens replacement was not due to age and I have a slight loss of corneal epithelium: references being related to not using the lens in the manual. thanks!

      Recently cataract surgery was done for my eyes.
      Alcon multifocal lens is implemented.
      My eye sight is 20/20 now and I don't have problems in the three regions.
      I am 63 and regularly play shuttle badminton.
      My reflexes and vision during play is perfect and I think my vision can be compared to a younger person.

    Hello Sir, the Tecnis Synergy lens just received FDA approval here in the U.S on May 7. Have you done a review of this lens yet? I understand this lens has had good results with less halos and glare.

      shortly, Synergy is a new hybrid technology and much better in terms of contrast sensitivity and gives better visual acuity. Detailed review will follow on my channel.

      @@aniketnavalkar5974 it is possible, but IOL exchange is not desirable as it is quite traumatic procedure.

      IOL exchange may destroy capsule - it depends on time after surgery and eye conditions. ask your doctor who will examine your case
