I agree with your list in the short-term outcome. I would like to see long-term clearview 3 results. Excellent video. Mind numbing choice of lenses. I require cataract surgery in the near future so I’m trying to educate myself on the different options. Will my choices be more limited due to the fact I have very little vision in my right eye? Been like that since childhood. Was often referred to as a “lazy eye” when I was growing up. Can’t say that I ever felt my sight wasn’t good enough for every day activities. My good eye seems to have coped pretty well. Is there a particular lens I should perhaps do a little bit more research into. I have mild 75 astigmatism and a 5.75 reading prescription.
If my right eye is still 20/20 but my left eye needs an IOL due to cataract. Will a monofocal suffice since I can still use my right eye for reading near? Or will I have to close my left eye to focus. ? Leaning towards PanOpix
I have had my PanOptix for 4 years now and I could not ask for anything better no glasses for anything and can read the smallest print up close. Sure glad I went with them. Just make sure you choose the right doctor that does a lot of them. A lot of doctors try to talk you out of getting them.
@ I think I was a -75 and stigmatism but I got to where I could not see very good in my right eye at distance but could read with it really well after the Cataract developed. No prescription could fix it at distance. Now I am 20/20 far and near.
Great video and information. I would love to see one more category and that would be best lenses that most insurance would cover. I am about to have Cataract surgery and the over $2,000 per eye for premium lenses is outside my price range and that of many other people, unfortunately.
One thing that is under communicated: The side effect of halos, glare etc from diffractive IOLs can be mitigated with a mix and match approach. I have panoptix in my non-dominant eye with what I would say severe dysphotopsia, but with my natural eye also open I barely see any halos. I would say almost none. In the future, hopefully not until many years, I will implant an EDOF IOL or monofocal in my other eye knowing that my overall vision will be amazing with no (minimal) dysphotopsia.
Hi, how is your near vision? I have 20/20 far vision in my left eyes and 20/40 in my right with some opacity, I feel OK with my distance vision with my bouth eyes open, I'm thinking in multifocal for my right eye because my far vision doesn't bother me with both eye open, I am very picky too, what do you think?
@@drjorgegalue I have amazing vision from 35 cm to infinity. Crisp/ in focus. I know mixing IOLs work very well, as long as you know how to mix and there is not too big difference in refraction between them. Some doctors do that but for some reason some think that both eyes must be the same. I need +1.75 glass to see crisp up close with my natural eye, but since my panoptix see crisp my binocular up close vision is perfect. Never use readers or feel any imbalance.
Please clarify what is meant by A+ vs A-. Do you mean going from 20/20 to 20/30 or worse? I am a pilot and need to see 20/20 distant and at least 20/40 near and intermediate, but preferably 20/20 at all distances. I also have astigmatism of 3 in left and 4 in right eye. What lense will work for me?
Ask your doc about the envista envy. I’m 5 days post op and can’t believe how well I can see. I also had a raging astigmatism and this lens is apparently one of the best for correcting it. This is a newer lens that just got FDA approval in Oct, but obviously underwent serious testing and trials prior to. Read up on it! I can’t wait to get my other eye done.
Thanks for the informative video. I just got back from an evaluation. My Dr. at first didn't recommend LAL because he said my pupil wasn't big enough after dilating. Then he rechecked it and said it was 6.1, which passes the minimum. Will that be ok. Should I be concerned. Thanks
I just had the vivity lens but in one eye. The other eye can see better up close do now they work together but I’m still struggling to see it’s blurry at distance and straining my eyes. And yet when the other eye gets done I want to be able to not where reading glasses so hopefully they can do a different lens? I’m only 4 days post op. I had laser back in 2008 and since Covid vaccine I ended up needing glasses and then two years after that they discovered cataracts. So the other eye is just starting but I’m afraid I will still need glasses for distance and up close down the road

Please tell me what your thoughts are on the Ray one EMV by Rayner , made in the UK. Developed by Graham Barrett A Monifocal lens that offers EDOF. Thank you
I use them since 2020. Using mini mono-vision gives excellent quality and range . Big plus is also that you can fix a non perfect result with glasses. Using multifocal lenses is risky, expensive, and quality far vision can be compromised. My other favourite lens is a Hoya vivinex .I aim them the same as a Rayone EMV . As of today I would have either lens in my own eyes

I got Symfony in 2018. I was 42 at the time. Got it explanted last year. No near vision. Severe PD and glow around surfaces and general lack of sharpness. Now I have PanOptix in both eyes.
Dear doctor, what lens do you recommend for RK patients ? I had an RK treatment in 1991 (nearsighted -4,5) but now I'm farsigthed +2,5 and even +3,5 for small print. Many thanks Steven
Hi, for most patients I’m targeting plano OU since I’m setting the expectation that this is my preferred lens for the best far vision. However patients will sometimes want some added near vision, so I may target -0.75 or -1.00 in the non-dominant eye
Hi. I am a senior too.... Just had my pre op for removing my cataract. Talk to your ophthalmologist ( office visit ) many options. I reviewed so many online prior to my office visit, had many questions which were answered. I am going with the Extended range of vision IOL in a couple of days.
I didn't get to choose. Or know that I could. My doctor never discussed lens with me. When I asked him about it, he said "I'm the doctor and I'll make that decision as to what is best for you." 😢
I have the Panoptix lens and I don't know what the big deal is at night. Sure there is a minor halo effect but it remains very near to the source so it doesn't take up much of the view. It seems like the 3,4 or 5 "rings" that you see around a source of light are not solid rings. The rings are made up of dots which make rays out from the light source (remember the "star" filters you could buy for nighttime photography?) It's like that except made up in many dots or tiny dashes. Relax - no big deal. Go for the no glasses required for the rest of your life.
Fortunately, there are statistics that show the truth about trifocal lenses. Just because it's OK for you, doesn't mean it's OK for anyone else. There are also optical requirements to see if such lenses are even suitable for a particular patient.
Thank you for the detailed description of the “halo” effect that so many videos and articles describe. sounds like you are describing the radiating star-effect that I would use when I was taking 35mm SLR photos!
My experience has been different. I’m 2 months out and night driving is…an adventure. The halos are very pronounced, and light spikes shoot across my entire field of vision from every light source or reflection (road signs, etc.) Additionally, the glare and light spikes also occur during bright sunlight, just to a lesser degree. While my near vision - 6 ft or less - is great, beyond that I’m worse off and more uncomfortable than I was with classes. At this point, I regret having the surgery.
I'm not thrilled with mine either. The surgery caused many floaters. Plus objects in my peripheral are not clear making me unsure while walking. Not happy.@charleslee9725

You can’t ever predict who will hate a multi focal lens. I have had to remove them. And I no longer implant them. People who want to be eyeglass free have a serious first world problem. Having done missionary cataract surgery for blind people in the third world, I have lost sympathy for whiners who come in expecting perfect vision without glasses, near and far, like they were 20 years old again. If you can see very sharp and clear with a monofocal lens implant and glasses after cataract surgery, count your blessings! Toric IOLs are a different story, getting good distance vision without glasses by correcting the astigmatism rarely pisses off a patient! I’m happy to use torics. But I’m not spending hours of chair time trying to placate an unhappy multi focal IOL patient. I have tons of patients in my practice who got multi focals elsewhere and are still wearing glasses!! I have also had some multi focal patients who are very pleased. It’s too much of a crap shoot for me, there’s plenty of Ophthalmologists who will implant them and if my patient demands them I just send them down the road to a colleague.
your comment reminds me of a 20/20 episode on A Type Personality tri-athletes that are aging and going on 3-4th knee rebuilds, expecting same results, making for a frustrated surgeon, lol addiction to exercise...

If my right eye is still 20/20 but my left eye needs an IOL due to cataract. Will a monofocal suffice since I can still use my right eye for reading near? Or will I have to close my left eye to focus. ? Leaning towards PanOpix
I got Panoptix in both eyes and I love my new vision.
Is LAL+ good for a person who had a cataract removed who also had a vetrectomy?
I have had my PanOptix for 4 years now and I could not ask for anything better no glasses for anything and can read the smallest print up close. Sure glad I went with them. Just make sure you choose the right doctor that does a lot of them. A lot of doctors try to talk you out of getting them.
@@jbell6745 what was your vision prior to surgery? Such as in diopters or prescription strength? Do you know if you also had astigmatism?
@ I think I was a -75 and stigmatism but I got to where I could not see very good in my right eye at distance but could read with it really well after the
Cataract developed.
On this list, the Aspire is covered by insurance
Some UT docs charge $15,000-20,000 for both eyes.
Or get mono vision
Just wait until you go on Medicare and it doesn’t cover any of it
One thing that is under communicated: The side effect of halos, glare etc from diffractive IOLs can be mitigated with a mix and match approach.
100% Agree!
I’m 51 with quite bad near sightedness and astigmatism. I have a consult for lens exchange surgery tomorrow. My job requires me to be in front of a computer 8-10 hours a day, I am physically active and do t do a lot of night driving. What do people suggest?
Eye Dr said my right eye has cataract needs to be operated on first new lens, then 3mnths later have left eye done, quite concered about cataract surgery, my long distance is going,
In my opinion for me A+ quality vision, I don’t mind reading glasses
Dear Doctor my left eye injured when i was 10 yrs in 2004 and we left without do anything but vision becomes decreasing and black part of my eye becoming white then it operated on 2004 like cataract, then again operated 2012 now i am wear a glass with the power + 2.5 i think that too not clear, and i can't able see the sun light or focus light directly in my left eye, without spectacle for left nothing visible clearly there is no problem on right eye, so there is some lens in my left eye which is fixed while operation so now i want to change the lens and i want spectacle free, is this possible, please suggest ❤
The diameter of the IOL is 5.75mm. Worries me for younger folks with large pupil who will see the IOL edge.
What’s the cost of Rayner EMV? Thinking of this or PureSee as options
Puresee iol?
Sir I am 33 now, had implantation of symphony bifocal iol in left eye, now I want to get 2nd eye operated for cataract, I am not able to decide, which lens I should opt
So young! There are many options to choose from. How’s your vision with the Symfony lens?
@@DoctorKrad I find my day vision good but night vision is compromised it feels like multiple sparkles around the light source, I doubt if I get right eye IOL with multifocal then I will feel like blind in low light conditions. And as discussed with a doctor for getting monofocal implant, then in one eye I will feel more brightness than the other along with use of glasses.
@@TradeNiftydailywhat abouth Eyhance?
I got Symfony in 2018. I was 42 at the time. Got it explanted last year. No near vision. Severe PD and glow around surfaces and general lack of sharpness. Now I have PanOptix in both eyes.
@@aniketnavalkar5974 how is your vision now ?
Dear doctor, what lens do you recommend for RK patients ? I had an RK treatment in 1991 (nearsighted -4,5) but now I'm farsigthed +2,5 and even +3,5 for small print. Many thanks Steven
I too did Rk and do not know what lenses to select
Hi Dr. Krad, do you typically mini-mono all/the majority of your LAL+ patients?
Hi, for most patients I’m targeting plano OU since I’m setting the expectation that this is my preferred lens for the best far vision. However patients will sometimes want some added near vision, so I may target -0.75 or -1.00 in the non-dominant eye
😂 i doctor i have diabetic
can you pleased please 🙏 advice which len vivity clear view 3 or panoptix?
i really don’t want any glasses at all please 🙏
I'm a senior. I love softball. Whats the choice for me. Hitting and catching a ball needs sharp vision
Hi. I am a senior too.... Just had my pre op for removing my cataract. Talk to your ophthalmologist ( office visit ) many options. I reviewed so many online prior to my office visit, had many questions which were answered. I am going with the Extended range of vision IOL in a couple of days.
@@shirleyott2669 Please let me know the outcome
I didn't get to choose. Or know that I could. My doctor never discussed lens with me. When I asked him about it, he said "I'm the doctor and I'll make that decision as to what is best for you." 😢
Such doctors give the profession a bad reputation
Acrysof or Clarion?
Here in the UK 🇬🇧 LALs aren’t currently available so I am considering the J&J TECNIS Toric which is getting positive reviews….as usual great information from Dr Krad and really appreciate your opinion 👍
Tecnis Toric is a very good lens. Best of luck to you!
@@DoctorKrad UPDATE: just completed the second eye and for an EDOF lens so far, all is looking pretty good, even able to read my phone & ipad most of the time except in low light where reading glasses are required also, no night halos or excessive glare so far…….#LifeChanging
Don't go for Toric , it rotates inside the eye and can move ... Have you heard of J&J Pursee ..heard it is a very good lens ... probably I will go for this one
Hi Dr. Omar kif Halik ? I did a cataract surgery last week on my left eye and had B&L monofocal IOL... My age is 42y and I have no diabetes... do you think my right eye will develop a cataract too ? Currently nothing... also my left eye has many redness after the surgery.. is that normal ??
The surgery cost here is super cheap... it costed me only 430 USD 😅😅😅😅😅😅
Speedy recovery. We are similar ages and have similar problems. I'm also considering getting an Eyhance monofocal implant in one eye. My other eye is still fine. Did you experience vision differences between your eyes after your surgery? I would be very happy if you talk about this issue.
@@atalayonline Thanks.... my good right eye is -2 ( nearsighted), I requested from doctor to put medium/far vision for my left eye whih called ( mono vision ) now my both eye have different vision strength but so far I don't have problem....it made me even happier as now I can see further object like tv etc with the left eye..... I might do lasik for the right eye but don't know if that possible or not.... we can have further communication if you want...
@@Mo2008dithank you, What kind of visual impairment does he have in his right eye? I guess your left eye's middle and long distance vision is better than your right eye?
@@Mo2008di If you compare your two eyes in terms of the vividness of colors, which eye sees the colors more vividly?
@@Mo2008di I don't think you should have LASIK done on your right eye, because cataract surgery on eyes that have had LASIK can be a little more complicated. The less problems you have for cataract surgery, the more successful results you will get, of course, your doctor's opinion is more important.
Yes
Doctor, for an out of towner, what would I expect to pay for replacement / IOL surgery to get these LAL+ lenses? If I had to pay out of pocket? Here in Canada, we seem not to have the range of availability, and at 60 I feel I would rather get the latest, best tech than whatever our surgeon has available.
What about mid-vision lenses vs far vision?
Wow very informative
Listened to your commentary with interest , I am 82 I got my cataract operation one year ago , I have stigmatisation, I got the operation under nhs conditions , e.g. basic lens , I require glasses for distance vision and reading , a nuisance I know , but saying that my distance reading is excellent I can read a number plate at 36 metres almost twice the distance required to drive , with reading glasses I can easily read the smallest letters on the chart , I have a slight sparkle with headlights at night but no haloes , if I had been much younger I would have enquired and went private for a different lens , but considering all the facts I am delighted with the results,
The disscussed for my vsion there 3 types low quality they Start at $200 and it's covered by the Canadian Goverment thats the surgery pluss weather you have it done at the Hospital or his clinic here IOn Toronto Canada . the the next Level up $200 on top and the 3rd $200 this Vibrant color's No Glasses. dr David Assad he qouted this to me after i seen him.
Panoptix was supposed to be the best lens but after watching this video I have changed my mind.
I have Panoptix toric lenses in both eyes. My best friend got the same lenses. It was the best decision I've ever made.
Thank you!
